INDUSTRIAL WAREHOUSE New Jersey worker in pineapple accident (a worker, 39, walked away after being buried under 680 kg of pineapple boxes – the factory worker is in a stable condition and was able to walk away after the accident, the cause of which is unknown – the man was taken to hospital to check for internal injuries – the man never lost consciousness and was not seriously injured) CONFINED SPACE Cudahy man in critical condition after Milwaukee industrial accident (a worker, 43, is in critical condition with what police are calling non-life threatening injuries after an industrial accident – the accident happened at about 7:45 a.m. – he was cleaning a cement mixer when he became trapped inside – emergency crews extricated the worker after 20 to 30 minutes and that worker was taken to a Hospital) CONFINED SPACE FATALITY in BARGE Worker At Port of Palm Beach Dies in Barge Accident (a man working was killed in barge an accident in Slip 3 at the port – initial reports had Riviera Beach Fire Rescue and hazmat crews on hand to investigate a possible chemical emergency, but those report seem to be erroneous – the worker, likely working construction at the port, reportedly went into the bottom part of a barge and never came back up – early reports indicate that the man was trapped in the barge, where he died – another worker did go to check on him, but found the man lying face down, according to police – the initial fears was that the man had succumbed to chemical fumes and passed out from them, but hazmat officials confirmed there was no such emergency) ELECTROCUTION Worker dies from injuries after electrical accident in Bayers Lake (a worker, 39, has died after being electrocuted while working at a building – at 1:30 p.m. the man suffered an electrical accident and was rushed to hospital, where he died) RADIATION Japan nuclear lab accident affected 30: Agency (the number of researchers exposed to low-level radiation in an accident at a nuclear laboratory has hit 30, with human error likely worsening the problem – the accident happened as 55 people were working at the laboratory – the researchers were carrying out an experiment that involved firing a proton beam at gold when the accident happened – the agency, which had initially said six researchers were exposed to radiation, announced late Sunday that 24 more people were affected – there was no widespread radiation leak, although two researchers were exposed to up to 1.7 millisieverts of radiation, a dose roughly equivalent to a strong medical X-ray – none of them required medical attention) FERTILIZER MIXING FATALITY Employee dies following fertilizer-mixing accident (a worker, 23, died from injuries sustained while mixing fertilizer in a mobile production unit on the grounds of the plant – he was blending water, ammonia and polyphosphate in the mobile unit on the afternoon of May 18 when he sustained injuries – he started working for the company early last year) HOTWORK FLASH FIRE @ GRAIN ELEVATOR Iowa worker injured in grain elevator fire (authorities are investigating the cause of a flash fire at a grain elevator that injured a construction worker – the worker was taking apart equipment with a torch at the top of the elevator when the fire occurred – he was burned on his arms and face – authorities did not reveal how serious the burns were – the cause of the fire is not yet known) STEEL PLANT BURNS Steel Workers Burned (three workers at a steel mill were burned in an apparent accident – they all suffered burns – their conditions and weren’t released – a bucket containing molten steel apparently exploded, injuring the workers – authorities are still trying to determine exactly what happened) FATALITY HOTWORK EXPLOSION on TANK Worker killed in bunker tank blast (a worker, 60, was killed while another one is in serious condition when the bunker tank of an asphalt plant exploded – the other worker, 44, is still undergoing medical treatment – they were installing pipes at the top of the bunker tank using a welding machine when the explosion happened – according to reports, the heat from the welding machine might have caused the explosion – a fire started after the explosion, causing the victims to suffer from serious burns in different parts of their bodies) FATAL FALL from TRUCK @ LANDFILL Yell County worker dies after fall at landfill (a county employee, 72, died after falling at a county landfill – he was killed after falling 18 to 20 feet from the landfill trailer and suffered severe injuries and was transported to the a hospital where he later died – authorities are unsure as to what caused the fall – he was a trash compactor operator for the County for more than 27 years) ARC FLASH Man suffers electric burns in explosion (a worker, 37, was injured in an electrical explosion at a factory – the worker received burns to his hands and face following an explosion and was taken to a Hospital for treatment) ELECTROCUTION on OIL FIELD Worker electrocuted in oilfield accident near Carlsbad (a worker, 23, died from electrocution on an oilfield – he was reportedly working with a crew to transfer a blowout preventer with a hoist bearing vehicle – the hoist made contact with an overhead power line while in transit, and the worker was knocked to the ground – personnel on the scene attempted to resuscitate him but were unsuccessful) CONFINED SPACE FATALITY – ATMOSPHERIC – One Entrant and Three would-be-rescuers Poisonous fumes kill 3 laborers inside factory waste dump (three workers died after allegedly being exposed to poisonous fumes in a waste dump – a fourth worker fell unconscious inside the waste dump in the tragedy which occurred in the morning when one of them had gone to clean the area – a worker fell unconscious after entering the dump in the morning – three other laborers who went to his rescue, too, fell unconscious inside the dump – when the four were finally rescued from the dump by other factory workers, it was found that three had succumbed while one was still unconscious) LINEMEN INJURED – ELECTRICAL and POLE Electric line workers injured restoring power (two electrical line workers providing assistance to restore power to tornado-ravaged areas have been injured, including one who suffered burns during an electrical charge – one worker was reportedly electrocuted and taken to a hospital – the next morning, another line worker was injured when a power line snapped and struck him in the face – the worker hurt by an electrical charge was taken by ambulance to a Medical Center for treatment of burns and was reportedly in serious condition – the lineman injured by the wire wasn’t transported by ambulance and instead was released to his coworkers in good condition for further medical care) CHEMICAL REACTION 5 Neb. meat workers hurt after chemical reaction (five workers at a beef processing plant were exposed to dangerous chemicals during an accident that forced part of the plant to be evacuated around 10 p.m. an April 14 after bleach was mistakenly pumped into a container of acid at the plant – the mixture of chemicals caused a reaction that produced toxic gas – part of the plant was evacuated for more than an hour while the ventilation system was used to clear the gas – five workers were treated for exposure at a hospital and released) REFINERY FIRE Fire breaks out at CPC plant in Kaohsiung (a fire broke out at a residue desulfurization unit, but there were no casualties – a preliminary report indicated that there were emissions of hydrogen gas from the plant after the fire broke out – the fire was caused by a fracture in a pipe in the factory’s residue desulfurization unit II – the fire started around 12:33 p.m. and was extinguished at about 1:05 p.m. by a team of 52 firefighters and 22 fire trucks dispatched by the city’s fire department) FATAL EXPLOSION China probes blast at explosives plant (china’s top procuratorate is investigating whether duty-related crimes may have contributed to a deadly blast at an explosives plant – the explosion occurred inside a three-story workshop of a plant – 33 people be handled internally.” – initial reports suggested a “serious leak” of sodium chloride – used in softening water – that can be harmful in a gaseous state) FATAL EXPLOSION One killed in Negev plant explosion (one person was killed and six more were moderately injured in an explosion at pharmaceuticals plant – no other details) AMMONIA Ammonia leak forces evacuation of Fort Worth plant (an ammonia leak at a business forced an evacuation, but no one was injured at about 8 a.m. at a food processing and packaging plant – plant officials had started an evacuation when firefighters arrived, but three workers were unaccounted for at the scene – hazardous material crews searched the business and found the three plant workers in protective gear attempting to stop the leak – crews allowed the ammonia vapors to dissipate and the workers were later allowed back into the plant) AMMONIA Ammonia leak at Emporia plant sends 7 to hospital (seven people have been released from a hospital after an ammonia leak at a food plant – officials said the level of ammonia that was leaking was well below what’s considered lethal – the leak temporarily disrupted production in part of the processing plant before it was found and stopped) AMMONIA Leak sends 12 workers to hospital (an ammonia leak sent a dozen people from a nearby business to a hospital – a dozen employees showed signs of ammonia exposure, for which symptoms include respiratory problems, sore eyes and headaches – a total of 12 employees were transported to hospital as a precaution and later released – the source of the ammonia was a leaky valve located on the roof of the neighboring plant – the windy conditions at the time pushed the vapor cloud across the street to the operation – the employees were outside working when they were exposed to the gas) CHLORINE Six firemen in hospital after chlorine exposure (six Fire and Rescue Services personnel were hospitalized after they suffered breathing discomfort while containing a chlorine leakage – a fire tender from the Fire Station, with a six-member crew, responded to the distress call – the personnel entered the Water Pumping House and found that the leakage was from the chlorine pipeline coming from the cylinder running parallel to the water pipeline for drinking water purification purpose – the leak was mended and they took out the chlorine cylinder and decided to dissolve it in a water tank in the vicinity on the campus – they lowered the cylinder and released the chlorine into the water from the cylinder – while doing so, the personnel were exposed to the chlorine and reported breathing difficulties at 6.50 am) CONSTRUCTION FATAL PARKING GARAGE COLLAPSE Prince Frederick Man Identified as Worker who Died in Construction Accident (a worker, 57 was killed when part of a garage under construction at a Mall collapsed – he died while trapped under a 50,000- to 55,000-pound piece of concrete with a steel beam running inside of it – a second worker survived the collapse – the men were working on the renovation when the concrete fell on them – there were eighteen other workers at the garage when the accident happened – the garage was under renovation and not being used for parking at the time of the accident) TRENCHING Construction worker in hospital after he is pinned underground outside Pratt & Whitney (a construction worker was flown to a local hospital after a construction accident 20 feet below the ground – the worker was working with a crew digging pipeline that parallels a Highway – he was working 20 feet below the ground when a piece of metal machinery shifted and pinned him between the machine and the shoring – crews extricated the man who was then flown by helicopter to a Medical Center – he is expected to survive) ELECTROCUTION Bridge worker dead in accident (a worker, 35, helping crews prepare to paint the bridge died after being taken to a hospital – other workers found the man unconscious and tangled in safety netting – the man had burns on his hands, which is an indication he might have been shocked by electrical wires – workers have been making preparations for weeks for the project, including installing equipment and industrial tarps to keep paint out of the river and dismantling large decorative lights to store while the work is ongoing) PFAS SAVES TWO MORE!! Accident left 2 workers dangling from Newburgh bridge (an accident left two construction workers dangling over the Hudson River – the two men were working on the underside of the bridge when temporary supports gave way, leaving them dangling by their safety harnesses – the men were able to climb to another part of the bridge and walk away unharmed – the men were stringing steel cables to support a containment system designed to keep debris from falling into the river) FATAL FALL Worker dies after falling off Chicago roof (a man, 55, has died after falling off a roof while working – he died from multiple injuries after falling) FATAL FALL Worker dies after 7-story fall (a worker, 29, is dead following a seven-story fall at a construction site – he was breathing very slowly when rescuers arrived and was taken to a nearby trauma center in critical condition and later died – no other details) FATAL FALL from BARN Police identify worker killed in fall at Seiple Farms (a construction worker, 28, died during a barn erection at a farm – he was killed around 6:45 a.m. at a construction site) EXPLOSION One hurt in Carpentersville explosion, fire (a home-construction worker was hurt in an explosion and fire at a house – he suffered burns on his arms and was taken to a Hospital – the 10:45 a.m. blaze likely was related to the construction in the bathroom of the house – although the fire is under investigation, authorities believe it started with a gas explosion – the fire extended from the first floor and into the attic – the house was badly damaged by the blast and fire, and it was deemed uninhabitable by the local building department) ROOF COLLAPSE Worker hurt in Simpsonville roof collapse (a worker was hurt when part of the roof collapsed in a former cotton mill – firefighters were called to the scene at about 11:30 – injuries are not believed to be life threatening – the workers are doing some reclaiming of some old lumber – there was a “partial collapse of the roof area”) CRANE FATALITY 4 killed in crane crash in Dhaka (four people have been killed as the chains securing the crane came loose while rescuing a truck – the accident happened at around 9:30pm – two more people have been injured in the accident – a goods-laden truck turned upside down in front of a Market after it ran into the traffic island in the evening – a crane reached the spot at 9:00pm to rescue the truck – at one stage of the rescue operation, the chains of the crane got apart and it almost upended – the four were killed as the chains and the crane hit several onlookers gathering there to see the operation) TOWER CRANE COLLAPSE Crane Collapses And Crashes Into Flats In Russia (a huge construction crane has collapsed on a block of flats in Russia, smashing through several floors of the nine-story complex – seven vehicles parked outside the apartment block, which is home to up to 300 residents including 37 children, were also damaged – there were no reported injuries after the accident – the cause of the crane’s collapse is unknown but it is thought it may have been down to strong winds) MISCELLANEOUS FLUNG from BACK OF PICK-UP TRUCK Water Department worker falls off pickup in traffic accident (an employee of the Water Department was rushed to a Hospital after he fell from a motor pickup that was involved in an accident – he was flung from the vehicle during the impact – the incident occurred at approximately 7:45 a.m. – vehicle was heading in a western direction while the other came from a bi-road and collided with the right rear – in the process, the worker who was one of two passengers traveling in the cargo bed of the vehicle, fell from there – he was transported having suffered head injuries) STOP-DROP-ROLL! Car Wash Cashier Burned When Dress Catches Fire (a car wash cashier was severely burned when her dress was ignited by an electrical heater – her dress touched a heat coil and caught fire – she ran in a panic and a co-worker knocked her down and smothered the fire – another co-worker sprayed her with a hose – she was in critical but stable condition at a Medical Center) ZOO FATALITY Young zoo worker, 24, who was mauled to death by TIGER as she fed it in its enclosure (A young zookeeper, 24, suffered serious injuries and went into cardiac arrest following the attack – zoo owner said the keeper broke strict safety protocols – it was ‘inexplicable’ why she entered the tiger enclosure) Forest worker dies in freak accident with tranquilizer gun (a forest worker died after allegedly being accidentally shot with a tranquilizer gun by a co-worker – the tranquilizer gun was fired by a forest department employee at a herd of four to five elephants which had entered the area in search of food – the forest worker who was accidentally shot was taken to a hospital where he was declared dead) HOTWORK Small Fire Breaks Out at Salinas High School, Students Using Torch to Cut Metal (a small fire was started at a High School just before noon – the fire started in the metal shop area, which is next to the gym – students were working on their projects and cutting sheet metal with a torch when the fire was started – it is believed that the equipment malfunctioned and caused the fire – part of the school was evacuated and the fire was contained to the workshop area – nobody was injured in this fire) |
