LOTO/GUARDING FATALITY Worker killed at Endicott Clay (worker, 42, died when he was crushed by a machine shortly before 6 a.m. – upon arrival, rescue workers found that the machine had crushed a male worker)

MOBILE EQUIPMENT FATALITY Man Killed in ‘Massive Crush Injury’ Accident at Rock Recycling Business in Sun Valley (worker, 21, at a rock recycling location died after being crushed in an industrial accident – worker had already died from what was described as a “massive crush injury” when emergency personnel arrived at the site at about 1:45 p.m. – a front-loader was in the process of dumping its load onto a dirt hauler when it backed up and ran over a worker while moving to another portion of the yard)

FORKLIFT Worker critically injured in forklift accident ID’d (worker, 62, was seriously injured in a forklift accident – there was some sort of accident in which the worker got his arms caught under a forklift – he was operating a forklift when another part of the forklift came down on both his arms)

FORKLIFT FATALITY Worker dies in forklift accident (worker was involved in an accident with a forklift and died of his injuries)

DOCK PLATE FATALITY Man crushed in workplace accident (truck driver, 30, has died despite desperate attempts to free him from under a loading ramp which collapsed onto him – it is believed he was killed instantly when he became trapped under the ramp at the rear of his trailer – workers from a site nearby tried to use a forklift to free the man)

FATAL FALL w/ ELECTROCUTION Magic Valley worker ‘unresponsive’ after electrical accident (worker apparently fell and touched part of a transformer – the lineman joined the company more than 19 years ago — died at a local hospital – the man’s partner pulled him away from the electrical equipment and performed CPR – emergency workers rushed the man to a local hospital)

FATAL FALL Worker killed after falling from platform barge (welder, 44, died after he fell 55-60 feet from a platform barge at a business)

SANITATION Keys sanitation worker may have been crushed to death (a sanitation worker, 59, may have been crushed to death by a garbage truck – driver of the garbage truck told deputies shortly before 10 a.m. when he didn’t hear anything from his co-worker, so he got out of the truck and found him unresponsive on the ground next to a dumpster)

CHEMICAL FATALITIES Toxic fume leak kills 4 workers (four scrap workers died after inhaling toxic fumes that leaked from waste chemical transported in drums – the drums were transported by a company which reportedly manufactures agro-chemicals, which include fungicide, herbicide and pesticide – three of the workers died on the spot and the fourth died en route to hospital)

HOTWORK Cell phone tower near Heritage High catches fire (insulation on a cell phone tower caught fire – fire started as maintenance workers were conducting welding operations on the tower just before noon – flames quickly spread along the insulation of wiring inside and outside of the tower – firefighters got to the scene and found “a massive amount of fire at the base of the tower, with wires burning along the full length of the tower” – no injuries – extensive damage to the tower, and it is completely disabled)

HOTWORK Employees evacuated after fire at Briggs & Stratton in Poplar Bluff (a fire broke out after some welding sparked dust to catch fire near a paint oven – the sprinkler system activated and the fire was quickly put out – employees were evacuated – no one was injured)

AMMONIA 3 hurt in ammonia leak on Northwest Side (three workers were taken to an area hospital after an ammonia leak shortly before 3 p.m. – three workers were transported to a local hospital, where their conditions were stabilized)

AMMONIA Five ill after ammonia leaks again at Ghaziabad meat factory (five women workers fell unconscious in a meat exporting unit after inhaling what was suspected to be ammonia gas – they were rushed to a nearby hospital where their condition was declared stable – management claimed it was conducting a safety drill and said opening an ammonia valve was necessary to convince workers that a leak had indeed taken place – police rubbished these claims, saying it was an accident due to negligence and that they would legally pursue the factory management)

ACID Acid spill after forklift punctures warehouse tub in Timaru (800 litres of formic acid has leaked at a warehouse after a tub was punctured by a forklift – area has been evacuated as crews deal with the spill -the acid was being neutralized with lime before it would be gathered up and disposed of in a safe and environmental manner)

CONSTRUCTION

FATAL FALL One dead, one injured after two workers fall from construction scaffolding in Campustown (two construction workers fell from scaffolding while working on a new apartment complex – one worker died and the other was injured and sent to a Medical Center – a ladder was required to gain access to the second-story roof, where the two workers had fallen approximately three stories off of scaffolding)

FORKLIFT FATALITY Hattiesburg man dies in forklift accident (worker, 49, died while making a delivery at a construction site – he was pronounced dead at the scene where he was pinned under a forklift around 9:15 – he appeared to be unloaded materials off a flatbed truck when the forklift flipped and pinned him underneath)

TRENCH FATALITY Construction worker killed after South Anchorage trench collapses (construction worker, 23, died after a trench collapsed – the construction worker had been working on a sewer pipe in a trench that measured roughly 7 feet deep by 15 feet across when it collapsed and buried him – co-workers tried to extricate him and once they got him out his injuries were just too severe and he died on scene)

WORKZONE FATALITY I80 Accident Kills Road Worker (flagger, 44, was struck and killed by a truck in a construction zone about 5:50 p.m. – worker was flagging traffic in the construction zone when a truck pulling two trailers drifted onto the shoulder, hit a guardrail and then hit the worker)

ELECTROCUTION Worker dies after job-site accident (worker, 42, was connecting downspouts to sewer lines on the site of a new public maintenance facility for the city when he came into contact with an energized underground power line that had been exposed by a backhoe operator working in a trench on the site – he was transported to a local hospital and later died of his injuries)

Construction worker, 60, dies following New Haven water rescue (construction worker, 60, fell into a Harbor while working on a new dock has died – he had been treated for brain cancer a year and a half ago and may have suffered a medical emergency that caused him to fall – he was only in the water for a very short time when he was pulled out by his co-workers, who began to give him CPR – he was assisting a welder in the front of the barge, handing him metal rods, when he disappeared)

FRONT-END LOADER FATALITY Storm worker killed in accident (worker, 38, working to clear debris left by the ice storm that devastated the area in February was killed – he was operating a construction loader stacking brush around 2:49 p.m. – the drive shaft of the loader broke, causing the equipment to lose its brakes and steering – it traveled down a hill, crossed a driveway and overturned, ejecting the worker from the vehicle)

PFAS SAVES TWO MORE! Dangling workers rescued from downtown VCU building (emergency crews rescued two men dangling from rigging ropes after their scaffold dropped – the workers were pulled back onto the roof following the incident – one of the workers was taken to a Medical Center for observation, the other was treated at the scene – those men were connected to their safety harnesses before they being pulled to safety)

Two hurt in National Steel Car accident (two workers were hurt in an industrial accident at about 6:45 a.m. – a 185-kilogram piece of steel was being maneuvered by a worker when it slipped off its carrier – worker moving the steel attempted to stop it from sliding with his hand, but was unsuccessful and his hand was “slightly injured” – the steel hit the ground and injured a second worker in the area – the second man was taken by ambulance to a Hospital with non-life threatening injuries)

BRIDGE COLLAPSE 3 hurt, one seriously in Ridgway bridge collapse (bridge undergoing a $2.2 million overhaul partially collapsed injuring three contractors, one gravely)

MOBILE EQUIPMENT FATALITY Man dead after industrial accident at Oakhurst in Sydney’s west (worker, in his 30s, died about 9.30am after he became trapped in machinery at a construction site)

HOTWORK EXPLOSION 2 burned in gas-line explosion in Bellevue (investigation into the explosion that injured two workers at a commercial building will focus on how they ignited a gas line – workers were soldering in a utility hole at the building when there was an explosion – the plumbers pulled themselves out, still on fire, and other workers doused the pair with water – one worker, 45, was in satisfactory condition – the second man, 36, was in serious condition and transferred to the intensive-care unit)

ELECTROCUTION Man dies from electrocution while working on school AC unit (contract worker, 54, died after he was electrocuted while working on a new air conditioning unit – he contacted a live electrical panel while working shortly before 2 p.m.)

TRENCH Workers rescue teen from trench collapse near Maryville, IL home (man, in his 30s, working to repair a water line became trapped when the hole he was working in partially collapsed – he had just completed repairs when he slipped causing a portion of the seven foot deep hole to collapse around him – people on the scene tried unsuccessfully to free the man on their own for about 15 minutes before calling 911 for help – it took rescue crews almost 90 minutes to free the man from thigh high mud that had built up around him)

MISCELLANEOUS

FARMING Farm worker injured in accident with hay baler (farm worker, 21, has been critically injured in an accident with a hay baler – he was working on the large hay baler when he was injured around 11:30 a.m. – he was taken to a Medical Center with critical injuries – a preliminary investigation has determined that the accident was caused by a jack failure)

MOWER FATALITY Leland Grove worker killed in lawn mowing accident identified (worker, 44, was killed in a lawn mowing accident – he was driving a commercial mower a little after 1 p.m. when the mower overturned on a slope next to a creek – he was pinned underneath the machine)

Worker hurt in bagel store blast, report says (worker suffered serious burns when a can of aerosol cooking spray exploded – the can fell and rolled under a grill – employees didn’t notice as the grill heated the can before it ultimately exploded, causing a fire – female worker suffered burns to her hands, legs, face and thigh)

ACETYLENE CYLINDER EXPLOSION Sydney mechanic suffers serious burns after gas bottle explodes at workshop (worker, 42, has suffered serious burns after an explosion at a car workshop – he was left with burns to his face and body and injuries to his hands and legs after the explosion – in addition to the serious burns, the man is understood to have suffered multiple fractures to his arms and legs and lost fingers on one of his hands – it is believed the man was working with an acetylene gas bottle when it exploded)

HOTWORK Welder hurt following a fire at work (worker was working on a fuel tank when it exploded – worker was treated for minor burns at the scene – workers tried putting out the fire before firefighters got there – took fire crews 20 minutes to put the fire out – only minor damage to the building)

MARBLE SLAB Teen Crushed by Falling Stone Slab (a teenager, 17, was killed when a stone slab weighing about 650 pounds fell on him outside a business that makes countertops – the 17-year-old was found at about 6:40am – business was closed when the accident occurred, and it’s not clear why the teen was at the site – it appears the boy was trying to walk between marble and granite slabs when a massive slab fell on top of him)
