cut the marble from its quarry into sections for counter tops – the estimated weight was 7,000 pounds) CRUSHING FATALITY from COLLAPSED RACKING Victim in fatal tile store mishap identified (worker, 53, died after being trapped under the debris of tiles and cement bags after a rack collapsed in the warehouse around 10:15 a.m. – he died at the scene – he was employed by a company specializing in warehouse rack inspections and repairs) FORKLIFT vs. PEDESTRIAN Worker injured in forklift accident in Elk Grove remains hospitalized (worker, 60, remained hospitalized after being seriously injured when was trapped underneath a forklift – he had stepped outside of an office door near a loading dock when the forklift rolled over the lower part of his body – forklift was headed into a large delivery truck before the man was struck – the driver, another employee of the frozen food manufacturer, was not injured – other co-workers unsuccessfully tried to free the man who was lying on his back with his two legs, from his knees to his feet, trapped underneath the forklift – two paramedics and an EMT arrived and used hi-lift jacks and cribbing, or blocks of wood, to raise the forklift about 3 inches and slide the man out from underneath – his injuries aren’t believed to be life threatening, but “they are life-altering”) OIL RIG FATALITY Coroner says oil rig worker died from trauma in Johnson County (coroner says a man working on an oil rig died after an accident led to blunt force trauma – coroner said he cannot determine what caused the injuries) GUARDING FATALITY Police identify worker killed in Phoenix industrial accident as Richard Johnson (worker, 31, clothes got entangled in a metal fabrication machine in a fabrication plant – he was working his last shift at the company and was about to start a new job in Vegas) YARDER – HEAVY EQUIPMENT Worker freed from logging equipment following accident (worker from a logging company was injured when the equipment he was operating fell over, trapping him inside – it took firefighters from multiple agencies about 40 minutes to extricate the man – described his injuries as “potentially serious” – worker was using a log yarder — a piece of equipment used to move logs from the area where they are cut to a landing where they can be loaded onto trucks – when firefighters arrived, the yarder was partially on its side, resting on top of another piece of equipment) MINE FATALITY Contract worker killed at northeast Nevada gold mine (officials are investigating the death of a contract worker in an accident at an underground gold mine – employee was killed in the accident in a vent shaft at the mine – no other details have been released) UNLOADING TO WRONG TANK Delivery driver in burn unit after Holly Hill chemical accident (a truck driver critically injured when the chemicals he delivered reacted and spewed noxious vapors was in a burn trauma intensive care unit – authorities began trying to find out what led up to the driver pumping 1,000 gallons of bleach into each of two tanks containing sulfuric acid – the dangerous mixture affected several residents living near the business after chlorine fumes permeated the air – eight people at the scene, including the driver, ended up being hospitalized – County officials warned 279 residences in a half-mile radius about the noxious vapor in the refinery explosion (three employees were hurt in an explosion at a refinery – one worker’s body was 75 percent burned while another two suffered less serious burns in the blast – no other details) FATAL FIRE @ CONCRETE PLANT Worker Burned In Farmington Hills Concrete Plant Fire Dies (worker, 37, has died of severe burns from a fire at a concrete plant where he was working – he was burned on his arms, hands and face when he was trapped in the Jan. 8 fire inside a large storage tower – six others who were injured in the fire have since recovered – he was part of a crew that was working to restore the tower) AERIAL LIFT Worker Injured At Iowa Food Plant (worker was seriously injured when he was on a lift extended upward when he became pinned between a pipe and the lift itself around 10 a.m. – another worker called 911 after lowering the lift – the employee was a maintenance worker taken to a hospital with a “serious, but what appears to be, non-life threatening injury”) FATAL BOILER EXPLOSION Four killed, four injured in boiler explosion (four people were killed and four others were injured when a boiler at a factory exploded at a dye factory where over three dozen workers were had just come in – a factory worker, told newsmen that it exploded due to a high steam pressure – no boiler engineer was present at the time) HOTWORK FIRE – 30 MINUTE FIREWATCH RULE Morning fire destroys Unionville business (a business has been destroyed following a hotwork fire – the owner was welding in the garage, then took a break – when he came back a short time later, the garage wall was on fire – building was completely destroyed) FIRE in OIL TREATER Two men burned in oil treater fire (two men, 27 and 30, were hospitalized with severe burns after an oil treater caught fire – both men were inside the structure housing the oil treater at the time it caught fire – one worker was reported to have jumped into a snow bank to extinguish the flames, and the other was put out by three individuals that were nearby,” according to a statement released by Mountrail Sheriff’s Department – one received burns on the lower half of his body while the other had extensive burns to his upper body, including his face and head – both men were reported to be wearing fire-retardant clothing at the time – no explosion was reported during the time of the fire inside the treater) Refinery Gas leaked) NITROGEN ASPHYXIATION LG Factory Accident: Nitrogen Leak At Display Manufacturing Facility Kills 2, Injures 4 (at least two people were killed and four others were injured after a nitrogen leak at a plant – of the four staff and contractors who were injured by the leak, one was in a critical condition – one of the workers died at the scene of the accident while another died while he was being taken to a hospital – the leak occurred when the workers and the subcontractor were carrying out routine maintenance work – the nitrogen gas cylinder was open by mistake, causing the accident, the report said, citing firefighters – Nitrogen is used to wash the display panels when they are being manufactured) PROPANE CYLINDER EXPLOSION Propane tank explodes at Sioux City scrap company (no one was hurt when a propane tank exploded at a scrap company at about 10:30 a.m. – facility processes scrap steel and the tank was inadvertently crushed – it is unclear how the tank was included in safety – leak was immediately contained and the air quality was checked before workers returned to the building) CHLORINE Crews On Scene Of Chlorine Leak At Berea Water Plant (a chlorine leak at a CRANE FATALITY Construction worker crushed in crane accident in Edwardsville (worker, 31, was killed in an accident involving a crane at a construction site – he was working on a large Grove HL150T crane at a building site about 11 a.m. when he was crushed by the weight of the crane’s boom – he was pronounced dead at the scene) RUNOVER FATALITY Incident at south Reno construction site claims a life (a worker was killed at a construction site around 9:00am when he was run over and killed by a loader truck) ELECTRICAL and FALL from LADDER Roofer In Critical Condition After Construction Accident In Newington (construction worker is in critical condition after he suffered an electric shock – the man, who works as a roofer, made contact with utility wires while scaling a ladder – he suffered a shock and fell approximately 20 feet to the ground below) Man Injured in Construction Accident (a construction worker is fighting for his life after being hit by a tree while clearing land – he had no pulse until the ambulance arrived and was transported to a Medical Center in critical condition with head trauma) HOTWORK WELDING KIT CAUSES FIRE AT AMAZON CONSTRUCTION SITE (a data site still under construction caught fire – welding equipment has been blamed for the fire – estimates put the damage at up to $100,000 – no people were hurt – fire was started when welding equipment ignited building materials which had been stored on the roof) TRENCH FATALITY Two workers killed at Yermarus Thermal Power Station site (two workers, 25 and 53, were killed on the spot and two others were injured after the walls of a trench collapsed on them at about 10.30 a.m. – the labourers were constructing a strong supportive wall for a 2.7-metre-wide iron pipe that was laid in the trench for supplying hot water from the main plant to cooling tower no. 2) TRENCHING Teenager crushed by digger bucket (a teenager, 16, working through his school holidays has serious leg injuries after a digger bucket disconnected and crushed him – the teenager remained in a Hospital for at least two days – the teenager was doing drainage work in a trench at a subdivision when the bucket “came off the digger and has fallen into the trench”) SCAFFOLDING COLLAPSE Scaffold collapses in downtown Lincoln (nobody was seriously injured when a scaffold collapsed at a construction site a little after 3 p.m. at the six-story site of a future student housing complex – it ollapsed on two vehicles, one a silver sedan and the other a pickup – a woman in the sedan complained of neck pain and was taken to a hospital – there were people in both vehicles, but they were already out of their vehicles by the time authorities arrived – Construction workers said the scaffolding was tied down, but wind had caught some of the plastic tarps attached to the scaffolding, causing it to catch like a sail and fall over) MISCELLANEOUS AMPUTATION Shop worker’s hand amputated after it got stuck in MEAT MINCER in horrific accident (a part time supermarket worker’s hand, 37, had to be amputated after it got stuck in a faulty meat slicer – the same shop began using the machine an hour after the gruesome accident – having washed it down with “Fairy Liquid” and hot water – he lost all his fingers and attempts to rebuild his hand failed – the machine should have had a guard to stop users putting their hand in a hole, but it had snapped and broken off about a month before the accident – staff had been using a plunger to insert meat since the breakage) CHAINSAW FATALITY Llandyrnog farm worker who died in tragic chainsaw
