dies in apparent industrial accident in Denver (a worker was
killed in an apparent industrial accident about 11:10 a.m. at a
distribution center – when firefighters and emergency personnel
arrived, a co-worker was performing CPR on the male victim – it
appeared that the victim{apos}s upper body was crushed – he was
pronounced dead at the scene – the cause of the fatal incident is
under investigation)
MINE FATALITY
Rescuers pull bodies from Mexico mine, others trapped (five
bodies recovered, others feared dead – rescuers searched for nine
miners feared dead on Wednesday, a day after a coal mine explosion
killed five others – the explosion on Tuesday morning was caused by
a build-up of methane gas)

Worker dies from scaffolding accident at old Amway Arena (a
worker, 58, died overnight after suffering injuries from a
scaffolding accident – he was struck with a metal scaffold, which

Gas well worker killed by heavy equipment at Smyrna drilling site
(a 23-year-old gas well worker died following a drilling site
accident – he was fatally injured at about 9 a.m. at a drilling site
– he later died during surgery)

Man seriously burned in fire at Rockledge laboratory (fire was
reported at a marine-related testing company – a man remains in a
hospital after being burned over 40 percent of his body following
the fire at the laboratory – the badly injured worker apparently
either drove or was taken to a walk-in clinic, before being rushed
to an area hospital and then airlifted to a burn center – they do
know that they were testing some fuel tanks for boats and there was
some fuel in the building, but the cause of the fire is undetermined
– investigation is under way)
FIRE FATALITY
Factory fire kills one in Tangerang (one worker was killed and
seven others injured in a blaze that engulfed a paint factory – the
deceased worker, 38, was trapped inside the factory along with the
seven injured workers when fire razed the building – co-workers
managed to escape and suffered serious burns – the fire started at
10 a.m. and quickly engulfed the factory compound – still looking
into what caused the fire)
ELECTRICAL FATALITY
Electrical Fire claims maintenance worker (a man, 36, died in an
electrical fire at a vacant hotel – he was pronounced dead at 12:20
p.m.- he was part of a crew that was doing maintenance work at the
closed hotel when the fire sparked about 11:20 a.m. – firefighters
found heavy smoke in the garage area of the building and later
located the worker in a mechanical room – the cause of the blaze
remains under investigation by the county fire marshal{apos}s office,
though the chief said it appeared to be electrical in nature)
UPDATE on FATAL EXPLOSION
DuPont fined for explosion (OSHA has cited the factory for 17
serious violations following an explosion at the plant last November
that killed one contractor and injured another – plant was fined
$61,500 after a chemical storage tank that a contractor was welding
ignited because highly flammable vinyl fluoride fumes had seeped in
from a connected tank, the investigation found – the resulting
explosion at the facility killed the worker instantly – rhe
contracting company also was cited for eight serious violations and
fined $55,440 – OSHA investigation found that the tank, which had
previously contained a polyvinyl fluoride slurry used in making
solar panels and some interior airplane parts, was supposed to be
empty and thoroughly cleaned prior to the welding work – it had been
emptied, but it was still connected to two other full tanks and
fumes seeped through connecting pipes – OSHA said the pipes should
have been disconnected, but they weren’t and that the tank should
have been vented to allow any trace fumes to escape, but it wasn’t –
workers conducted the necessary air monitoring in the area outside
the tank, but not inside the tank, where the fumes existed – the two
contractors had no idea that they were doing welding work on a tank
that had previously contained such a flammable chemical)
UPDATE on HYDROGEN SULFIDE FATALITY Hydrogen sulfide leak kills one,
hurts 2 others (one person has died because of a hydrogen sulfide
gas leak while performing maintenance work on a natural gas pipeline
– two other workers were injured when the gas leaked and the workers
were exposed to the highly toxic fumes – the three workers were
using a wand steamer on the gas production line to remove ice fog –
the steamer caused the accidental release of the hydrogen sulfide –
the dead worker was an employee of an oil and gas exploration firm –
the second injured worker was rushed to a hospital where he is still
being treated, while the third worker escaped unharmed – one of the
Mounties who responded to the call and was conducting a road block
nearby the site was also injured and sent to the hospital for
treatment – the other RCMPs who responded used breathing packs as
they helped the two workers)

Fire Sparks Up at Natural Gas Processing Plant in Midland County
(no word on what caused a fire at a natural gas processing plant –
fire crews were called to the plant just after 3 a.m. – the call
went out as a small explosion, inside one of the compressor
buildings – a compressor caught fire and crews were able to turn off
a valve and put the flames out – no one was hurt)
HYDROGEN EXPLOSION

truck explodes at Iowa nuclear plant (a truck delivering
hydrogen to a nuclear power plant has exploded, sending the driver
to a local hospital – the incident posed no threat to the public or
the plant – the facility continues to operate safely – its internal
fire brigade and local firefighters responded to the blaze)

Man severely injured in Fremont forklift accident (firefighters
rescued a 50-year-old man trapped under a forklift – firefighters
found the man with one leg completely trapped in a forklift{apos}s wheel
well – crews stabilized the forklift, then used airbags to raise the
vehicle enough so they could extricate the victim from underneath
the equipment – it took about 25 minutes to free the man from the
forklift – he was transported to a Hospital and suffered severe
traumatic injuries)

WALL COLLAPSE FATALITY
Ministry probes fatal industrial accident (investigators were
piecing together the circumstances leading to the death of a
contractor, 50, who was killed by two large cement slabs – he and a
colleague were working on a porch at a home when two pieces of
cement came loose and fell on his chest, trapping him for about 40
minutes – he was initially able to speak to paramedics, he lost
vital signs before firefighters managed to free him – his co-worker
injured his ankle in the incident)
WORKZONE FATALITY
Worker hit by car ~~ 2nd time in a week (a vehicle struck a road
crew flagman, 40, about 4:30 p.m. – the windshield of the car was
shattered when it struck the man who was flagging traffic – he is
the second worker from the same crew to be hit in less than a week –
the worker was wearing a vest – on April 26 about 8:50 p.m. a fellow
road crewmember, 58, was hit by a vehicle driven by a local man in
what police called an "alcohol-related" accident – he died of his
injuries at a Medical Center)
WORKZONE
One injured in Washington County crash; trooper, construction worker
unhurt (a state trooper narrowly escaped injured in a
three-vehicle crash – the accident occurred about 11 a.m. in the
southbound lanes – the trooper was out of his vehicle at the time of
the crash – he had pulled off on the side of the road in a
construction zone and was speaking to a man working in a bucket
truck when the officer saw a sport utility vehicle approaching at a
high rate of speed – the trooper and the man jumped out of the way
before the SUV slammed into the back of the patrol car, which was
pushed into the bucket truck – neither the trooper nor the
construction worker were injured, but the driver of the SUV was
flown to an undisclosed hospital)
TRENCH MACHINE FATALITY
Local woman killed in freak work accident (worker, 41, was
killed when a 2,000-pound trenching machine fell on her while she
was working a cable job – she was using the machine to install cable
when it tipped over and landed on her – a co-worker contacted a
nearby resident who called for help, but she was declared dead at
the scene)
DUMP TRUCK
Worker injured in dump truck accident (a man, 37, was
hospitalized after the truck he was steering plunged down a 12-metre
embankment – he was freed from the driver’s cabin of the severely
damaged dumper truck by local fire fighters after the incident on a
construction site – he suffered multiple injuries and was
transported to a clinic)

Construction workers injured in fall at Minnechaug Regional High
School construction site (three construction workers were
injured at the construction site of the new Regional High School –
one worker was on the roof fastening his safety harness when he slid
off the roof and took a second worker down with him – they fell from
a peaked portion of the roof onto a third worker on the ground – all
three workers were taken by ambulance to a Medical Center – the men
appeared to have suffered broken bones, but their conditions did not
appear to be life-threatening)
Poisonous Gas Leaks in Northern India, One Dead (a worker in a
factory was killed after poisonous gas leaked in the unit -incident
took place at a plastic-making unit, housed in the basement of a
building, where one worker died and two others were admitted to a
local hospital unconscious – officials found drums full of various
chemicals used to make plastic bags and other such items)

2 ONG workers injured by rollover accident (two natural gas
workers were injured when they were hit by a vehicle while working
on a piece of pipeline – there were three men performing routine
maintenance on the pipeline – one man was standing above the ditch,
in which the other two men were working, when a Chevy Blazer rolled
over in the ditch and hit the two men – both men in the ditch were
injured – one was treated for contusions and released – the other
was ultimately transported to a Medical Center with back injuries)
CRUSH FATALITY
Falling Glass Causes Death of Florida Construction Worker (a
construction worker died after hurricane-resistant glass and other
materials fell on him while inside a container truck – rescuers had
to cut a hole into the side of the container to try and reach the
trapped worker; however, he is reported to have died at the scene –
he was delivering windows to a vacant home and was in the back of
the 18-wheeler when the two glass windows weighing 3,000 pounds fell
and crushed him – another worker, also reported to have been in the
truck when the scaffolding and glass shifted, is said to have
suffered an arm injury)
TORNADO FATALITY
Worker killed in New Zealand tornado (a 37-year-old construction
worker, a father of two young children, was killed and 14 other
people required hospital treatment when a 200-kilometre per hour
tornado wreaked havoc – the man who died, was hurled into the air by
the winds and slammed into a concrete car park – a student nurse
tried desperately to save his life)

Fire destroys Calvary Chapel Church in Enid (a fire broke out at
a Church when some plumbing work was being done – a workman’s torch
caused a fire that destroyed the church – a worker doing plumbing
work started the fire but the man was able to get out of the
building unharmed)

Trench collapse in Hamilton buries worker up to his knees (a
trench collapse buried a contractor up to his knees, but he escaped
serious injury – he was performing work for the township sewer
department around 11:10 a.m. when the accident occurred – they were
running the sewer line and the worker was working in the hole and it
collapsed and he became trapped up to his knees – co-workers were
able to free him from the hole outside a home before the arrival of
firefighters – he was taken by ambulance to a hospital for
evaluation but not believed to be seriously hurt)
UPDATE on CRANE FATALITY
Jury Awards $1.45 Million in Bellevue Crane Collapse Case (jury
awarded $1.45 million in damages to a man, 39, left permanently
disabled in a deadly construction crane collapse in Bellevue four
and a half years ago – he was awarded a total of $1.45 million in
economic and noneconomic damages with the jury placing 70 percent of
the blame for the collapse on an engineering firm and 30 percent on
the general contractor – his wife also was awarded $55,000 in
noneconomic damages – he was operating the 210-foot tall crane
during construction of the Tower 333 when the crane collapsed,
killing a resident of a nearby building)

Worker from Bloomfield Construction Business Killed in Accident in
Union (a man was killed when the six-ton equipment he was
operating fell down an embankment – he was smoothing part of a 1.45
mile path)

CHEMICAL REACTION
Hotel worker hospitalized after mixing cleaning chemicals (a
hotel maintenance worker was transported by ambulance to the
hospital after inadvertently mixing cleaning chemicals which, when
combined, created a chlorine vapor – accident occurred at 10:12 a.m.
– firefighters found the worker on the first floor where he was
experiencing non life threatening respiratory irritation – an odor
of chlorine was detected by firefighters wearing protective clothing
who checked for toxicity and ventilated the area – eighteen guests
staying at the hotel were allowed to remain in their rooms after it
was determined that the chemical vapor was isolated – the chlorine
vapor was released when the worker poured a cleaning solvent from a
larger container into a smaller one which contained residue of
another incompatible chemical)
CHEMICAL REACTION
Investigations into Summerside chemical accident (investigations
are underway following the chemical accident that took place – after
spending more than eight hours in hospital being monitored, the
building{apos}s carpenter was back at work – in all, 22 people were sent
to hospital when two chemicals were accidentally mixed together,
causing a noxious gas – one person was still in hospital after a
release of chlorine gas)

Two badly injured after accident at C.V. Days site (two carnival
workers, 26 and 19, received severe electric shocks when one of the
rides they were helping to set up hit an overhead power line – one
was in fair condition and one was treated at the hospital for minor
burns and released – worker was apparently holding a metal cable
attached to one of the rides being erected by other workers- the
ride touched an overhead power line and received an intense shock –
one was conscious but appeared to be in severe distress, with
visible burns on his body, when emergency medical personnel arrived)

Worker seriously injured in fall at Snow Park Niagara Falls (a
worker, 56, was seriously injured in a fall from near the top of the
snow ramp – he was performing work on the ramp at the time of the
accident – a foreman and other workers told police they did not see
the victim fall, but heard him hit the ground – he appeared to
strike his head and was unconscious)
Woman burned in freak accident improving (a worker suffered
several burns after a gas line exploded – she was cutting grass when
her lawnmower hit a gas line that had been hidden by some dirt –
that caused the explosion, and ignited a fire – she suffered burns
to her hands and face, but officials say her long sleeve shirt and
goggles prevented more serious injuries)
