Worker Dies in Accident at Disabled Japan Nuclear Plant (worker, 52, died after the hatch at the back of a truck closed on his head – man who was working with the victim told plant officials that he found him slumped at the back of a 7.9-meter-long (32-foot) tank truck used to transport underground water and dirt dug from a frozen wall construction site to a dump site – co-worker told officials that minutes before the incident, the victim indicated to him that he had finished cleaning and inspecting the truck’s tank and wanted him to close the hatch – co-worker said he then closed the hatch using a lever behind the driver’s compartment)

GUARDING/LOTO Jenison worker seriously injured after hand caught in machine (worker, 55, is in serious condition after his left hand became caught in a machine at a maker of cellulose insulation – he was operating a wool compressing machine at the plant when his left hand became entangled in a roller mechanism – others at the plant freed him from the machine before rescuers arrived and he was taken by ambulance to the hospital)

GUARDING/LOTO Worker seriously injured in reported accident at Carpenter Technology (maintenance employee was working on a piece of machinery outside one of the company’s production buildings around 11:20 a.m. when he suffered a serious injury to one of his legs)

FORKLIFT FATALITY Woman dies after struck by shipping equipment at Port Elizabeth (worker, 49, was struck and killed by a piece of equipment that moves shipping containers at 1:20 p.m. – she was struck by the top loader while walking – medical personal responded and pronounced her dead at the scene)

SILO COLLAPSE FATALITY 19-Year-Old Worker Missing After Silo Collapses at Quarry in Loudoun County (worker, 19, has been on the job for less than a year – he was unloading material from the silo into a truck when the silo “split” – rescuers were unable to get inside the silo to search for the man because the 70- to 80-foot structure was unstable)

ASPHALT PLANT FATALITY Worker dies in Waycross industrial accident (worker, 35, got trapped in a piece of equipment at a asphalt plant and died – coworkers had freed the victim and County Coroner pronounced him dead – he was killed while performing welding work on an asphalt silo – he was cutting away a piece of metal, safety equipment in place failed and he was crushed)

GUARDING/LOTO FATALITY Man killed in Harrison industrial accident (worker, 59, was working with a piece of machinery when his right leg became entangled in the equipment – a co-worker called 911 after hearing him scream for help – when first responders arrived on scene, worker was deceased)

BURNS Fiji worker burned in sugar mill accident (sugar mill worker was severely burned and is in a stable condition – the Union’s assistant general secretary says the burned worker was instructed to clean out a tank but was not trained to do the technical task while other staff failed to check the temperature of the liquid)

MINE FATALITY South Africa: Harmony Mineworker Killed in Accident (worker was killed in a rail-bound accident at a gold mine – all tramming operations on the level where the accident occurred have been stopped)

EXPLOSION 33 Injured in Missouri Plant Explosion (molten aluminum hitting water is the apparent cause of explosions that injured more than 30 employees at an aluminum plant – OSHA says it’s investigating two explosions that occurred at the Aluminum plant – OSHA says the preliminary cause of the explosions appears to be the result of molten aluminum contacting water – agency says it also investigated the facility in June after a worker was burned, and that an investigation is ongoing – preliminary information shows 33 employees were injured in the explosions, with most suffering ringing ears and eye and throat irritation)

FLASH FIRE Flash fire injures 3 at Arlington plant (fire officials say there was a flash from an oxygen-acetylene torch that was being used on a bearing that caused burns to two workers – one worker received second and third degree burns to 45 percent of his body – a second worker received second and third degree burns to 27 percent of his body – third worker refused transport)

AMMONIA Tank spews toxic fumes after blast (worker, 44, is in the Intensive Care Unit at a Hospital after he was exposed to ammonia following a chemical leak and explosion at the plant – two workers were in a production room where a 1.6m-long gas cylinder was being filled with compressed ammonia when the tank ruptured and exploded at 8.35 a.m. – the other worker escaped unhurt – the gas cylinder which ruptured was in a ground floor room about half the size of a basketball court – force of the blast caused the false ceiling to collapse and rocketed the cylinder across the room, smashing machinery and releasing ammonia fumes into the air)

EXPLOSION @ MINE Explosion at Sudbury’s First Nickel injures workers (four workers were injured in an explosion at a mine – one worker required medical aid, but the injury was nothing serious – no major injuries, and everyone on shift was able to go home when they left the mine – no other details)

methylene chloride FATALITY Feds investigate after local man dies deglazing bathtub (OSHA says it is investigating the death of a worker who apparently was overcome with chemical fumes while stripping the glaze off a bathtub – preliminary reports indicate the worker may have been exposed to methylene chloride while deglazing the bathtub in a poorly ventilated bathroom – OSHA said the worker did not appear to have been wearing gloves or using a respirator while applying methylene chloride to the tub)

HOTWORK FIRE Welding starts fire at Valenzuela warehouse; 1 dead (worker, 45, died after fire hit a fabrics warehouse – he was on his way out of the warehouse when he suffered a heart attack)

Rodeo: Cause of refinery fire under investigation (fire, which started around 3 p.m. and prompted the county’s incident warning system to issue an alert just before 3:15 p.m. – advised people with respiratory sensitivities to avoid the area or stay inside but added that most people should not be affected – no injuries were reported)

FALL from DOCK Barge worker injured near Valero (contract barge tankerman was air lifted following an incident shortly after 11 a.m. – the patient is a 60-year-old male who fell approximately 20 feet into the water)

CATASTROPHIC FAILURE of NITROGEN TANK Tank Explodes, Building Partly Collapses at German Plant (a nitrogen tank exploded and part of a building collapsed at a chemical plant injuring several people – 10 people were injured, two of them seriously)

AMMONIA Ammonia leak at Poland Spring plant sickens 2 workers (a leak of anhydrous ammonia leads to an evacuation of the plant around 10:30 a.m. – two employees of the bottling plant were taken to a hospital after an ammonia leak about 10:30 a.m. – two employees complained of headaches and were evaluated at the scene by emergency personnel before being taken to a Medical Center – they have since been released)

AMMONIA Eight casualties after ammonia leak at Spalding abattoir (an ammonia leak resulted in six casualties being treated at the scene by paramedics – two members of staff were also taken to hospital)

AMMONIA Ammonia leak at Compton leads to 90-minute evacuation (an ammonia leak in the mechanical room at an Ice Arena led to a brief evacuation of the building at about 12:05 p.m. – an employee went to a Wellness Center after inhaling some ammonia – the leak was only in the mechanical room – building opened again at about 1:30 p.m.)

TRENCH FATALITY Mass. worker killed after becoming trapped in trench (construction worker was killed when he became trapped in a water-filled trench during a sewer project just before 8 a.m. when a water main broke and the trench filled with water)

ELECTRICAL Worker collapses after being shocked at condo complex (construction worker is nearly electrocuted while working at a condominium complex)\

AERIAL LIFT FATALITY Worker killed when boom lift flips (worker, 40, was killed when this 85-foot boom lift flipped and he was ejected – he was pronounced dead at the scene – it appears he began to move the equipment while on a sloped area of ground around the house – boom lift flipped over, ejecting him from the basket – he landed on the ground about 25 feet away from the basket and approximately 105 feet from the house)

FORKLIFT Worker seriously injured after being hit by forklift and pallet at Barangaroo worksite in inner Sydney (worker, in his 40s, was seriously injured after a pallet of scaffolding reportedly fell on him after he was hit by a forklift – he suffered multiple injuries after he was struck by a forklift carrying pallets of scaffolding – a pallet then fell off the vehicle and onto the worker)

WORKZONE Road worker run over by dump truck in Hanover Township (road worker was taken to a hospital after a dump truck ran over his legs and pinned him in a construction zone – accident occurred at 2:14 p.m. – a tow truck used a crane to lift the vehicle off the man)

BULLDOZER FATALITY Pipeline worker killed in construction accident (pipeline worker, 54, was killed when a bulldozer he was operating rolled on top of him – he died at the scene – he was operating the bulldozer at 11:48 a.m. on a steep grade when it rolled down the hill, turning numerous times before coming to rest in an upright position – he was part of a construction crew working on a 36-inch natural gas pipeline)

FALL Worker injured at Brookfield’s Manhattan West site (construction worker suffered serious injuries after he plummeted 40 feet to the ground right before 2 p.m.)

Worker killed in New Town accident (construction worker, 65, died after a gravel pile gave way and swept him under a piece of machinery – he was standing on the ladder of the front end loader he’d been operating when the top of a large pile of sandy gravel slid down, knocked him off the vehicle and buried him underneath it – he’d worked there for “a couple of months” before his death – accident happened around 2:30 p.m.)

TRAIN CROSSING FATALITY Worker killed at fort (construction worker was when the equipment vehicle he was operating was struck by a train – accident was reported around 6:15 a.m. at a railroad crossing)

FATAL BEE STING Worker dies after crew attacked by bees in Riverside (worker has died after a crew was attacked by a swarm of bees – two other people were also stung – there were two different construction projects underway at a parking lot where the hive was located in an underground sprinkler box on the south end of one construction site – workers accidentally disturbed the bees’ underground hive, and an electrician was stung by the bees)

TRENCH FATALITY Trench collapse kills Parkman man (worker, 54, died after a 10-foot-deep trench collapsed on top of him – he and another worker were in the trench that hadn’t been reinforced as safety rules require when one side collapsed – they were working on a storm-water culvert project when the accident happened – the other employee wasn’t hurt)

AERIAL LIFT Scissor lift collapse at construction site near Altrincham leaves two men injured (worker was seriously injured and a second was also hurt while working on a platform at a construction site – the contractors were working when their scissor lift fell, injuring the two men)

ELECTROCUTION w/ LADDER Southern Illinois contractor electrocuted on Friday (contractor, 23, for an area home supply store was fatally shocked at a home – the two were shocked after an extension ladder they used to access the roof had come into contact with a high-voltage power line – both men were on the ground when officers arrived – worker, 23 was transported to the hospital where he was pronounced dead – co-worker, 21, was airlifted from the scene and his condition is listed as stable)

MISCELLANEOUS

AMPUTATION Worker Loses Foot in China’s Third Escalator Accident in a Week (cleaner, 35, loses left foot after escalator traps him – doctors were forced amputate a man’s foot and part of his leg after it was shredded in a mall’s escalator – his leg became caught between the revolving stairs and the floor plate)

TOLL BOOTH FATALITY 61-Year-Old Toll Worker Struck and Killed on Mass. Pike (toll worker, 61, was struck and killed just after midnight – he was crossing in front of an EZ Pass lane at the tolls when a 2010 Toyota Tacoma struck him as his car was exiting the toll booth)

BURNS Teen critical after suffering burns in workplace accident at North Geelong KFC (worker, 16, is in a critical but stable condition after suffering horrific burns to his face and body – he was cleaning the kitchen when the incident occurred, with most of his body receiving third-degree burns – he had been working at the restaurant for nearly two years and that he was an experienced cook)

FALL Window Washer in Critical Condition After Plunging Six Stories (employee had been working from a chair while cleaning windows at the hospital when he fell – worker suffered two broken legs and internal injuries, but it expected to be OK)

FATAL FALL City worker, 23, plummets to his death from a tenth-storey roof terrace (worker, 23, has died after he plummeted from a tenth-story roof terrace – he fell from the top floor at around 7.45pm)

HOTWORK MFD: Sparks from welding torch start fire at metal recycling center (no one injured in fire at facility – sparks from a welding torch reportedly started a fire in an industrial metal shredder around 10:45 a.m. – Investigators said the fire was caused by sparks from a welding torch used in the area just before the incident)
