INDUSTRIAL LINEMAN ARC FLASH Oncor crew worker injured; causes power outage in Rusk (worker was injured when he attempted to change out a power pole – an electrical flash occurred, injuring the worker – he was airlifted to a Hospital Burns Center, and his condition is unknown at this time) STRUCK BY FATALITY Worker fatally struck by vehicle on South Laredo Street (worker, 68, was fatally struck by a vehicle just after 6 a.m. – the victim was picking up garbage and moving a barrel across the street when he was struck – he worked for a food processing company in the area – driver initially fled the scene of the crash, but was taken into custody a short time later – charges were pending against the driver) AMMONIA Ammonia leak at Tar Heel Smithfield packing plant hospitalizes 15 (a packing plant was evacuated because of an ammonia leak and some of the workers were transported for medical treatment – a hot water tank tilted over and hit he top valve of a reserve ammonia tank, which caused the leak – the leak was minor and is now contained – a portion of Highway 87 was closed because of the leak – more than 2,400 employees were evacuated from the entire operation – eight people from the plant were transported to a Medical Center for breathing problems – two people were taken to a County Hospital and five were transported to a Regional Medical Center – of the eight transported, one was admitted for observation over night) EXPLOSION Republic Steel says damage caused by Monday morning explosion was minimal (three workers hospitalized following an explosion were released from a local hospital – explosion took place after workers in the plant’s melt shop tried to reignite a furnace that was shut off over the weekend – damage to the melt shop was minimal – a small water leak may have affected operations at the plant, but said it is too early to comment on the explosion’s cause – two were treated and released almost immediately and another was released later in the day – a fourth worker was treated at the scene) CONFINED SPACE FATALITY Man dies, two injured while cleaning asphalt tank (one killed, two injured in quarry accident – a worker was overcome by fumes while cleaning an asphalt tank at the quarry – upon arrival, firefighters saw workers on top of a tanker attempting to rescue two victims inside – the victims were cleaning the inside of tanker when they grew ill – a third worker was able to self-rescue prior to FD arrival – he was treated and transported to area hospital – the remaining two victims were rescued from the tanker by firefighters and were also transported to area hospitals – one of the victims, a 58 year old male, succumbed to his injuries and later died) ELECTROCUTION One man dies of electrocution at metal plant in Georgia (worker, 29, died from electrocution at a smelter plant – he was taken to the hospital, but he died soon after arrival – electricity supply stopped for one of the ovens in the plant – workers called for electricians to have them find the fault in the electricity cabin at the factory – he was hit by 1,000 volts of electricity when he was trying to repair the damage in the cabin) FIRE Worker burnt in Malaga fire (worker, 33, has been rushed to a Hospital with burns to his upper body after he was injured in a fire about 11am – a fire investigations officer is at the scene to try to determine what caused the blaze – fire caused about $300,000 damage to the building and business – the injured man – who has worked at the business for four year – is currently in a critical condition) CYLINDER EXPLOSION EXPLOSION TEARS HOLE IN ROOF OF WAREHOUSE NEAR BUSH IAH AIRPORT (a series of explosions were seen, heard and felt outside of a manufacturing facility just before noon – company manufactures and machines oil drilling parts – nearly 20 workers were inside the building when some said they heard an acetylene tank leaking – some of the tanks that were inside of the building exploded) ELECTROCUTION Lucas city employee electrocuted (a city worker, 65, died while installing a security light – the man and another city employee were doing electrical work when he died – the other worker called for help but the man died at a Hospital) FATAL EXPLOSION Rosneft Halts Achinsk Oil Refinery After Explosion Kills Seven (six people were killed in a fire at an oil refinery -repairs could take up to six months – the fire had broken out at a fractionation unit of the refinery, which normally produces 140,000 barrels per day of oil products, causing fatalities and injuries – seven people had died, seven were injured and two others were missing – plant had been shut down on May 11 for regular maintenance, and its planned restart had been brought forward from June 19 to June 12-16) FATAL EXPLOSION 1 killed, 2 injured in explosion at Thurston Co. fireworks facility (one person was killed and two were injured in an explosion at a fireworks facility – three injured victims were found at the scene – the most seriously injured of them, 74, died before he could be airlifted to a Medical Center – worker, 25, was airlifted to a Medical Center with burns and trauma – the co-owner of the business was taken another hospital to be treated for burns – explosion happened while a truck was being loaded with fireworks to be taken to a show) CHLORINE Chlorine Leak At Columbia Water Treatment Plant (a chlorine leak occurred at a City Water Treatment plant around 7:44 PM – when crews arrived they found a leaking valve on a 1-ton chlorine cylinder – the chemical scrubbing protection system activated and contained the spill in the area of origin – fire department Haz-Mat team members went into the area and were able to stop the leak – no injuries were reported) CONSTRUCTION Both sides of 405 reopen in Seal Beach after construction accident (a steel beam fell in an accident that injured 2 workers, one of them seriously – two workers were injured in the incident when a steel beam apparently fell in a construction area about 3 a.m. – construction crews were setting beams over the southbound lanes of the 405 freeway when one of the 135-foot steel beams bumped into another – the beams were being moved by forklift, and weigh about 60,000 pounds each – the impact of the beams sent at least one of them crashing onto the southbound lanes of the freeway – one worker, 52, fell about 25 feet – he was in serious condition – worker, 46, appeared to have been struck by the I-beam and suffered a chest injury – his condition was moderate) CONCRETE POUR COLLAPSE Parkway worker believed to have been crushed during concrete pour (a worker, 40, was crushed by a massive steel form – worker was believed to be assisting with a concrete pour on a bridge deck – he was helping to remove the form used for the pour when it is believed a gust of wind suddenly lifted the massive steel form as it was being raised by a crane and crushed him) EXCAVATION Berkshire Gas investigating cause of gas leak at UMass Amherst (a gas leak evacuated hundreds) TUNNELING FATALITY Tunnel to reopen after worker’s death (worker, 25, died about 7 miles into the tunnel which is part of a larger sewer project – a contractor, he died of a crushing blunt force traumatic injury of the head) CRANE FATALITY GHS Student Killed in Construction Accident (a work site accident claimed the life of a 16-year-old construction worker – he was struck by the boom of a construction crane and was pronounced dead at the scene at 11:41 a.m. – the job foreman, a relative of the deceased, was operating the crane) CRANE FATALITY Crane Accident at Fluor-ICA Fabrication Yard in Mexico Kills Seven Workers (a crane collapse caused a man-basket with eight workers to fall 40 meters (130 ft) to the ground, killing seven of them, at a fabrication yard – one worker is in serious condition after the accident – the workers were being lifted by a 600-ton crane, but that the accident cause is “unknown”) COLLAPSE Worker’s chest crushed by falling beam at Erin Mills Town Centre (a worker, 37, was critically injured due to a structural collapse – a steel worker was working on the roof of a construction site at the popular mall around 11 p.m. when a roof truss collapsed, causing some beams to fall – one of the beams fell on him, trapping him for about 10 minutes – he suffered serious chest and head injuries – he remains in critical condition) FORKLIFT FATALITY Worker killed by forklift in Queens (a worker, 54, was killed when a forklift fell on his chest at a construction site – he was found unconscious at the site at about 10:30 a.m. – he was taken to a hospital where he was pronounced dead – it wasn’t clear how the incident happened) WORKZONE DPW Worker Hit by Passing Car (Department of Public Works employee was struck by a passing vehicle while working in front of the municipal building – motorist did not stop – accident occurred at about 2:00 pm. – worker suffered minor injury to his shoulder) SCAFFOLDING COLLAPSE 5 roofers injured when scaffolding collapses at East Deer work site (five roofers who fell during an apparent scaffolding collapse didn’t sustain life-threatening injuries – two workers were flown to hospitals by emergency medical helicopters, and a third man was taken via ambulance to an area hospital – at least one other was treated at a hospital and released and officials said one worker declined treatment – the workers were replacing the roof when a portion of the scaffolding collapsed around 9:30 a.m. and they fell about 20 feet) COLLAPSED 4 Construction Workers Injured In Foster City Partial Building Collapse (four construction workers were injured when a building they were working on partially collapsed around Construction Worker On Busy Highway (a construction worker was taken to a local hospital following an accident in which a street sweeper backed into him – accident occurred on a portion of the highway that’s closed because of the construction) CRUSHED Worker injures leg in Clearwater construction accident (a construction worker, 32, suffered a severe leg injury when the piece of machinery he was working in flipped and came to rest on his leg at about 3 p.m. – he was on a piece of machinery similar to a Bobcat when the accident happened – the vehicle flipped into an open elevator shaft and came to rest upside down on one of his legs – he suffered a severe lower left leg injury) TRENCH Construction worker rescued from Falls Church trench after being trapped for eight hours (a construction worker had to be rescued after the trench he was working in collapsed, trapping the man for about eight hours – crews were working on replacing a water line when the 15-foot trench the man was standing in collapsed and covered the man’s feet in about 2 to 3 feet of dirt around 3:30 p.m. – a secondary collapse occurred not long after rescue workers arrived and further trapped the man in the trench, which was then about 20 feet below ground level, covering him in dirt and mud as high as his chest) FLASH FIRE Explosion injures worker at south Lincoln home (a fireball burned a construction worker as it tore through a two-story house blowing off doors and blasting through windows at 11:52 a.m. – three construction workers were renovating inside the unoccupied house when the fire started – one was taken to a hospital with non-life-threatening injuries – one of the workers was staining the floor in the basement while another went up to the garage to make more of the was working – he died at the scene – state medical examiner’s office will perform an autopsy) MOWER Landscaper suffers serious injuries in mower crash (a landscaping worker, 25, suffered serious injuries when his mower hit a metal fence at the bottom of steep embankment – the fence prevented his half-ton mower from going over a 10-foot wall into onto the pavement – apparently the man jumped from the mower just before he and the machine hit the fence) TREE TRIMMER ELECTROCUTED Man electrocuted while tree-trimming remembered for loving his wife, kids with all his heart (a tree-trimming accident killed a worker, 35 – he was strapped in a tree at a residence and was approximately 35 feet high and trying to get to a limb, when a metal strap on his body hit a live wire, electrocuting him) EXPLOSION Three injured after pool pump explosion (three maintenance workers were taken to a hospital after a pool pump explosion at an apartment complex – all three do not have life-threatening injuries and are expected to recover – one of the workers inhaled chlorine, while another was struck by a shrapnel – the third worker was injured due to pool chemicals entering the worker’s eye) EXPLOSION FATALITY 26-year-old worker dies in an accident at a gas station in South Bulgaria (worker, 26, dies in an accident at a gas station – incident occurred when the man was changing tank at the gas station – he died of a blow to the head and smashed chest) FLASH FIRE Aerosol can blamed after worker burned (apprentice, 19, suffered burns to his face, neck and hands after an incident about 3pm – it appeared the man had thrown an aerosol can into a fire – the man had been taken to a hospital in a serious but stable condition – the burns were not considered life-threatening) BIODIESEL EXPLOSION Biodiesel explosion destroys barn north of Waterloo (damage is estimated at $250,000 after fuel exploded inside a barn – farm hand was attempting to make a biodiesel – it overreacted – the farm worker didn’t get the mix right and it blew up – he was badly shaken but not physically hurt as he escaped the barn) FATAL EXPLOSION Australian worker dies in fridge explosion (a man has died in hospital after a gas explosion erupted while he and another man were working on a refrigerator unit at a hotel – the two men, aged 34 and 52, had been working in the cellar but were unaware of a gas leak – something sparked an ignition as the men were removing a motor from above a refrigeration unit – they suffered burns to 40-50 percent of their bodies, including facial burns and internal
