INDUSTRIAL MINE FATALITY One man dies in mining accident (a worker, 33, died after a shaft ceiling collapsed following a micro blast in the coal mine – emergency service workers managed to get him out of the rubble soon after the incident, but he died on the way to the hospital – 2 that two workers were trapped under the rubble in the incident – the other one had bodily injuries) FORKLIFT AMPUTATION Marina Worker Loses Legs In Forklift Accident (a marina worker, 54, lost both of his legs after a boat forklift overturned – the forklift was approaching a seawall when it struck the worker who was standing on the dock – the forklift fell into the water, where it had the worker pinned against a fence – worker was airlifted to a nearby trauma center in critical condition) CRUSHING FATALITY Denham Springs man dies in industrial accident near Baker (an industrial accident left one worker, 41, dead around 10:30 p.m. – the plant makes steel pipe for the oil and gas industry – worker became pinned between two large pipes as one rolled forward during the course of his work) ELECTROCUTED Florence Utilities worker electrocuted (a utility worker, 43, was electrocuted while working on a utility line – accident occurred at 12:20 p.m. as he worked on a line about 60 yards off the road – co-workers performed CPR at the scene and also used an automatic external difibulator (AED) in an attempt to revive him) MINE FATALITY 160-foot fall kills dozer operator at Wyoming coal mine (an accident involving a bulldozer that plunged off a 160-foot-high wall killed a worker at the Coal Mine – the accident happened around 2:30 a.m. – dozer operator, 44, was ejected from the cab and killed – he was the only person inside the bulldozer – he was a two-year employee of the mine – the mine accident was the third causing a fatality in the United States in as many days – the last time three people were killed in the mining industry over three days was in 2002) FATAL EXPLOSION @ Fireworks Factory Fireworks Factory Explodes In Vietnam, Several Workers Dead (a fireworks factory exploded, killing about two dozen workers – at least 20 workers were also injured, much lower than the initial report of 100 or more – according to an initial report, some explosives at the fireworks factory self-ignited in a storage room, which caused a fire and additional explosions) GUARDING FATALITY Port Allen man dies in Mer Rouge rice plant accident (a man has died from injuries after an accident at rice and grain plant – he fell into one an auger – workers were able to remove him from the auger, but before rescue workers could arrive on the scene, he died from his injuries) DRILLING FATALITIES Four workers killed in Oman oilfield accident (four workers died in an accident at an Oil field – an investigation is under way to find the cause of the accident – the exact nature of the accident was not known – it was also not known if the accident occured at the oil or gasfield) RADIATION Six workers at Japan’s Fukushima nuclear plant doused with radioactive water (six workers were doused with radioactive water from a desalination system – the fluid splashed onto the men when they accidentally removed a pipe connected to the system – the pipe was reconnected and the leak stopped within an hour of the initial incident) EXPLOSION Explosion injures worker at saltwater disposal plant (worker, 32, was flown to a hospital after he was burned in an explosion at a saltwater disposal plant just before 5 a.m. – the man was flown to a Medical Center for treatment for burns to his arms and face – cause of the explosion is not known – nine of the tanks exploded as the fire continued to burn so the firefighters let the flames die down before approaching) SCISSOR LIFT FATALITY Worker fatally crushed at south Phoenix business (a worker has been fatally crushed by a lift – the worker was inside the refrigerator section of the facility when the accident occurred – he was on a scissor lift and his clothing got caught in on the controls, lifting him and crushing him) ELECTRICAL EXPLOSION 3 injured, 2 seriously as explosion rocks stadium in Mallorca (two workers suffered serious injuries when a generator exploded in the parking lot of the stadium – a third person suffered lesser injuries – incident occurred around 3:20 p.m. when the two workers were adjusting a generator in the stadium parking lot to increase electrical power ahead of the World Cup match – it is thought that one of the workers may have touched the electrical box with a tool, causing the explosion.) FATAL EXPLOSION HOTWORK on DRUM Man Killed in Welding Accident in Joliet Township (a man, 52, has died from injuries he received in a welding accident – he was cutting off the top of a 55-gallon drum when the substance inside exploded, sending the lid into the man’s head – victim was taken to a Hospital where he was pronounced dead at 2:23 p.m.) HOTWORK Fire Damages Industrial Building in Fond du Lac (fire damaged an industrial building – when crews arrived on scene, they found flames and heavy smoke coming from a hydraulic pump house – firefighters were able to put out the blaze, but there was damage to the building and the hydraulic pump – investigators believe the fire was an accident, started by workers who were using a torch to cut a metal chute from a piece of machinery connected to the building) FATAL EXPLOSION in LAB Burn victim in Dow lab explosion dies (a man, 51, burned over half his body in a laboratory explosion has died from his injuries – he suffered his burns during an explosion inside the laboratory – he was working alone in a lab when the explosion occurred shortly before noon – he was flown by helicopter to a Hospital where he died – state Fire Marshal said she was unable to say what caused the explosion, but that the accident was still under investigation – the sprinkler system inside the laboratory contained the fire and that the fire alarm alerted everyone so they were able to get out of the building unharmed) FIRE 5-alarm fire wipes out hazelnut drying plant (no one was hurt in a five-alarm fire that broke out at a hazelnut drying plant, but hundreds of thousands of pounds of the nuts were lost in the blaze – the first call came in around 2:20 a.m. – both a building and attached silo were on fire – it took crews around eight hours to get it fully extinguished – estimate around 400,000 pounds of hazelnuts were lost – cause is under investigation) AMMONIA Ammonia gas leak in Andhra plant; 30 hospitalized (at least 30 workers at a private agri-marine processing plant were hospitalized after inhaling leaked ammonia gas from the plant – incident occurred at around 10am when the workers at the plant inhaled the gas which leaked out while processing of shrimps on the plant premises – at least 30 workers fell unconscious and were immediately shifted to government hospital and a private hospital and treated upon – their condition is out of danger) AMMONIA Firefighters find leak at Tip Top factory (firefighters spent almost two hours at an ice cream factory finding an ammonia leak, then clearing the fumes – four engines were sent to the plant around 2.30 am after reports of a leak in the factory’s laboratory area – the leak was small and the fumes easily ventilated – no-one was hurt, and they finished the job around 4.30am) CHLORINE Chlorine leak puts over 300 in hospital (about three hundred residents were overwhelmed by chlorine gas when a cylinder at a water purification plant sprung a leak – more than 300 people had been brought to the hospital as a result of inhaling the gas – some had received treatment and returned home – of the two patients receiving treatment in the Hospital’s ICU, one had been sent to a ward, while the other remained due to a wheezing problem which the patient had for a long period – the patients included those who were employed in the project site as well – the leak had occurred while attempting to fix a new chlorine cylinder) CONSTRUCTION FATAL FALL from SCAFFOLDING London man named as victim in Waterloo construction accident (a construction worker was killed when he fell off an apartment building – a man who was working on the roof of the 12-story building fell onto scaffolding several floors below – he was doing masonry work on the edge of the roof) CRANE FATALITY Survivor recounts horror of fatal crane collapse (a crane collapsed at a construction site – a survivor of a fatal construction crane collapse said the crane’s ladder had snapped, sending a worker crashing down – three workers were killed in the incident, which occurred at an under-construction luxury hotel – police said the crane operator had “lost control” leading to the collapse – the crane had hit five workers, killing three, when it lost balance) WORKZONE FATALITY Man who died after being run over by a car while working down an unguarded manhole (worker, 37, was halfway down the hole when he was hit by a slow-moving car at 10:35 am – he was taken to the Hospital where he died) WORKZONE FATALITY Parish worker killed in Vinton accident (worker, 51, was performing maintenance on a roadway around 10 a.m. when he was struck by an SUV traveling eastbound – he was pronounced dead at the scene by the Coroner’s Office – he was an employee in the Public Works Department for nearly 20 years – impairment does not appear to be a factor in the accident) TEMP STAIRS COLLAPSE Four workers hurt as stairs collapse at Southport home (four workers were injured when a staircase in a home under construction collapsed, trapping them in the basement – two sets of temporary stairs in a home under construction collapsed as they carried a cast-iron tub to the third floor – the tub and two staircases toppled into the basement, with one of the staircases falling onto the men – workers fell between 15 and 20 feet and suffered serious but not life-threatening injuries – one man had a broken leg, one had broken ankle and another suffered head pain and complained of a lack of feeling in his arm – one of the workers was expected to be treated and released – the second man was being evaluated for “non-critical” injuries in the incident) FALL from SCAFFOLDING Man injured in Clayton Park work site accident (worker, 25, tumbled several metres off some scaffolding around 10:20 a.m. – worker sustained leg injuries, but they are not life-threatening – he was taken to hospital by ambulance) WIND TURBINE COLLAPSE Workers escape as wind turbine collapses at Belfast Harbour (the stem of a wind turbine collapsed on to a digger as it was being moved – no injuries) CONVEYOR 3 workers injured in accident at Seattle office construction site (three workers were injured in an accident at an office building construction site – the three were working near a conveyor belt when it malfunctioned – two people working in a pit about 75 feet below street level suffered ankle injuries – they were lifted by crane to waiting ambulances – a third worker at street level suffered a head injury – all were taken to a Medical Center and were expected to survive) WORKZONE FATALITY Town worker killed in accident (a town highway worker, 69, has died of his injuries after the supervisor, 52, backed a town pickup truck over him during a paving project – supervisor struck the worker while driving a truck in reverse to pack pavement on the shoulder – he told police he didn’t see the worker and thought he was on the other side of the road) FORKLIFT Worker struck with girder in construction accident (worker, 20, was injured when a 21-foot-long steel girder slipped off a forklift and hit him at a construction site at 3:30 p.m. – he was stationed in the foundation of a home – he was transported to the hospital with injuries to his head and leg) EQUIPMENT FATALITY Man killed in industrial accident (one person is dead in an industrial accident around 2:30 p.m. at a construction site – a man was struck by a piece of heavy equipment – accident happened while workers were digging up a section of road – it appears they were putting in a new water or sewer line) CRANE FATALITY Plant engineer crushed to death by crane (a construction worker, 42, died instantly when he got crushed by a crane while working at a project site – the accident occurred when the crane lost balance – four others who were at the top of the crane to ensure it was balanced were able to climb down upon an alert just before the accident) ELECTRICAL and FALL from LADDER Construction Worker Hospitalized After Electrocution at Forever 21 Store (worker, 25, was on an A-frame ladder when he was shocked and he fell to the floor, approximately 12 feet – when paramedics got to the store, he was on the floor in cardiac arrest – by-standers were performing CPR – after being shocked by the paramedics, the man got a strong pulse back and was taken to a Hospital – he also regained consciousness and started speaking) TRACKHOE Pa. construction worker hospitalized after mishap (a construction worker was flown to a hospital after a piece of construction equipment fell on him – the man was working at a bridge construction project when a 36,000 pound trackhoe excavator overturned into a ditch – the man’s legs were pinned by the machinery and the excavation materials – it took rescue crews about 20 minutes to dig him out – he was taken to the hospital, but there is no word on his condition) TRENCH WG&L employee sent to Phoebe after falling into hole (a man fell about six to eight feet, which resulted to them having to use a skid and backboard along with ropes to get him out of the hole around 10:30 a.m. – it took the fire department about 15 minutes to rescue the worker – he was sent to a Hospital with a possible broken leg) HOTWORK Davenport firefighters battle fires caused by worker’s torches (firefighters had to battle two fires caused by construction workers using torches – first fire occurred at 1:27 p.m. – workers were using a torch under the roof of the building when material caught fire – no one was injured – second fire occurred at 2 p.m. where workers are renovating an empty space – workers were using a torch in the upper area of the ceiling when insulation caught fire – the fire was quickly extinguished but firefighters were on the scene about three hours making sure the fire was out of the old building, and that all of the affected insulation was removed) Miscellaneous WORKER KILLED BY FALLING ORANGES (a seasonal worker, 30, was fatally injured when a wooden enclosure, pushed by the weight of oranges, collapsed and fell on him – he was rushed to the Hospital after the accident but died later the same day – he was working at a wooden structure used as a receiving point for fruits but it collapsed and fell on him) LAWNMOWER FATALITY Cleveland Department of Public Works employee dies after lawn mower accident at Archmere Park (a Department of Public Works employee, 65, injured in an accident has died – crews were sent to the park at about 10:15 a.m. for a report of an overturned lawn mower – victim was taken to a Medical Center, where he was pronounced dead – he worked as a real estate maintenance worker and had more than 13 years of experience) FALL through SKYLIGHT UM Worker Falls Through Skylight onto Boiler (a longtime University employee, 61, is in serious condition at a Hospital after falling through a skylight and landing on a boiler inside the heating plant on campus – he was checking new equipment on the roof before plummeting 15-20 feet through the glass window – he was checking a flue gas damper on the roof, before he fell through the glass skylight) ANIMAL ATTACK Worker injured by tiger (an employee at an animal park had her arm severely mauled by a tiger – it was all a terrible accident as the result of the female worker violating a safety protocol and placing her hand inside the cage of an adult male tiger – the woman was wearing a large goose down jacket, which according to the park, got bunched up inside the cage wire not allowing her to get her hand back outside the cage wire fast enough – that’s when the tiger grabbed her hand pulling her left arm through a 4-inch square hole) TRACTOR FATALITY Farm worker dies after tractor rear-ended (a farm worker, 58, has died of injuries suffered when the tractor he was driving was rear-ended by a pickup truck pulling an empty livestock trailer – the tractor was marked with slow-moving vehicle signs) CHAINSAW cuts LPG Line Worker burned, tanks catch fire at Jackson County body shop (eployees of the body shop building, an auto repair building and also nearby auto sales building were all evacuated from the area – the fire started when a body shop employee was cutting brush around the tanks and a LP vaporizer with a chain saw -the employee hit a copper gas line with the chain saw – the pilot light in the LP vaporizer caused the leaking liquid LP to ignite – employee did suffer some burns to an arm and refused medical transport from the scene but did seek medical treatment on his own) AIR CONDITIONER FUMES FATALITY DC hospital worker fixing air conditioning overcome by fumes, dies (a worker died after he was overcome by fumes and at least four others were treated – the worker responded to a mechanical room in the east building to fix the air conditioning and was overcome – the worker and a colleague were taken to the emergency department, where one worker died – the other worker is expected to be released – a total of five people were taken to the emergency room – as a precaution, some the building was evacuated, but patients were able to return after an hour – the cause of the leak is under investigation) ELECTRICAL Tree worker jolted by electrical wire in Lunenburg (a tree service worker was injured after a branch he was cutting made contact with a power line around 8:14 a.m. – when emergency workers arrived on the scene, they found the tree worker about 20 feet up in a tree, and he was awake and able to communicate with emergency crews – he was transported by ambulance to a Medical Center) |
