LOADING DOCK FATALITY Gardner man dies in Shawnee FedEx accident (a man, 62, who was injured at a loading dock has died just before 7 a.m. – he suffered critical injuries when he was pinned between a trailer and a loading dock at the company’s plant and was transported to a Medical Center where he later died from his injuries – in March, a worker, 31, was killed after falling under the wheels of a truck in an accident at the same facility)

FORKLIFT Beer Store worker injured (an employee at the warehouse was treated in a hospital for a leg injury after the incident – worker at the warehouse was struck by a reversing lift truck – worker fell and then the part of the truck struck the worker)

RAIL FATALITY CN worker killed near Tisdale, Sask. (rail worker, 39, is dead following a train accident just after 6:30 p.m. – he was taken to a hospital where he was later pronounced dead – no details)

FATAL FALL from CRANE Worker killed in accident at Port of Oakland (a worker was killed in a fall from a crane and was pronounced dead at the scene)

MINE FATALITIES Colorado mine accident kills 2, injures 19 (an accident at a mine killed two workers, 34 and 59, and injured 19 – carbon monoxide was blamed for the deaths – authorities were trying to determine the source of carbon monoxide blamed for killing two men and injuring others at the mine – investigators were looking at whether a small blast on Saturday at the underground gold, silver and lead mine may have been the cause of the carbon monoxide problem – nineteen other miners were taken to a hospital and four were admitted – they were listed in fair condition)

LOTO/GUARDING Two injured in workplace accidents (around 10:40 a.m. a 62-year-old employee became trapped up to the waist in production equipment at a bakery – by the time the firemen arrived other employees had removed the injured man from the machinery)

POWER PLANT Incident at Beaver County power plant injures worker (a worker at a power plant was flown to a hospital after an apparent leg injury – he appeared to have broken a leg – he works in mechanical maintenance and was injured working on a project)

EXPLOSION Worker facing surgery after gas bottle explosion (a man, 44, is set to undergo surgery after suffering serious injuries in a gas bottle explosion – he had been working at agricultural and industrial equipment manufacturer when he sustained injuries to his face and a hand in the blast at 11.20am – he was taken to a University Hospital and is expected to undergo surgery)

EXPLOSION Shelbyville blast prompts evacuation, severely injures one worker (a worker was seriously injured in a fire that prompted the evacuation of a nearby jail workhouse, animal shelter and school – law enforcement evacuated a half-mile radius around the burning building – there were reports that tanks at the facility exploded about 10:30 a.m. – one person was brought to the hospital by LifeFlight – some biodiesel was stored in large tanks around the back – by late afternoon, the fire was mostly out and people were being allowed to return)

FATAL REFINERY EXPLOSION 1 killed in Coast refinery blast, blaze (authorities are investigating the cause of an explosion and fire that killed one worker, 50, at a refinery – an operator at the Cracking II Processing Unit died after something detonated inside a furnace and sparked a fire – the operators were outside the furnace at the time, and the fire was contained to a small area – the victim had worked as an operator for five years and was among about a dozen workers in the area when the explosion happened – there were no other injuries)

MECHANICAL PRESS AMPUTATION Albury worker’s hand crushed in press (a worker whose hand was crushed in machinery at a factory was in a serious condition in hospital – the man, believed to be in his early 20s, was taken to the hospital after his hand was caught in a metal press – he suffered a serious hand injury)

FALL from LADDER We Energies worker hurt after falling inside manhole (worker was hurt after falling inside a manhole – worker was working inside the manhole around 10:15 a.m. – he was on the ladder inside when he fell off and hit his head – he suffered a cut on his head – worker was conscious, but transported to a hospital for minor injuries)

LOTO/GUARDING w/ AUGER Plant worker injured in auger incident (employee was airlifted to a Hospital after both his legs were caught in an auger around 10 a.m. – his legs were severely damaged in the accident)

ELECTRICAL Duke Worker Injured by Live Power Line (worker cleaning up from Sunday’s storm, was rushed to the hospital after he came in contact with a live power line just before 5 a.m.)

FATAL EXPLOSION @ REFINERY Antwerp refinery blast kills two contractors (an explosion at a refinery has killed two contractors and led to an evacuation of the site – the blast occurred as maintenance was being carried out on a steam generation system in a gasoline production unit – the accident did not cause a fire and there was no loss of containment)

ELECTRICAL Man injured in electrical accident at dam (employee, 54, was critically injured as a fire started at a switch in the Pump Generating Plant –  he was flown to a Medical Center with burn injuries – another employee was treated locally for smoke inhalation – he has worked for the bureau since 2008 – he was listed in serious condition in intensive care unit – at the time of the incident, four pumps and five pump generators were undergoing maintenance – the accident is believed to have been caused by a switch gear failure)

AMMONIA Residents Take Shelter From Ammonia Leak (an ammonia leak kept nearby residents inside their homes for more than hour – no one was injured in the incident, which started before 4 p.m. at the citrus packinghouse – residents were told to take shelter indoors until about 5:30 p.m., when employees stopped the leak)

CHLORINE Chlorine Dioxide Fumes Send 19 To Blytheville Hospital Sunday Morning (nineteen people working at a steel mill were exposed to chlorine-dioxide – employees and contractors that were taken to a Medical Center had to be decontaminated before being admitted to the hospital – the chemical is used to control bacteria in water treatment systems – nine people were admitted for observation and two were taken to another hospital)

CONSTRUCTION

COLLAPSE FATALITY Fatal Mall Accident Traps Construction Workers in South Africa (a mall under construction collapsed, trapping dozens under the rubble – at least one construction worker died and 26 others were injured – at least 100 search-and-rescue officials were at the scene trying to locate the trapped workers – the mall was reportedly under construction and scheduled to open in the spring)

MOBILE EQUIPMENT FATALITY – COMPACTOR Construction worker dies after being trapped by equipment in Southeast Texas accident (a construction worker, 49, has died after being trapped under some dirt compacting equipment – he was critically injured and died hours later – he was driving a compactor on a pile of dirt when the equipment overturned)

EXCAVATOR Worker lost an eye in horrific work accident (worker lost an eye and part of his scalp during a horrific accident – he was severely injured on his first day working for the demolition firm – he lost his left eye and part of his scalp when he was hit by a falling excavator bucket – he also broke his eye socket, cheekbone, jaw, nose, left collarbone, several ribs and his left leg, as well as suffering a punctured a lung severed nerves on his bottom lip – the excavator was being operated by another worker to break up slabs of concrete and feed it into a crusher – investigations carried out by the HSE following the accident found the company lacked training and supervision in the use of the excavator)

TUNNELING FATALITY Construction accident kills 5 in E China (five workers died and another one was injured in a construction accident – accident happened 19 minutes after midnight when the employees were carrying out tunneling work for water supply – the injured worker is in stable condition – investigation into the cause of the accident is under way)

FATAL FALL from ROOF due to WIND GUST Worker Identified In Fatal Construction Accident (accident happened at around 2 p.m. at the construction site – worker, 23, was in the process of installing metal roofing on the new building, approximately 24 feet off the ground, when a gust of wind blew him and a piece of the roofing off of the roof structure – he was pronounced dead from the injuries he had sustained in the fall)

TRENCHING Update: Quick work by police may have saved workers’ life (fast work by two police officers may have saved the life a construction worker, 40, who was trapped for over three hours in a collapsed trench – workers were extending the sewer line to an assisted-living facility under construction about 200 yards south of the accident site – worker suffered shoulder and leg injuries and was medflighted to a Medical Center)

SCISSOR LIFT Worker injured in scissor lift accident at LAX (a man, 22, operating a scissor lift at the Airport was briefly pinned against a building and sustained minor injuries – incident occurred just after noon at the airport’s Terminal 8 – he was taken to a nearby hospital with back pain and “minor torso trauma” and was listed in good condition)

WORKZONE FATALITY Highway Worker Killed; Another Hurt After Beltway Crash (a crash killed a 25-year veteran of the State Highway Administration and injured another along I-695 – worker, 50, died at the scene – worker, 38, was also hurt in the accident – the driver of the car was taken to a Medical Center because of injuries she sustained in the crash – the car was traveling on the outer loop of I-695 when the car veered off the roadway and onto the shoulder – at that point both worker, who were both working on a roadway sign, were struck)

WORKZONE Two road workers struck by car on Highway 20 in Lachine (two road workers were seriously injured when they were struck by a motorist at 11:35 p.m. where work on the overpass has been taking place – workers, 44 and 47, were from a private company under contract and suffered serious injuries but their lives are not in danger – the 33-year-old driver was arrested on site and will be charged with driving under the influence causing bodily harm)

RIDING in PICKUP BED OSHA launches investigation into Pearland road construction accident (two workers were injured after they were thrown from the back of a moving pickup truck at a construction site – driver of the truck told officers he was headed north when he became distracted and ran into a dirt mound – truck was reportedly traveling fast enough that the truck vaulted the truck into the air – the two workers riding in the open truck bed were thrown from the vehicle during the accident)

CRANE FATALITY Tokyo construction worker killed by falling beam (a construction worker, 57, was killed when a large concrete beam weighing 800 kilograms (1,764 pounds) fell from a 20 meter crane above and crushed him – he was pronounced dead on arrival due to severe head trauma – the concrete beam was simply described as getting loose from the construction crane, although it isn’t clear what he was doing at the time of the accident or why he was in harm’s way under the heavy load)

FATAL FALL from SCAFFOLDING Construction worker dies after falling from scaffold at NYU building in Greenwich Village (worker, in his 40s, was rushed to a Hospital where he died – he had been working on the scaffold outside the sixth floor when he fell about 11:20 a.m. – he landed on the roof of the neighboring two-story building – coworkers said the victim was hired less than three weeks ago – his job was to mix cement and lay bricks)

FALL Worker hurt in roof fall (a man working on the roof of the building fell off while working on the gutters – victim was transported to a Hospital around 9:30 a.m. with “serious injuries”)

BOOM TRUCK into POWERLINES Hundreds without power and one injured after power line accident (a bucket truck containing a roofing worker struck a power line – worker was taken to the hospital with severe burns – no other details)

FALL from ROOF Update: Worker injured at Fort Mill school site in critical condition (a construction worker, 28, at the new Elementary School fell off the school’s roof – he fell from an upper roof onto a lower roof before landing on the ground – he was airlifted to a Medical Center and listed in critical condition)

NAIL GUN Roofer treated after injury with nailer in Bayonne (a worker, 49, injured on the roof accidentally shot himself in the knee using a pneumatic roofing nailer, and was unable to descend due to the severity of the injury – firefighters used a 75 foot Ladder (Bucket) Truck to gain access to the roof of the 2-story building, provided medical attention along with EMS, and secured the worker to a backboard – he was transported to a Medical Center by EMS Ambulance)

CRANE into POWERLINES Worker injured by electric shock when crane hits power lines in Oklahoma City (a crane operator was electrically shocked when his crane hit power lines around 5 p.m. – did not know the injured worker’s condition)

FALL from SCAFFOLDING Worker injured in fall into hole (it took a high-angle rescue to get a construction worker, 22, to the hospital after he fell more than 20 feet from a scaffolding at around 3 p.m. – about 15 rescuers trained in high-angle rescues used a system of ropes to raise the worker on a litter to the waiting ambulance)

TRENCHING FATALITY Asian worker dies in pit collapse (a worker was killed and another was seriously injured while installing connecting pipes inside a pit for a project to extend sewer lines – a part of the pit collapsed on them while they were working and trapped them under the rubble – the incident occurred at about 9 am, when two workers were performing their duties inside the pit – rescuers were able to save the first worker, 35 – he was transferred to hospital to receive medical treatment for his severe injuries – the second worker, 32, was found dead)

Miscellaneous

ELECTRICAL Worker severely burned during construction work at University Hospitals (a man doing construction work while inside the bucket of a lift truck was severally burned around 8:30 a.m. – rescue workers were raised up using a lift truck of their own and got the man down – he was taken to a Medical Center, where he underwent surgery in the afternoon – the victim was 50 years old and had severe burns – it appeared the man was working on a garage at the hospital complex)

FALL from RIDE Deputies ID worker injured after fall at convention center (worker was critically injured after falling 20 feet while setting up a carnival ride – he was working on the ride at an expo at the Convention Center when he fell – unknown why/how he fell – he was taken by ambulance to a Medical Center)

FATAL HOTWORK FIRE Welding spark causes fire at popular Vietnam bar, at least 6 killed (a fire broke out at a popular bar and killed at least six people – the bar is under construction and an initial investigation indicated the blaze began when sparks from welding hit an electric wire – the six victims were rushed to a hospital but were pronounced dead)

HOTWORK No blaze when I left, welder tells inquest into fire deaths (a welder who carried out works at a disused factory shortly before it caught fire said there was “absolutely” no fire when he finished the job and he would go to his grave saying that – he was giving evidence at Coroner’s Court at the combined inquest into the deaths of two firefighters – they died fighting a fire at a disused ink factory – Coroner’s Court heard that the metal fabricator had been asked by his friend to weld shut the main factory door because people had been dumping rubbish in the premises – the two men went to the building to carry out the welding works at 10am on the morning of the fire – no metal sheet was used to prevent sparks escaping while the welding took place – 90% of the welding carried out happened on the outside of the building – He said that when he finished inside he looked around to check and “absolutely nothing” was on fire)
