INDUSTRIAL BURNS Two workers injured in accident at iron plant (two workers, 28 and 30, were injured when hot waste coming out of wet scrubber tower of the kiln fell on them around 11.10am – the hot waste in the kiln got jammed – the victims suddenly opened an outlet resulting in the accident – one sustained around 90% burn injuries but the other worker managed to escape with minor injuries) MINE Worker Hurt in Mine Accident (a worker has been hurt in a mine accident around 11 a.m. – the worker was hit in the back with a cable and was taken to the hospital with non life-threatening injuries – no word on what caused the accident) SUBSTATION Worker airlifted after being crushed at Duke substation in Hildebran (a man was airlifted to the hospital after an accident at a substation – he was working at the substation when he was crushed by a piece of electrical equipment that weighed about 2,000 pounds – he was taken to an Elementary School by ambulance and then flown to a Medical Center with potentially life-threatening injuries – accident was reported around 10:30 a.m.) UPDATE now a FATAL EXPLOSION Worker burned in waste water plant explosion dies (worker, 58, burned in waste water plant explosion dies – he was either welding or getting ready to weld in a tank at a wastewater treatment plant – something exploded, burning him over 96 percent of his body) COMPRESSED AIR Two hurt in accident at Tower Automotive (two workers, 60 and 61, suffered injuries when an air line they were working on separated at 8:05 a.m. – it appears they were working on a large, high-pressure air line that ended up separating – both men were taken to a Hospital by Ambulance and later both were flown to out-of-area hospitals for treatment of their injuries – no immediate word of their conditions) FLASH FIRE Worker injured in flash fire at Autoliv plant (a worker, 29, was injured after a small explosion on the roof of a plant – the incident happened about 10:30 a.m. – the man was working on an air conditioner on the roof when a flash fire erupted and burned his arms and face – he was airlifted to a hospital and is expected to survive) OIL WELL EXPLOSION Oil well explosion puts five workers in hospital with burns (five workers have been injured in an explosion at an oil well – the workers suffered burns and were taken to a Hospital – the explosion caused a small fire, which emergency responders quickly put out – the explosion also damaged a pickup truck and a portable petroleum-chemical storage tank – no residences needed to be evacuated) UPDATE Worker crushed when forklift used instead of ladder (business has been fined more than $67,000 after an employee, 33, was seriously injured while being raised on a forklift to close a faulty roller door – he was crushed when he got trapped between the roller door and forklift backrest – he spent a week in hospital with injuries to his ribs, back, right shoulder, chest, stomach and lungs – he was unable to work after the accident – court heard the motorised roller door at the warehouse was faulty and employees sometimes were lifted by a forklift to reach the door when opening and closing it – employee operating the forklift on the afternoon of the accident did not have a valid license and lifted the other worker too high, causing him to be crushed against the door) SANITATION FATALITY Sanitation Worker Killed on the Job in Oyster Bay (a sanitation worker, 45, died after a car struck him – he had just finished emptying trash containers into the truck’s compactor just after 7 a.m. when he was struck – he was walking along the driver’s side of the truck when he was struck – he was transported to a local hospital with severe head trauma but was pronounced dead shortly after arrival) EXPLOSION Man hospitalized after small chemical explosion (an employee was hospitalized after a small chemical explosion – a chemical leak caused the small explosion – one worker was taken to a Hospital with burns – condition remains unknown) HOTWORK Fire at sewage plant causes $400,000 in damage (four area fire departments responded after a water filter caught fire at a sewage treatment plant – the fire broke out just before 10 a.m. when a worker, 45, was welding the filter at the sewer plant) HOTWORK Cause of fire at Trane Company released (Fire Department said the fire was started with a torch during demolition work at the facility about 6:50a.m. – a contractor was doing demolition work with a torch when he started some insulation on fire – about 30-employees had to evacuate the building – everyone made it out safely) REFINERY FIRE St. Paul Park Refinery Assesses Damage from Fire (refinery is assessing damage from a fire – the fire started in a crude oil pump around 11:30 a.m. – firefighters from the refinery and city extinguished the fire by 2:30 p.m. – preliminary estimates show that the outage at the unit will last about three to four weeks, and repair costs are expected to be no more than $10 million) CHEMICAL Roads blocked due to reported leak at BP refinery (roads were blocked off after a reported leak at a refinery around 11:18 a.m. – authorities have not said what kind of leak or chemical release) REFINERY FIRE Marathon puts out fire at Galveston Bay refinery (the fire in the 451,000 bpd refinery’s hydrotreater unit burned for about a half-hour – no injuries were reported) Rensselaer Falls man hospitalized after industrial accident (an industrial accident sent the owner, 61, of a sand and gravel company to a Medical University for treatment of what hospital officials called critical injuries – he was trapped in a front loader he was operating when a screening plant collapsed on it – he was loading stone and gravel and a piece of equipment that separates stone and sand tipped onto the cabin of the loader and trapped the victim) TANK OVERPRESSURE EVENT Explosion rocks Oxford roofing plant, no one injured (an over-pressurized tank is to blame for an explosion at a roofing plant – an over-pressurization asphalt conversion tank caused an explosion at the plant around 10:15 a.m. – there was no fire – it was not clear why the tank was over pressurized – there were 125 working at the time of the explosion, and no one was injured) EXPLOSION Many Feel Blast From Explosion At W. Va. Gas Plant (officials are investigating after an explosion at a natural gas processing plant – explosion happened around 1:20 a.m. – no employees were injured – the fire was confined to a small area and allowed to burn itself out – they did shut down roads and evacuate homes and businesses as a precaution – company is still trying to determine the cause of the fire – evacuees have returned home) EXPLOSION Fire Destroys Oklahoma Chemical Plant,Claims No Life (a fire destroyed a chemical plant and caused small explosions forcing the evacuation of about a dozen people from nearby homes, but resulting in no injuries – the fire erupted about 10:00 p.m. about three hours after the last employees left the facility – facility had nontoxic but highly flammable chemicals that are used in the oil and gas production industry – 10 to 12 people were evacuated from homes near the facility as a precaution – authorities are trying to determine the cause of the fire) AMMONIA Ammonia leak at ice plant traced to valve left open (an ammonia leak that forced residents out of their homes has been blamed on a valve left open at an ice-making plant – the leak was discovered around 8 p.m. at the ice plant, the same building that was the scene of a fire in May that also forced some people from their homes – a valve was inadvertently left open by workers at the plant, which is in the process of being decommissioned – “Our Hazmat team shut it off but it took a while to finalize” – “There are so many valves and lines, and they were trying to figure out which ones (the workers) were working on” – the leak forced the evacuation of four city blocks for several hours, but no injuries were reported – the all-clear was given just before midnight) HYDROSTATIC RELIEF VALVE Mechanical failure may be cause of gas leak at Jackson Co. plant (a gas leak is being blamed on a mechanical failure – a pipe started leaking Anhydrous Ammonia around 8:45 AM – no one was injured – it took county firefighters and environmental crews about an hour to contain the small leak – leak originated from liquid Anhydrous Ammonia leaking from a transfer line that linked a 30,000 gallon tank to a rail car – they suspect a relief valve on the pipe malfunctioned – officials believe it was due to a mechanical failure because the pressure in the system, whenever it builds up too high, it’ll release, but the pressure on the gauge did not indicate that the pressure was high enough for release) Maintenance Leads to Ammonia Leak at John Deere Plant (Fire Department responded around 10:15 p.m. to the Product Engineering Center for a report of an ammonia leak – workers evacuated after the leak started while performing maintenance – the spill was contained and cleaned up by 11:30 – there were no injuries reported as a result of the spill) AMMONIA Six hospitalized by ammonia leak at Grantham factory (an investigation has been launched after six factory workers were taken to hospital following a gas leak – a small ammonia leak from a refrigeration pipe occurred – leak was traced and fixed within two hours of being discovered) FORKLIFT Fork lift falls on man in industrial accident (a man reportedly suffered a chest injury when a forklift fell on him – responders were called at about 5:35 p.m. – the man hurt in the industrial accident is reportedly 20 years old) FORKLIFT Metal object pierces man’s knee in forklift accident (a piece of metal was extracted from a worker’s, 36, knee by specialized firefighters after a work accident about 8:30 a.m. after a man on a forklift was impaled by a metal object – employee was driving a forklift that was hauling a trailer when, for an unknown reason, the trailer broke free) PALLET JACK Crews free man following forklift accident on city’s west side (a man suffered serious injuries after becoming trapped under a forklift around 6 p.m. – the man was rolling a pallet of goods from inside a semi truck onto a lift gate when the load shifted, knocking him off the lift gate – the man’s left foot became trapped beneath the load, leaving him hanging upside down – responders were able to free the man after about 30 minutes – he was taken to a Hospital with a serious foot injury) CONSTRUCTION WORKZONE – EXCAVATOR Worker injured in excavator incident in Kilbride (a construction worker has been taken to hospital with apparent lower leg and foot injuries after he was struck by an excavator doing roadwork just before 1 p.m. as workers were doing street repairs in the area – worker apparently had his foot and lower leg pinned under the excavator – the man, who appeared in his mid-20s, was conscious and alert, speaking to emergency officials) Worker pinned between water truck & street sweeper (a construction worker became pinned between a water truck and a street sweeper – no other details) TRENCH FATALITY Montreal construction worker dies after work-site accident (a construction worker, 34, was buried in a landslide of dirt, wood and paving stone died of his injuries – accident happened at 10 a.m. at a residential construction site where the construction crew were excavating – a retaining wall built to contain the dirt on the steeply banked hill at the back of the site gave way and buried the worker – co-workers dug him out and pulled him free – he was rushed to a hospital in critical condition) TRENCH FATALITY Worker Killed in Accident at Sewer Line Construction Site in Middletown (a worker at the construction site of a sewer line was killed – he initially was stuck in around 3 feet of dirt – but when an inspector returned after making a 911 call he was found buried in dirt – rescuers showed up within minutes but when they reached the worker he was dead) EXCAVATOR FATALITY Newark man killed by earth mover in Vallejo construction accident (a worker, 48, was struck and killed by a large earth-mover at a construction site – he had been part of a surveying team, which was sharing the site with workers who were grading it for the eventual construction of three homes – “One man was working with a surveyor (equipment) and the other had the stick they use as a marker, and this big Cat scraper came around and as he started to make the turn, he didn’t see the person with the marker, and struck and killed him”. “He looked back and saw someone lying on the ground, so, he stopped and jumped down to check on him and called for an ambulance.”) TRENCH Rochester construction worker in fair condition after trench accident (a construction worker, 30, is in fair condition after falling into a trench at about 4:15 p.m. – a pile of dirt became unstable causing the worker to fall into the trench – he became trapped under the dirt but co-workers were able to quickly get him out of the hole) TRUSS COLLAPSE Three workers injured when trusses collapse at Timberwood Church (three employees remained hospitalized after an early morning construction accident – the workers suffered injuries when a series of trusses collapsed at the site – the workers were listed in fair condition – a series of trusses, perhaps a dozen, collapsed at the site where an addition is being built to the church) EXCAVATOR FATALITY Construction accident kills worker in Bethesda (a construction worker, 42, died when an excavator toppled over, pinning him inside around 8:35 a.m. – a construction crew was working at a home there when an accident caused one worker to become pinned between an excavator and the house – two workers freed the trapped worker; however, he died following the accident – the two other workers suffered non-life-threatening injuries) UPDATE on FATAL PARKING GARAGE COLLAPSE Westfield Montgomery Mall construction accident due to malfunctioning jack (the failure of a jack assembly led to the collapse of a portion of a mall parking garage that killed one worker and seriously injured another – investigators say that that the scaffold legs of a jacking tower at the construction site where the accident happened were not being used properly, causing it and a portion of the garage to come down – worker, 57, was killed in the accident and another worker suffered serious injuries) FALL from SCAFFOLD Scaffold accident leaves two workers injured in Phoenix (two men working on a home under construction have been injured after falling from a scaffold – the men were working at a height of about three stories when the scaffold either slipped or collapsed – the two men fell to the ground and appeared to have a number of injuries, but no apparent broken bones or life-threatening injuries – both men were conscious and talking at the scene) FATAL FALL from SCAFFOLDING Student worker dies after fall (a construction worker, 60, died after falling from scaffolding into a barrel – he fell backwards off the scaffolding about 8 feet into a 55-gallon barrel – emergency personnel were dispatched at 8:35 a.m. and cardio-pulmonary resuscitation was begun before the man was transported by Ambulance to a Medical Center) WORKZONE FATALITY Road worker killed as driver adjusts car radio (a man who has admitted to hitting and killing a traffic worker with his utility in February has apologized to the victim’s family through his lawyer – he pleaded guilty to causing death by negligent driving and is due to be sentenced next week – he was adjusting the radio in his ute and did not see the worker, who was holding a stop sign – he was travelling about 53 kilometres an hour in a 40 kph zone – the worker, 62, died in the Hospital from head and chest injuries later in the evening) CONFINED SPACE FATALITYChicago storm sweeps construction worker to his death (a construction worker, 25, who was in a sewer died when he was swept away by a flash flood as a thunderstorm pelted the city – he was a private contractor working to reline a sewer – he went missing around 8:30 p.m., right as torrential rain hammered the region – his body was recovered several hours later – the storm dumped up to three inches of rain on the city and pounded some areas with hail) CRANE Crane Topples Through Roof Of McDonald’s Restaurant In Consett (a crane collapsed on to a McDonald’s restaurant – contractors working on a new drive-through area were lowering a new fridge on to the site when the crane toppled – the main part of the restaurant, which was open to the public with a number of customers inside, was evacuated immediately afterwards – no-one was hurt in the accident, which happened just after 1pm) CRANE FATALITIES 2 die in crane collapse (the accident took place at around 1:30 am – two construction workers, both 25, were killed when a crane collapsed while moving embroidery machines from a multi-story building – a crane collapsed after its rope snapped while it was transporting embroidery machines from the 10th floor of the 13 story, leaving the two workers critically injured – they were rushed to a Hospital where on worker was declared dead upon arrival and the other died at around 3 am) Miscellaneous FARMING FATALITY Bayham man killed while harvesting tobacco (a farm worker, 54, died while running an automatic tobacco harvester – he was found in the middle of a field underneath a tire of the machine and taken to a hospital where he died – he was operating the vehicle by himself) EXCAVATION GAS LINE EXPLOSION Gas explosion injures one (a gas line explosion left one city employee injured – the water department was changing water meters out when one of their employees struck a residential gas line while digging – the employee digging suffered some minor injures including singed nostrils and eyebrows – he was taken by a city truck to a Medical Center where his condition is unknown at this time) EXCAVATION and ELECTRICAL Workers doing landscaping sustain injuries after explosion on Clematis Street (two city workers who were doing landscaping sustained injuries after they were shocked – two workers were digging a hole and hit a live power line, blowing a transformer, setting off an explosion and a small fire – investigators are trying to determine whether proper procedures were followed – before digging, workers are supposed to call 811 so people can come in and set up flags on the ground to mark where certain electrical lines are located to prevent shocks – it is not clear if those procedures were followed – the workers were in “stable” condition) HOTWORK Preliminary cause of 5400 sq meter fire at Almaty flear market unveiled (preliminary conclusions on the causes of the massive fire at a flea market cites welding works being the cause of the fire – the welding was done in one of the containers) LABORATORY Workers evacuate Lancaster lab after chemical leak (a hazardous materials team responded when compressed ammonia gas leaked from a container in a laboratory – about 6-7 pounds of ammonia gas leaked from the container just before noon – workers left the container under a venting hood and evacuated the lab
