INDUSTRIAL MINE FATALITY One killed, two injured in W.Va. mine roof collapse (miner, 45, was killed in a mine collapse that left two other miners injured -he died when the roof and ribs of the mine came down about 9 p.m. – the cause of the collapse was unclear – two other miners were injured in the accident – one remained in the hospital as of Monday; the other was released – the injured workers were pulled out by other mine workers) CRUSHING FATALITY York Coroner’s Office: Worker died during industrial accident (a long time employee, 63, at a wire company died after an industrial accident at 8:50 p.m. – he died of asphyxiation when a spool of wire that weighed several thousand pounds fell on his chest) PARKING LOT FATALITY Worker Leaving Area Power Plant Struck, Killed In Parking Lot (power plant worker, 55, was struck by a vehicle in the plant’s parking lot died at a local hospital – he was walking to his car in the lot when a vehicle driven by another employee struck him) MINE FATALITY BHP halts work at Blackwater coal mine after worker killed (a worker at the mine was killed in a truck accident involved a raptor transporter vehicle) LOTO/GUARDING FATALITY Industrial accident claims life in Kingsbury (worker, 41, died in an industrial accident after his sweatshirt became wrapped around a turning shaft and he was pulled off the ground – he was discovered by a co-worker before 6:30 a.m. – he was found hanging three feet off the ground and died from a broken neck along with asphyxiation – the machine lifts bags into the air and carries them to another side of the plant as part of the packaging process – several employees were involved in turning off the machine and cutting him free) LOTO FATALITY NYCHA worker killed in building accident (a maintenance employee, 31, died after a faulty hoist fell on her) FATAL EXPLOSION 3 Workers Killed in West Texas Oil Field Rig Explosion (an oil field rig exploded killing three workers – a pulling unit crew was at the site when the explosion happened – one other worker at the site was not hurt in the explosion – the workers who died were employed by an independent contractor) FATAL EXPLOSION Brent Industries explosion: Name of worker killed in blast released (worker, 35, was killed in a fiery plant explosion around 3 p.m. – blast sent several others to the hospital with injuries – as many as six workers sustained chemical burns in accident – cause of the explosion remains under investigation) LOTO/GUARDING American Apparel worker injured when hand catches in machinery at Hawthorne facility (worker in the dyeing plant of a clothing manufacturer was injured when one of his hands got stuck in a machine at 5:18 a.m. – an Urban Search and Rescue team was summoned to get the unidentified man’s arm out of the “large machine” – extrication was completed by 6:36 a.m. and he was hospitalized in stable condition) PALLET Hatfield Meats worker injured by falling hams (worker was injured after a pallet of ham fell on his head – extent of the man’s injuries is unclear but the man was conscious and alert when paramedics arrived) VEHICLE FATALITY Worker killed when truck rolls down embankment in Burbank (driver of a rock mover truck was killed when the vehicle rolled down an 80-foot embankment in a landfill – a private contractor was performing roadway services about 2:45 p.m. when the accident occurred – truck rolled down the embankment for an undetermined reason, and the driver was ejected from the truck) FATAL EXPLOSION 1 killed, 1 injured in Lea County oil field explosion (one oil field worker has been killed and another has been seriously injured in an oil field explosion around 3:30 p.m. – worker was killed in the explosion and another worker sustained injuries to his chest, torso, and right leg and was air lifted for treatment – the workers were loading oil field related materials into perforated pipe which was being installed into the drilling pipe when an explosion occurred – victims were injured by metal and shrapnel, but there was no fire associated with the explosion) WELL SITE FATALITY Calfrac employee killed in incident at Progress Energy well site (worker lost his life at a well site – he was conducting completion operations – the incident occurred when the company was conducting a fracking operation) FALL thru SKYLIGHT Worker falls through skylight at Auburn business (a worker, 48, who was trying to clear snow and ice from the roof of a business fell through a snow-covered skylight – he was one of several people clearing the roof of a trucking company about 9 a.m. – he fell about 25 feet before landing on boxes that broke the impact of his fall – he rolled off the boxes before ending up on a concrete floor – injuries described as not life-threatening) Enwin worker taken to hospital with apparent broken ankle (utilities worker was taken to hospital with an apparent broken ankle as a result of being struck at low speed by a truck – the worker was removing a bin from a stationary truck while doing work at around 3 p.m.) Worker suffers burns at Oakland food plant (worker, 24, was hurt when a generator malfunctioned – he had been performing maintenance on a generator when some type of explosion occurred – he was splashed with hot liquid that had come from the generator, burning his face and chest) FIRE Worker injured in car parts factory blaze in Bradford (worker suffered burns in a fire at a car parts manufacturer – some 60 staff were evacuated from a foundry after a tank full of oil, which is used in the cooling and hardening of smelted pins, caught fire at about 9.30am – flames then caused part of the building’s extraction fan system to catch alight and the fire spread into a roof area) BOILER EXPLOSION Factory boiler explodes, 11 hurt, 3 seriously (eleven workers were injured, three seriously, when a boiler exploded at an animal feed factory – boiler used for preparing animal feed exploded on the fourth floor of the six-storey factory – eleven workers sustained burns, three seriously – explosion damaged large areas of the fourth and fifth floors of the building – damage was initially estimated at 150 million baht) ELECTROCUTION Worker electrocuted on Merritt Island (subcontractor was electrocuted around 2:50 p.m. – worker was on scene replacing electrical poles and that the victim was in a “bucket” working when he was electrocuted – when Fire Rescue workers had arrived on scene, co-workers had removed him from the bucket and had attempted to resuscitate him – the victim was pronounced dead on scene) EXPLOSION Small blast in St. Paul waste treatment plant sends flames through the roof (explosion sparked a fire at a waste treatment plant leaving one person slightly injured – the blast was caused when a contract employee cut into odor-fighting equipment as it was being demolished – worker was taken by a fellow contract employee to a Hospital for treatment of minor injuries) FIRE 2 hurt in St. Clair County welding supplies shop fire (two people have been injured in a fire at a welding supplies business – one person suffered minor burns – the other had facial burns) FIRE Exxon says fire at Torrance, Calif., refinery quickly extinguished (fire brigade had quickly extinguished a blaze at the 155,000-barrel-a-day refinery – there were no injuries in the second blaze – no word where in the refinery the fire broke out – the fire did not affect the refinery’s current operations) WATER TANK EXPLOSION Water tank explosion blamed for Texas Panhandle plant accident, 3 hurt (investigators blame a water tank explosion for injuring three workers at a corn flour plant – authorities are trying to determine what caused the tank to explode – three workers were transported to a hospital) FLASH FIRE Man Burned in Yerington Oil Plant Accident (Fire Department is investigating a flash explosion that injured a man at an oil plant around 8:15 a.m. – he was filling a tank on a commercial delivery tanker when fumes ignited causing the flash fire – two people on scene gave the victim first-aid and extinguished the flames before responders arrived – he suffered burns to his face and hands and was transported to a Medical Center by ambulance, and then flown to a hospital for advanced medical treatment – no one else was hurt in the incident) EXPLOSION No injuries in Kalamazoo Co. plant explosion (explosion and fire at a plant just after 12 p.m. – nobody was injured in the incident – damages to the property are unknown at this time) FATAL EXPLOSION @ ASPHALT PLANT Explosion in northern Mexico asphalt plant kills 3 (an explosion inside an asphalt plant has killed three workers and gravely injured three more – the blast occurred about 7 p.m. inside a reactor where asphalt is mixed with rubber – blast in turn caused an asphalt tank to burst, killing three people standing nearby – while they had not yet determined the cause, officials said that the reactor somehow overheated) DUST EXPLOSION Fire reported after explosion at plant in Grandview (an explosion around a dust collector at about 4:30 a.m. severely damaged a wall in the building – fire crews reported a building explosion with fire and three injured employees – fire was brought under control in about 20 minutes – three people suffered minor injuries) FORKLIFT FATALITY Person killed in forklift accident in Lower Hutt (worker was killed in an incident involving a forklift at a food importing business – it appeared nobody saw what happened to him) CONSTRUCTION FALL Construction worker air-lifted after school construction accident (a construction worker was air-lifted to an area hospital after a work-related fall at school currently under construction – worker took a fall of about 14 feet and was air-lifted to a Medical Center by Life Star) FALL w/ IMPALEMENT OSHA investigating accident that impaled construction worker (worker, 61, was impaled on a metal rod after he fell 15 feet at a construction site – a Life Flight helicopter ambulance flew the victim to a Hospital Trauma Center for treatment after crews spent more than one hour extricating him) Construction worker killed in glass accident (construction worker has been killed at a villa after a large sheet of glass fell on him) EXCAVATOR Worker Injured At Gastar Well Pad in New Martinsville (worker was injured in an accident involving an excavator at a well pad site – preliminary reports indicate the man’s leg was injured – no other details available) FATALITY Construction Worker Killed in Work Accident (worker was killed in a construction accident when a concrete beam fell on him during the renovation of one of the apartments – attempts to save his life failed, and he died due to the severity of the injuries) CRANE FATALITY Crane accident in Dubai kills 1 (crane workers allegedly dropped box on the floor unaware they were people inside – one worker died and at least two others were injured – the accident occurred when a crane was used to transport workers up to the seventh floor at the site – crane workers allegedly dropped the box on the floor, unaware that they were two people inside) FATAL FALL from SCAFFOLDING Mainland worker falls to death on Galaxy Macau Phase II site (worker, 55, accidentally fell to his death from scaffolding around 4:05 pm from a height at 12 metres – investigation found that the worker’s safety belt had not been secured properly at the time the accident happened when he was working from scaffolding) HOTWORK Construction Crew’s Torch Sparks Fire At Santa Cruz Hotel (a construction crew using a torch caused a fire at a hotel – arriving crews found a “moderate” amount of smoke coming from the roof of a two-story building – firefighters reached the roof of the building, found the source of the blaze was at a corner under construction by a roofing company and extinguished the fire – it appears fire started by accident and was caused by a roofing crew using a torch to make repairs) AERIAL LIFT FATALITY Painter killed in Northland scissor lift accident (painter died when a scissor lift that he was working fell over at about 1:30 p.m. – two painters were working on the lift at a height of about 25 feet – as the lift tipped over, one painter fell and landed on a truck – a portion of the scissor lift then fell on top of him – the second painter was uninjured) MISCELLANEOUS FIRE 4 injured in 2-alarm fire in commercial Newark building (a warehouse worker is recovering after miraculously being pulled from the rubble of a huge fire and explosion – investigators still don’t know what sparked the explosion that led to a fire that burned through a commercial building – three people were injured and taken to the hospital – firefighters also searched for three hours for a missing person before finally locating him – a ceiling and wall had collapsed on top of him and firefighters had to pull him from the rubble – worker is expected to survive) TREE CUTTING FATALITY Delta worker killed on the job by falling tree (worker has died on the job after being crushed by a tree – he was clearing trees when one fell on him at around 9 a.m. – he was pronounced dead at the scene) MRI MACHINE EXPLOSION 1 Worker Still Hospitalized After Paramus Animal Hospital Explosion (two of the three workers injured when an MRI machine exploded at an animal hospital last week has been released from the hospital – the third person hurt remained hospitalized in critical but stable condition – RI machine exploded inside the hospital, prompting the evacuation of 60 animals – three contractors were making repairs on the MRI machine at the time – explosion caused a ceiling in a small room to collapse but none of the animals were hurt because they were in a different part of the building – still trying to determine what caused the explosion) Worker critically hurt in diesel shop incident (worker, 20, remained in critical condition two days
