INDUSTRIAL MINE FATALITY BC Coroners Service confirms identity of deceased worker from Teck’s Coal Mountain Operations (worker, 59, a water truck driver was filling his truck with water when he was impacted by frozen rock which fell from a separate vehicle – he was transported to hospital but he could not be resuscitated – the fatality occurred around 2 am) FATAL FALL from CRANE BASKET Worker dies at transmission line site north of Terrace, B.C. (a worker contracted as part of the crews working on a transmission Line was killed in an accident – two workers were in a man basket that was part of a 22-ton mobile crane on rubber tires at the work site – the man basket was suspended from the crane and about 85 feet off the ground – they were putting conductors in place on a tower when the accident happened – it looks like a worker signaled for the crane to be lowered and something happened – the bucket was swung around and hit the boom and the worker was thrown out to the ground below – no reports on whether anything happened to the second man in the basket) CONFINED SPACE FATALITY w/ H2S Torquay man dead in workplace accident (a 40-year-old man died in the pit of a sewage lift station – he was pronounced dead at the scene and it’s believed that he was overcome by H2S gas while inspecting the station – an investigation is ongoing by the RCMP, Occupational Health and Safety and Worker’s Compensation) CRUSHED between RAILCARS Man dead after being crushed between railway cars (worker, 34, was crushed between two railway cars at around 9 p.m. – he was on a moving train that was backing up when one of the cars derailed – he was somehow caught between two cars) LOTO FATALITY w/ FALL from LADDER Worker, 26, dies in conveyer belt mishap (worker, 26, died in a cement factory – he suffered fatal injuries at 9:50 p.m. when attempting to unblock a conveyer belt – he mounted a ladder and attempted to push the conveyer with a pole when an unidentified object struck him on the head, causing him to fall from the ladder – he was later confirmed dead at hospital) FORKLIFT BASKET Bradford worker suffers massive head injuries (worker suffered massive head injuries after a metal basket he was riding in was dropped to the ground by a forklift – he was unconscious when fire and ambulance arrived on the scene and was transported by ambulance to hospital with critical injuries – preliminary investigation shows the victim was situated in a metal basket that had been raised from the floor using a forklift – basket containing the victim became dislodged from the fork lift and fell to the ground) FLASH FIRE caused by STATIC Two men injured in flash fire (two men were injured in a flash fire – reports indicated an explosion occurred at the resin and coating manufacturing company – one of the victims was wounded and shirtless – Medflight took him to a General Hospital – no damage to the facility – two workers were mixing a resin in a container with other chemicals – investigators believe that static electricity ignited the vapors of the chemicals) FATAL EXPLOSION w/ CYLINDERS Four workers killed in factory cylinder blast (four workers were killed and 18 others injured when a gas cylinder exploded at a gas company’s factory – the explosion at the cylinder gas refilling factory occurred at approximately 12 p.m. when the factory workers were busy loading medical oxygen gas cylinders onto a truck – a gas cylinder at the factory’s filing station had leaked, causing four others to leak and explode, said one of the injured victims – as a result of the impact felt due to this cylindrical explosion, the workers flew from inside the gas cylinder truck and landed on the nearby footpath) CAUSTIC Worker hospitalised after Alcoa Pinjarra refinery accident (worker at a refinery suffered severe burns from an accident involving caustic soda – the man was admitted to the burns unit at a Hospital after a spill in the precipitation area at the refinery – caustic soda is used at the site to extract alumina from bauxite – the worker is reported to have lost sight in his left eye) Worker at Strongwell Chatfield Division hurt on the job (accident sent one man to the emergency room with a hand injury – the man’s fingers on one hand were penetrated by fiberglass) GUARDING AMPUTATION Worker’s arm severed at North Strabane manufacturing plant (a worker, 63, had his right arm severed in an industrial accident – he was adjusting drill bits on a machine at about 9:30 a.m. when his arm became stuck in a spindle and it was severed at the elbow – he was flown by medical helicopter to a Hospital or treatment – investigators think loose clothing might have caused the machine to pull his arm into the spindle) CHEMICAL BURNS w/ thionyl chloride Corrosive’ chemical burned worker’s lap area, fire chief says (six people were brought by ambulance to the hospital after being exposed to the “corrosive” chemical thionyl chloride – the most seriously injured was a male employee who “was complaining of burns to his lap area” after the chemical spilled on him – he was “immediately placed into a shower for decontamination from the substance,” then brought by ambulance to a Hospital – five other employees were hospitalized as a precaution after reporting “throat and breathing irritation”) CHEMICAL BURNS Martinez: Tesoro workers injured in acid spill, second in a month (workers were injured in an acid spill in the same unit at a Refinery – the two employees were treated at a hospital after they were exposed to sulfuric acid – two contract employees suffered sulfuric acid burns – the workers both wore chemically protective clothing – the workers were taken by ambulance to a Medical Center for serious but not life-threatening injuries – the workers were exposed to the chemical at a unit during planned maintenance activities – the workers were wearing protective safety equipment and were decontaminated in a shower before being taken to the hospital, and that one has since been released – incident comes less than a month after two other workers were burned with sulfuric acid in the alkylation unit – cause of the incident is under investigation and is believed to be unrelated to the chemical release that occurred on Feb. 12) No injuries reported in Neb. gas pipeline blast (no injuries have been reported from an explosion on a natural gas pipeline – the line erupted about 8:30 a.m. – no damage to any structures has been reported) CONVEYOR AMPUTATION Recycling plant closed as bosses probe accident which saw worker’s arm trapped in machinery (worker, 19, who suffered horrific injuries when his arm was trapped in a machine is now able to sit up in his hospital bed – his arm was crushed in a moving conveyor – he was airlifted to hospital by air ambulance after emergency crews were called to the recycling plant – he was a temp agency worker who had been at the business since December) CRUSHED Hovis worker hurt in accident (a worker, 23, was taken to hospital after a piece of machinery fell on him – he suffered undisclosed injuries but Police said he was in a critical but stable condition – he was air-lifted to the a Hospital) HOTWORK Chip mill fire fails to slow production at Woodland Pulp (a fire broke out at 1:47 p.m. in the three-story-high metal building that houses the chipper and debarker – workers had been performing routine maintenance, cutting and welding metal, when a spark got into the walls where there is a great amount of dust – workers put out the fire, but five or 10 minutes later the operator of a crane in the building saw some smoke – they tried to put the fire out again, but it was difficult to get to it inside the walls – the fire caused extensive damage -the damage could amount to about $1 million) HOTWORK Friday night fire in Clark County destroys sawmill (a sawmill is a total loss following a fire around 7:45 pm – when firefighters arrived, the sawmill was already smoking heavily – crews spent 8 hours putting out the fire, and had to have water trucked in to continue fighting the fire after the city’s water supply ran low – building’s owner says he was welding and that a spark may have started the fire – the loss is estimated to be between $1.5 million and $2 million – no one was injured in the fire) HOTWORK FATALITY Man injured in STO oil tanker fire dies (the man, 61, injured in an oil tanker fire incident has died, after receiving treatment – the fire incident occurred while he was welding on the deck of the oil tanker) AMMONIA Tonawanda ammonia leak contained (an ammonia leak at a bottling plant sent dozens of emergency workers, though much of the leak was contained in the interior of the structure where few employees were present – the leak, caused by an overheating tank that holds a liquid form of ammonia, was halted before it could spread by a relief valve – no injuries reported, nor were evacuation procedures necessary – “the relief valve served it’s purpose,” he said. “If it wasn’t for the relief valve it could have been far worse.”) AMMONIA Workers evacuated after ammonia leak in Brooklyn factory (an ammonia leak in a meat factory forced the evacuation of 1100 workers -facility was evacuated after about 100 litres (26 gallons) of ammonia leaked into the roof space of a chiller room at about 10.50am – workers were let back into the factory after about 90 minutes, although the affected area remains sealed off – firefighters used breathing equipment and protective suits at the scene to clear the leak and monitor air and wind conditions – one firefighter was taken to a Hospital with a minor eye irritation) CONSTRUCTION SCAFFOLDING COLLAPSE Six Injured in Scaffolding Collapse; State Inspecting Accident (a scaffold collapse sent six construction workers to the hospital – two workers that were taken to the hospital with significant injuries, some broken extremities – the construction crew was up three floors outside on the scaffolding – they were placing stucco on the side of the building when the scaffolding crashed – the scaffolding was wrapped in this plastic sheet as the crew worked) FATAL FALL Air ambulance called as man dies in Boughton Lees after falling from roof of Eastwell Manor (a worker, 51, has died after falling from the roof of a luxury hotel – he suffered multiple injuries after plunging to the ground at the four-star hotel – the man was a sub-contractor fixing the roof) FATAL FALL from ROOF Cabot Man Killed in LR Industrial Accident (worker, 51, has died in an industrial accident – he was found lying on the parking lot next to a building where he had been working on a roof – another man who had been working with him told police they were about 200 feet apart as they used a tape measure to take measurements of the roof- the other worker said he had turned around and then did not see him anymore – he then ran to the side of the building where his coworker had been and saw him face down on the parking lot – he was pronounced a short time after being taken to a hospital) TRUCK UNLOADING FATALITY Illinois man killed in work-related incident in Cedar Rapids (a man, 28, died after steel pipes slipped off of a trailer, struck him, and caused fatal injuries at a Country Club at 3:51 p.m. – the victim who was unstrapping steel pipes secured onto a trailer – the accident happened when some of the pipes slipped and struck the victim, causing traumatic injuries – medical treatment began and he was transported to a nearby hospital for treatment where he later died) Three workers die in tragic accident (three construction workers died when a concrete beam collapsed on them at a construction site – the contract workers had been working on the new highway stretch which is now under construction – two workers had died on the spot while the other had succumbed to his injuries at a hospital) WELDING BURNS LIRR worker recovers after welding incident (a worker is recovering after being injured while working on the tracks – the worker was burned while he was welding – worker is in the hospital, but is expected to be OK) UPDATE on HOTWORK FIRE Report: Features Caused Rapid Spread of Fort Detrick Fire (the fire last summer in the world’s largest high-security research lab was blamed on a welder’s torch – a welder’s torch sparked a fire that caused $10 million in damage at the world’s largest high-security research lab, still under construction at Fort Detrick – two people were hospitalized for smoke inhalation and released the next day – about 300 others were safely evacuated from the 800,000-square-foot building – workers did not have the appropriate permits for work with an open flame at the site – fire alarm system had not yet been activated in the building, though its occupants were able to evacuate within 15 minutes – a welder was working at the construction site with a fire blanket and fire extinguisher when a fire started in an epoxy-coated room he had left just minutes earlier – he saw soot, then fire coming from a supply air duct nearby) HOTWORK???? Welding work under investigation following fire (fire investigators were looking into whether welding work was to blame for a huge blaze that tore through an apartment building construction site, threatening nearby buildings, prompting evacuations and forcing officials to call in about half the city’s on-duty firefighters to prevent its spread – fire officials were looking into preliminary reports that workers were doing torch work at the site shortly before the fire was reported – the fire was fueled by wood frames and other materials at the site) HOTWORK Blowtorch, spray paint set off Naperville worksite blast (no one was injured and no damage was reported in the blast that occurred about 11:52 a.m. at the indoor sky-diving facility, which is under construction – a lighted welder’s torch that came too close to a box full of spray paint caused an explosion that startled motorists and passersby – no one was injured and no damage was reported – 4-to-6-foot-tall flames shot up from the second floor of the sky-diving center – the blast occurred when a construction worker’s welding torch accidentally set fire to a cardboard box containing canisters of spray paint) HOTWORK Fire at IKO Industries likely accidental (fire department investigators are fairly certain they know what caused the massive blaze that had crews calling in a third alarm – the blaze shut down several nearby businesses and fire crews remained at the scene for a few days to monitor hot spots – officials now say a contracting crew was dismantling a piece of equipment, called a Saturator, with a cutting torch and believe something ignited) SCAFFOLDING COLLAPSE UNCG employees recovering after accident (two employees who were injured when a scaffold they were working on fell over are in good condition – they are maintenance workers – the men fell from 15 to 20 feet – an ambulance took the men to a Hospital – one suffered bruises and was likely to be released from the hospital that day – the other received a head injury, but is conscious and in good condition – he was expected to remain in the hospital overnight for observation) FORKLIFT FATALITY Colorado City teen dies in forklift accident (a 14-year-old boy was killed following a forklift accident – the boy was working at some type of construction site operating the forklift – he was on the forklift when it started to tip over – he jumped to get out of the way, but the forklift fell on him) Miscellaneous MOWER FATALITY Man killed in mower accident at Snohomish County golf course (a worker at Golf Course was killed when a lawn mower flipped over on top of him at about 1:20 p.m. – victim was found dead at the scene – it appears the man was riding the lawn mower when it flipped over on top of him) DUMP TRUCK GATE FATALITY Longtime North Bend city employee Phil Bowman dies in work accident (a worker with the city’s streets department was fatally injured when he was struck by the gate of a dump truck at a work site in the park – he worked for the city for more than 20 years, and had been recognized for his service by the Mayor in September 2013) Forklift accident at Stump Pass Marina damages 6 boats (a shipwreck on land damaged a number of boats at a marina – it was all because of a faulty forklift – the forklift was reaching to get a boat from the third shelf when a bolt snapped, causing the fork and the boat to crash down on everything below – no one was hurt – ironically, the marina has been under new ownership since last week – they ordered a new forklift, and it was delivered about an hour after the accident happened) |
