FATALITY Man killed in Colonie industrial accident (worker, 61, was killed in an industrial accident – he was alone at the business when he was pinned between two pieces of equipment and died instantly from severe trauma to the head – person who was familiar with the victim stopped by the business to see him and found him pinned)
ANHYDROUS AMMONIA UNLOADING Worker life-flighted to Fargo following industrial accident (truck driver, 43, has been life-flighted following an industrial accident at approximately 9:40 a.m. – he was unloading hydrous ammonia from his semi truck, “As he took a safety cap off a coupler, it sprayed” – the anhydrous ammonia sprayed him in his facial area resulting in a chemical burn)

PRCS FATALITIES – 5 Would-be Rescuers, Ammonia TankFive killed in ammonia tank accident in Sri Lanka rubber factory (five people were killed when a worker at a rubber factory fell into a tank of ammonia and colleagues, and residents died trying to save him – would-be rescuers were overcome by fumes as they tried to help the worker and they too fell into the tank, which contained liquid ammonia – 18 people had been admitted, of whom five had died)
West Longview industrial worker injured Friday afternoon (worker was seriously injured in an industrial accident at about 2:50 p.m. when he was struck by a large piece of equipment – no other details)
FATALITY TOSHA investigating death of worker at Bush Brothers’ facility (female worker has died – authorities have not said how she was killed – she was a contractor at the plant but had been working there for several months)
FATALITY Worker killed at Bonnell (worker, 45, died Wednesday of injuries sustained in an accident at the plant – he was injured by a multi-ton fire door around 9 a.m. at the aluminum extrusion plant – he was airlifted to a Hospital where he died – second major incident at the plant in the past few years – June 2016, an explosion in the casting house rocked the plant, causing some injuries but no fatalities – in 2004, a 49-year-old maintenance employee was killed at the plant when he was crushed by the arm gripper of an extrusion press)
Employee hospitalized after accident at Voestalpine plant (worker at the iron ore processing plant was injured in a work-related accident – coworkers called 911, and the victim was taken to the hospital – employee was hospitalized and is currently recovering from minor injuries)
LOTO/GUARDING FATALITYWorker killed in accident at Bush Brothers (a temporary worker, 40, was killed in an accident involving a lift used to hoist heavy pallets of canned goods – she had been working at the plant for about six months through a temporary agency – victim apparently died instantly in the accident – she was cleaning in the area of the machine when she accidentally set off a sensor that caused it to lift, wedging her between the machine and a metal post)
OPP investigate fatal industrial accident at PSI on Walker Road in Tecumseh (worker, 24, was killed and a second man injured in an industrial accident at a concrete manufacturer around 6:30 a.m. – the other man’s injuries as serious, but added he is expected to survive)

UPDATE on FATAL COM DUST EXPLOSION Jury awards $33.1 million to burned plywood worker (a jury awarded $33.1 million to a worker who was badly burned in an explosion at a plywood plant – the plant supervisor survived the blast and fire that killed two of his co-workers in 2014 – jury assigned 51 percent of the blame to the company that designed the plant’s dust collection system, and 26 percent of the blame to the maker of the spark detection system, according to the jury verdict – the plywood plant was found to be 23 percent responsible in the blast – legal claims against the employer were resolved prior to trial for a confidential amount)

FORKLIFT FATALITY Worker crushed by forklift in Dolores (worker, 38, died after being crushed by a forklift while he was working in a fruit and vegetable company – Paramedics attended the incident but despite their efforts he was eventually pronounced dead at the scene – the forklift truck used to lift pallets and boxes was destabilized and fell on top of the worker)
FORKLIFT Mackay worker’s leg crushed in forklift disaster (a meat worker injured when he was run over by a forklift have spent days at a Hospital wondering if he is going to lose part of his leg – his leg was caught between two wheels of the forklift and a second forklift was required to lift the machine and free him)
HOT WORK FIRE Crews put out small welding fire inside BMW plant, no one injured (a small welding fire forced workers to quickly evacuate – firefighters were able to quickly put out the flames – fire happened near some construction in an assembly hall – no injuries were reported)
HOT WORK FIRE Wind whips welder spark-ignited fire (firefighters were on the scene of a wind-driven fire all afternoon and well into the evening after sparks from a welder ignited brush off a County Road – a well was being drilled in the area, and the well casing was being welded when sparks from the welder caught area brush on fire – the fire, which began about noon, spread to a neighboring property and burned approximately 100 acres of brush and pastureland by evening)
UPDATE on HOT WORK FIRE Welding caused fuel terminal fire, report finds (a potentially catastrophic fire at a fuel terminal last July was caused by welding work on an in-service diesel tank that was not properly monitored and should not have taken place, according to an investigation report from the fuels inspectorate – report highlights several breaches of industry safety standards by SOL in carrying out “hot works” on a fuel tank filled with 15,000 barrels (525,000 gallons) of diesel fuel – fire at the facility took more than eight hours to contain and prompted a mass evacuation of neighboring homes – the state of the tank and the scope of the work, in this case, meant it should have been taken out of service and emptied before the repairs took place – its contractor did not follow proper protocols, failed to adequately plan the works, failed to station a “fire watch” supervisor on the job at all times and ignored a safety alarm that could have alerted them to the danger much earlier)

ELECTROCUTION with FORKLIFT Construction worker killed, 2 hospitalized after electrocution in Johnstown (one person has died and two others are in the hospital after an electrocution – construction crews were repairing the sewer system when the incident occurred shortly before 10:30 a.m. – a machine the workers were using touched high-tension lines that were carrying 23,000 volts of electricity – electricity went down in the machine and electrocuted one worker and shocked two others – all three were taken to a Hospital, and one worker died in the Emergency Department)
ELECTRICAL Worker badly burned at Westside Regional Medical Center (worker for an electrical contractor was burned outside a Regional Medical Center – he was working on electric lines and was shocked and badly burned after he cut into a pipe holding wires that deliver power to the hospital)

EXCAVATOR FATALITYWorker dies following accident at Wynn casino construction site (construction worker, 56, died after he was injured – he was operating an excavator at the south end of the construction site taking apart a trench box when the incident occurred around 3 p.m.)
Falling beam injures worker in Battle Creek (an equipment malfunction is to blame for a construction site accident that injured a worker, 64, shortly before 8:30 a.m. – firefighters found him pinned underneath a 24-foot beam – the beam was part of a system to move equipment around and that system failed causing the beam to fall about 10 feet, hitting the construction worker in the head and pinning him down – someone used a forklift to raise the beam and free the worker)
