rotor at the time- no other details) Great Dane Worker Injured in Accident (assembly line worker, 23, was seriously injured while moving a trailer within the plant – worker was treated at a nearby hospital and then later transported to an Indianapolis hospital) FATAL FALL UPDATE: Accident kills man at Old Town Mill (worker, 54, lost his balance and fell about 25 feet into the large vat that holds wood chips – nobody witnessed the accident, but workers found his body shortly after it happened – he was not a mill employee, but a driver for a Trucking company) Injured Monsanto worker recovering (contract worker is recovering after suffering a serious leg injury – he was a member of the contract hydro blasting crew and was injured when a pipe on which they were performing maintenance shifted and trapped his leg) Cal-OSHA investigating Mare Island accident (machine shop employee was taken to a trauma center after being crushed by a steel I-beam that fell from its cradle just before 11 a.m. – the beam fell from its cradle and hit the worker in the legs) Worker, 25, suffers minor injuries after industrial accident (worker, 25, was injured after an industrial accident inside a Steel Plant’s water treatment unit – a contract worker was cutting a tree inside the Water Treatment Plant when a big branch (cut) fell down on him – he died on the spot) CRUSHING FATALITY Employee dies after Tamuning workplace accident (worker, 27, was pinned by granite slabs that they were apparently offloading – the site was slippery when the worker lost balance trying to unload the container and four pieces of granite fell) FORKLIFT Man’s Injuries Minor In Tecnocap Accident in Glen Dale (a workplace accident just before noon took one worker to a Hospital with minor injuries – the victim was a vendor for Lift Truck contractor who was performing maintenance on a forklift when he was injured in an as-yet undetermined accident at the plant site) FATAL FALL Worker dies in Ctg ship breaking yard accident (worker, 22, died after falling from a scrap ship in a ship breaking yard) CONFINED SPACE FLASH FIRE Man severely burned in explosion at Lowell gas station (an explosion at a gas station severely burned a worker, 50, who was repairing an underground pump – worker suffered serious burns on his head, face, arms, hands, and legs – he was working on a pump in an underground hole at around 10:30 a.m. when the explosion occurred – the hole had been cleared and gas had been shut off to the pump, but there may have been residual gas vapors in the hole – vapors ignited when the man started cutting pipe, causing an explosion that engulfed him in flames – his co-worker, who had been outside of the hole, pulled him out – no one else was injured, as the explosion was contained within the hole) Worker burned at Shell refinery in southern Ontario (employee is in stable condition after he was burned – employee had been performing a routine activity at one of the units – no other details) FALL from LADDER Man sent to hospital with head injury after industrial accident along Caledon border (industrial accident at a train yard around 11 a.m. has sent one man to a hospital – initial reports indicate a worker sustained serious head injuries after falling off of an eight-foot ladder onto a forklift – his injuries have been determined to not be life-threatening) COAL BUNKER COM DUST EXPLOSION Northern Power Station explosion hospitalises worker in South Australia (worker, 24, remains in the hospital after sustaining burns in an explosion at a Power Station – employee was airlifted for treatment of burns to his face and hands and is in a stable condition – two firefighters sustained minor injuries while tackling the blaze, which started in a coal bunker and sparked the explosion) FATAL BOILER EXPLOSION Rice mill worker killed in boiler blast (worker, 35, was killed and two others were injured in a boiler explosion at a rice mill – no word on cause) HOTWORK Welders Start Fire at RV Storage Facility (welders installing a vault triggered a blaze – the fire was caused by welders who conducting some sort of operation on the vault – no injuries were reported) FATAL WELDING EXPLOSION Welder killed as lorry explodes (gas welding worker, 34, was killed after the oil tanker he was working on exploded – police said pressure built inside the tank, causing it to burst – he was working on an oil tanker which had developed an overhead crack – suddenly the tank exploded after the fire from the welding equipment created a pressure inside it and finally giving way – welder was killed on the spot) EXPLOSION Explosion causes fire at chemical plant in Pennsylvania (an explosion and fire were reported shortly before 11:30 p.m. – a shelter-in-place order was issued, but it was lifted after officials decided air quality was acceptable and there was no danger to residents – fire was extinguished about two hours after it began – there were no injuries) COM DUST EXPLOSION Small explosion, fire at Prairie du Chien manufacturing plant (fire and small explosion around 11 a.m. began at the bag house – building suffered minor damage and there were no injuries – cause of the fire is under investigation) AMMONIA UPDATE: West State Now Open-Ammonia Leak Cleared (there were no injuries in the leak – several houses in the area were evacuated – a large plum of ammonia could be seen in the area around 11am) CHLORINE Chlorine leak injures five at Apple data center (five employees were taken to a local hospital for treatment – workers could have been exposed to fumes after the chemical, used to clean its cooling systems, was spilled inside the building – all of the employees have since been discharged from the hospital) FORKLIFT Man Injured in Fork Lift Accident (worker was sitting on the forklift using it to start unloading the gravel on the hill when his load shifted – as the man was attempting to go down the hill when the heavy load toppled the forklift forward – driver inside wasn’t properly wearing his seat belt so he left his seat and suffered a serious head injury – as police investigated they discovered the driver actually fastened the seat belt around the back of the forklift’s chair – if he had been wearing it they don’t think he would have been injured) CONSTRUCTION Worker Killed in Museum of African American History Accident ID’d (construction worker, 35, killed while working on metal panels – he was critically injured and died later that afternoon – it is not immediately clear how he was injured – no other workers were hurt) WORKZONE FATALITY ODOT worker killed in Grady County after being struck by vehicle (DOT worker, 47, was killed after being hit by vehicle in work zone – he was a transportation equipment operator and had been with ODOT since May – crews were weed-eating when a vehicle left the roadway and hit the worker – OHP is still trying to figure out what caused a driver to veer off the road) AERIAL LIFT FATALITY Construction worker killed while working on overpass project (construction worker, 35, killed while working on repairs to an overpass – he was apparently pinned by a hydraulic lift against the roof of the bridge overpass at about 8:20 a.m. – he was pronounced dead at the scene) WALL COLLAPSE FATALITY Worker killed in Orangeburg by falling concrete block (worker, 70, was killed when a part of a wall he was working on fell on him when he and another man were taking down a wall at a closed car wash – witness said he was hit by a concrete block when the wall fell in the wrong direction – he died at the scene) CRANE FATALITY UPDATED: Construction worker dies in Hammondsport accident (construction worker, 50, died in a work-zone accident – an employee of a Paving company was found dead at the scene – a large steel beam broke free from a clamp and when it broke free, it struck her in the back and hit her in the head – workers were using a crane to hammer steel beams into the ground) WORKZONE FATALITY Pregnant road worker killed by truck (pregnant worker, 33, working on road construction died in a truck accident – the accident occurred just before 10:00 am – she was flagging vehicles away from the construction when the accident occurred) FALL from SCAFFOLDING Maintenance worker falls into water tower, seriously injured (worker doing maintenance on a water tower was seriously injured when he fell into the tower around 5:45 p.m. – he was employed by the primary contractor on the maintenance project, which involved cleaning, repair and painting the tower – he fell from one platform to another inside the tower, which could have been up to a 70-foot fall) ROLLOVER Injured INDOT worker airlifted to hospital (DOT worker, 24, was injured in a construction accident is in stable condition – he was driving an asphalt roller in a parking lot at approximately 3 p.m. – when the roller reached the soft shoulder of the lot, it became unstable and rolled into a drainage ditch, flipping over in the process – worker was pinned under the roller) GRASS CUTTING Mahoning County worker injured mowing grass in Austintown (County worker mowing grass was struck by an SUV – it went out of control around 8:30 a.m. and ran into a tractor being driven by a county employee – SUV then struck a nearby pickup truck – injuries do not appear to be life threatening) WORKZONE FATALITY Construction worker hit by vehicle on Oregon 22 in Salem; highway closed (construction worker struck and killed – driver accused of DUI – incident occurred just after midnight) CRANE Crane drops huge air conditioning unit down 30 stories in Midtown, damaging building and injuring 10 people (crane hauling an air conditioner to the top floor of a 30-story building dropped the heavy load smashing parts of the building and injuring 10 people – workers were loading the shipping-container-sized machine into a mechanical room when a rigging strap broke – the conditioner bashed a hole near the roof, then sheared off chunks of the building as it fell at 10:43 a.m. – 10 people were hurt, two of them construction workers, but the injuries were not serious) EXCAVATION EXPLOSION Repairs made to gas leak that sparked fire in Flint (gas leak has been fixed that sparked a large fire, injured two men and closed stretches of a road and freeway – a worker struck an underground gas pipe while working on a bridge reconstruction project – leaking gas caused an explosion and fire that burned for a couple hours – two motorists suffered burns during was checking ear tags on cattle when he was run over by a feed trailer) Police name worker killed at tow shop in Boardman (worker, 48, was under a pick-up truck trying to unlock the transmission when a shift in weight caused the vehicle to fall on him) ZOO Komodo dragon bites Omaha zoo worker (zoo worker was bitten by a Komodo dragon and treated at a local hospital – worker had been released and should recover fully – worker needed a couple stitches to close up the wound on her hand – the animal is about 4-feet-long and weighs about 10 pounds) ELECTROCUTION Worker dies after driving into 11,000 volt cable (driver, 27, of the tipper truck died after he tried to leave the vehicle when it struck overhead cables – the tipper bed was up when it collided with the high-voltage overhead cable – the driver was said to be initially unhurt, but was killed when he tried to get out of the cab while the tipper was still in contact with the cable) ARC FLASH Durban hospital investigates explosion that left one person injured (hospital is investigating the cause of a minor explosion at the facility where a man sustained burn wounds – explosion was the result of a short circuit that occurred on
