AMPUTATION in CONVEYOR OSHA visits Emmett sawmill following accident that severed worker’s arm (accident happened just after 7 a.m. while the worker, 24, was working at the sawmill – employee was standing on the sawmill’s floor when he for unknown reasons stuck his hand in the head of a moving conveyor)

LOTO FATALITY w/ ROBOT Robot kills worker at Volkswagen factory in Germany (a contractor, 21, was installing the robot together with a colleague when he was struck in the chest by the robot and pressed against a metal plate – he later died of his injuries – a second worker was outside the cage, and was not harmed)

FORKLIFT FLASH FIRE Worker Burned In Industrial Accident At Lincoln Facility (worker was burned in a possible explosion just before 7:30 a.m. – worker, who was on a forklift at the time of the accident, has been airlifted to a Medical Center suffering from second to third degree burns to 10% of his body – unclear what caused the accident)

FORKLIFT FATALITY Worker killed by forklift at Federal-Mogul Corporation (worker, 25, has died after being run over by a forklift – he was walking in front of the forklift possibly securing an empty barrel)

FORKLIFT and ELECTRICAL Workers electrocuted after tractor hits power line at Columbus manufacturing facility (two workers at a manufacturing facility were taken to the hospital after being electrocuted when a front-end tractor loader made contact with a power line – the two injured men, who are contractors from an outside vendor, were moving a large air handling unit through a parking lot outside the facility – victims were walking along the unit as it was being transported using the front-end tractor loader – as the two men, and another worker inside the tractor, maneuvered the unit, the loader hit overhead power lines, electrocuting the two men on the ground)

ELECTROCUTION Siemens Employee Dies in Electrical Accident at AK Steel Ashland (contractor performing routine electrical maintenance work was electrocuted – he was found unconscious and efforts to revive him were not successful – County Coroner has reported that the worker appeared to have been electrocuted while performing testing work at a substation when he came in contact with a wire)

FATAL HOTWORK EXPLOSION Four dead in fuel tank explosion at Togo factory (four people were killed when a fuel tank exploded at a cement factory – six workers were carrying out welding work on the roof of the tank when the tragedy happened – the foreman and 13 workers in total had been working to repair the tank before the explosion)

HOTWORK One injured in rail yard fire (a fire caused minor injury to one worker – all employees exited the building before firefighters arrived – contractor was installing “fall protection” in the paint booth area, a process that requires welding – one of the employees was on a scissor lift welding when the fire started, a fire apparently sparked by a spark that came into contact with air filters in the paint booth – worker on the scissor lift told firefighters the fire spread so fast that he was unable to access the scissor lift’s controls and he climbed down the side of the scissor lift to escape suffering minor burns in the process – another employee standing watch on fire guard told firefighters that the fire spread so quickly that he was unable to extinguish it with an extinguisher – firefighters used about 2,500 gallons of water extinguishing the fire, which was also in part extinguished by the building’s sprinkler system)

GUARDING/LOTO Industrial accident critically injures man in Big Sandy (worker, 40, was critically injured at a soil plant – he was crushed in some type of machinery in the plant)

MINE FATALITY Authorities ID coal miner killed in accident (coal miner, 55, with 10 years of experience died when he was crushed when a door came down on him underground – the cause and manner of death is pending autopsy results)

ELECTRICAL Transformer explodes at BPU plant in Quindaro, no one injured (a 69kv transformer exploded at a power plant – no one was injured in the explosion)

EXPLOSION No injuries in small blast at Flagler County fireworks plant Friday (small explosion at a fireworks plant started a fire to a building where gunpowder is pressed into pellets – two employees were in a small steel and concrete building when a fire sparked – no one was burned or suffered any blast-related injuries – explosion occurred during a process of pressing gunpowder into pellets, which generates heat – fire was extinguished in about three minutes)

FORKLIFT FATALITY Deputies: McCracken Co. man dies in forklift accident (worker, 87, is dead after an apparent forklift accident around 12:45 p.m. – worker had been run over by a forklift – worker was found dead next to a forklift – neighbor reported that as he was driving past, he saw smoke coming from a barn and stopped to investigate. The neighbor said that he found the worker lying on the ground and the forklift running, in gear up against the barn wall with its tires spinning – determined that the worker was trying to jump-start the forklift while standing between the front and rear tires – the forklift was in gear and started while he was standing between the tires and was unable to get out of the way and he died from his injuries after being run over by the forklift)

FORKLIFT FATALITY Man killed in forklift accident at lumber yard in Caldwell County (worker, 55, was run over by the forklift, which apparently was in reverse – forklift was not carrying a load at the time of the accident – there were no witnesses to what happened and the deceased was believed to be driving the forklift)

CONSTRUCTION

Construction worker killed in accident identified (worker, 66, died in a bulldozer accident after he was “apparently trapped in a work area the employee had been using the bulldozer to dig in”)

SAW KICK BACK FATALITY Worker killed in accident at Sikorsky Airport (worker using an gasoline-powered saw to cut a water main in a trench when the saw apparently kicked back and caused a fatal injury to the man’s neck – worker was pronounced dead at the scene)

DROWNING Worker killed in river accident was from Batavia (construction worker, 31, drowned when he fell into the Genesee River – he was in a small work boat that tipped over underneath a bridge – officials say a safety line he was attached to became stuck on an abutment, trapping him under water – crews recovered his body about two hours after the accident)

SCAFFOLDING Scaffolding falls on worker at new Atlanta Falcons stadium site (worker is being treated for a “severe head injury” after metal scaffolding fell on him at the construction site – worker was injured during “scaffold removal”)

UPDATE on WORKZONE FATALITY Woman charged in auto crash that killed Ohio utility worker (driver, 23, has been charged with aggravated vehicular homicide and other counts in a crash that injured several utility workers – police say they filed charges against the driver after completing their investigation of the April 22 accident – driver was arrested on three charges of aggravated vehicular assault and operating a vehicle while impaired)

WORKZONE FATALITY Construction Worker Killed In I-95 Crash (construction worker, 62, was struck and killed in a crash involving a tractor trailer in a work zone after 1:30 a.m. – a tractor trailer ran into the work zone and crashed into two other vehicles – one of those vehicles then ran into and killed the worker)

CRANE FATALITY Update: Worker Killed in Construction-Related Accident Identified (construction worker, 41, was killed when a piece of metal rebar fell 130 ft from a crane and hit him on top of his hard hat – he was pronounced dead at the scene – initial reports indicate there was some sort of mechanical failure)

ELEVATOR FATALITY Fatal elevator accident in Istanbul (construction worker, 28, dies while repairing an elevator – he died on the spot – no other details)

WORKZONE FATALITY Worker killed during construction accident (construction worker, in his mid-20s, on I-40 was hit and killed by a dump truck just after 9:15 p.m. – crews have been re-paving a large section of I-40 over the past few days)

CONCRETE AUGER Construction worker injured on Interstate 80 near Wanship (worker, 52, sustains severe injuries to lower legs when he fell into a cement auger – he was working on equipment in the construction area at approximately 6:15 p.m. when he somehow got entangled in the equipment – his injuries as “critical, but not life threatening”)

AERIAL LIFT FATALITY Worker Killed In Fall From Cherry Picker Truck In San Jose (worker fell from a cherry picker and died – he worked for a subcontractor assigned to work on the city’s streetlights – he fell around 9:40 a.m. as he worked on a boom truck – emergency crews pronounced the man dead at the scene a short time later)

TUNNELING WINCH FATALITY Worker killed at tunneling site (contract worker, 38, who was operating a winch was killed when the equipment malfunctioned at about 7:55 a.m. – workers were removing a large piece of a micro-tunneling machine from the tunnel when “a winch came loose, killing the employee who was operating the winch” – unknown reasons, the mishap caused the winch to travel toward the tunnel entrance – worker who was killed was on the winch when the accident occurred)

Plumbing Contractor Injured on the Job at Golisano Children’s Hospital Dies (plumbing contractor injured while working has died – he was apparently struck in the head by a metal pipe fitting he was working on – incident happened around 11 a.m. in a mechanical room that houses the hospital’s plumbing and air handling equipment – two other contractors who were working nearby were uninjured)

POSSIBLE LIGHTENING FATALITY Worker possibly killed by lightning strike in Port Orange (investigators are trying to determine if lightning caused the death of a man who was killed while on the roof of a home – the 911 caller said a worker had been hit by lightning as storms passed through – victim could not be revived)

TRENCH FATALITY Man Dies after Construction Site Trench Collapse in Lakeville (incident occurred at approximately 3:45 p.m. at a construction site – a machine-dug trench collapsed on the worker, 51 – the excavator operators was the man’s brother – trench was roughly six to eight feet deep – worker was helping to install drain tile when the walls of the trench collapsed in on him)

STEAM ROLLER Worker Run Over by Steamroller at Eureka Construction Site (construction worker, in his 40’s, was run over by a steamroller – he was “alert, oriented and seemed fine” when firefighters arrived just after 11:15 a.m. it rolled over the man’s chest, legs and pelvic areas)

Toowoomba worker crushed by falling steel (worker, in his 30s, has been injured after being crushed by falling steel poles at a construction site – he suffered head and abdominal injuries and was taken to hospital in a stable condition)

WORKZONE and CONFINED SPACE Workers In Manhole Hit By Truck (two construction workers were hospitalized after they were injured when a truck came crashing through some barricades – one of the workers was pinned in the manhole – both men were taken to an area hospital with non-life threatening injuries)

LADDER and ELECTRICAL Ladder Hits Power Line Injuring Construction Worker (construction worker, 35, was injured – no one actually witnessed the incident, it appeared that the worker was moving an aluminum ladder and touched a power line with the ladder – other workers heard a “popping” noise, immediately went to his location, and found him on the ground unconscious and with a leg wound – they began first aid and called 911)

FATAL FALL from ROOF Tennille man killed in accident (construction worker, 22, died after falling 20 feet while working to install a roof on a building – was one of the workers installing the roof of a new building when he fell about 20 feet onto a concrete floor – the victim and another employee were putting insulation on beams that would eventually support the roof when the victim fell)

WORKZONE ALDOT worker struck by vehicle (worker is recovering after he was hit by a passing vehicle as he was doing final surveying work when he was hit by part of a vehicle – accident happened before 11 a.m. on I-565)

TRENCHING FATALITY Construction worker dies on site of new Allsup’s (worker, 63, died around 2:00 pm while installing fuel tanks – the worker was crushed by dirt walls and the fuel tanks and declared dead at the scene)

MISCELLANEOUS

TREE TRIMMING Worker injured when part of tree falls on him in Littleton (worker, 26, was injured when part the tree he was working on fell on him – victim had been working on an 80 foot pine tree when a section of the top fell off and hit him in the head – he was taken to a nearby hospital with life-threatening injuries)

BUS LOT FATALITY Bus runs over, kills DART driver at northwest Dallas bus lot (a bus driver, 55, was hit and killed by a bus in the transit lot – he had worked at DART since November – accident was reported about 5 a.m. at a lot where drivers begin their shifts and get their routes – victim was outside his bus when another driver pulled out and struck him)

WORKZONE FATALITY Lawn worker struck, killed by 18-wheeler along Highway 146 (a lawn contractor, 20, working alongside a highway was struck and killed by an 18-wheeler – he was weed-eating along highway when the big rig veered off the road and struck him – investigators said they believe he may have had a medical issue – no signs of intoxication)

Worker killed while unloading car hauler (a driver fatally struck a worker, 37, who was unloading vehicles from a tractor trailer on the side of the road just before 11 a.m. – he was inside the back of the truck removing the chains from a car on the top rack when an approaching northbound driver failed to stop and drove up the lowered ramp and hit the worker)

UPDATE on WOOD CHIPPER AMPUTATION Worker loses leg after wood chipper accident (a tree maintenance company has been ordered to pay thousands of dollars to an employee who had his leg amputated in a workplace incident – ordered the company to pay $35,000 in reparation to the victim whose right leg was pulled into a wood chipper machine – investigation revealed the company could have extended the chute of the chipper so there was less chance staff would come into contact with it)

DROWNING Worker installing pump drowns in Westchester County lake (worker, 31, installing a pump fell from a canoe and drowned – he and another worker from a landscaping company were installing an irrigation pump for a private residence – he lost his balance and went into the water – he attempted to retrieve the canoe, which sent it farther into the lake – co-worker went back to the residence and called 911 – he was not wearing a life jacket and was struggling in the water as if he couldn’t swim)

CHLORINE Maintenance worker treated for chlorine inhalation (emergency rescue crews treated a maintenance worker for chemical inhalation at a townhouse complex after he was overcome with chlorine fumes when he walked into a pool shed on the property – they believe there was a malfunction of an automatic pool system that mixes the chemicals together to treat the pool – crews responded and evacuated one nearby building while they investigated and aired out the shed)
