MANLIFT fatality Worker dies as firm wins safety award (a fatal cherrypicker accident befell a lines company leaving one immigrant worker dead and another critically ill – on worker died and another was critically injured just hours after their employer won a supreme award for workplace health and safety – the two men were crushed by a toppled cherrypicker while doing lines work – they were part of a team of four workers replacing an overhead wire at a site when the cherrypicker they were using toppled down a bank – a 34-year-old man, who suffered broken legs and serious chest injuries, was flown by rescue helicopter to a Hospital in critical condition in intensive care – a paramedic pronounced a second man dead at the scene)

UPDATE, now a FATAL EXPLOSION 2 workers injured in Tenn. plant fire die (two employees have died from injuries received in a fire at a factory, bringing the total number of workers killed in accidents at the plant this year to four – investigators said Wednesday evening they have reached no definitive conclusions about the cause of three accidents at the facility this year)

RAILYARD Injured Metro-North Worker in Stable Condition (an employee was struck by a train in a rail yard – the mechanic, 55, who was injured has been working for the railroad for 31 years – a locomotive hit him while he and other workers were replacing engines in the rail yard – the accident happened at 5:55 p.m. – he was airlifted to a Medical Center shortly after the accident suffering internal injuries and broken ribs – he is currently in stable condition at the medical center)

UPDATE on CONVEYOR GUARDING INCIDENT Steel worker dragged through CD-size hole (a steel worker, 23, sustained horrific injuries but miraculously escaped with his life after being dragged through a gap no wider than a CD case – he was working for a structural steel fabricator as a plate welder when his overalls got caught on a high-powered conveyor system for moving heavy steel beams – his clothing snagged on the machine and he was forced though an opening just 125mm wide on the machine head, suffering horrific injuries that have caused lasting physical and psychological damage – he suffered a ruptured stomach and bowel, broke his back in two places, shattered his pelvis, and fractured both hips, his right arm and several ribs – the HSE investigation into the incident found there was no guarding in place to protect the worker, a serious safety failing for both his employer and the machine supplier – both the employer and machine manufacturer pleaded guilty to safety breaches and magistrates have remitted the case to Court for sentencing)

PALLET JACK Man Injured in Industrial Accident (a worker got his foot caught in between a pallet jack and a fork lift – a witness reported the man’s foot became stuck when his foot slipped off of the emergency brake – the victim was taken to a Hospital – the extent of his injury was unknown)

FALL Paper mill worker seriously injured in 12ft plunge at building site (the 41-year-old was off duty when he had the accident – he suffered serious head injuries after falling 12 feet at the demolition site at a paper mill – the employee was off duty and was not authorized to be in the area on Saturday at 10.30pm)

UPDATE Heinz fined after work accident at baked bean factory (company fined after work accident at a baked bean factory – a worker seriously injured their left hand in a metal working machine – he had been employed by the plant for more than twenty years, but was not given any refresher training on safe working practices in  that time)

CRANE FATALITY Factory worker dies of crush injuries (a worker, 57, was killed as he worked 14ft above ground at a steel factory – he is believed to have been working on a crane when the tragedy happened -he suffered serious crush injuries to his chest and lower body – an air ambulance, with an emergency doctor on board, rushed to the scene and firefighters used a cherry picker to reach the injured man and bring him to the ground – he was certified dead at about 3.15pm)

CONFINED SPACE FATALITY – ONE ENTRANT and ONE “WOULD BE RESCUER” – Oxygen Deficiency Iranian workers get suffocated in petrochemical unit (two workers got suffocated by polyethylene material in a petrochemical company – one worker got suffocated while cleaning one of the silo tanks which contains polyethylene and the other worker while attempting to save the first worker)

UPDATE on LOTO/GUARDING Crushed worker back in surgery (a maintenance worker, 48, whose right arm was crushed in a conveyor belt will have more surgery after two breaks to his arm  – he was working on the conveyor belt when his right arm was pulled into the machine and became trapped – he lay helpless for about 30 minutes before a fellow maintenance worker, who had been working in another area of the plant, found him and called the emergency services – he sustained two breaks to his arm – one break was between the hand and forearm, and the other between the upper arm and shoulder – he endured incredible pain to tell his rescuers how to unbolt the housing around the machinery that had trapped him)

MINE FATALITY Another coal miner in Colombia killed (a cave-in at an illegal coal mine killed another miner, marking the second mining death in the past two days, and officials now say 63 miners have been killed so far in 2011 despite an increase in government regulations and safety inspections)

MINE Three People Hurt in Accident on Mine Property in Kanawha County (three people were hurt in an accident on mine property in Kanawha County (it happened about 7:45 p.m. – an inloader operator hit a coal truck, causing it to rollover – the driver of the coal truck was trapped for a short time, he was flown to the hospital – his injuries are not considered life threatening – the operator of the inloader and a security guard who was trying to help, were also taken to the hospital – they are also expected to be okay)

REFINERY FIRE Linden refinery worker critical after being burned in fire (a worker, 41, at a Refinery was in critical condition with burns over about 20 percent of his body after a fire started in a furnace in a crude unit used to heat crude oil as part of the refining process – the worker suffered burns on his legs, face, hands and arms and was airlifted to the burn unit after medics determined he apparently inhaled superheated air through his nose – the unit heats crude oil as part of the refining process – the refinery fire department responded and extinguished the fire)

(an oil storage tank exploded in the port just metres from where a cruise ship was berthed – the explosion blew the lid off the tank and sent a fireball high into the air – the buckled steel lid lay on the quayside away from the tank, indicating the force of the explosion – two people who were carrying out welding work on the tank were injured in the blast – one of them, 40, suffered serious, extensive and life-threatening burns and is in critical condition – the other injured worker, 25, did not suffer serious injuries – some passengers on board the cruise ship also sustained injuries as a result of the explosion – a half-metre piece of steel from the tank landed on the ship and some passengers suffered minor burns and fractures – several photos @ link)

HOTWORK FIRE & PROPANE TANK EXPLOSION Meat packing plant saved from fire (firefighters were able to keep flames from destroying a meat packing plant – the fire started around 7:30 by a worker using a blow torch – he set the torch down near flammables – a small propane tank exploded and sent fire into an addition in the back of the old building as well as the attic – the USDA is on the scene and they will determine if the meat packing plant can re-open)

FATAL EXPLOSION @ Refinery

UPDATE on Incident – I have seen many news reports on this incident, some say two deaths and some say four, so I went to the Police Department that responded and got their official statement. Multi agency statement in relation to incident at Chevron Oil Refinery, Pembroke Dock (Paramedics have confirmed that four people lost their lives as a result of the incident, Officers are now in the processes of informing the next of kin and will support the families throughout this difficult time. Our thoughts are now with the family, friends and colleagues of the deceased.)

AMMONIA Evacuation at Thunder Creek Pork due to ammonia leak (after readings were taken at the site, deputy chief made a decision to evacuate the plant at 1:48 p.m. – staff were allowed back into the building at 2:30 p.m. – the leak itself had stopped but there was still product in the lines – firemen wore fully encapsulated class a hazmat suits and were in contact with Canutec, a national advisory service which provides technical information during a dangerous goods emergency)

Ammonia Leak Investigated at Sharonville Plant (Fire and Hazmat crews get the call for the smell of ammonia at a business – crews responded to a Freezer business on Sharon Road just before 9 a.m. – a worker opening the business for the day reported the ammonia smell – crews located the leak in a small area of the large building – they closed that area off and ventilated the rest of the building – no one needed to be evacuated from the business or surrounding area and no one was hurt)

AMMONIA Plant Evacuated After Ammonia Leak (an ammonia leak at a dairy caused a small fire {not sure if this is accurate reporting – I have contacted the FD to see if they can clarify} and led to the brief evacuation of the plant – no one was injured in the incidents – Fire Department was called to the dairy about 9:20 a.m. after anhydrous ammonia began leaking – a one-inch pipe that was leaking from a valve that had ruptured in a maintenance room – that rupture caused a small fire that was extinguished before the Fire Department arrived – the plant evacuated all 136 employees as a precaution – plant workers got to an emergency shut-off valve to stop the flow of the hazardous vapor and firefighters ventilated the area of the dairy where the leak occurred and to check air quality around the plant – there was no evacuation of homes and businesses near the dairy)

AMMONIA Fish Kill At Port Of Catoosa Caused By Ammonia Leak (a release of ammonia vapors from a barge loading at the Port has resulted in dead fish in the immediate vicinity of the incident – the vapor release came as workers on a towboat loading the commonly-used fertilizer chemical dropped a hose into channel – the fertilizer is harmful to fish that swim near the spill because oxygen is depleted from the water by the fertilizer – while conducting routine operational and safety procedures a company employee discovered evidence of an ammonia release from a barge)

CHLORINE Chlorine leak contained; evacuated residents return home (after an evacuation lasting nearly seven hours, an estimated 100 people were allowed to return to their homes – a chlorine tank at a water treatment plant developed a leak around 7:15 AM. – those who live closest to the plant say it was nearly impossible to breathe as that toxic gas spread – hazmat teams took turns throughout the morning donning their protective suits and heading into the building – a one ton chlorine cylinder had about 1,400 pounds in it – several people were taken to the hospital and treated, though there were no reports of serious injuries – not known what caused the leak)

FORKLIFT FATALITY Police investigating death of man killed in forklift accident at Random House (investigators are looking into the death of a  man, 44, after a forklift crash at Publishing House – he was killed when the forklift he was driving hit a shelving unit in the warehouse around 3 a.m. while moving pallets of books – employees and crews from the fire company worked to free the worker, but he was pinned between the shelf and the forklift – he had worked for the company for nearly two years)

UPDATE on CONFINED SPACE FATALITY Officials: Asphyxiation, not fall into silo, killed Mohawk man (authorities say the man who had fallen into a silo died of asphyxiation due to suffocation – the accident happened around 9 a.m. when a worker, 24, fell into a silo filled with cottonseed – he was working on a ladder inside the silo, when the tether – the device attaching him to the ladder – came undone, causing him to fall – emergency workers tried for hours to rescue him from the silo but by the time they got to him around 2 p.m., he had died from asphyxiation due to suffocation – he was a temporary worker assigned by a temp agency)

FRONTEND LOADER FATALITY Worker killed in machinery mishap (a construction worker was struck by a front-end loader and was taken to hospital with serious injuries – the man was hit in the head by the front end loader- he was later pronounced dead at a Hospital – the man, who appeared to be in his mid-30s, was part of a construction crew who had been working since March on sewer, water main and road reconstruction)

TRENCH Injured worker pulled from 10-metre-deep trench (firefighters put together an improvised rescue plan to lift an injured construction worker out of a 10-metre deep pit – he was stuck at the bottom of the hole in severe pain, after being struck in the chest and left arm by a five-metre-long section of concrete pipe that was being lowered into the trench at about 11 a.m. – workers were doing water and sewer line repair work – the injured man wasn’t trapped under the pipe, but he couldn’t get out of the pit because of his injuries – firefighters used one of the construction company’s pneumatic shovels to lower a rescue basket into the hole and then bring the man up to a waiting crew of paramedics – the victim was stuck in the trench for nearly an hour before he was brought to the surface)

TRENCH Worker buried waist deep in trench (a construction worker experienced a few scary moments after a trench he was digging collapsed – the man, who is 35 to 40 years old, was repairing drains at a private residence at the time of the accident – while he was digging the hole it collapsed on him – he was at one point waist deep – they used a vacuum truck to take the dirt out around him – the worker, an employee of a private draining and plumbing company, was able to lift himself out of the hole once the dirt was removed – he was checked out by paramedics at the scene and that no injuries have been reported)

SCISSOR LIFT & ELECTRICAL Construction Worker Suffers 11,000 volt Shock in Work Accident (a construction worker, 24, suffered serious burn injuries when he was electrocuted in a work accident – the steel erector was working on the construction of new poultry units, when the scissor lift he was operating came into contact with an overhead power cable – he suffered serious personal injury following the resulting eleven kilo volt shock, including burns to his hands and abdomen; he also suffered some internal muscle damage during the incident)

FLASH FIRE Two Firefighters, Worker Injured in New Orleans (LA) Fire (three people were injured, including two firefighters, after a worker cut a gas line to a home sparking a flash fire – around 8:15 a.m., firefighters were called to the home where three contractors were renovating a home when one worker cut the gas line from the supply side, causing a flash fire and burning his face and his upper body – he suffered first- and second-degree burns and was airlifted to a burn center)

SCAFFOLDING Woman Hurt After Wood Falls From LES Scaffold (a woman was rushed to a Hospital with head injuries after being hit by rotten wood falling from scaffolding – the incident happened just after 8:30 a.m. – a Buildings Department official said the wood had deteriorated and was blown off the structure by wind – a man who said he worked for the scaffolder said it had been up for more than three years – managers had ordered the scaffolding removed after the accident – FDNY was unable to say how serious the woman’s injuries are)

CRANE Crane tips over in Long Beach mini-mall, no one injured (a crane being used by a roofing company tipped over at a mini-mall – no one was injured – accident occurred about 7:30 a.m. as the company was preparing to re-roof the mall – when the crane operator tried to raise the first load of material, the crane tipped half-way over, coming to rest against the edge of the building – authorities called in a city tow truck to put the crane upright)

ELECTROCUTION Tree-service worker electrocuted in Ormond Beach (a tree service worker, 36, was electrocuted while working at a home – he was working with a crew when the accident occurred about 11:40 a.m. – he had scaled a large palm tree while wearing a harness and spikes and was cutting fronds when one of the fronds got caught on a nearby power line – he was attempting to pull the frond from the power line when he was shocked)

LAWNMOWER FATALITY Golf course worker crushed by mower (a veteran groundskeeper at a golf course died in a lawn mower accident when he veered off the cart path and tumbled down a hill – the mower then rolled on top of him, killing him – coroner says he died of blunt force trauma to the head – he kept the grounds at the course for more than 20 years)

FUELING FIRE Man Burned by Pressure Washing Truck (a pressure-washing worker, 47, was burned on both arms when his truck caught fire – he was hospitalized with second-degree burns on his arms after the power washing truck he was working with to clean a private dog park caught fire – he was taken to a Medical Center with severe burns on his arms and his after the fire around 9:30 a.m. – they were cleaning the dog park and one worker was pouring gas into the machine – the machine was off but the gas ignited)

CO2 leak sparks evacuation at Phoenix McDonald’s (a carbon dioxide leak forced the evacuation of a McDonald’s – the fumes caused a pregnant employee to pass out in a stairwell – her co-workers said she had been feeling lightheaded and dizzy, both of which can be indications of exposure to too much carbon dioxide – several other workers reportedly had been experiencing symptoms – the leak was coming from soda machines in the basement of the building – the pregnant woman was taken to a local hospital for treatment – other employees were treated at the scene)

CONFINED SPACE RESCUE Workers taken to hospital after being ‘overcome’ by fumes (the three men were cleaning toilets with hydrochloric acid at when they were ‘overcome’ by the fumes – they were taken to an Infirmary for treatment – they were using fluid with 10-15% hydrochloric acid to clean a toilet – but paramedics had to call Fire and Rescue Service just before 9am as they were unable to get into the confined space to reach the workers – firefighters used breathing equipment to take the men – aged 64, 57 and 23 – into the fresh air where they were treated before being taken to an Infirmary)
