INDUSTRIAL FATALITY UPDATE Worker killed in accident at Wrigley plant (worker, 54, was killed in an accident – she has been with the company for 1 1/2 years – she sustained life threatening injuries from a work-related accident at the plant and later succumbed to those injuries and was pronounced as deceased at the scene – no details about the accident) POWER PLANT FATALITY Worker killed in accident at Comanche Generating Station in Pueblo (worker, 25, died in an industrial accident – workers were moving a large transformer when the contractor got caught up in the equipment and was crushed in the moving operation) TRUCK UNLOADING FATALITY Deceased Worker Identified in Fatal Farmington Industrial Accident (a bundle of aluminum being removed from a truck fell on top of the driver, 65 – he received a serious head injury and was transported to a Hospital where he later died) TRUCK MAINTENANCE FATALITY Worker Crushed By Large Trailer At Local Facility Dies (worker died after he was crushed by a large trailer while performing routine maintenance on a vehicle – it appeared that a mechanic was repairing the brake system on a commercial truck and trailer, when the safety devices failed, causing the trailer to back over the mechanic) FATAL PRESSURE TEST EXPLOSION Worker Killed in Carol Stream Factory Explosion (worker, 35, was testing a 200 gallon air tank for leaks just after 11 p.m. when the container exploded – another worker had minor injuries but refused treatment – there was no smoke or fire associated with the blast but there was structural damage to the facility) FORKLIFT FATALITY Death of worker at Harford plumbing distribution center under investigation (worker, 33, suffered life-threatening injuries from a fall around 11:20 a.m. – he had been operating a forklift in the distribution center – another forklift “bumped” into his forklift, and he fell backward about 20 feet to the floor – he was pronounced dead at the scene) Forestry worker hurt in Rai Valley accident (a forestry worker, in his 50s, was airlifted to hospital after becoming trapped between two trees – he had moderate injuries to his lower abdomen – he had been felling pine trees when he got trapped between two of them) MINE FATALITY Worker Killed in Freeport Indonesia Mine Accident (a construction worker, 27, died in an accident at a mining complex after being hit by a truck in an underground mine) FATAL FALL @ PAPER MILL Worker dies from fall at Natchitoches Parish paper mill (worker, 57, has died of his injuries after a fall while working at a paper mill around 11 a.m. – worker fell 15 feet suffering serious injuries – he was airlifted to a hospital where he died – no details on what he was doing or what he fell from have been released) AMPUTATION BlueScope Steel worker’s hand partially amputated in industrial accident (worker, 46, was airlifted to hospital after partially amputating his hand – the male worker caught his hand in machinery – he had been freed from the machine and relocated within the steelworks – he had a crush injury to his right hand with fairly significant damage – it was partially amputated – it is believed the man received the injury while trying to retrieve an item that had become stuck in a roller) TREE CUTTING FATALITY Utility worker killed by tree (worker, 27, died clearing trees in the power-line right-of-way at approximately 2:30 p.m. – worker died at the scene) WWTP TANK COLLAPSE 10 workers killed as effluent tank collapses in Ranipet (ten workers were killed when a tank of an effluent treatment plant collapsed on the compound wall of an adjacent factory where they were sleeping – one of the two tanks of the common effluent treatment plant collapsed on the compound wall – nine workers and a watchman, who were sleeping near the compound wall, were buried alive in the sludge – one worker who was awake when the tank collapsed, managed to run to safety) FATAL EXPLOSION Fatal blast at Indian chemical plant (one worker was killed and a further four injured after an explosion at a chemical manufacturing plant – the incident was caused by a short-circuit in a scale being used to weigh a barrel of 4-hydroxybenzohydrazide – all four of the injured workers have now returned to work) ELECTROCUTION ZESCO WORKER ELECTROCUTED (worker, 41, a lines man, died after his body was said to have stuck to the transformer after he was electrocuted – crew was installing a transformer – in the process, he was electrocuted while on top of the transformer and he died on the spot) ARC FLASH Two hurt in American Refining Group accident in Bradford (two employees were injured when an electrical arc flash occurred as they were working on electrical equipment – the incident occurred at approximately 6:30 a.m. in the motor control center of the crude unit – two male employees were working in the facility, which is undergoing expansion due to growing electrical requirements at the refinery – after the arc flash, the employee with less severe injuries was able to radio for assistance – one individual was treated and released later that morning, while the other individual remained hospitalized with non-life-threatening injuries) EXPLOSION Investigators probe blast, fire at Wellsville insulation plant (fire started in the mulch area of the plant with an explosion – there were 10 people working the night shift when it happened, and all of them got out safely) ACID TANK RUPTURE Tank ruptures at Hopewell Honeywell plant; no injuries (a tank ruptured during cleaning – a tank which was not in service was being cleaned for inspection when the tank became over pressurized and the top ruptured – the tank, which normally contains a type of sulfuric acid, was being cleaned with water – rupture did not cause a fire or explosion – no injuries were reported) AMMONIA Hazmat Teams Respond To Ammonia Leak In Frankford (a 1,000 gallon tank was leaking a small amount ammonia – no evacuations or injuries were reported – crews closed roads surrounding the plant – leak was brought under control a short time later – no other details) AMMONIA Ammonia leak at Speedway Coca Cola plant prompts evacuation; shutdown (an ammonia leak at a bottling plant forced employees to evacuate, and prompted a significant emergency response by Fire Department HAZMAT teams – shortly after 10 p.m. , 25 workers in the plant noticed a strong ammonia smell, called 9-1-1, and immediately evacuated the facility – two HAZMAT teams identified an 80,000 gallon tank as the source of the ammonia leak – ammonia is used in the refrigeration processes at the plant – readings eventually dropped from 700 parts per million (ppm) to about 150 ppm) CHLORINE Waxahachie chemical-blending plant evacuated after chlorine spill (a chemical-blending plant in was evacuated after a chlorine spill was reported around 2:20 p.m.) CONSTRUCTION ELEVATOR FATALITY OSHA to investigate after worker, 45, crushed in construction accident (worker, 45, was crushed in a construction accident at about 3 p.m. – he as about six floors, or 60 feet, above ground and was working inside a crane during the construction of a high-rise residential building – he was leaning into a shaft where a cage, or “elevator,” was descending, pinning him – workers were able to free him before additional emergency crews arrived – he was taken in critical condition with injuries to his upper body to a hospital, where he later died) AERIAL LIFT FATALITY Plainfield construction worker dies in Wilmington accident (construction worker, 33, has died after being pinned between a beam and a lift – he was in the lift he was working in at the time – workers performed CPR until firefighters arrived – he was taken to a hospital where he was pronounced dead) CONCRETE FORM Construction Worker Hospitalized Following K-State Accident (one person was transported to a local hospital after a construction accident just after 10:30 a.m.- a concrete form fell on a worker – worker was transported to a Regional Health Center with what are described as ‘non-life threatening injuries’) AERIAL LIFT FATALITY Construction Worker Killed in Industrial Accident, Another Injured (construction worker, 26, is dead and another is recovering following an industrial accident around 7:45 a.m. – police found a worker with life threatening injuries when they got to the scene – he was taken to a Hospital where he later died – another employee received minor injuries and went to the hospital via private vehicle – the two men were demolishing a brick wall with sledge hammers while standing on a scissor lift – that’s when the brick wall collapsed, causing the lift to fall – the deceased fell out of the lift and hit his head on the ground) BACKHOE FATALITY Worker Killed in Accident While Working on Galleria Oaks (construction worker, 61, was killed when he was struck by a backhoe attachment on a work site – worker was trapped between a backhoe attachment and the tractor it was attached to – initial investigation concluded that he was standing behind the tractor when the backhoe swung around and struck him, killing him instantly) CRANE Crane topples at Carteret construction site leaving one worker injured (construction site accident left one worker dangling from a toppled crane – crane toppled over hitting a cell phone tower on its way down – worker was expelled from the crane in the accident, and was left hanging approximately 20 to 25 feet above the ground – the worker was still dangling from his harness when emergency personnel arrived at the scene – he suffered head injuries and a broken leg, but was conscious and responsive) HEAVY EQUIPMENT FATALITY Worker killed when pinned between tractor and dozer in Ohio (worker, 40, attempting to pull a piece of equipment out of deep mud was killed when the stuck tractor suddenly lunged forward and pinned him between the tractor and a dozer – he was between the pieces of equipment hooking up a chain so the tractor could be pulled from the mud with the dozer) WORKZONE Road worker struck on Y Felinheli bypass by Gwynedd Council truck (worker suffered serious injuries in the accident which partially shut the road for more than two hours – worker was rushed to hospital with serious injuries after being struck by a lorry while doing maintenance work at roughly 7.40am – he was performing repair work on the road when the council lorry struck him) Construction worker injured at VMI Cormack Field House (construction worker with a HVAC company was injured while working on a renovation project – worker was in critical condition) TRENCH Trench collapses in Castaic, briefly traps worker (worker had to be extricated from a collapsed trench – situation was not life-threatening – the trench collapsed pinning the worker’s feet and part of his legs – worker was taken to a hospital, but only for purposes of evaluation) CRANE FATALITY Worker killed after piece of crane fell on top of him on Chicago jobsite (part of a crane fell atop a worker – workers found a co-worker that had become pinned by part of a crane – workers were able to free the man who was unresponsive when emergency personnel arrived to the site – paramedics spent several minutes treating the man before transporting him to a nearby hospital where he was pronounced dead – the man was working about six stories up on a 24-story luxury condominium when a part of the crane fell atop his chest) HOTWORK ASU football stadium fire caused by construction (construction work going on at the press box resulted in the fire – roof of the former press box had a plywood floor down and they were up there welding and cutting it looked like and it caught the plywood on fire) MISCELLANEOUS MOVIE SET FATALITY Accident kills worker on Taiwan set of Martin Scorsese film (worker was killed in an accident on a movie set – two others were injured – worker died when the ceiling collapsed in an 83-year-old structure on the set – eight workers were preparing to fortify the structure when the incident occurred) STRUCK BY MONORAIL Newark Airport worker struck by monorail train, suffers leg injury, authorities say (mechanical worker, 51, was injured after being struck by a monorail train – he was working on the AirTrain track at about 11:10 a.m., when he was struck by a moving AirTrain – due to the impact, the worker was knocked down to the lower catwalk portion of the structure – worker suffered a non-life-threatening injury to his left leg and was conscious) TIRE FATALITY WorkSafeBC Investigates Fatal Accident at Kamloops Fountain Tire (worker was reportedly fatally injured while on the job at a tire business – worker was injured between a vehicle and a tire – what exactly happened or how it happened isn’t yet known) LPG EXPLOSION Seven hurt by explosion at LPG station (seven technicians were injured by a gas explosion when they were carrying out an annual inspection of valves at an LPG service station – one was reported to have been seriously burned) FURNACE EXPLOSION Plant worker suffers minor burns in Tuesday morning explosion
