INDUSTRIAL LOTO FATALITY Autopsy planned for worker killed at Cartersville plant (worker, 40, was cleaning a machine at a plastics plant when he got caught in a roller bar and was dragged through the machine – he was dead when police officers arrived at the scene) MINE FATALITIES 2nd deadly accident at a mine site in 2 days (there has been another deadly accident at a mine site, the second in two days – a fuel service contractor was killed on a coal mine haul road – driver was hauling a load of diesel fuel when the truck overturned in a single vehicle accident – it’s not known whether the death is mining-related or due to natural causes – other fatality was a worker was killed while reportedly conducting a pre-shift exam when he was struck by rock sloughing off the mine wall) FATAL FALL thru SKYLIGHT Worker Dies after Falling Through Skylight (worker fell 15’ through a roof skylight at a storage shed) CRUSHING FATALITY Longshoreman dies in accident at Port of Anchorage (a longshoreman died after he was pinned between two pieces of equipment – he had been loading military equipment into rail cars when he got “pinched” between two pieces of military equipment – he was on top of a rail car when the incident occurred and died immediately) EXPLOSION Two hurt in reported explosion at New Jersey’s Picatinny Arsenal (an explosionat a research facility in a military arsenal injured two civilian employees about 11 a.m. – one of the two explosives technicians who was hurt has been released from the hospital – one of the workers was flown to a nearby medical facility, while the other was taken by ambulance, both with injuries not considered to be life-threatening) FRACKING FATALITY Worker Dies Fracking (worker, 28, died at a hydraulic fracturing operation – he was killed in an industrial accident and was part of a team working a well-site when something went wrong just after midnight – he was pronounced dead at the scene – he was working in the nitrogen pumping division – apparently high-pressure iron failed, with the worker in the immediate vicinity at the time) UPDATE now LOTO FATALITY Autopsy Report Released In Wrigley Plant Worker Death (worker killed last month was crushed to death by the machine he was working on – autopsy report shows he died of blunt force trauma to his chest, and the compression left a deep gash along his torso – accident happened the night of February 3rd, while he was working on a destacker machine) COAL BUNKER FIRE 1 worker injured in coal fire at Burnsville power plant (a coal fire at a power plant injured at least one person and fire crews are working to put out the blaze – the blaze in the generating station was reported around 7:30 a.m., in the Unit 4 coal bunker – one employee was injured and taken to the hospital, but the severity of their injuries was not noted – about 50 employees were evacuated, but have since returned to the plant, which is still operating – cause of the fire isn’t known) FIRE Propane tank explosions suspected in fire on Milwaukee’s south side (a two-alarm fire and propane tank explosions ripped through an industrial building at 7:52 a.m. – no injuries were reported – fire was concentrated in a separate building where propane and other chemicals were stored and away from the main processing facility – cause remains under investigation by the Fire and Police departments) HOTWORK Former Grain Mill owner cited for fire code violations (a cutting and welding torch sparked the flames – no ‘hot work’ permit was obtained – 30 fire departments called to fight fire at the grain mill – Owner of the Grain Mill is cited for several fire code violations – cited for a total of five violations, including failure to maintain fire extinguishers, failure to obtain a hot works permit and failure to possess a permit for LP-gas – a cutting and welding torch (hot works) that was being used to remove machinery in the building sparked the March 6th fire inside the grain mill – March 26th is the one-year anniversary of the Boston fire that killed two firefighters on Beacon Street. That fire was caused by cutting and welding that was being done on a building next door. The work there was also being done without a permit. COMBUSTIBLE DUST Dust Collector Causes Fire, Explosion at Wrigley Plant (a fire and explosion in a dust collection system – flames were seen coming through the roof, and employees had to be evacuated from the plant – starch used to make candy products was reportedly burning – no injuries were reported and most of the fire was extinguished by 7:30 pm.) EXPLOSION Water tank explosion blamed for Texas plant accident, 3 hurt (investigators blame a water tank explosion for injuring three workers at a corn flour plant – authorities are trying to determine what caused the tank to explode – three workers were transported to a hospital – further details on their conditions weren’t immediately available) CHEMICAL REACTION EXPLOSION @ PAPER MILL Federal authorities investigating chemical leak, explosion at Menasha paper mill (authorities are investigating a chemical leak and explosion at a paper mill that forced people out of their homes – no one was injured in the blast but dozens of homes south of the plant were evacuated shortly after 8 a.m. – an “organic polymer” was being unloaded at the plant when it leaked and came into contact with another chemical, sodium hypochlorite – a more concentrated form of household bleach – both are used in cleaning water from the paper making process – the two reacted, creating heat and an explosion that blew off a portion of a roof of the plant, as by design, where the chemicals are located – evacuation order was lifted by early afternoon) AMMONIA Ammonia leak at Alliance Group Lorneville Plant near Invercargill (two men were taken to hospital suffering the effects of ammonia inhalation, following a leak – they were treated and discharged – the leak had been isolated by about 8:20pm and a decontamination corridor was set up at the scene – fire service were alerted to the leak by a gas alarm which was triggered at 6:25pm, and 111 calls stated there was a definite leak) AMMONIA TRUCK UNLOADING DuPont workers sprayed with liquid ammonia taken to hospital (two employees have been transported to the hospital after a hose sprayed them with liquid ammonia around 7:30 a.m. – two men, 36 and 45, were sprayed with ammonia and were taken to the hospital – their injuries are not severe – the employees were unloading a tanker when a hose came loose and squirted them both with liquid ammonia – the men’s personal protective equipment prevented further exposure and more serious injuries) CONSTRUCTION Cupertino: Construction worker injured at Apple job site (worker was injured in an accident at the construction site around 3:02 p.m. – worker had been trapped beneath a large building block and was transported to an area hospital) FATALITY Construction accident at post-Katrina project kills worker (a construction worker has died after an accident on a structure to block storm surge from a canal that breached during Hurricane Katrina – he died from injuries received while removing forms from a concrete wall) FATALITY Construction Worker Killed in Worksite Accident (a work site accident claimed the life of a man employed by a local construction company – he was running a machine and was thrown from it – he was found unresponsive by a fellow employee at approximately 11:45 a.m. – emergency response personnel pronounced him dead shortly after arriving on scene) WORKZONE FATALITY Update: Tolland Public Works Employee Killed Identified (a Public Works employee was killed in the accident while fixing pot holes – Reports were that the employee was hit by a truck being operated by another employee) Worker injured while repairing bridge on I-95 in Florence (a subcontractor doing work for the I-95 bridge project was injured and remains in stable condition at a hospital – he fell into a portion of the bridge that was taken out for repairs) CRANE Source: Crane may not have lifted load properly, causing accident on 520 bridge that injured 8 (an investigation was launched into the construction accident at the bridge that sent an overhead sign crashing down on a Metro bus, injuring eight people – a crane dropped a large steep pipe that hit an overhead highway sign, which then crashed down on the bus – a source with knowledge of the accident told Q13 FOX News the crane was not plumb, meaning it wasn`t sitting directly above the load when it lifted the load off the flatbed truck, causing the pipe to swing and hit the sign) FATAL FALL from BRIDGE Man, 34, killed in Highway 520 project fall experienced worker (worker, 34, was fatally injured in a fall on the Highway 520 project – he was a carpenter who had worked seven years with prime contractor – he fell off the west edge of a bridge deck, landing 60 to 70 feet below on a hard surface at around 4:30 p.m.) WORKZONE ODOT Worker Struck by Vehicle, Injured While Filling Potholes (a seven-year DOT employee remains in a Columbus hospital after he was hit by a driver while working to patch potholes on I-70 – two other workers were briefly hospitalized for injuries after they leapt over a guardrail to avoid the driver – he suffered a broken leg and is continuing to be evaluated – Police at the scene said he will likely be charged with failure to control his vehicle – it’s not clear if texting or distracted driving had anything to do with the crash) CITIZEN FATALITY from CONSTRUCTION SITE Plywood blown from condo conversion fatally strikes woman, 37, walking on Manhattan sidewalk (women, 37, was killed after a piece of plywood hit her and smashed her head against a wall – she was chatting on her phone on W. 12th St. when she was fatally walloped by a 4-foot-by-8-foot wooden sheet torn loose from a fence by wind gusts of nearly 40 mph) FATALITY Construction worker killed in glass accident at Albufeira villa (construction worker has been killed after a large sheet of glass fell on him – the man was killed immediately in the accident around 7.30pm) CRANE Worker breaks legs during Barrow Island cyclone prep (worker suffered severe injuries during cyclone preparations – a crushing injury was caused by a crane leading to two broken legs and the possibility of amputation – unions have suggested the injury may be linked to high winds) FATAL FALL from BEAM LR industrial worker falls from steel beam, pronounced dead (worker is dead after falling from 11 feet from a beam – he was working approximately 11 feet in the air on a beam while placing bolts into steel – it was then that it is believed he lost his footing and fell, striking his head on the concrete slab below – he was transported to a hospital where he was pronounced dead upon arrival) BACKHOE FATALITY Police Investigating Death at Binghamton Contracting Company (construction worker, 42, was killed backing an excavator out of a building – it’s not yet clear exactly what happened, but investigators say no other vehicles or workers were involved – first responders arrived on scene shortly after 7 a.m. and found him dead) FALL from ROOF with PFAS Worker falls from house under construction (a worker was taken to a hospital after falling from the upper level of a house under construction – extent of his injuries wasn’t immediately known – worker was wearing a safety harness – worker struck a wooden support on the exterior of the house and dislodged it in the fall – accident happened at around 2 p.m.) ELECTROCUTION Columbiana man electrocuted in Struthers (worker, 58, was electrocuted while working on the roof of a building when he touched a 40,000 volt line as he and his son were installing a new roof on a clothing store – he died instantly from electrical injuries) ELECTROCUTION Worker electrocuted at RAK construction site (electrician died died after he was electrocuted while trying to connect power to a construction site – worker was rushed to hospital in critical condition and died just after he was admitted for treatment) ELECTROCUTION – ARC JUMPED from POWERLINE to EQUIPMENT Man electrocuted near Bloodvein, Manitoba (worker, 37, died after being electrocuted just before 11 p.m. while he was operating a piece of heavy duty equipment as part of the construction of a new road – there was some kind of arcing from a power line – machinery didn’t actually come into contact with the wires but the arc was enough to cause the equipment to stall – he got out to see why and when he went to get back into the machine, he grabbed the door handle and was electrocuted) VIDEO: Scaffolding comes crashing down after high winds in Portland FALL from SCAFFOLDING Construction worker seriously injured in fall from scaffolding at site of new synagogue in Hackney (construction worker, in his 30s, was seriously injured after falling from scaffolding – suffered chest injuries in the accident) FALL into TRENCH Man airlifted after fall into trench (worker, 29, sustained non-life-threatening injuries in a construction accident around noon – he reportedly lost his footing and fell about 15 to 20 feet into a 20-25-foot trench being dug for a sewer line – he was injured during the fall – he was not trapped in the trench and there was no collapse of earth) MISCELLANEOUS OSHA Investigating Accident Involving Garbage Collection Driver (employee, 18, suffered potentially critical injuries after being struck by a vehicle while picking up garbage in the neighborhood – Medics took the worker to the hospital in critical condition) TRACTOR Worker taken to hospital after getting pinned under tractor (worker at a fruit farm has been taken to the hospital after rolling a tractor – she was pinned under the tractor when crews arrived, but was conscious and talking to rescue crew members – no word yet on the extent of her injuries – sheriff’s office says her injuries are not expected to be life-threatening) HOTWORK EXPLOSION cutting on DRUM Worker severely injured in explosion at Cadott business (worker, 24, was severely injured in an explosion – full extent of the man’s injuries isn’t known – he was attempting to cut open a 55-gallon drum with a blow torch when the drum exploded – drum was not full, but contained some oil-like substance) |
