OIL RIG 1 dead, 9 missing in Azerbaijan offshore oil rig accident (one oil worker has been reported killed and nine more are missing after part of their offshore oil platform broke off in strong winds – a 150-meter (490-foot) section of walkway fell into the sea amid 90 mph (145 kph) winds and says 10 workers are missing – five of the victims had been working on the installation and another five were in a cabin that was torn off the structure)

TRAIN AMPUTATION Pan Am Railway worker loses arm in Glenville accident (worker, 22, had his arm cut off by a train shortly before 5 a.m. – he had slipped on some ice, causing his arm to go across a rail where it was run over by the train – his arm was cut off below the shoulder and was recovered and sent with the victim to a Medical Center – worker was stationed alongside the tracks near a switch that controls the rails at the time of the accident, and the train was moving at a slow rate of speed)

CRUSHING FATALITY @ LANDFILL Worker Crushed to Death in San Juan Capistrano Land Fill Accident (construction worker, 38, was crushed to death at a landfill about 3 p.m. – investigators found a wrench and hydraulic fluid near the accident scene, leading them to suspect that he might have been working on the hydraulic lift of the dump truck before he got crushed under it)

MINE FATALITY Driver killed by truck’s tailgate in freak accident at Rix’s Creek Mine (mine worker, 53, has been killed after a truck accident at a coal mine – the truck driver was removing mud and grime from his truck when the tailgate on the truck swung down, striking him in the head)

CONFINED SPACE FATALITY – DIVER Crews work overnight to recover body from Braintree water tank (rescue crews worked through the night to drain a 1-million-gallon municipal water tank to recover the body of a diver who was killed while doing a five-year inspection of the tank – firefighters rescued a second worker who was spotting the diver and jumped into the 45-degree water in an attempt to help when the diver’s equipment failed – the rescued worker was in shock and had been rushed to the hospital – workers were contracted by the town had been inspecting the tank when something went wrong – the diver radioed to the spotter that he was losing air before communication and video contact cut out – the spotter, who was not wearing diving equipment, then jumped in to try to save the diver)

WORKZONE FATALITY Road Worker Struck and Killed While Flagging Traffic (worker, 60, had three traffic cones in front of him while he was flagging motorists through  eastbound lane – a gray 2014 Chevrolet Malibu approached the scene, when the flagger raised his hand in an attempt to signal the oncoming Chevrolet to stop the car kept going, striking him with the vehicle’s left side)

CONVEYOR AMPUTATION Hamilton Coco Paving worker injured in conveyor belt accident (worker is in hospital after getting caught in a conveyor belt – his injuries were originally thought to be life-threatening but have since been deemed non-life-threatening – he suffered injuries to his arm and leg around 10:30 a.m. – around 11 a.m. the man had been freed and sent to hospital)

FATAL FALL into SAND HOPPER Man found dead in hopper of Atlanta Sand & Supply (worker, 62, died in a sand hopper just after 5:20 a.m. – employee had fallen into the sand hopper at the company and was pronounced dead at the scene)

FATAL FALL in to MOLTEN CHOCOLATE Death by chocolate: factory worker in russia perishes in freak accident involving confection (worker, 24, fell into a vat of molten chocolate – her remains were found within a tank located inside of the confectionery plant where she worked – some believe that the young woman may have been reaching for a cell phone that fell out of her grasp – others feel that while emptying sacks of ingredients into the large mixer, she could’ve accidentally toppled into her chocolate death)

Worker dies at Trenton cheese plant (employee was killed in an incident on one of the production lines at the facility around 5 p.m. – no details)

Dumbarton factory worker injured in horror crane accident (worker was injured when his arm became trapped in a crane lift – he is said to have been suspended in the air by the machine – colleagues rushed to help the employee who is understood to be a production line worker – he had suffered a fractured arm)

LOTO – ROBOT FATALITY Alabama factory worker dies two weeks before her wedding after being crushed to death by robot (OSHA has announced 23 safety violations issued to an auto parts manufacturer after a bride-to-be was crushed to death by a robotic machine – worker, 20, was killed June 18, two weeks before her planned wedding day – the assembly line had stopped and she entered a robotic station to clear a sensor fault when the robot restarted abruptly, crushing the young woman inside the machine)

LOTO – GRADING MACHINE FATALITY Worker, 26, is crushed to death when his body is dragged into soil machine at compost firm (worker, 26, was crushed to death at a compost site when his body was dragged into a grading machine – he was found with his legs hanging out of the heavy industrial machine)

DRILL RIG EXPLOSION Murfin explosion under OSHA investigation (an explosion at a drilling rig -two of the five persons injured in the explosion – five employees were taken, but two of them were flown by air ambulance to a hospital – the explosion occurred some time after 7 a.m. – the explosion occurred in a warming hut on the unit – they were bringing up a pipe joint and gas released out of that pipe joint migrated into their warming hut, and something electrical sparked the explosion)

AMMONIA In Zillah a faulty valve leaking ammonia at a fruit warehouse caused many residents to evacuate (at about 9:42a.m. the plant ammonia team made entry to the warehouse and were able to shut off the valves around 10:37 a.m. – the report is 8000 pounds, all the ammonia did not leak out it’s going to take some time to figure out how much the way their system is set up, they claim it was a faulty Solenoid valve – one report of a person transported to a hospital to get checked out)

AMMONIA all roads are back open after ammonia leak in sanger (a roof collapse caused an ammonia leak at a business – the weather could be a factor in the collapse of the plant – it pulled down some piping causing a significant ammonia leak – 10 employees were inside at the time but got out safely – no homes in the area were evacuated – “The weather right now– it’s obviously cold and rainy and wet and it’s really pushing the ammonia down on the ground, so the ammonia is actually staying a lot lower to the ground than we like it. Normally when it’s hot outside you’ll see it rise a little bit faster. This right now, as you can kinda see in the background, it tends to stay lower to the ground so people will smell it more.”)

HYDROGEN Hydrogen tank fire extinguished at WaferTech in Camas (a 3,000-gallon tank containing hydrogen caught fire and company officials are not sure how it started – Fire Department extinguished the fire about 50 minutes after arriving and dousing the tank with cooling water – the tank eventually vented, releasing a large cloud of hydrogen vapor – there were no injuries)

CHEMICAL REACTION Fire crews respond to chemical leak at Hantsport factory (we determined that it is a peroxide-based chemical that’s used in the paper industry for bleaching, so it was a reaction of two peroxides that’s causing off-gassing of basically pure oxygen – two chemicals got mixed together that caused this reaction)

FORKLIFT FATALITY Man killed at N.J. recycling plant when forklift falls on him, police say (worker, 52, was killed at a recycling plant after being trapped under a forklift – he died from injuries sustained during the accident – EMS responded to the recycling facility around 4 p.m. where they found the man pinned under a forklift and seriously injured – workers at the plant used an excavator and a front end loader to lift the forklift off the victim to allow rescue workers to treat him – investigators were told the victim was operating the forklift, which became stuck at some point – the victim was then somehow ejected from the forklift, at which point the vehicle then fell on him)

CRANE FATALITY Indianapolis man killed in accident at IPL construction site near  (worker was killed in an accident on a construction site around 2 p.m. – the worker was struck in the head by an I-Beam/catwalk after a rigging device failed – this is the second death for the contractor at the construction site this year – a contractor was killed back in March when he was pinned between a trailer and a truck)

SEWER PLUG Two Hurt in Manhole Accident (two men were injured, one of them seriously, in a construction accident – the men were working in a manhole when a plug used to block a sewer pipe blew out and hit them – one of the men suffered a serious leg injury and had to be strapped to a backboard and carried out of the site, while the other suffered a wrist injury – both men are expected to make full recoveries – it isn’t exactly clear why the plug dislodged from the pipe and struck the men)

Santa Teresa worker killed in industrial accident (worker, possibly in his fifties, was killed at a wind blade storage site after 12:45 p.m. – the supervisor at the site said he was working at the site and was shifting a 350-pound steel block and it crushed his chest)

FATAL FALL Construction worker killed in fall (construction worker, 50, was killed after falling about 20 feet about 5 p.m. – he was taken in critical condition to an area hospital, where he later died of blunt force head trauma)

FORKLIFT FATALITY OSHA: Vehicle doing snow removal killed worker at Deep Tunnel site in Summit (worker, 55, who died at the site of a Deep Tunnel stormwater reservoir had been struck by a vehicle that was part of a snowplowing operation – the man, a carpenter craft worker, died about 2 p.m. – he died of multiple injuries caused by being struck with a forklift)

BULLDOZER FATALITY Worker killed in construction site accident (worker, 27, died after being ejected from a bulldozer at a student apartment housing construction site – he was pronounced dead at the scene at about 12:30 p.m. – he fell off of the bulldozer, which then ran him over)

UPDATE: Victim in house collapse identified as Ocean City resident (worker, 24, was injured and trapped in the water in an accident while a house raising project was underway – he uffered a fractured right femur and lacerations to his head – he was working on a crew raising a house and was hurt in a collapse that knocked him into the bay near the bridge, with a chain trapping him in the shallow cold water)

CRANE FATALITY Crane truck accident leaves 2 workers dead, 2 seriously injured (the platform of a crane truck overturned at a reinforcement construction work site killing two workers and seriously injuring two others – around 1:29 p.m., two workers, both of them brothers, crashed to the ground 8 meters below when they were aboard the carrier to engage in reinforcing panels on the external wall of a two-story plant before being pronounced dead at a nearby hospital – another brother of theirs and their co-worker sustained serious injuries)

FATAL FALL from LADDER Worcester electrician killed in fall from ladder (electrician, 55, died after a fall from a ladder)

TRACKHOE BATTERY Worker injured in explosion at Peterborough construction site (construction worker was taken to a Regional Health Centre for treatment of non-life threatening injuries after a battery on an excavator reportedly exploded – the man reportedly suffered facial injuries)

HOTWORK FIRE Contractor fined for fire at Simplot headquarters (OSHA has fined a contractor more than $20,000 for safety violations connected to a fire – two construction workers were six stories up on a aerial lift when a welding blanket caught fire in the basket of the lift – workers on the ground were able to swing over a boom, which the men in the basket used to jump from the lift to the sixth floor of the building – workers were not injured – OSHA investigated the incident and said that molten and burning zinc dripped onto a tarp which was not appropriate for the type of welding work being performed – OSHA also determined that the fire extinguisher the workers used was inappropriate for the situation, and that the workers were not wearing required respirators while working – fined $20,484)

FORKLIFT FATALITY One dead, one injured following construction accident downtown (one man is dead and another is in the hospital as the result of a construction accident – the two men were working from the bucket of a forklift about 75 feet above the ground when the bucket tipped over and the men fell out – one man died at the scene while the other was taken to a Medical Center with critical injuries – both men were in their 50’s)

SNOW PLOW FATALITY ITD worker killed in snow plow accident in Boise County (State Police and the Sheriff’s Office are investigating a snow plow accident that killed an DOT employee, 62 – a passerby came by the sheriff’s office to report the accident around 10 a.m. – the state trooper who responded found that a man had been sucked into the blades of the plow – the plow is the type that has rotary blades)

Poultry farm worker dies after industrial accident (worker, 50, succumbed to his injuries less than two days after he was crushed by a metal silo while working at the Poultry Farm)

MANHOLE EXPLOSION Two SEPTA workers injured in manhole fire in West Philly (two workers were injured in a manhole fire – one worker was transported to a Burn Unit with first-degree burns to his hand and face – the other worker was released after being treated for minor injuries – the two workers opened the manhole cover to investigate reports of an odor in the area – they attempted to find the source of the odor – cause of the fire is still under investigation by the Fire Marshal’s Office)

Teen Catches Fire While Welding At School In Arbuckle (teenager is recovering in the hospital after he suffered burns on more than 70 percent of his body – “It was pajama day and somehow a spark got in his shirt and all of a sudden, he flamed all the way up” – “He started running through the hallways. Then, one of the teachers had like stopped him and I guess she tackled him to the ground and started patting him down to like pull him down and then she got the fire blanket and covered him” – teacher received minor burns and was treated at a different hospital and released)
