INDUSTRIAL

LIME @ WWTP Worker exposed to lime in wastewater plant accident (a man was trapped and exposed to lime while making repairs to a silo at a wastewater treatment plant – Fire Department responded to a call around 9:30 a.m. to extricate the man – according to a coordinator at the facility an employee became trapped when lime came rushing out “where it wasn’t supposed to” – by the time firefighters arrived on the scene, employees had already removed the man from the area below the silo where he had been making repairs, and were trying to clean the lime from him – he was taken to a hospital by ambulance and two other employees were taken via private vehicle)

LATHE Worker catches little finger in lathe at Oerlikon Fairfield plant (employee got the little finger of his right hand caught in a lathe – no other details)

GUARDING/LOTO Accident Sends Tyson Employee to Hospital (around 11:00 a.m. emergency workers received a call about an employee whose arm was stuck in a piece of machinery – when officials arrived, the woman had been freed and was with the on-site nurse – her injuries are not life threatening, but she was transported to a Medical Center after complaints of arm pain – she has since been released)

REFINERY LEAK Worker burned in toxic gas leak (worker was burned and staff evacuated from a refinery after toxic gas escaped – leak happened at 3:40 a.m. in a petrol-production unit and the injured worker received a chemical burn – the leak was stopped very quickly and there was no risk to the local population or the environment from the leak of butane and hydrofluoric acid, which is highly corrosive and an acute poison)

REFINERY FIRE Major fire in Vadodara oil refinery plant brought under control (there was major fire preceded by an explosion in the FCC plant of the oil refinery – firefighters immediately brought the blaze under control – fire brigade sources said there was a blast in the FCC plant of the refinery which was followed by a blaze and the fire has been brought under control)

REFINERY FIRE Fire at Kuwait refinery doused (no casualties – a fire described as “limited” which broke out at a refinery has been extinguished – the fire team of the refinery was able to control the fire and extinguish it within 20 minutes after activation of the emergency response plan at the refinery)

FATAL EXPLOSION @ HYDROPOWER PLANT Gas Blast At Hydropower Plant In Siberia Kills Two (a gas explosion at a hydropower plant has killed two people and injured at least one – the accident occurred during repainting work at the facility – it’s not clear what caused the explosion)

Fumes hospitalize FedEx mechanic (one person was hospitalized in an incident involving hazardous materials at 12:15 p.m. – a HAZMAT team located a 50-gallon drum containing an unknown liquid substance – facility was evacuated and sealed off – substance was tested, but it’s still not known what it is – one worker, a vehicle mechanic, was hospitalized to rule out exposure concerns – no other injuries were reported – air quality inside the facility was tested, and workers were allowed to return)

CONFINED SPACE – SEWER- H2S Two men overcome by fumes, rescued from Long Grove sewer (rescuers lifted two incapacitated men out of a sewer at about 12:15 p.m. – the pair had been hired by a property owner to jet-rod his sewer, but in doing so released a gas strong enough to put their lives in danger – when the owner went to check on how the job was going he found one of the workers unconscious, trapped in the sewer and immediately called 911 – the men were trying to climb out of the sanitary manhole when a gas overpowered them – that gas was hydrogen sulfide created by the breakdown of the sewage they were rodding out, and now at an intense 120 parts per million – one of the workers apparently made it to the top of the ladder, but behind him, his co-worker fell back down, apparently from about halfway up – the man at the top climbed back down to help, but was immediately overcome by the fumes – wearing breathing equipment, a rescue technician climbed down and attached a rope to the second victim — the crew on the surface then pulled him out)

CONSTRUCTION

Construction worker suffers burns, lacerations in Toms River accident (a road construction worker, 27, operating a piece of machinery suffered burns and lacerations to his leg and was hospitalized – he had his leg pinned for about three minutes in the unspecified piece of equipment while doing paving work – he was attempting to clean some asphalt from the machine when his feet became caught under a belt and metal – fellow workers were able to free him by using a torch and pry bar, cutting the metal that was trapping him – he was flown to a trauma unit and is in critical condition)

WORKZONE FATALITY Beiseker Accident Involving Highway Workers Fatal; Charges Pending (criminal charges are pending against a truck driver after two highway maintenance workers were hit following a collision – `one of the workers, 18, died at the crash site – the other worker, 19, was taken to hospital with critical injuries and remains in intensive care – the transport truck went through a stop sign and struck a car – the truck and car next slammed into a parked pickup truck, then all three vehicles veered into the two workers standing nearby – the truck driver, 35, faces charges of dangerous driving causing death and dangerous driving causing bodily harm)

Worker Killed At Electric Boat in Quonset Point (a man working at a facility has been killed in an accident – the employee was fatally injured around 11 a.m. – he was taken to a Hospital, where he was pronounced dead – he was employed by a subcontractor working for the general contractor constructing a new manufacturing building – no other details)

WORKZONE Construction Worker Hospitalized After Fulton County Accident (worker, 53, was struck by a motor vehicle – she was reportedly working as a “flagger” on the West end of a construction zone before the accident occurred – driver, 41, told Deputies that he began attempting to avoid striking a vehicle that was stopped in the construction zone – he allegedly decided that in order to avoid causing an accident, he would drive into the Eastbound ditch – but subsequently struck the flagger – she was treated by paramedics on scene and transported to a Hospital before being Life-flighted to another facility – investigation allegedly revealed that the driver’s license had been suspended at the time of the accident)

Construction worker injured by tree branch (a construction worker, 47, is in hospital in serious condition with a head injury after a cement truck knocked a tree branch on to him – the accident happened at a construction site around 10: 45 a.m. – he suffered minor facial lacerations, was conscious at arrival of paramedics)

FATAL FALL from ROOF Worker Dies After Fall From Oklahoma Hotel Roof (a construction worker, 37, was killed after he fell from the roof of a hotel – he fell about 40 feet from the roof – he had been working alone at the time of the accident – a passerby discovered the worker lying on a concrete sidewalk – he was taken to a hospital, where he was pronounced dead a few hours later)

COLLAPSE Construction accident occurs at Temple U. in Philly (a six-story building under construction partially collapsed – one construction worker suffered non-life-threatening injuries – he is hospitalized in stable condition – initial reports indicate a failed connector caused a fifth-floor beam to fall to the fourth floor – firefighters rescued several workers who were stuck on the upper floors)

FALL in ELEVATOR SHAFT FDNY rescues building worker from elevator shaft on Staten Island (firefighters rescued a man who had fallen down an elevator shaft while working on the construction of a new courthouse – he was in the elevator shaft on the sixth floor of the structure being built when he fell one flight to the fifth floor – the worker may have suffered a broken ankle and/or arm)

CEMENT TRUCK CHUTE Construction worker hospitalized after being pinned to truck (a construction worker, 47, has been hospitalized with serious injuries after being struck by a cement chute that pinned him to a truck shortly after 1 p.m. – the man was treated for serious chest injuries after the chute of a cement truck him in the chest – the man was momentarily pinned between the chute and the truck until co-workers could pull back the chute)

CONCRETE FORM Worker Injured in Pine Construction Accident (worker was crushed by a 1,000 pound concrete form when it fell on top of him – the worker was attempting to move the concrete form without a brace when it toppled onto him – other workers rushed to lift the 9-foot tall form from his body and managed to pull him out from under it – he was taken by ambulance to an area hospital for treatment)

ELECTRICAL Cable worker seriously burned after accident in Spotsylvania (a cable company subcontractor was airlifted to a burn center after an apparent accident – worker was in a hole digging with a shovel just before 9 a.m. when he apparently hit a live power line – the resulting explosion caused severe burns over most of his face and torso)

WORKZONE FATALITY Investigators Continue to Probe 270 Crash that Killed Worker (an accident left a construction worker, 57, after a dump truck loaded with concrete struck him near the ramp from I-270 WB at approximately 9 a.m. – he was part of a crew working on a construction project in the area)

STEAMROLLER Steamroller crushes man’s leg (a man, 52, had his leg crushed by a steamroller  around 11:30 a.m. – he was doing asphalt work when his leg got crushed by the steamroller, which was being driven by another employee – paramedics treated the man and took him to a hospital –  heis in serious, but stable condition)

Crane falls on Zurich passenger train (a crane fell onto a passenger train entering a station – the crane collapsed during track work near the station, it’s harm striking a train arriving from the airport – the cause of the accident remains unknown – none of the some 250 passengers on board were hurt but the incident caused hundreds of thousands of francs worth of damage)

MISCELLANEOUS

CONVEYOR Worker hospitalized after Portland harbor incident (a seaman aged in his 50s was flown to a hospital with extensive injuries to his right hand after an accident on a vessel in the harbor – police officers attended the industrial accident at the port berth five, where they found a man with his right hand caught in a conveyor belt – he was injured while unloading mineral sands – he had serious fractures and severe lacerations to his right hand – the patient was in a stable condition)

LOTO/GUARDING FATALITY in BALER Worker crushed to death at Redwood City Grocery Outlet – (worker, 43, at a Grocery Outlet was discovered crushed inside an industrial cardboard baler – the man’s body was discovered by a co-worker at 10 p.m. – he was declared dead at the scene)

LOTO/GUARDING FATALITY in SHREDDER New details on industrial accident at W. 34th St. that killed worker (a worker, 43, was pulled into an industrial shredder – an autopsy is being performed on the man – the victim was working at the business and placing dry styrofoam into an industrial shredder – the shredder then melts the dry styrofoam into a foam, which is then placed into bags and stored on site – the victim, working alone at the time, reached inside the machine with his right hand and the blades of the shredder pulled him into the machine, causing his fatal injuries – it took about two hours for a rescue crew to extricate the worker from the machine, which was mounted on the back of a box truck)

AMPUTATION from CABLE Update: Man’s arm severed in Quyon Ferry accident (an employee, 33, working aboard a ferry had an arm severed when a cable snapped – a crew of four men were working on the new ferry, which is not yet in service and is tied up alongside a dock, when the cable snapped under tension, severing the worker’s right forearm – one man jumped into the water to retrieve the limb)

LATHE FATALITY @ UNIVERSITY University worker is killed in accident with lathe (a technician, 51, was rushed to hospital following the incident at the University’s School of Ocean Sciences – he suffered what proved to be a fatal head injury – he was airlifted to a specialist unit where he later died)  NOTE:  Last year there was a another FATALITY involving a late at a University in the USA

CRUSHED by BACKHOE Victim in backhoe accident identified (a man, 54, was killed in an industrial accident – he was pinned underneath an backhoe on property owned by a landscaping company – the victim was changing a tire on the backhoe when it fell on him)

Worker injured by chain saw (a tree service worker, 24, was flown to a Medical Center following a chainsaw accident – he owns a tree service business and was clearing branches from a tree when the accident occurred at 5:46 p.m.)

BURNED Golf course worker in critical condition due to burns (employee, 47, was badly burned when he tried to light a large pile of brush for disposal – he was one of several workers at the golf course – the extent of his burns weren’t explained by police, but they did say he was airlifted to the burn ward of a Hospital – he’s in critical condition)

CAR WASH Worker’s foot crushed by car wash conveyor (a worker’s foot was crushed by a conveyor at a car wash – no other details)

FORKLIFT Carnival Conquest departs Mobile two hours late; two workers hurt in forklift accident (two workers were taken by ambulance after minor injuries when heavy luggage fell from a forklift – the workers were hurrying to unload the last two busloads of luggage that arrived late after being screened there – at 5:40 p.m., shortly before the accident, workers could still be seen transporting loads of luggage on small forklifts from the terminal to the ship)
