INDUSTRIAL LOGGING FATALITY Nelson man killed in logging accident (worker, 35, was killed in a logging accident – he was struck by a log while doing a job they’ve called “choker”) MINE FATALITY Goldfields loses second worker (worker, 63, was killed while operating machinery at a remote goldmine) Worker hurt at aluminum plant in Wapakoneta (a worker suffered a hand injury in an industrial accident – he suffered non-life threatening injuries in the accident at the manufacturing facility at about 8:20 p.m. and found the worker conscious and talking in a first-aid room) LOTO/GUARDING Monarch employee in Florence airlifted to hospital (a worker was airlifted to a Hospital after being involved in a workplace accident – he got his arm trapped in a machine – fire crews worked to maneuver him out of the machine, and he was then taken by med-flight to a Hospital – no word yet on his condition) LOTO/GUARDING FATALITY Worker, 24, dead after industrial accident in rural Manitoba (worker, 24, is dead following a workplace accident shortly after 2 a.m. – he became caught in an industrial machine – emergency medical services were dispatched, but the man was declared dead at the scene – the employee was running a radial telescopic stacking conveyor at the time of the incident – machine is designed to telescope and transport rock and sand on a conveyor belt – it’s unclear what happened, but the worker, who had been with the company for about six months, was familiar with and trained in the procedures of running the crusher, which is commonly used at sand and gravel pits) SAW ELECTRIC SAW ACCIDENT (worker’s arm was cut by an electric saw – his co-worker stated that the man lost control of the saw as he was cutting a piece of wood – the 39 year old furniture maker was given first aid and then rushed to a Hospital) LOTO FATALITY Police investigating fatal industrial accident at Milwaukee business (worker, 38, was crushed by a machine around 1:30 a.m. at a cement and construction business – he died while making a repair in a concrete mixing tower – he oversaw production and would have been the first to inspect any machine malfunction – when staffers tried to radio him., there was no answer – a supervisor later found him pinned under a machine – he worked at the plant for 5 years) FORKLIFT Worker injured in forklift accident (worker, in his 50s, was injured when a forklift crashed into him at a warehouse at 9.10pm – he was treated at the scene and taken to hospital with lower leg injuries – unclear what had caused the accident, but it seemed the man had been walking around the warehouse when the forklift hit him and injured him) SULFURIC ACID BURNS E. Texas steel worker suffers sulfuric burns, flown to hospital (worker, 55, at a steel plant had to be flown to an area hospital after she was severely injured in an accident – the woman suffered serious injuries from a sulfuric burn) Piling worker’s leg crushed at Weymouth job (a worker, 50, was airlifted to a hospital after his leg was crushed between two pieces of steel – it is believed the incident involved heavy sheet piling) MANLIFT FATALITY @ MINE Authorities investigate mine death (an accident claimed the life of a contractor – he was in the open cabin of a cherry picker at the mine, around 15 metres off the ground when he was crushed between it and a metal structure above – he suffered head and neck injuries and died at the scene) FORKLIFT Man’s leg stuck in forklift at Friendly’s H.Q. (firefighters worked to rescue a worker whose leg became entangled in a forklift at a plant – a fork truck had backed over the employee and that caused the man’s leg to become stuck – Fire Department Rescue Squad was called in to help get the man out, which took quite a while – worker was taken to a Medical Center, though his condition is not known at this time) ELECTRICAL Worker Burned In Fire At Hialeah Car Dealership (a worker at a Service Center was rushed to a Trauma Center with severe burns – it began when some electrical work was being performed at the scene – Fire units had to wait for Florida Power and Light to de-energize the building – the extent of the injuries to the worker is not clear at this time) EXPLOSION Explosion and Fire Fought at Ill. Brass Plant (an explosion caused injuries to two workers – an injured worker with a bandage on his arm was transported via helicopter to a Hospital and another worker was also transported for injuries) LOTO/GUARDING FATALITY Cullompton man was crushed to death at paper mill (worker, 41, was crushed to death after being “flipped” by a moving belt into machinery at a paper mill – he became stuck in the machinery at the mill – he had been running his hands along a belt to check for creases in some paper – he suddenly “cartwheeled over” and landed on the machinery – he suffered multiple injuries which included crush wounds to his chest that were consistent with being trapped in rotating machinery) HOTWORK Second fire in as many months at Calgary roof-tiling company (a fire forced a dozen people to leave the site of a roofing-tile company – it was the second fire in as many months at the facility – this fire was smaller than the one in March that heavily damaged part of the building – it’s believed it was started by a contractor dismantling some equipment with a cutting torch) BIOFUELS PLANT EXPLOSION New Mexico biofuel plant catches fire (a biofuel facility caught fire prompting authorities to briefly evacuate an area that includes about 350 homes – a series of small explosions was reported at the plant but there was no immediate word of any injuries – the cause of the fire was not yet known – the half-mile area around the plant was evacuated because of the threat of blasts from the methanol on the premises) FIRE Paper mill fire in Hamilton, Leicester, was an accident (a blaze that burned for more than 12 hours at a paper processing plant started accidentally – investigators said the fire at the factory, which makes toilet tissue for supermarkets, started in a machine used to dry paper – at its height, more then 50 firefighters were involved in containing the blaze – it spread through ducting and aluminum sandwich panels used for cladding and insulation to the roof – plant was safely evacuated prior to the fire service’s arrival – no-one was hurt in the incident) EXPLOSION Sayreville warehouse blast injures 3 (an explosion of undetermined origin blew out a portion of the warehouse – the blast at the food packaging manufacturing plant slightly injured three people – the explosion, about 7:30 p.m. is of undetermined cause) EXPLOSION Explosion, fire causes damage to Ball Metal Beverage Container plant (workers were evacuated overnight after a fire broke out – the fire began in an oven and spread to the factory’s ductwork – an explosion in the ductwork then caused the fire to spread throughout the entire ducting system and out the roof – four firefighters received minor injuries from the blaze and were treated on scene – the fire was fueled by accumulated residue from the ovens caked inside of the duct work – no employees were injured) FORKLIFT FATALITY – WARNING! GRAPHIC CONTENT Man dies in freak forklift accident (a man died after his head was crushed by a forklift in a workplace accident – when crews arrived, they found that the victim had already died – the victim was driving the forklift when it suddenly tilted and landed on its side, crushing the victim’s head) CONSTRUCTION BULLDOZER FATALITY – LOADING onto FLATBED Construction Worker Killed When Bulldozer Overturns (a construction worker, 67, lost his life after a bulldozer overturned at approximately 9:45 am – upon arrival Officers observed a man lying on his back with a bulldozer on top of him – unfortunately, life saving measures were unable to be employed – further investigation revealed, that the worker was attempting to load the bulldozer on to a flatbed trailer at the time of the accident – it is not clear how the equipment toppled over and off the trailer at this time) FALL and IMPALEMENT on REBAR Fall injures worker at Wastewater Treatment Plant in Detroit (a worker was impaled by a steel rod when he fell at a Wastewater Treatment Plant – he has been discharged from the Hospital – the worker’s torso was punctured after he fell on some reinforced steel at the plant – he was working on a fire alarm upgrade when he fell – “He didn’t fall very far” – “He impaled himself on a steel rod that was uncapped. It entered the chest cavity.”) CRANE SET-UP Worker injured during crane set up in Charlottetown (a worker suffered a fractured wrist and is in the hospital for observation after part of a crane hit him on the head when it fell to the ground – at about 8 a.m. part of a crane boom fell a few feet during the set-up procedure and nothing was suspended or hanging in the air at the time – two workers were involved in the accident and one was still in hospital for observation as of Monday after he was hit in the head – that worker also had a fractured wrist and got stitches to his tongue while the other worker had a sore wrist) FATAL FALL from AERIAL LIFT Construction worker dies after falling from 4th floor hotel (worker, 35, died after falling from a Hotel construction site lift – he was on a platform holding supplies that were being lifted to the fourth floor – accident happened around 7:30 a.m. as the man was working at the site – he was trying to put red and black wire spools inside the fourth floor when the platform became unstable – he was not wearing a safety harness at the time of the incident) FALL from LADDER into MANHOLE Worker slips from ladder, falls in downtown Denver manhole; injured (a man was rescued by firefighters after falling through a hole at a construction site – he fell off a ladder into a manhole and likely suffered a broken leg – the worker, who reportedly weighs about 300 pounds, may have suffered other injuries as well, although he was conscious after the fall – he was taken to a local hospital to be treated and evaluated) RUN OVER CONSTRUCTION WORKER INJURED AT CORBETTS FARM DEVELOPMENT (a construction worker was severely injured around 9 a.m. when his legs were run over by a cement truck while working at a residential development – the man, approximately 52 years old, is in stable condition with “what appeared to be non-life-threatening but severe” injuries) AERIAL LIFT Worker hurt when lift tipped into building (an accident left one injured after a construction lift tipped onto two wheels at 10:11 a.m. – the lift tipped into a Church with one construction worker in the basket – the worker was on his way to the roof with shingles when the lift tipped, crashing the basket into the side of the church – first responders removed the construction worker from the lift and he was taken to the hospital – the injuries sustained by the worker in the basket could include a broken arm) WORKZONE FATALITY Road maintenance worker killed, 5 injured after hit by express bus (a road maintenance worker was killed and five others injured after they were hit by an express bus – the noon accident happened as the victim was directing traffic at the newly paved road – the bus lost control and swerved to the right hitting the victim – another bus which was following closely behind could not brake in time and hit a van and a four-wheel drive vehicle where the five others were standing – two of the five were seriously injured while the other three suffered light injuries) FATAL FALL Worker dies in fall at San Jose construction site (a construction worker helping build town homes at a site fell to his death – the three story fall – it’s the second such fatal accident at a Bay Area construction site in two days – Cal/OSHA spent hours at the accident site trying to determine how the man slipped around noon – he was unloading sheet rock before he fell from a height of three stories – it was unclear if he was perched on the building or the forklift crane holding the materials) EXCAVATION Worker hurt as gas blaze sparks chaos (a construction worker was taken to a hospital with serious burns and 150 people were evacuated from a shopping plaza after a gas pipe ruptured and caught fire during earthworks – the man, in his early 20s, was “in a stable condition” – the fire burned for just over an hour before fire crews were able to bring it under control, while police managed the evacuation of shoppers and traders amid early concerns the fire might spread) HOTWORK Downtown Building Catches Fire (a building under demolition caught fire – crews had been using welding equipment earlier in the day and suspect that was the cause of the fire) WORKPLACE VIOLENCE Construction worker allegedly killed by co-worker in hammer attack (a man was taken into custody by police investigating a deadly hammer attack at a construction site – a heated argument between two construction workers turned physical when one of the men began beating the other in the head with a hammer – it happened during a renovation project at an apartment building – after the attack, the injured man stumbled out of the building and collapsed in the street – he was transported to an area hospital where he was pronounced dead) Miscellaneous HOTWORK Owner of Eastside auto body shop sustains second-degree burns in fire (owner, 63, of an auto-body shop was in serious condition with second-degree burns after a fire – he was working with a torch when somehow, his clothes caught fire about 7 p.m. – worker, 59, saw his boss run out of the shop and ran after him, tearing his burning clothing off – both were taken to a Hospital, owner in serious condition and worker in good condition – a third worker in the building was not hurt) ELECTROCUTION Sussex worker electrocuted after falling on live rail (a railway worker, 46, died after he fell onto a live rail at a train depot – he was electrocuted after he fell onto the rail in the early hours at 3.15am – the man suffered burns after he was exposed to an electrical current – paramedics worked to resuscitate the man but he was pronounced dead at the scene) HOTWORK Welding sparks cause shipping container fire (sparks from a welder are believed to have been the cause of a fire which destroyed a shipping container – worker was welding beside the shipping container when he noticed smoke coming from within) |
