INDUSTRIAL MINE Investigation underway after worker hurt in Sask. gold mine ( miner, 58, was working at the mine site when the ground beneath him collapsed – crews had been doing some blasting at the time – the man was trapped for about an hour and was submerged up to his neck when his partner found him – he was released soon after he was taken to a hospital) SKID STEER FATALITY Worker killed in Utah meat-packing accident (worker, 52, was operating a skid-steer loader at the back of the plant – the bucket on the loader became jammed in the top of a trailer – while trying to dislodge the bucket, he was crushed between the loader and the trailer – he died at the scene) PAPER MILL FATALITY Georgia Pacific, OSHA investigating fatal accident (an employee, 39, at the mill has died from injuries sustained during a workplace accident – she died that night following the accident – no other detail will be provided while an investigation into the cause of the accident is being conducted – a worker was killed at the facility in August of 2003 – OSHA concluded the fatality occurred when an employee was clearing paper from an operating conveyor – worker apparently fell onto the conveyor and was thrown into an operating “beater pit” where paper was being shredded – OSHA recommended a fine of $102,000 – another worker was killed April 10, 2006 when an employee was assigned to weld pinholes on the outside of a pressurized metal tank – two other employees, standing on catwalks above and below the welder, served as firewatch(s) – during the welding operation, a vacuum-pressure relief device discharged steam and paper stock, causing one watchman to fall 30 feet – OSHA recommended a fine of $63,000) UNLOADING FATALITY Worker dies in freak accident at Yeo Hiap Seng factory (worker, 48, died from severe internal injuries caused by the accident- he was unloading goods from a shipping container when the trailer it was on moved, pinning him against a concrete barrier – the trailer was carrying more than 10 tonnes when its driver allegedly reversed into the worker, who died in a Hospital from serious internal injuries) GUARDING/LOTO Man Critically Injured In Industrial Accident (a man was injured in an industrial accident at a Dairy – the worker got his hand stuck in a machine, but was able to pull it out before responders arrived around 12:30 a.m. – he may have lost some fingers in the accident – he was taken to a Medical Center with critical but non-life threatening injuries) FATAL FALL from OIL RIG Coroner Investigating Death of Oil-field Worker (an oil-field worker was killed at about 3 p.m. in an accident on a oil rig – he was fatally injured when a platform extension fell – he was pronounced dead shortly after paramedics arrived) BURNS @ Aluminum Plant Man Injured In Aluminum Plant Accident (a worker at an aluminum plant is recovering from serious burns he got in an accident at the plant – the man stumbled into a pot of hot aluminum – he suffered burns to at least 40 percent of his body – this is the second major accident at the plant in a year-and-a-half – in April of last year a worker, 44, was killed when he got sucked into a scrap-metal grinder) MINE All 6 workers injured in accident at Oak Grove Mine released from area hospitals (all six workers injured in an accident at a Mine have been released from area hospitals – five of the individuals were seen at the hospital and released, while one was kept overnight for observation before being released – the six individuals transported to area hospitals were among nine people traveling out of the mine on a mantrip when the mantrip struck a piece of equipment) FURNACE EXPLOSION Pat Heung tofu factory blast kills one, injures four (a worker died and four men were injured, one critically, in an explosion that gutted a tofu factory – a furnace was believed to have exploded – the five men were working in the factory when the explosion occurred in the 80-square-meter cubicle that housed the furnace) ELECTRICAL Minntac Worker Hurt in Electrical Accident (a worker, 52, is hospitalized with serious injuries after an electrical accident – he was hurt in a small explosion that happened while he was working on an electrical apparatus – he has worked at the plant for about 17 years) REFINERY EXPLOSION 3 hurt after oil refinery explosion (three people were injured by an explosion at an oil refinery – one of the injured is in critical condition – causes of the explosion remain unknown) STEEL MILL FATALITY Man dies in accident at AGY plant (an employee was killed in an accident as he was attempting to remove some steel I-beams – a 30-foot piece of channel iron fell approximately 7 feet and struck him in the head – he was transported to a hospital where he was pronounced dead) FATAL PROPANE EXPLOSION Death Toll from Mexico Gas Plant Blast Climbs to 5 (the death toll from an explosion at a natural gas plant climbed to five on Wednesday after another body was found among the rubble – two other people have been hospitalized with serious injuries – the cause of the blast at the gas plant has not yet been determined – homes and businesses in a two-kilometer (1.25-mile) radius of the industrial park were evacuated) UPDATE on FATAL LAB EXPLOSION North Shore man dies follow North Andover plant explosion (worker, 51, died on Oct. 9 after he was caught in the epicenter of a fiery explosion in a laboratory the previous day and suffered extensive burn injuries – according to an investigation by the state Fire Marshall and Fire Department, the explosion was caused by the exposure of trimethylindium, a pyrophoric chemical, to the air – “Either there was a malfunction of the container or human error” – the fire at the plant following the explosion was quickly contained by the plant’s sprinkler system and the fire department) Explosion blasts hole in roof at Ramsbottom waste plant (a gas bottle was accidentally shredded at a waste management site and the explosion blew a small hole in the roof – incident did not cause a fire) AMMONIA Firefighters respond to North Portland ammonia leak (no one was reported injured in the incident, which happened at a metallurgical business – employees of the business showed up for work and began turning on their furnace equipment – an ammonia alarm sounded shortly before 4:30 a.m. and the employees evacuated – no evacuations were ordered – cause of the leak was not immediately known) AMMONIA Ammonia leak contained at Warsaw business (the leak started on the roof of a Foods plant around 12:30 p.m. – the leak activated emergency plans for the business, and fire response teams responded to the scene – the leak is believed to have been a very small amount in a small pressurized line – no injuries were reported, and the emergency response system worked exactly the way it was supposed to) AMMONIA CYLINDER EXPLOSION 10 injured in explosion from ammonia leak (at least 10 persons suffered minor injuries in their panicked attempts to escape an ammonia cylinder explosion – they were admitted to a Hospital – the injured were out of danger – the cylinder exploded around 11:00 a.m. inside an Ice and Cold Factory, which was closed, and the poisonous gas spread to an adjoining five story building) CONFINED SPACE ENGULFMENT OSHA investigating how 5 workers got trapped in grain silo in NC (only one worker remains hospitalized and is being held for observation – the five men were doing maintenance work inside the grain silo – at least one worker was up to his neck in grain, which he describes as having a quicksand effect) CONSTRUCTION CRANE FATALITY Man dies in Maplewood crane accident (worker, 34, died when a large crane he was operating fell during construction – the crane tipped on its side – he appeared to be moving a large beam when the outriggers to the sides of the crane gave way in the soil – other workers tried to dig him out of the cab, which had at least one broken window where dirt had fallen in – emergency responders provided medical care as they worked to free him from the crane’s cab using saws and other tools – after he was extricated, he was transported to a Hospital where he died) FRONT-END LOADER FATALITY Transport Quebec worker killed in work accident (a worker, 54, died just before noon – it appears the worker was crushed by a front-end loader he was attempting to lift onto a flat-bed truck) DEMO Worker injured at former heavy water plant site (a man was taken to hospital with injuries that were not considered to be life-threatening following an industrial accident at the former heavy water plant site – an employee working on the demolition of the former plant site was removing some fencing, when the accident occurred – they cut the last stretch of fencing there was tension on it and the pole struck him on the head) FORKLIFT Worker injured on Shetty Hospital site (a construction worker is being treated after an industrial accident on the site of a hospital – the man sustained a serious injury to his right ankle whilst working alongside heavy equipment – other sources have indicated that the young worker’s foot was severed by a forklift) MANLIFT in WORKZONE Two I-69 Construction Workers Badly Hurt In Accident (two construction workers were badly hurt when the hydraulic lift they were inside was hit by a semitrailer – the workers were on the lift underneath a bridge over the highway when it was hit – the semi driver told police he had slowed in the construction zone when the truck’s passenger side hit the lift’s bucket – the impact knocked one worker onto the roadway, while the other remained inside the bucket – both were taken to a hospital in serious to critical condition) FALL from COLLAPSED SCAFFOLDING Worker Hurt In Ft. Lauderdale Construction Accident (one man was rushed to the hospital after a beam collapsed at a construction site where workers are building a two story home – he was pouring concrete for a second floor when a board he was standing on gave way – firefighters arrived to find collapse debris and the injured man who had fallen approximately 25 feet – he was taken to a Medical Center as a trauma alert) UNLOADING REBAR FATALITY Worker killed at 49ers stadium construction site (a worker at the construction site died when a bundle of rebar he was unloading fell on him, the second workplace fatality at the stadium in recent months – accident occurred at 6:45 a.m.) WORKZONE Mowing crew worker falls from truck in motor vehicle accident (a mowing crew worker was airlifted to a hospital after a motor vehicle accident – a driver wrecked their vehicle into the back of a mowing crew pick up causing an employee to fall out of the vehicle and hit his head – the crew member’s injuries are considered non-life threatening) CRANE FATALITY Fatal workplace accident in Waterloo on Friday (a worker died while doing some masonry work on a building – the man fell several stories after being struck by a crane) ELECTROCUTION Worker electrocuted at Upper Arlington construction site (the electrical worker was in a room in a parking garage under construction when other workers reported hearing a loud noise around 10 a.m. – they found the worker touching the electrical panel – he was taken to a Medical Center where he later died) ELECTRICAL WorkSafe B.C. investigating after worker electrocuted at Vancouver job site (a worker was shocked at a job site – the worker was completing some concrete finishing around 11:30 p.m. – worker was using a tool to smooth out some concrete when a malfunction somehow led to an electrical charge connecting with the tool and shocked the worker) HOTWORK Police and fire briefs for Oct. 18, 2013 (a fire at a Field house caused the evacuation of a handful of students, faculty and staff from the building but did not injure anyone – an investigation pointed to a blow torch used during construction work as the source of the blaze that began in parts of the wall and ceiling – the use of a blow torch sparked the first flames as a roofing crew was covering an old skylight at the structure – fire was contained to a heating, ventilation, air conditioning and cooling room, but the structure’s HVAC system spread smoke throughout the building) FATAL FALLS Four labourers die in accident at Vedanta’s thermal plant (four laborers died when a pulley’s wire broke up at the 1980 MW power plant – they were working to erect the 270 metre high chimney – some workers were doing welding and grinding work at the height of about 150 feet by standing on a pedestal inside the chimney – a wire of the pulley attached to the pedestal broke due to a mechanical failure and the two workers fell down – the other two laborers who were left hanging from the pedestal were pulled out after an hour and were also found dead) CRANE Crane collapses onto overpass in Southeast Fresno (it is unclear how the accident happened, but the crane is blocking all lanes of the overpass – the equipment toppled over around 10:30 p.m. – it was being used for work on a section of railroad tracks – no one was injured) CRANE FATALITY Victim ID’d in deadly Butte crane collapse (a man, 46, died from injuries in crane collapse – he was in the bucket of the extended crane with another man when it tipped over – he was thrown from the box, while the other man sustained minor injuries – he was taken to a HealthCare facility before being flown to a hospital, where he later died) Miscellaneous MAINTENANCE WORKERS Struck by Train Trainee was operating BART train that struck workers, NTSB says (a trainee was operating a train when it struck and killed two workers who were inspecting the tracks – the driver/operator was one of two trainees on board with a management trainer when the accident occurred – three others on the train had mechanical duties – the trainee operator had sounded the air horn before the accident – the train was moving at 60 mph to 70 mph when it made an emergency stop – the operator had heard a radio report warning of workers on the track) MANLIFT FATALITY Carnival cruise-ship worker dies while doing ship maintenance (a cruise-ship worker died in an accident while working on the side of a ship at a port – accident happened about 10 a.m. as the worker was performing maintenance on the side of the ship – he was elevated in a cherry picker performing maintenance work on the side of the vessel, he became wedged between the cherry picker and a platform holding one of the ship’s lifeboats) CRUSHED FATALITY Worker dies after being pinned under tractor in northwest Harris County (a worker has died after being pinned and trapped under a tractor at a heavy equipment repair shop – one of the workers was apparently repairing a tractor when something went terribly wrong, trapping the person – employee was pronounced dead at the scene) BUCKET TRUCK Bucket truck accident injures worker (an accident involving a tree service company’s bucket truck which rolled from the roadway and into a ditch while an employee was in the bucket, leaving the employee trapped underneath – he was still conscious when rescuers loaded him into an ambulance – he was airlifted to a Medical Center with unknown injuries) PROPANE Country Fair food vendor worker burned in propane fire (a propane heater fire burned a worker, 27, at a one of the food vending booths at the Country Fair – she suffered burns of her face and hair when the propane device she was lighting flared up – she was setting up at the calamari booth when the incident occurred at about 9 a.m. – she was transported to a Hospital with first- and second-degree burns) TIRE INFLATION FATALITY Man killed while inflating tire (a farm equipment worker, 31, died after he was struck in the head by a tire – he died while attempting to inflate a tire at about 4:30 p.m. – tire bead came away from the rim and the whole thing flew approximately 15 feet in the air, striking the worker in the head, causing major trauma and death) |
