GAS PLANT Worker injured at Campbell County gas plant released from hospital (a worker who suffered head injuries in an accident at a gas processing facility was released from the hospital – a fire broke out at the facility and the worker suffered head injuries serious enough to require transportation by helicopter to a hospital – crews extinguished the blaze before firefighters arrived)

RAIL ACCIDENT Worker dies in accident on Alabama Warrior Railway (a worker has died in an accident – details on how the accident happened weren’t immediately available – a safety stand-down has been ordered at the railway before workers return)

MINE FATALITY Officials ID Sheridan worker killed at Montana mine (a worker, 58, at a mine was killed in an accident involving a truck used to haul coal – no one else was injured in the accident – the 230-ton truck crashed through a berm about 7:30 p.m. killing the driver)

FATAL FALL (or FATAL LOTO???) Man killed in Hull lift tragedy ‘on first day of new job’ (worker, 37, was crushed under a lift on what is believed to have been the first day of his new job – the lift engineer was working when it is thought he fell almost ten metres down an elevator shaft – moments after his fall, he was crushed by the lift – it is not yet known how he came to fall)

FORKLIFT FATALITY Worker crushed in Expressway Spares accident (worker, 40, died as he was loading a large industrial engine onto a forklift when the engine toppled forward trapping the man underneath – it is not known what caused the engine to fall – he died at the scene, remaining trapped underneath the engine for more than two hours)

FORKLIFT FATALITY Worker killed in accident at Fife warehouse (a worker, 58, at a warehouse was killed after a 11,500-pound piece of equipment fell on him – he reportedly worked at the business for more than 20 years – accident happened just before 11 a.m. at the warehouse – worker was helping to move the heavy piece of machinery used to bend metal when it toppled over on him – he was pronounced dead at the scene)

PLATFORM Oil worker airlifted to Aberdeen hospital after accident on rig (worker was airlifted with a knee injury – injury is not believed to be serious)

CRUSHED FATALITY Man crushed to death by slabs of granite (worker was crushed to death by giant slabs of granite shortly before noon – no other details)

CRANES COLLAPSE 4 injured, 1 seriously hurt when cranes collapse at VT Halter Marine in Pascagoula (five workers have been injured, one seriously, at a marine yard after two cranes collapsed – four workers were taken to a Hospital and one was taken by helicopter to the a Medical Center – incident happened at about 2:15 p.m. – it was caused when multiple cranes were in the process of moving a section of a boat -“ For some reason, they suffered catastrophic failure”)

TRUCK Nalcor investigating Muskrat Falls accident (a trailer struck someone who was working on the construction of a transmission line – worker was between the back of a pickup truck and a trailer that was attached to the truck – driver of the truck was unaware that the other worker was between the two vehicles and didn’t see the worker before moving the truck forward – worker was struck by the trailer as it moved forward and was knocked down to the ground)

FATAL EXPLOSION Two killed, one critically injured in fire accident near Sivakasi (two workers, 30 and 55, were killed and one was critically injured in a fire accident at a cracker unit – the injured, 19, has been admitted to a Hospital with 100 per cent burns – friction caused while handling chemicals led to the fire – while the injured sustained burns, the other two were fatally knocked down by flying rubble following an explosion)

STEEL MILL FATALITY Steel Mill Worker Dies From Burns (a worker who received third degree burns while working at the steel mill has died from his injuries – a contractor with an Environmental firm was cleaning out a container of waste with a backhoe – friends of his say the waste he was cleaning was a dust-like powder that is extremely hot and fell on him while he was cleaning – his doctor told his family that over 90% of his body had been burned)

CRUSH FATALITY Man Crushed by Glass in Aurora, Colo. (a worker died after being crushed by nearly 4,000 pounds of glass – the victim was helping unload wooden crates of glass at the business when the 4,000-pound load fell on top of him, killing him at the scene – no one else was injured)

CONFINED SPACE FATALITY in RCAR Hydrogen sulfide gas suspected in deaths of Pekin Agridyne workers (autopsies should determine if H2S gas, rather than carbon monoxide, killed two men as they prepared to clean a railroad tanker car at an agricultural food supplement plant – investigators, however, already know the workers were not wearing protective masks – workers, 29 and 37, were identified as the men who died while apparently doing their regular jobs at the plant that produces and transports corn-based animal food supplements – one had worked at the plant for about a year – fire officials relayed a third worker’s report that one of the deceased jumped through the roof hatch of the cylindrical metal car to rescue a down worker, who fell through the hatch when he opened its door – tanker still contained several inches of a liquid corn-based product – a specially trained Fire Department rescue team, which arrived within several minutes of the accident report about noon, entered the tanker with SCBAs to pull the victims out by winch and rope – one worker was pronounced dead at the scene, one at a Hospital, where he had been taken unresponsive – before their rescue efforts, the firefighters scanned the interior and immediate surroundings of the tanker with gas detectors and were alerted to a strong presence of carbon monoxide, a potentially fatal gas that typically is produced by a mechanical source – investigators concluded that an “electrochemical interference in our detectors” may have produced a false report of carbon monoxide – “It could be that hydrogen sulfide (was present) at such an extreme level that it triggered” the false reading – neither victim was found wearing a mask)

Explosion at Bécancour chemical plant (an explosion at a chemical plant will not cause any lasting environmental damage – witnesses report a dark yellow cloud of chlorine gas rising from the plant at around 11:30 a.m. – one man was overcome by the gas, but is expected to make a full recovery)

LIFT STATION EXPLOSION McDonald’s sewer lift station explodes, investigation underway (explosion ripped through the small shed housing a lift station – no one was injured – detonation occurred around midnight and traffic was light at the time – most of the 10’ x 10’ x 8’ structure landed in the middle of the Road and the concussive force was strong enough to damage the freestanding McDonald’s sign many feet above the ground – explosion instantaneously burned itself out, just used up all its oxygen – investigation of the cause of the explosion was yet in its early stages – Fire Chief believe that gasoline somehow introduced into the sanitary sewer system may be to blame – a strong odor of gasoline was initially noticeable at the lift station – it was also found to be present at other points in the line as the collection crew began popping manhole covers up and down the Road – firefighters, meanwhile, were detecting varying levels of a flammable substance presumed to be gasoline)

HOTWORK on TANK EXPLOSION Fuel tank believed to be empty explodes (a fuel tank that was believed to be empty exploded at a recycling plant – explosion occurred when employees at the plant tried to dismantle the tank by cutting into the metal, which created sparks -the tank was thought to be empty, some fuel residue remained inside, fire officials said – the explosion shot the lid off the tank, and no injuries were reported)

AMMONIA Second ammonia leak at a Kitchener business in less than a week (for the second time in a week, an ammonia leak brought firefighters out to a business – no one was hurt – it’s believed to have been caused by a leak in a compressor – they were called out to the same business after an ammonia leak in a cooler)

FORKLIFT FATALITY Teen killed after being run over by forklift in Horizon (worker, 17, is dead after he was run over by a forklift at 5:45 p.m. at a junkyard – the teen jumped onto the forklift before falling off – the forklift then ran over the left side of the teen’s torso – he was taken to a Medical Center where he later died)

CONSTRUCTION

WORKZONE Tulsa City Worker Hit By Car Facing Months Of Recovery (a city worker, 29, was hit in a work zone – it will be months before she can walk again, and her injuries are likely permanent – a part time worker with the city, she was hit by a truck while directing traffic around other workers – crash left her with a broken pelvis and other injuries – accident was a hit and run and the only description is a man driving an F-150 that’s dark, with lighter gray on the bottom)

WORKZONE Wreck injures MoDOT truck driver on I-435 (a state highway worker escaped serious injury in a traffic accident – a tractor trailer cab rear-ended the worker’s MoDOT truck about 9:30 a.m. – DOT driver complained of minor injuries and was taken to a local hospital – the truck had a rear-mounted crash cushion that absorbed much of the impact – ahead of the DOT truck, several department employees were setting up traffic controls for a bridge inspection)

CONCRETE HOSE Worker Injured at W’burg Whole Foods Construction Site (a worker at a construction site was injured at 11:30 a.m. – he was transported to a Hospital and remains there in serious, but stable condition – injury occurred because the worker hit himself with a hose while placing concrete below ground at the site)

RR CROSSING Worker seriously injured in train-truck collision near Red Deer (a railway worker, 44, was airlifted to hospital with serious injuries after the loader he was operating was struck by a train – the construction vehicle was working on rail construction at the site when it was struck by a train – indications are, the loader crossed the track and was struck by the train – the man who was operating the loader was found unconscious and was extricated by fire crews and flown by air ambulance to a Hospital in serious but stable condition – no one else was hurt)

FALL from ROOF Worker seriously injured in fall at incinerator site (accident happened at the site of a new waste incinerator – worker was airlifted to a hospital with serious injuries after an accident at the building site of a new waste incinerator – the man fell about 30 feet)

FATAL FALL Construction worker dies after fall (worker, in his 20s, fell 22 stories and was pronounced dead at the scene – the worker fell from the 26th story to the 4th while working on a balcony)

WORKZONE 2 highway workers hurt in interstate crash (transportation officials are once again urging motorists to exercise caution in highway work zones after a crash injured two workers, including one critically – accident happened in an area where crews have been building a dike to divert flood water – the two workers were placing traffic cones to mark lane closures when a vehicle struck a state Department of Transportation truck – one worker, 43, is in critical condition – the other worker, 57, suffered non-life threatening injuries)

CRANE FATALITY Two killed in San Juan construction site accident (two persons were killed at a construction site when metal beams being lifted to the sixth floor of a multi-level parking building fell to the ground – accident happened at 1:30 p.m., when three 20-foot metal beams were being hoisted by a power crane – the beams landed on a makeshift cafeteria, crushing the victims – initial reports say a gust of wind may have caused the beams to fall)

WALL COLLAPSE FATALITY Father-of-two crushed to death in tragic accident on Melbourne construction site after strong winds topple brick wall (worker, 30, died when a gust of wind toppled a partly-built garage wall and trapped him underneath – it is believed the builder left the upper story of a townhouse development because he thought it would be safer working on the ground level when strong winds)

GAS WORK Gas blast breaks worker’s leg (a worker was taken to hospital with a broken leg following a blast during gas main work – it is believed work was being carried out on a gas main where a fractured pipe has caused an explosion, creating a gas cloud)

FATAL COLLAPSE Father Crushed to Death in Wall Collapse (a construction worker, 40, was trapped and killed when the last standing wall of a building under demolition collapsed on top of him – he was standing nearby the last standing portion of the building around 12:30 p.m. when it came crashing down, killing him – he was knocked into a trench, leaving him partially buried by debris – he was pronounced dead at the scene)

FORKLIFT FATALITY Man dies after forklift accident, son in critical condition (a father is dead and and his son is critically injured after a forklift accident – they were hired contractors, working on a forklift -“ There was some sort of malfunction on the equipment and they fell and hit the concrete – they fell about 15 or 16 feet – father was pronounced dead at a Hospital – the son is currently in serious condition)

MISCELLANEOUS

ELECTROCUTION Worker electrocuted at SJC truck stop (a man was electrocuted while working on the roof of the truck stop – the man, an electrician, was shocked about 1:30 p.m. in what was described as an industrial accident on the roof – he was in his mid 20s – he was transported to a hospital, but was reported in full cardiac arrest)

DISTRACTED DRIVER FATALITY Landscaping crew worker dies when equipment trailer is hit (a landscaping crew worker, 71, died from injuries suffered when he sought shelter from the rain in the crew’s equipment trailer, which was then struck by another vehicle – he received multiple blunt force injuries and died at a Medical Center – three other men suffered minor injuries in the 9:40 a.m. crash  – driver told authorities he failed to see the trailer as he attempted to retrieve an item from his vehicle console, police said – he was ticketed for reckless driving)

WOOD CHIPPER GUARDING FATALITY Police ID Florida Man Killed by Wood Chipper (a landscaper, 42, was killed by an industrial wood chipper – he and two other people were clearing brush when the apparent work accident occurred – police were still trying to determine exactly what happened – his co-workers weren’t with him when he went into the machine)

FATAL TIRE EXPLOSION Tire Explosion Kills Tuscola County Farm Worker (a farm worker, 44, was killed when a tire exploded as he was repairing it – he was trying to seal the bead on a tine rake tire about 8:00 p.m. when it exploded – he died later at a Hospital)

STREET SWEEPER FATALITY Worker dies after being pinned under street sweeper (a sanitation worker, 43, died in an accident after he was struck and pinned by a street sweeper – he had been working on his own street sweeper when he was fatally struck by another one at a Department of Sanitation garage at around 1 p.m. – he was pronounced dead at the scene – he had been with the department since 1999)

EXPLOSION Propane explosion sends 2 to hospital (an explosion rocked a restaurant – according to the fire chief and the state fire marshal, propane ignited inside the restaurant as two employees were lighting a stove – fire chief said a worker was unfamiliar with the gas control valves and left one of them open, letting the propane build up inside – explosion has been ruled “accidental human error” – both of the employees were injured in the explosion – worker, 50, suffered burns to his legs and was eventually air lifted to a Center, where he was last listed in critical condition – the other employee, 44, was burned on her legs and was treated and released at a Medical Center)

HOTWORK Fire Causes “Extensive” Damage to Johnny Rockets at Knott’s Berry Farm (restaurant suffered “extensive” damage because of a two-alarm fire – fire broke out around 2:20 a.m and the flames were under control within the hour – no one was injured by the fire that consumed the two-story burger joint – an arcade and laser tag area next to the restaurant also had some smoke and water damage – fire caused approximately $170,000 in damage – cause is still under investigation, but authorities believe it may have been caused by a worker that was welding)

HOTWORK Fire damage closes The Ranch Steakhouse & Grill restaurant (restaurant has suffered severe fire damage to its kitchen and rear after a fire broke out because of welding work – the blaze, which may have been caused by workmen welding, led to a serious amount of damage to the kitchen and rear of the restaurant)

HOTWORK Welding sparks Port of Providence boat fire (call came in around 11:10 a.m. for reports of a 70-foot fishing vessel fully engulfed in flames – a welding mishap started the fire – crews did use the department’s new foam pumper truck to help extinguish the flames – no one was hurt in the fire)

HOTWORK Fire engulfs Longview beer warehouse (owner of a beer distributor called damage to the business “too much” after a blaze – all employees evacuated the building before fire crews arrived at the plant – no serious injuries were reported – the fire originated in the warehouse after a piece of copper wire ignited the building’s spray foam insulation – air conditioning people were working on the insulation and caught it on fire with a torch)

HOTWORK Fire damages Canton shopping center, injuring one man (a rooftop fire damaged at a shopping center, causing minor injuries to one man and forcing the temporary closure – the accidental fire was started by construction workers using a torch to make roof repairs – one worker was treated at the scene by firefighter paramedics, but declined to go to a hospital)

SCAFFOLDING COLLAPSE 2 Workers Rescued From Dangling Scaffolding In Midtown (two construction workers found themselves hanging on for dear life after their scaffolding gave way – some 12 stories high, the two construction workers in safety harnesses found themselves dangling around 8:30 a.m.)

EXPLOSION Three hurt in plant explosion (an explosion at an industrial cleaning business sent three employees to local hospitals with burn wounds)
