INDUSTRIAL FORKLIFT FATALITY Worker dies in industrial accident in Waco, crushed by lift (worker has been killed in an accident at an industrial repairs facility – the victim was crushed by the hydraulic aerial lift that he was servicing) ELECTROCUTION Worker dies after power line accident in Seffner (subcontractor was shocked while working on a power line – he died from injuries suffered in a power line accident – worker was 30 feet in the air in a bucket truck and touched a live wire while using a saw at about 11:16 a.m. – injured worker was transported to a Hospital where he died) CRUSHED OSHA investigating McNeilus after machinery nearly crushes worker (worker was nearly crushed to death by a piece of machinery that fell on him, pinning him down – he had to be extricated and was flown to the hospital) LOTO/GUARDING FATALITY Worker killed after arm trapped in machine at Welshpool factory in Perth (worker, 58, died in a workplace accident after allegedly being caught in a large industrial mixer – the man’s arm had become trapped – he died at the scene) AERIAL LIFT FATALITY Alabama worker killed in Virginia accident (worker, 22, died after he fell 83 feet while cutting electrical wires – he was in the bucket of a truck when the accident occurred around 12:30 p.m. – the wires sprang back and sheared the bucket off the truck, and he fell to the ground) FATALITY Maintenance Contractor Killed At Braddock Steel Plant (contractor has reportedly died in an unspecified accident – the contractor was performing unspecified “planned maintenance” when he was killed – no other injuries were reported and the steelmaker wasn’t providing other details immediately) ELECTRICAL OSHA investigating service worker’s life-threatening injury (service technician, 43, was hurt while testing electrical parts and was being treated for life-threatening injuries – he was working at a commercial facility when he was injured) LOTO FATALITY Man, 31, killed in lumber mill accident near Big River, Sask (worker, 31, was injured while conducting maintenance on equipment and died before the emergency services crew arrived) FATAL HOTWORK EXPLOSION Man killed in South Akl explosion named (worker, 24, was killed when a tank explosion blasted him across the road and into a car park – another man was injured – witnesses say the worker was flung from the top of the tank, across the road, and into a car park – he was welding when the explosion happened) EXPLOSION Taser weapons factory blast injures seven, one critically (worker was critically injured and six hurt in an explosion – worker was taken by ambulance to a local burn unit with life-threatening injuries – the other workers sustained concussion-related symptoms and were reported to be in stable condition at area hospitals – workers were cleaning up and shutting down the production line for the weekend when the incident occurred – no word on cause) MINE FATALITY Worker killed in Webster coal mine accident (miner, 29, died in a machinery accident – he was performing maintenance on a continuous mining machine at 2:20 a.m. when it fell on him and crushed him – another miner was working with him but was not injured) LOTO/GUARDING Man crushed by carpet machine (worker who was maintaining a carpet forming machine, became trapped in a guarded area of the equipment – victim, only been in the job for two weeks after being promoted from operator to tool changer, was under supervision – however during the works the employee became trapped when the top hood of the carpet heating press descended on him – employee suffered severe crush injuries) CONFINED SPACE FATALITY Man dies in sugar plant accident (worker, 27, was killed on the job while he was part of a cleaning crew working in a large storage silo when a large chunk of solidified sugar became dislodged and crushed him) ASPHALT PLANT One hurt in Hamilton asphalt plant explosion (worker has been taken to hospital with burns after a small explosion caused hot asphalt to splash back onto a worker – some of the heated asphalt had splashed onto a worker attempting to clear a chute at the back of a workshop) FLASH FIRE Kyneton man airlifted after explosion (worker, in his 40s, was been flown to a hospital after suffering serious burns to the face and hands in an industrial accident – accident occurred while the worker was removing a fuel pump – a line of petrol ignited, causing an explosion which left the man with severe flash burns – injuries were not life-threatening and the man was airlifted largely as a precautionary measure) HF ACID LEAK Acid leak at Torrance ExxonMobil refinery being probed (state safety regulators are investigating a leak of modified hydrofluoric acid at a refinery – the leak occurred from a 3-inch nozzle as a truck containing hydrofluoric acid unloaded its contents into a vessel at the plant – during the operation, “a small leak” was discovered at a rate of 10 drops per minute – the leak was fixed by 6 p.m., that means the acid leaked for at least 15 hours) CONFINED SPACE FATALITIES – ONE Entrant and ONE “would be rescuer” Two die after falling into sewage vat at Eni plant (two workers, 33 and 37, from an engineering firm died after falling into an industrial sewage vat at a petrochemicals plant – they were carrying out a routine inspection of the vat at the plant when the accident happened – one of the workers fell while trying to set up a small surveillance camera by the vat’s edge, while the other died while trying to save him) CRANE FATALITY 1 killed in Eskom plant accident (one worker has died and four others left hurt after a crane collapsed at a Power Station) CONFINED SPACE FATALITY Three killed in accident at Andhra coffee plant (three workers were killed and two others critically injured in an accident at a coffee plant – workers died after they inhaled lethal gases while carrying out repair of a boiler – the injured were taken to a hospital, where their condition is stated to be critical) AMMONIA 8 people taken to hospital after ammonia leak at Brentwood rec center (eight people were taken to a hospital after an ammonia leak in a room with a refrigeration system for the ice rink – three were firefighters, four were construction workers and one was a city employee – firefighters were treated and released; the city employee was treated and released – one of the construction workers was treated for ammonia burns – none of the injuries was life-threatening – workers were checking the system when the leak was discovered) AMMONIA Nine injured in Galilee factory ammonia leak (nine workers were slightly injured after an ammonia leak sparked a fire at a factory – four of the nine injured were taken to a Medical Center for treatment after being exposed to ammonia – the factory’s automated safety systems sealed the leaking valves shortly after the leak was discovered – local residents were warned to stay indoors until the area is cleaned) FORKLIFT FATALITY Letcher County man killed in forklift accident (worker, 38, died after a forklift collapsed on top of him – he was working on a forklift when the forks gave way, crushing him – he was declared dead at the scene – authorities are still looking into what caused the forklift to collapse) CONSTRUCTION FATAL CONCRETE POUR Worker killed in construction zone ID’ed in Hancock Co. (construction worker, 54, was killed in an accident at a restaurant when a section of flooring collapsed – workers were pouring concrete on a raised deck – workers tried to rescue the worker but his injuries were too severe) CRANE Worker in crane collapse released from hospital (worker was operating the crane to pour concrete in the construction of a parking garage when the crane became unbalanced and toppled over – worker suffered moderate injuries – he was conscious as he was taken to a Hospital) ELECTRICAL Worker airlifted to hospital after electrical accident in downtown State College (construction worker was injured in an electrical accident on a construction site – he was airlifted to an unknown hospital – worker was pouring concrete from the second level of the building at about 9:30 a.m. when the equipment he was working with hit an electrical line) FOOT AMPUTATION from GRINDER Worker Loses Foot In Hand Grinder Accident (worker lost a foot in an industrial accident at a commercial strip mall at 8:28 a.m. – he had been working with a hand grinder when a malfunction occurred and the blade somehow fell and sliced off his foot – worker was airlifted to a Medical Center) EXCAVATOR Worker injured in construction accident in Freetown (worker, 52, suffered a serious leg injury when an excavator rolled over on its side, pinning his lower left leg under the weight of the heavy equipment vehicle – injuries suffered by the worker were isolated to his left lower leg – worker was transported with non-life threatening injuries – forklift from a nearby business was used to gently lift the excavator off the worker’s leg and firefighters slid the man out the cab) ELECTROCUTION with CRANE Man electrocuted in ‘freak accident’ (worker was electrocuted around 10 a.m. – construction crews were working on the sewer, getting ready to drill a hole underneath the roadway – when they were replacing that pipe the crane got into the power line – tThe crane was 85 feet in the air and had a cable lowered into a ditch where one man was working – attached to the cable was a hook and that hook was supposed to wrap around pipes so they could be lifted out of the ditch and replacements could be moved in – crane was back into power lines, and as the man reached for the cable, he was electrocuted) WORKZONE FATALITY Highway worker hit and killed on I-15 in Escondido (highway worker, 21, was killed when a suspected DUI driver crashed and overturned their SUV on a freeway exit – he was repairing sprinklers on the roadside at the time he was struck – accident occurred shortly after 2 p.m. – driver, 31, was on drugs when he was trying to get on I-15, when he ended up down an embankment) WORKZONE FATALITY Construction Worker Killed by Dump Truck (construction worker, 26, was killed as he was standing in the construction zone around 11:23 p.m. – a dump truck was doing pavement work on the northbound lanes when it started to backup southbound – worker was standing behind the truck and was struck) FATAL FALL Worker falls to his death at construction site (construction worker fell to his death – he was working at a 16-story construction project when he fell around 9:15 a.m. – the man was dead at the scene) CRANE Two Workers Injured After Wall Collapses At Home Construction In Greenwich (two workers were injured after a wall collapsed on them while working – one worker was seriously injured and may have life-threatening injuries after a wall collapsed on him and a co-worker about 4:50 p.m. – 12 workers were trying to lift a 25 foot by 15 foot wall made of 4-by-8-foot LCL lumber with three-quarter inch plywood sheathing – as the wall was lifted to about shoulder height, two of the workers went under the section of and the wall dropped) WORKZONE Road worker injured while placing cones on highway in Hawaii (road worker, 59, was injured last week after he fell while placing orange safety cones along a highway – worker was seriously injured at about 5:40 a.m. when he fell from a road crew truck – he suffered a knee injury from the accident and was taken to the hospital) WORKZONE Connecticut DOT worker injured after being struck in vehicle by tractor-trailer (Department of Transportation worker was injured after his vehicle was hit by a tractor-trailer – he was standing outside his truck on Interstate 95 when a tractor-trailer hit the DOT vehicle – he suffered injuries to his leg and arm, but they weren’t life-threatening) FATAL FALL from SCAFFOLDING Worker dies after platform collapses at Champlain ice bridge (worker, 44, had been working with a scaffolding, one of at least a few placed underneath the structure commonly known as the Ice Bridge, when it somehow gave way at one corner at around 12:20 p.m. – after an extensive search, the body was found shortly after 6:30 p.m. close to the Champlain Bridge) WORKZONE Worker hit by car while striping road (worker, 31, was seriously injured while striping a north side street – he was listed in serious condition before being flown to another hospital in the afternoon – he was running a motorized hand cart to stripe the center line at around 10 a.m. when he was hit by a sedan – the crash threw the worker and the painting machine into the eastbound lane where the machine was hit by another car – the impact of the second crash threw the machine on top of the worker – a second city worker was driving a pickup truck behind the painting machine as traffic control – the second car hit the pick up truck head on after it had collided with the painting machine, causing minor injury to the driver) FALL from SCAFFOLDING Painter on life support after scaffolding fall (worker, 49, sustained serious backbone injuries when he fell off a scaffolding while doing a painting job – five months and three surgeries later, there is no improvement in his condition) MISCELLANEOUS FATAL FALL Maintenance Worker Dies In Accident At Tulsa Fairgrounds (maintenance worker was killed and another was injured when they fell while working on a ride at the fairgrounds – the two were working a ride called the Skyride when the basket they were in fell – both men were wearing safety harnesses and were hanging about 90 feet off the ground – both men were taken to a hospital where one of the men was pronounced dead – the other man was reported in fair condition) FARMING FATALITY Elderly farm worker killed in tragic accident with combine harvester (worker, 71, was pronounced dead at the scene despite lengthy treatment from emergency
