FATALITY by TRAIN Contract worker killed inside Bokaro Steel Plant (a contract worker, 30, was killed after being hit by a goods train while trying to cross the tracks – he was attempting to go to the other side of the plant by riding on his bicycle on a stretch that was not manned)
AMPUTATION by TRAIN Railroad worker hurt in accident in Fort Wayne (a Railroad employee was injured in an accident at about 7 a.m. – he fell onto the tracks and had a hand and foot amputated – the accident occurred as a general merchandise train was moving onto a storage track)
Worker Dies after Serious Accident at Keppel Verolme Shipyard (a worker, 43, was fatally wounded while carrying out maintenance work on an installation vessel at a shipyard – no details)
CRANE FATALITY @ DRILL RIG Worker killed at Oklahoma well site (a drilling contractor employee, 51, has died during the drilling of a horizontal well – he was struck in the head when the counterweight rig block fell from the top of the crane – he was not struck by the rig block itself but a piece of the apparatus that was supposed to hold the block in place – he had just begun his shift and was standing on the drill deck when he was struck)
FORKLIFT related FATALITY Worker killed in accident at New Brunswick Pepsi bottling plant (a worker, 37, died in an accident at a bottling plant – the worker and a co-worker, both of whom were employees of an external contractor, were removing a floor guard that is used to protect storage racks p- the victim’s co-worker was operating an industrial lift truck and prying the floor guard with the forks of the lift truck – when the screws that held the guard in place let go, the guard hit the victim in the head)
HYPOCHLORITE CHEMICAL REACTION = CHLORINE GAS Eighth worker in hospital after gas accident (an eighth person has been hospitalized after an incident involving chlorine gas at a chemical plant – seven were initially hospitalized after the incident involving the bleach sodium hypochlorite; however, another worker brought themselves to the emergency room – the incident happened at around 4.45 pm while workers were loading a consignment of the chemical – during the procedure, an acidic substance was accidentally mixed with the bleach and a chemical reaction occurred, releasing chlorine gas within the building and into the air outside – it took eight units of the fire service more than three hours to contain the situation)
MINE FATALITIES Eight killed in Iran mine collapse (at least eight workers were killed after a coal mine collapsed – there were 40 workers in the mine when the accident happened in the morning – of them, the whereabouts of 32 others were not known – the possibility of an explosion in the mine could not be ruled out)
FATAL BOILER EXPLOSION Boiler blast kills 2 rice mill workers in Pabna (two rice mill workers, 30 and 32, were killed in a boiler explosion – the boiler exploded with a big bang when the worker were boiling paddy at the mill around 10:00 am)
FALL from CELL TOWER – PLEASE NOTE that although the article claims the worker “slipped from his harness”, I actually think the work just slipped and his harness is what saved him) Cell phone tower worker slips from harness, dangles 100 ft (a cell phone tower worker slipped out of his harness and was left to dangle high in the air for about a half hour – two men were working on a tower when one of them slipped out of his harness and could not make it back to the platform – the man’s coworker held on to him – he was also attached to a cable, and dangled 105 feet in the air while rescue crews got their ladder in place – the ladder extends to a maximum of 110 feet – firefighters safely helped the man to the ladder, and he was able to climb down to the ground)
Gearbox falls on truck worker’s head (a worker, 32, has been flown to a hospital with serious head injuries after an industrial accident – a large gearbox had fallen on the worker – paramedics spent about an hour treating the man before he was flown to a hospital in a serious but stable condition – the employee sustained major injuries to his head and face)
REFINERY FIRE Fire put out at Montreal’s Suncor refinery (police reopened the eastbound service road of Highway 40 after firefighters brought under control a blaze that tore through an oil reservoir of the refinery – the fire first occurred at around 9 a.m. and a small spill of flammable liquid near the highway was observed at around 10:15 a.m. – refinery firefighters struggled to control the fire, and called for reinforcements from the city Fire Department – the fire flared up again around 2:45 p.m., but firefighters were able to put it out later in the afternoon – the environmental impact of the fire was “minimal”)
FATAL REFINERY FIRE (w/ FATAL FALL) Two die after fire in RIL’s Jamnagar refinery (two persons have died following a fire incident at a refinery – the incident took place in the crude distillation unit (CDU) of the refinery – maintenance work was going on in the unit, and it was shut down for the purpose – when the fire broke out, one of the two persons doing maintenance work in the CDU fell from a height and died on the spot, while the other sustained burn injuries, and was admitted to hospital – he succumbed to injuries late Thursday night – the persons were employed by a contractor – the reason for the fire is not yet known)
EXPLOSION No injuries in N. Ky. plant explosion (an explosion at a northern Kentucky plant scattered debris but caused no serious injuries – the explosion occurred as a truck was offloading product into a storage silo on the side of the facility – all plant personnel have been accounted for, and there were no injuries to plant workers or firefighters)
RELIEF VALVE ACTIVATION Reports of minor explosion at Dow plant (it happened around 7:30 p.m. – the explosion was small, and no one was injured – there was no threat to the community, and no shelter in place was called – officials later said there was no explosion at all – instead, they said a safety device was responsible for making the loud noise – the cause of the incident is under investigation)
FATAL EXPLOSION from HOTWORK on STORAGE TANK Saudi- 6 die in Jubail plant explosion (six workers were killed in the explosion of a chemical waste plant – there was an explosion in the diesel and chemicals tanks – two trucks also caught fire – the fire broke out at 10.30 a.m. and was put out within two and a half hours – a firefighter sustained minor injuries – the explosion occurred while maintenance staff were doing welding work on a tank containing benzene and blended diesel)
EXPLOSION when cutting ACETONE DRUM Scrap metal plant explosion sends one to the hospital (a man was taken to the hospital following an explosion at scrap metal plant – the explosion happened about 11:30 a.m. as the worker was using a cutting torch on a barrel that contained acetone – the acetone ignited and caused an explosion that could be heard up to 500 yards away – worker was thrown an estimated 15 feet)
AMMONIA Firefighters cleaned after leak callout (two firefighters were decontaminated after an ammonia leak at a meat processing plant on Christmas night – two fire engines were called just before 10pm to the meat processing plant, where crews identified an ammonia leak – the crews were put on standby until the gas valve could be turned off – crew members wearing breathing apparatus and gas suits entered the building and were able to isolate the valves to the ammonia system – crews were at the meat processing plant for 1 hours)

CRANE Benicia Worker Severely Hurt In Accident With 3-Ton Steel Beam (a man, believed to be 19 or 20, is recovering at a hospital after a three-ton steel I-beam fell onto his legs while he was working at 9:20 a.m. – he had been pinned underneath the beam for about a minute after it fell from its chain-and-hook grip while being lifted with a crane – coworker used the crane to move the beam off of the victim – fire crews provided care to the man, who remained conscious and alert, and he was then transported to a hospital – he suffered severe injuries to both of his legs but is expected to survive)
CRUSHED by GARAGE DOOR FATALITY Construction Worker Dead After Accident at Former Home of “Burning Man” (a construction worker, in his early 20s, died at the still-ruined former mansion of “Burning Man”, who torched the house in July, 2009 – a man working for the home’s new owner was crushed by a large, roll-up garage door that he and a co-worker were removing – the garage door was large, RV-sized, and the man was pronounced dead upon arrival at a hospital)
FATAL SCAFFOLDING COLLAPSE Worker killed, 2 others injured during school construction accident in DeKalb County (a construction worker died and two others were seriously injured while working on a new elementary school – the cause was a 30-foot-high scaffolding that collapsed just before 8 a.m. – the men were taken to hospitals)
MANLIFT ENTRAPMENT Morgan Sindall worker crushed by cherry picker (a worker, in his 20s, was taken to hospital after he was crushed by a cherry picker he was using – he was working on a construction site at a school when he became trapped between steelwork and a cherry picker he was operating – he had to be cut free by fire crews at the scheme before being rushed to the local hospital with multiple injuries)
FATAL FALL Worker dies after falling at San Francisco construction site (a construction worker, 50, fell to his death after falling more than a dozen feet at a house under construction – the fall was reported at 10:37 a.m. where a three-story home is being built – he was standing on scaffolding near an unfinished stairwell at the time – after other crew members did not see him for about 10 minutes, they went looking for him and found the man lying on the concrete below – the distance of the fall was estimated to be somewhere between 12 and 35 feet – he was taken to a hospital, where he died)
FATAL FALL thru HOLE Man falls to death at Sengkang construction site (a worker, 32, has died after falling off a 27-story building under construction – his body was found at the foot of the building and pronounced dead on site by paramedics – the worker was plastering a wall on the roof of the building when he fell through an opening – he was part of a team of four workers)
FATAL FALL Syrian construction worker falls to his death in Basta (a construction worker fell to his death – he slipped while working on one of the high floors at a construction site and died immediately)
FALL thru ROOF Construction Worker Falls Through Roof (a construction worker fell through a roof and was knocked unconscious – he was helping to repair a roof when he fell through the roof – he fell approximately 25 feet onto a cement floor and bystanders were working on him when fire crews got there – there is no information on his condition)
FATAL FALL Airport worker falls to death (a contract labourer, 22, fell to death from the sixth floor of the new, under-construction airport terminal – he reportedly did not have a safety belt)
MISCELLANEOUS
FARMING FATALITY Sugarcane worker killed by harvester (a 14-year-old boy helping harvest sugarcane has been killed in a gruesome accident involving a combine harvester – the boy was working at a sugarcane plantation – investigators were shocked on arriving at the scene, finding body parts scattered in the field – the victim’s severed head and mutilated torso were found inside the harvester – the farmer, 38, told the police that he had recently hired the boy – the dead boy’s job was the collect the cane cut by the harvester for loading on trucks – he was new to the job and had been working with them for only a week – crew did not know about the accident until his mutilated body was found in the harvester – they suspected the boy had sneaked off to take a rest, and maybe gone to sleep in the cane and then been harvested along with the crop)
FATAL FALL from TREE NH tree worker falls to death in Mass. (a worker, 32, died in a tree-cutting accident on Christmas Eve – he was performing commercial tree work at a private residence when a tree limb gave way causing him to fall about 15 feet to the ground, where the limb landed on him – he was taken to a Hospital where he was pronounced dead – the large tree had been damaged in a recent storm and was being removed)
STREET SWEEPER Grapevine City Worker Injured by Street Sweeper (a city worker was injured by one of those street sweeper vehicles – early reports are that the worker was rolled over by the street sweeping vehicle – the worker had to be airlifted by medical helicopter for emergency medical care – Note, this intersection is also known as the most dangerous intersection in Grapevine)
CARBON MONOXIDE Worker hospitalized after CO found at Saskatoon hotel (a man was taken to hospital after carbon monoxide was detected at a hotel on Dec. 26 — the second such incident in two days – fire crews were called to the hotel just after 9:30 a.m. – a maintenance worker who worked in the basement was feeling sick – he was taken to hospital, although he wasn’t seriously ill – crews used gas monitors to find carbon monoxide levels of 120 parts per million near the boiler room – an unspecified problem with one of the boilers was determined to be source of the CO gas – the problem with the boiler was fixed and crews ventilated the basement until the carbon monoxide was at a safe level) |
