Worker unaccounted for after Texas plant accident (a contract
worker remains unaccounted for after a fiery explosion at a
petroleum plant storage unit – crews managed to cut off the source
of fuel that was feeding the blaze and, except for a couple of hot
spots, the fire was basically extinguished – the cause of the
accident is sought, but possible pipeline failure will be considered
– workers heard a hissing sound before the blast)
LOTO/GUARDING FATALITY
Industrial accident claims man (an industrial accident claimed
an unidentified worker – his arm was caught in machinery – when
emergency responders arrived the man was found to be deceased – the
man was working on a piece of machinery outside the shop and became
entangled in the power take-off portion of the machine – coworkers
said it happened very quickly, and by the time the machinery was
turned off by co-workers, the man had died – the man had been
employed at the business for nearly four months)

Work Accident
Results In Broken Arm (a food firm has been fined after a worker
suffered serious personal injury when his arm was pulled into a
moving conveyor – he was cleaning the conveyor, which had recently
been installed as part of a million pound improvement at the factory
when his arm was dragged into the moving machinery – he had to spend
nineteen days in hospital and undergo two operations after breaking
his arm in four places and fracturing his hand – it is likely he
will require more surgery in the future – an investigation by the
Health and Safety Executive (HSE) revealed that the machinery did
not have adequate guards in place – following the accident guards
were added to the conveyor at a cost of just £600)

Man killed in work accident at Rochester job site (a 24-year-old
worker was killed after he reportedly became tangled in the
machinery he was working on at a recycling job site – he was found
by a co-worker around 12:30 p.m. – they all took a lunch break and
he didn{apos}t come in – co-workers went to check on him and found him
caught in a conveyer device used by the laborers to recycle roofing
shingles – it appears that the worker{apos}s hooded sweatshirt got caught
in the belt – he reportedly was choked by the machine – he had just
been hired by the business on Jan. 13)
LOTO AMPUTATION
Linpac Packaging fined after severed finger (packaging firm
fined £5,000 after a worker, 49, had his finger severed – he lost
the top of three fingers on his right hand while trying to clear a
jam in a machine – he was clearing a blockage in a machine used to
dry plastic beads when the incident happened – he had removed a
discharge pipe to deal with the jam, when his hand came into contact
with a 1.7-metre-long rotating screw, known as an auger –
investigation into the accident found that workers were often at
risk of being injured by the auger as they had to deal with
blockages in the pipe once or twice a shift – {amp}quot;Unfortunately, the
company became used to the machine being blocked, and did not
realize the danger workers faced when they removed the pipe to clear
jams{amp}quot;)

Inquiry into Lincolnshire forklift driver accident (an
investigation has been launched into an accident at a shipping
company that left a worker with a spinal injury – a forklift driver
loading a shipping container was injured when his ramp jolted
suddenly – the driver was airlifted to hospital after suffering a
fractured vertebra which pressed onto his spinal cord causing a loss
of feeling in his legs)

Blossvale man killed in lumberyard accident (a man, 35, was
killed in an accident involving a wood cutting machine – he was
changing the blades on the machine when a co-worker accidently hit
the wrong switch and turned the machine on at around 5:30 a.m. – the
co-worker was unable to turn the machine off before he was killed –
the deceased and the co-worker have been long-time employees of the
lumber company)

Man Suffers
Toxic Burns in Accident at Work (an accident at work caused
serious personal injury to a fifty eight year old employee, who was
left with toxic burns following a chemical explosion – he was in the
process of mixing sodium cyanide pellets with hydrogen peroxide when
the volatile solution exploded, soaking him in the hot toxic mixture
– he has been unable to return to work following the incident and
has suffered permanent scarring, due to having skin grafts to his
arms and upper body – an investigation revealed that approximately
five times too much hydrogen peroxide was added to the sodium
cyanide pellets which resulted in an exothermic explosion)

Offshore worker sues after crane slams into platform (an
offshore worker has filed a lawsuit against the operators of a crane
after he was slammed into an offshore platform – he was employed as
a catering hand working on an offshore platform when the accident
occurred – he states he was injured when he was being transported in
a crane and the crane slammed into the platform – he is seeking
damages for physical, mental and emotional pain and suffering, loss
of wages, fringe benefits and wage earning capacity, physical
disability, medical expenses, punitive damages, interest and court
costs)

Plant fined for safety violation (a fertilizer plant has been
fined nearly $20,000 – the charges stem from an April 21 accident
where a worker was seriously injured after his arm got caught in a
conveyer belt pulley – the company was charged with failure to
ensure a safeguard on a conveyer belt pulley remained in place at
all times)

Utility worker suffers electrical shock in downtown West Haven
(a worker has been taken to an area hospital after being hurt by an
electrical shock on a utility pole – the worker had a burn on his
face – there were two linemen in buckets – after hearing a loud
bang, the witness saw one lineman use bolt cutters to free the wires
from the injured man; he then pulled him into his own bucket)
UPDATE on FATAL EXPLOSION
Families of 6 workers killed in blast sue Tesoro (relatives of
six of the seven workers killed in an April 2010 explosion and fire
at an oil refinery have filed a wrongful death lawsuit – the action
filed alleges the company deliberately ignored dangerous conditions
that led to the blast – a contractor who was burned but survived
joined the families in the lawsuit, which seeks unspecified damages
– lawsuit accuses the refinery of failing to inspect decaying
equipment and ignoring industry safety standards and federal laws
governing refinery safety – state Department of Labor {amp}amp; Industries
investigators determined the explosion was preventable – the
department fined the refinery $2.39 million – refinery has appealed
the fine)

(officials suspect a mixture of water or ice and molten aluminum
caused a small explosion at a recycling plant, sending one worker to

worker was using a front-end loader to place scrap aluminum into a
furnace to melt it down when an ice chunk or water got into the
mixture – the reaction caused the molten aluminum to spray like an
explosion – another employee took the worker to a Medical Center,

released – the employee suffered either second- or third-degree

plant burst into fire killing four factory workers – according to
one of the three survived workers, there were seven people residing
on the upper floor of the factory, and all of them were sleeping
when the incident took place – the worker said a large number of
flammable materials stored in the plant, including wood, had caused
the fire to spread easily)

roof on the structure were using a propane torch and accidentally

has been evacuated due to an ammonia leak – officials discovered
that a pipe connected to a 100-gallon container of anhydrous ammonia
had developed a leak – about 100 employees of the company were

OSHA investigating fatal accident at Hazelwood warehouse (OSHA
is investigating a workplace accident that killed a forklift
operator, 53, at a food warehouse – he was thrown from the forklift
and hit his head – a witness returning from his lunch break told
police he heard a commotion and was hit by falling boxes – he then
saw the worker being thrown off the forklift – he saw his body hit a
rail and his head and back hit the floor – he was able to sit up
right after it happened – he was talking and in pain when he was
loaded into an ambulance, but he died of his injuries less than
three hours later)

Crane collapses in
Yerevan killing one and injuring another (around 16:20 pm a
construction crane collapsed – according to preliminary information
the operator of the crane, 65, died and another person was injured –
no word on cause)
WORKZONE (M-DOT worker remains in
critical condition after being struck by vehicle (a driver struck
two DOT workers, 50 and 36, while they worked patching potholes –
one of the workers remains in the hospital in critical condition –
alcohol and speed were not factors in the crash – one worker has
worked for DOT for six years – he has been released from the
hospital, but his co-worker, a temp worker who has not been on the
job that long, remains in critical condition with head injuries –
the men were wearing the right safety gear, had the appropriate
signage on the scene and a flag crew – {amp}quot;as they turned around to
walk back towards their truck, one of the M-DOT workers was already
there, and the two turned around, as they did so they stepped in
front of the oncoming traffic and was struck by the female driver”)

building (two construction workers died after they fell five
stories through an open elevator shaft at a construction site – the
iron workers, 51 and 49, were rushed to a Hospital in serious and

clothing became entangled in a post-hole auger attachment to a
compact tractor – the accident occurred at 10:38 a.m. while he was
involved in digging a hole for installing new fencing at a park – a
co-worker immediately controlled bleeding)
SALT SPREADER AUGER
Worker freed from salt spreader (a worker salting pavement at a
shopping mall got his arm caught in the spreader machinery and was
taken to a Hospital – the man was using a large augur style spreader
{amp}quot;not one of the tiny ones you see attached to the back{amp}quot; of the truck
– somehow his arm was fed into the augur that feeds the salt to the
impeller, which broadcasts the salt – accident occurred shortly
after 9 p.m. at the shopping center – a passer-by saw him entangled
and called 911 – with the assistance of the passer-by and one of our
company officers, they shut down the truck, disconnected the
hydraulics and reversed the mechanism to disentangle him)
