LINEMAN ELECTRICAL Electrical accident sends Columbus contractor to hospital (an electrical worker goes to a hospital after making contact with a high voltage line – he was doing routine maintenance on a line at about 3 p.m. – after the ambulance left, the vest the man was wearing remained behind hanging on a sign and appeared to be visibly burned – the worker did not fall out of the bucket when he made contact with the line)

FALL into GRAIN DRYER Worker injured after falling into grain dryer (worker, 54, fell through a vent door of a grain dryer and was knocked unconscious – he was taken to a Hospital with serious, but non-life threatening injuries)

FATAL FALL from OFF-SHORE RIG Accident Claims One in BP North Sea Incident (a man has died after an incident on a North Sea oil platform – the death had resulted from a fall – he was part of a team carrying out routine maintenance onboard)

CRANE Worker injured after steel beam falls on leg (worker was injured after a steel beam feel on his leg – he was taken to a Medical Center with life-threatening injuries – he was using a remote-control crane to store large metal beams when a steel beam fell on his leg – injury was to the lower part of the employee’s leg)

FALL on CELL TOWER Update: Oklahoma workers identified, one remains in critical condition after fall from cell tower (worker was treated and released from the hospital – one victim remains in critical condition – three men were rushed to a hospital after an accident at a cell tower – two workers were on the cell phone tower when a rope snapped, hitting one of the men in the face – the force knocked him unconscious – two men were stranded near the top of the tower for about 15 minutes while a third worker tried to get them down – as the third worker climbed to secure a new rope, the two men slipped again)

ELECTROCUTION Police: Worker electrocuted in Clinton Township (worker was electrocuted just before 6 a.m. – worker was moving some equipment and came into contact with high-powered lines)

SILO COLLAPSE Grain silo accident causes rail havoc (rail services face disruption after 600 tons of grain held in a mill silo burst open and landed on the railway tracks – nobody knows yet what caused the accident in the 30-year-old silo)

PAPER MILL FATALITY Worker dies at International Paper US plant (maintenance worker, 58, has died at approximately 2.30 pm – he was pronounced dead at the scene – he was working in the Paper Mill when he was involved in what appears to be an industrial accident – there was an incident [with] a piece of equipment he was working on which claimed his life – he was an employee at the mill for more than two years)

CONFINED SPACE FATALITIES Poisonous Gases Kill Two at Sewage Plant in Chennai (a sanitary worker and his colleague were asphyxiated to death while braving poisonous gases in the valiant bid to save other friends, engaged at a Sewage Treatment Plant (STP) – without protective gear, the workers were involved in the maintenance of the STP – two of them died after inhaling noxious gases from the outlet valve – a six-member team of sanitary workers employed by a contractor, was involved in cleaning work at the STP while one worker, 21, lowered himself into the valve chamber of primary clarifier – when the valve was operated around 4.30 am, a spindle rod snapped resulting in sewage water and toxic gases filling the chamber – he swooned inside the 12 ft deep chamber – workers, 26 and 27, rushed into the chamber were also swooned immediately – sensing trouble, worker, 28, entered the chamber and pulled out the two out – he went again for the third time into the chamber despite the warnings of other workers – all the four who were affected were moved to a Medical College Hospital – doctors declared the two entrants brought dead on arrival and the other two were admitted for treatment – the condition of the duo was stable)

LINEMAN ELECTRICAL LADWP Worker Remains Hospitalized In Critical Condition (a Department of Water and Power worker remained hospitalized in critical condition with serious injuries he sustained while working on overhead power lines – a journeyman line worker was working with his crew to replace a power pole as part of LADWP’s Power Reliability Program – just before 11:30 a.m., the worker made contact with electrical wires that carried 4,800 volts of electricity – he was knocked unconscious as a result – he was rescued by co-workers who lowered him from the pole)

SLIP/TRIP/FALL Man Seriously Injured In A Hamilton Industrial Accident (a worker suffered a serious head injury on the job – a driver slipped and hit his head on the back of a truck – no other details have been made available)

FATAL FALL Quarry Worker Dies After Fall (a worker, 54, at a Quarry died of a broken neck after falling 10 feet at about 6 a.m. Tuesday – he entered a storage building to obtain wooden planks from within the rafters – he climbed onto pallets to reach the rafters, lost his balance and fell approximately 10 feet, landing on his head and neck – none of the other workers witnessed the accident)

MINE FATALITY Worker dies in mine accident (worker, 29, died in a private mine after a section of rock fell from a ceiling over him at around noon – he was trapped under the rock and seriously injured – colleagues at the worksite pulled him out and he was taken to a Hospital – efforts to treat him were unsuccessful and he died)

MANLIFT FATALITY Worker killed in Lyttelton port accident (worker, 40, has died after a scissor lift toppled over at the port – he was the sole occupant of the lift when it fell to the ground where he died at the scene)

BARGE Man injured in barge accident in Red Wing (accident left a barge worker, 30, badly hurt after he was crushed inside the Lock and Dam and airlifted to the hospital – he was crushed between a lock wall and barge when the water pushed it against him – he was standing on a lock wall ladder waiting for a tugboat to pass through – that’s when the barge moved too close and pushed up against him)

STEAM Haverhill worker burned (worker was transported to a Hospital around 5 p.m. – the injury was some sort of hot water or steam burn – he was then flown to a Hospital to be treated for his injuries but no other information was available)

Man burnt in Broadwater sugar mill blast (inspectors are investigating how a man was burnt during a boiler blast at a sugar mill – the worker, aged in his 40s, suffered burns to his face, neck and arms and was flown by a rescue helicopter to the a Hospital in a stable condition – the incident was not an explosion and the problem occurred when the boiler fuel ignited in what’s known as a ‘flash-back’)

HOTWORK 95% of Sterling fertilizer building lost in Wednesday fire (a fire that broke out at a fertilizer plant has destroyed 95% of the facility – fire erupted at the production facility just after 3 p.m. – the blaze was started by a welding accident inside the facility – no serious injuries were reported)

REFINERY FIRE Blast at BP Refinery in Indiana Sparks Fire, Shakes Homes (an explosion at an oil refinery rattled nearby homes and sparked a fire that was later extinguished – it didn’t cause any major injuries or halt production at the facility – the explosion was caused by “an operational incident” on a processing unit about 9 p.m. and was extinguished by the plant’s fire department within a couple of hours)

LOTO 2 hurt in accident at Omaha plant (two people have been hospitalized following an accident – one person who was critically hurt may need attention from the trauma center; the other person suffered serious injuries – it appears the men were working on a hopper for a hydraulic trash compactor, when a piece of equipment fell on them – both individuals suffered injuries to their lower extremities)

AMMONIA Update: Tuesday ammonia leak caused Koch evacuation (a toxic gas leak caused an evacuation of about 200 employees at a Nitrogen plant – plant remains shut down – preliminary report from the U.S. Coast Guard’s National Response Center database also noted the leak of anhydrous ammonia was believed to be more than 100 pounds, or more than 2,000 cubic feet of the gas – an atmospheric release of ammonia happened at 6:30 a.m. during a start-up procedure – there were no injuries)

CONSTRUCTION

PAVER Public construction accident sends worker to hospital (construction worker was ran over by an asphalt roller – he was raking loose asphalt near a roller when another employee returned to the roller to continue smoothing out the road – the driver of the machine reportedly could not see the employee standing nearby and put the machine into gear and began moving forward – roller ran over the man’s foot knocking him down and continued for several more feet – because of the noise level of the machine, the driver was not able to hear the other employees yelling at him to stop – injured employee was taken to a local hospital with life threatening injuries – he suffer multiple internal injuries and broken bones)

SKID LOADER Man in bobcat trapped under dump truck (fire crews had to free a worker in a bobcat loader was trapped under a dump truck – he was trapped in the bobcat under the front axle of the truck shortly before 1 p.m. – firefighters had to cut off the top of the bobcat’s cage and steady the truck before they could get the trapped worker out – he was transported to a hospital with serious leg injuries)

CONFINED SPACE FATALITY Two workers die in construction accident (a construction accident has left two workers, 19 and 46, dead when a construction crew was working on a new sewage system for the city – it was a toxic combination of fumes inside a manhole that caused the workers to pass out, then drown in the water below – the sealant we figure is what put off the fumes with the reaction of PVC liner, the fumes reacted with each other – father of the 18-year old believes his son realized something was wrong, and went into the manhole as well to help get him out – he suffered from asthma and believes that had to do with his death – both men passed away at a local hospital later that day –

“I stood beside my boy for 4 hours while they tried to get a pulse, tried to get anything,” said Taylor.

“It was a clean new manhole, fresh water in it, it was a fluke, 100 % crazy accident,” said Taylor)

FALL Worker injures kneecap in 4-story fall at Missoula construction site (a man injured in a four-story fall at a construction site has been released from a Hospital with a minor injury to his kneecap – the crew member had left the hospital with possibly a broken or dislocated kneecap and planned to seek further treatment from a specialist – the man works as a framer for a company that’s a subcontractor – he fell an estimated 40 feet after making a wrong move – he removed a piece of plywood that shouldn’t have been removed and then fell through the hole)

WORKZONE PA. TURNPIKE WORKER INJURED IN CRASH (worker was struck by a tractor trailer around 11:00 a.m. – investigators initially believed the worker was hit by a tractor trailer – however, police now say the big rig hit a dump truck the worker was in – worker fell out of the dump truck after impact and suffered minor injuries and was being treated at the hospital)

SCAFFOLDING COLLAPSE Construction Site Fall Injures Two Workers on Santa Barbara Mesa (two construction workers came crashing down when a scaffolding fell apart while they were working on a two story home – one had back and arm injuries – the other man had a leg injury, but gave a thumbs up to a co worker as he was taken away on a gurney – unknown why the scaffolding collapsed – they were 14 feet above the ground at the time working on a framing project)

FALL from ROOF Roofer plummets at WSN (a construction worker is in a medically-induced coma after falling from a roof at approximately 9:30 a.m. as he was installing roofing panels – he fell approximately 30 feet and suffered multiple broken bones – the man worked for a subcontractor on the project – the worker has a broken shoulder, leg, foot, ribs and arm, and he’s got a punctured lung)

TRENCHING FATALITY Labourer Killed After Soil Caves in (worker, 30, and another worker, 28, were digging a ditch when the accident occurred at around 3 pm. – they were in a 10-feet ditch and the wet mud collapsed on them – they rescued one, who had suffered severe injuries, after a one-and-half-hour operation – the other worker had suffocated to death under the debris – neither the company nor the contractors had taken any precautionary measures during the work)

FALL thru ROOF Worker falls through roof at Schenck High School during construction job (a roofing subcontractor was hospitalized after falling through the roof of a High School – he fell through a section of roof decking and landed on a file cabinet and possibly a table near the auditorium area of the school at about 8:30 a.m. – extent of the man’s injuries was not immediately available, but the worker did not appear to lose consciousness or break any bones)

MISCELLANEOUS

FORKLIFT FATALITY 50-Year-old farm worker dies in accident (a farm worker, 50, died when the heavy farm machinery he was driving plunged into a slurry pit – he was transferring heavy slurry from one pit to another when his vehicle went into the pit and became almost completely submerged – emergency services were called just after 8am after fellow farm workers found him)

PLANE TUG Worker hurt at Pittsburgh International Airport (airline employee was hurt seriously in a vehicle accident outside the airside terminal -the crash happened at 8:56 p.m. with a vehicle that tows airplanes, although there were no other immediate details – there weren’t any aircraft involved)

ELECTROCUTION Electric shock kills worker (an engineer, 24, has died after receiving a fatal electric shock while working at a school – he was working on a cooker when he suffered a massive shock)

MOWER DEEP Worker Injured In Fall In Hartland (a seasonal employee for the state Department of Energy and Environmental Protection was injured after he fell about 20 feet onto rocks while cutting grass – he was using a large walk behind mower when the accident occurred)

MOWER Two Ste. Genevieve workers injured (in two unrelated events, two Road and Bridge Department workers were injured while performing their duties – one worker was struck in the back of the head by a random piece of wood flung by a boom mower – the wood traveled through the rear window of a second mower where it impacted the driver of the mower – the other accident took place later when a machine that unrolls tarp on a dump truck managed to remove the fingernail of a road worker)

Man Killed in Accident (at approximately 2:40 PM a trailer fell on top of a worker, 22 – he had been fatally injured in the accident – early investigation indicates he was driving a small tractor while pulling a twenty-five foot empty flatbed trailer – trailer came loose and collided with the tractor he was on when his tractor struck a parked vehicle – trailer then rode over the back of the tractor and pinned him down)

GARBAGE TRUCK FATALITY Worker hit and killed by SF city truck in the Bayview (a Department of Public Works garbage truck struck and killed a crew member, 52, just after 9 a.m., while the crew was cleaning up trash dumped illegally on streets)

FLASH FIRE Employee error blamed in Reno museum flash fire (a minor explosion during a science experiment at the museum burned several children and forced the evacuation of the museum – the flash fire injured 13 people at the science museum happened when an employee applied the chemicals in the wrong order during a tabletop demonstration about the mechanics of tornadoes – a  3-5 second blaze erupted after the presenter poured alcohol on a cotton ball that had been dusted with boric acid and partially ignited)

FORKLIFT FATALITY Christiansburg Forklift Accident Kills One Man, Seriously Injures Another (worker, 50, was killed and another seriously injured in a forklift accident – they were using a forklift with a bucket to cut trees around 11 a.m. – one was operating the forklift when the machine started to turn over – according to witnesses, he tried to get out of the cab and run to safety, but was hit by the lift’s elevated bucket – he was pronounced dead at the scene – the other man was in the bucket when the forklift overturned – he, 22, was airlifted to a Hospital with serious injuries)

BIG BOX STORE FATALITY OSHA will not review Menards fatal accident (the victim, 38, was shopping with his wife when a pallet of flooring ceramic tiles fell from a storage shelf and struck him in the head – the tiles weighed up to 600 pounds – a forklift operator had completed placing the pallet on the top of an upper storage rack before the accident – the tile fell about 15 feet)

FORKLIFT Cape Coral landscaper seriously injured by forklift (a landscaper, 59, working in a construction zone along I-75 was seriously injured after an accident with a forklift – he was standing in the east shoulder of a construction zone – a forklift operated by another man was laying down grass sod on the same shoulder of the construction zone – the landscaper bent down to pick something up from the ground as the forklift was approaching but it wasn’t able to stop and struck his pant leg which caused him to fall to the ground “and his right leg was struck by the front left tire of the forklift”)
