RUNAWAY TRUCK FATALITY State worker killed in port accident remembered as dedicated colleague (a state worker, 46, was killed while trying to stop an out-of-control truck at the Marine Terminal – he noticed his truck rolling backward shortly after he had parked it about 10:15 a.m. near Berth 14 – he attempted to get into the vehicle and to stop the vehicle from rolling, and in the process he was struck and killed by the truck – he was pronounced dead at the scene – he had worked for the environmental service for more than seven years)
FALL from LADDER Man rescued from maintenance shaft in Houston area (a worker has been rescued after falling about 20 feet into a maintenance shaft at an industrial site – the man fell off a ladder – a paramedic was lowered into the hole to rescue the injured man, who was safely removed and appeared to have suffered a shoulder or arm injury)
CONFINED SPACE FATALITY – HYDROGEN SULFIDE INJURED SARAS REFINERY WORKER REGAINS CONSCIOUSNESS (an employee, 23, of a contractor regained consciousness in intensive care – he woke up in the intensive care unit following the accident that cost the life of his colleague, 23 – the accident happened while they were carrying out maintenance work on a column at the Dea3 plant in the refinery – the workers knocked over by a jet of hydrogen sulphide – a third worker was injured after falling down a stairway)
GUARDING FATALITY Man dies after falling into pasta machine (a man, 26, is dead after he fell into a machine while working at a pasta factory – he was the future son-in-law of the owner of the business – the man’s shirt became caught in a pasta-making machine, pulling him in head-first – paramedics were brought in to deal with any of the staff who were still in shock after the accident – the owner{apos}s brother said his sister was admitted to hospital for shock)
MINE FATALITIES Six Miners Killed after Bucket Plunge (rescuers retrieved the bodies of six workers after they were buried by rocks from a bucket that plunged down a shaft in a coal mine – initial investigations indicated that the six workers in the shaft were immediately killed after the rope lifting the bucket broke, showering six tonnes of rocks on them after a 500-meter fall in the mine – one worker, who was trapped in a pit near the ground, was pulled out alive two hours after the accident)
FORKLIFT FATALITY Accident claims Mansfield man{apos}s life (a worker, 28, died in an accident at a recycling plant – he was moving bales of paper with a forklift when the load became top-heavy and caused the forklift to overturn – he jumped off the forklift but couldn{apos}t get out of the way in time – the forklift fell on him – he was a temporary worker at the plant)
EXPLOSION Worker hospitalized after accident at Maybrook firm (an employee suffered severe burns when one of the manufacturer’s furnaces blew up – the worker was loading molten metal into the furnace around 12:30 a.m. when the furnace exploded – the victim’s injuries were unknown at press time)
FATAL EXPLOSION Blasts & blaze kill two in Rourkela gas plant (at least two persons were killed and another was missing following a major blaze triggered by an accidental explosion in a gas plant – one more person was admitted to a Hospital in a critical condition – one of the deceased was identified, while the second one{apos}s body was charred beyond recognition)
AMMONIA Ammonia leak reported at elevator in Horace (residents were forced to shelter in their homes after an anhydrous ammonia leak – the leak happened at an Elevator just before 10 a.m. – the leak was contained around 10:40 a.m. – emergency responders had to transfer 30,000 gallons of anhydrous ammonia from the leaking tank to a good tank to get it under control – a small ammonia cloud was visible but dissipated after a short while)
EXPLOSION Accident At Kilgore Plant Sends Three To Hospital (three workers were sent to a Hospital after an accident at their workplace – the workers were mixing chemicals when the accident happened – two workers were burned, another was knocked unconscious – all were expected to be okay – the three workers were taken to the hospital by staff members, rather than an ambulance – this is the same facility where an explosion hurt six workers back in September – the plant was cited for 14 safety violations and fined $348,000 – over the last 25 years, five workers have died on the job)
Offshore oil platform housing over 700 workers sinks in Gulf of Mexico (an offshore oil platform housing 713 workers and containing 2,075 barrels of diesel and 82 barrels of kerosene, began sinking into the Gulf of Mexico – the platform was operated by Mexican state-owned Pemex – no injuries have been reported – the trouble began when water pumps failed and the system supporting the platform became flooded)

Driller dies when sinkhole opens beneath him (an experienced well driller, 52, died after a sinkhole opened beneath him and his truck fell in on top of him – a second-generation well driller was pulled from the sinkhole – the accident occurred on privately owned property and was discovered by his partner around 11:30 p.m.)
FATAL EXPLOSION Fireworks explosion kills five workers in Hawaii (five workers were killed after an explosion ripped through an underground munitions storage bunker – the facility was stocked with powerful display fireworks, which continued to detonate and feed a fire – the explosion scorched trees and brush 40 feet away from the entrance – many of the fireworks were the large caliber aerial type used in public displays, which had been brought into the United States illegally – officials have not specified what initially sparked the blast)

TRUCK FATALITY Worker dies in industrial accident (a serious industrial accident at a construction site resulted in the death of one site employee, 47 – he was in the unloading area behind Gate 6 at the construction site, directing arriving trucks for unloading – the worker, who had worked for about two years and held a valid blue card, and the driver of one of the trucks in the queue didn’t communicate sufficiently – the driver allegedly geared the vehicle to reverse pinning the employee against the front of another truck at its rear around 8am – he was stuck between the two trucks and was certified dead at the hospital)
WORKZONE Injured worker identified (a worker, 20, is reported in serious but stable condition after he was hit by a car in the driveway of a home – he was working in the driveway when a car turned into the driveway and hit him with the left front side of her vehicle – he was air-lifted to a Hospital with reported head injuries)
FATAL FALL from RADIO TOWER 2 workers fall from radio tower and die (two people are dead after an accident at a radio tower – the two who died are construction workers who fell from the tower, which was under construction – there were five people working on the tower, but no other injuries were reported)

FATAL FALL from FORKLIFT PALLET Store worker found hanging on forklift (authorities are investigating the death of a worker, 61, who was found hanging upside down from a forklift pallet in the store{apos}s warehouse – he was pronounced dead at a Medical Center about an hour after another employee found him – it{apos}s not clear how long he had been hanging by both feet from the forklift pallet, about 10 feet above the warehouse floor)
Men trapped in container (a man, 30, trapped for nine days in a shipping container is recovering in hospital after being found with a co-worker{apos}s corpse – he was discovered inside the container at the Terminal, next to another man who was pronounced dead at the scene by paramedics – investigations indicate that both men are believed to be port workers who were trapped inside the container by accident while they were taking a rest – the two had entered the container to take a nap after having dinner and a smoke midway through their 12-hour night shift on April 1)

Two Seriously Injured in Explosion of Food Truck on West Street (a food truck accident that caused propane to explode in the truck – a collision between a food truck and an SUV caused an explosion that sent two men to the hospital with serious burns – they were taken to a Medical Center with non-life-threatening injuries)
3 Mauldin workers hospitalized after insecticide canister leaks (three employees were hospitalized after a pressurized canister ruptured and released fumes at a pest control business – firefighters and the hazardous materials team were called – the substance involved was identified as orthene – a seal had broken on a can of orthene, a common insecticide, and leaked onto the shelving of a materials storage container located on the building’s premises – two technicians and a manager discovered the spill when they opened the doors to enter the container – they felt dizzy and lightheaded) |
