LOGGING & SEATBELT Worker injured in logging accident (rescue crews had to extricate a worker who suffered two broken legs after rolling over eight times in a logging skidder – he was operating a skidder about 75 feet up an embankment when the machine starting going backward – the skidder flipped as many as eight times with the man inside – skidder landed upright, but he wasn’t buckled in)

Accident at Pekin wastewater plant injures worker (a worker suffered a serious but non-life-threatening injury when a piece of heavy piping fell on his leg at the Wastewater Treatment Plant – a team of workers was removing a flange from a large pipe used to connect chlorination tanks at about 11:15 a.m. when the object fell about 2 or 3 feet onto the victim, pinning him to the ground in a hole dug for the work project – workers used heavy machinery to lift what was described in a police news release as a large iron cap from the victim’s leg)

ENGULFMENT in trailer Worker killed in accident at Boston cotton gin company (worker, 32, was crushed by cotton seeds at a plant and died – he was unloading cotton seeds from a trailer when he fell into the back of the trailer and seeds began pouring on top of him – it took investigators between 15 and 20 minutes to find him)

CRANE 1 injured in Clark County industrial accident (an indoor crane fell on a worker and trapped him – his condition was not immediately known – co-worker said the crane was picking up a panel when it fell over)

MINE FATALITY Foreman killed in West Virginia mining accident (a foreman, 49, at a mine was killed when he was struck by a piece of rock – he was operating a roof-bolting machine at the time of the accident shortly after 9:30 p.m.)

MINE FATALITY Worker killed in accident at Tharisa mine processing plant (worker was killed in an accident at the processing plant during routine maintenance)

Windsor worker suffers fractured feet in accident at CS Wind (a worker, 47, is recovering in hospital after an industrial accident around 2 a.m. – a piece of machinery had pinned the feet of an employee – worker was freed and taken to hospital for treatment of multiple fractures in his lower legs – worker was wearing protective footwear at the time of the accident)

FLASH FIRE Worker injured at East Hartford natural gas fueling station (a maintenance worker has been burned in an explosion at a compressed natural gas fueling station – the worker was doing maintenance on a compressor just after noon when a spark ignited a fire – worker suffered burns to his head, hands and torso)

ELECTROCUTION Firefighter: Missouri worker fatally electrocuted (worker has died after being electrocuted at a facility for a battery charger manufacturer – he was taken to an area hospital where he later died – it’s unclear what caused the accident)

UPDATE on GUARDING/LOTO FATALITY in CONVEYOR Hillshire Brands hit with fine for May death of worker caught, killed in machine (worker, 49, was killed in an accident when some of his clothing became caught in machinery – he was cleaning up at the end of his shift in the meat-packaging department when he apparently became entangled in a conveyor belt and died of accidental asphyxiation – his job was to clean industrial equipment used to make sausages – following an investigation through the MI-OSHA, the company was fined $6,300 each for two violations classified as “serious”)

FATAL FALL @ POWER PLANT Contractor dies at Wygen III plant after 80-foot fall (a contractor, 25, at a power plant died after he fell about 80 feet from a roof while helping to install a new panel of some sort at about 9 a.m. – “The man … had a safety harness on but it’s believed it wasn’t attached” – the men were working on the roof to “catch a panel” that was going to be installed when they came upon a situation that required the men to shuffle where they hooked up their harnesses – “One switched and turned around and his partner was not there” – bystanders attempted first aid on the scene, but paramedics pronounced him dead upon their arrival)

Worker Seriously Injured at Acument Plant (worker, in his mid 40’s, was reportedly seriously injured in a work-related accident in the 7 o’clock hour this morning – reported to be a maintenance worker who had only been on the job for a matter of a few months – latest accident comes just a few months after a man was killed at the plant back in June)

FATAL EXPLOSION Police ID Worker Killed In Lapeer Shop Explosion (worker, 34, died in an explosion at a machine shop that also injured three other people around 2 p.m. – it appears that a cylindrical tank containing an unknown type of gas became over-pressurized and exploded – explosion reportedly occurred behind the business)

Roof collapses at factory, killing worker (worker was buried and killed under the rubble of the factory after the roof collapsed – he was buried under the rubble of the factory for 55 minutes before firemen pulled him out)

FATAL FALL @ WWTP Dyeing factory worker drowns in ETP pond (worker, 55, drowned after falling off the factory’s effluent treatment plant (ETP) – he was cleaning the top of the treatment plant when he lost his balance and fell into the water inside the plant – the water was deep, about 30 feet, rescuers failed to recover the body – workers work at the ETP without life jackets as those are not provided by the factory, and there is no railing to prevent such accidents)

FIRE Worker Injured in Chemical Fire Explosion Identified (worker is in the critical care burn unit with burns to 65-percent of his body – investigators believe the fire began when a worker was transferring some kind of chemicals – officials put the damage to the building at $1 million)

PROPANE EXPLOSION 4 injured in explosion and fire at Turano Bakery garage (a propane tank explodes at a Bakery garage, injuring 4 people – it destroyed a truck maintenance garage – explosion occurred around 2:15 p.m. as workers were changing a propane tank in a truck – workers thought the tank was empty, but it was full and shot into the air, causing the brick walls of the structure to collapse)

CONFINED SPACE FATALITY – One Entrant and One Would-Be Rescuer 2 workers die of suffocation while cleaning sewage treatment plant (two workers died of suffocation after inhaling noxious fumes while cleaning the sewage treatment plant (STP) – two others, another cleaner and a fireman who was involved in the rescue of the workers, also fell unconscious on inhaling the fumes – both are currently recuperating in the critical care unit of a hospital – two workers had entered the STP to clean it, without wearing any safety masks or protective clothing – one started feeling faint and fell unconscious inside the tank – another worker entered the tank to rescue him – the duo panicked as they started feeling suffocated and they unable to come out of the tank – at this point, a third worker jumped into the tank to rescue the duo, but he too inhaled the noxious gas and fell unconscious – fire brigade that reached the spot removed the three men in an unconscious state from the tank – a fireman, who was involved in the rescue operation, also fell unconscious as he too inhaled the gas)

TANK EXPLOSION Tank explosion rocks Cheapside (there were no significant injuries in the explosion at an oil storage tank – incident remains under investigation – after roughly three hours of fighting an oilfield fire the Fire Dept. was able to extinguish a blaze that had burned two fiberglass oil tanks and destroyed a truck)

HOTWORK Flames Damage Verizon Wireless Tower (workers were welding on the tower and heard “two loud noises” – worker stated he looked up and saw flames and a lot of smoke – all power to the tower was cut off – a coaxial cable caught on fire as welders worked – fire was up too high on the tower for firefighters to extinguish – no injures were reported)

FLASH FIRE Flash fire at Ashe County plant injures 2 employees (two employees suffered neck and face burns, while another employee who witnessed the fire was taken to the hospital after suffering a panic attack – two victims will be released from the Hospital, while the third victim was taken to a Medical Center – the flash fire happened as one of the employees was pulling rags from one container to another – building wasn’t damaged)

FATAL EXPLOSION One dead, three injured in Lapeer plant explosion (a worker, 34, is dead and four others injured in a plant explosion at about 1:30 p.m. – injured workers were transported to a Hospital)

EXPLOSION No one injured in fertilizer plant explosion (the cause of an explosion at a dry fertilizer plant continues to be investigated and the plant will be closed for a short time – no idea how the explosion occurred and did not have a damage estimate at this time – accident happened as product was being taken up an elevator leg into a blending bin – a resulting fire smoldered for some time before the fertilizer was pulled out from the plant’s tower)

AMMONIA FATALITY After weekend’s fatal ammonia leak, MKs call for swift action (the leak is suspected to have been caused by factory workers who accidentally cut two pipes – the tragic ammonia leak left a firefighter dead and injured 20 others – the leak, which occurred at a Poultry Processing Plant is suspected to have been caused by factory workers who accidentally cut two pipes linked to the 8-ton ammonia tank that leaked – firefighter, 34, was killed in the rescue effort that followed – “The failure probably occurred when upgrading the ammonia tank”)

FORKLIFT FATALITY Woman (27) dies in tragic workplace accident (worker, 27, was killed after the forklift she was operating “tipped over” – incident occurred in a premises around 9.30am – the forklift tipped over and she was trapped in it in some way)

FORKLIFT Forklift strikes gas line at North Arlington moving company (a forklift struck a gas line at a moving company prompting an evacuation around 7 a.m. – employees were unable to find the cutoff valve to the gas line)

CONSTRUCTION

FATALITY Worker dies in construction accident in Warwick (worker, 45, was killed during a construction accident as he was working on the state roadway project shortly before 2 a.m. when a large piece of construction equipment fell on him)

LOTO AMPUTATION Lexington road worker’s arm ripped off in accident (a road worker had his arm ripped off when he was checking a piece of equipment that stopped working – the machine kicked back on just as the man grabbed it, amputating his arm at the elbow)

WORKZONE FATALITY Worker who died in Causeway accident had worked for company only a short time (worker, 57, died when his construction vehicle was knocked off the Lake Pontchartrain Causeway – he had worked for the company only a short time – the truck he was driving slowly as part of a construction crew on the bridge was struck by a garbage truck – he was driving the large truck very slowly and protecting workers in front of it when it was hit from behind by the garbage truck at about 3:30 p.m. – the construction truck, which provides a buffer between traffic and workers, is designed to withstand impacts from the rear, but the force from the garbage truck sent it over the bridge railing and into the lake – the garbage truck, which caught fire, flipped and hit a Causeway police car that was in front of the construction truck)

EXCAVATION HOTWORK FLASH FIRE 2 workers burned in Newburyport construction accident (workers were welding in a trench near a gas line when a flash fire burned two workers – residual gas in the trench lit, causing the fire – one of the workers, contractor, was taken to a local hospital with severe burns to his face – the other worker, employee, helped the contractor out of the trench and was taken to a local hospital with minor injuries)

EXCAVATOR FATALITY Construction Worker Killed In Accident At IAAP Site (the search for a construction worker, 30, is over following a deadly accident at an Army and Ammunition Plant – around 11:30 a.m. a construction worker using an excavator along the bank of a lake slid into the lake – the worker was trapped inside the equipment in the deep lake – his body was later recovered by a dive team around 5:30 p.m.)

WORKZONE FATALITY – BLIND SPOT Construction worker killed in Turnpike crash (a worker has been killed in an accident at a construction site on the Maine Turnpike – worker was in the blind zone of a dump truck when he was hit and killed – incident happened in a lane that was closed for construction)

TRUSS COLLAPSE Construction workers recovering from accident in Sun Valley (worker who was seriously injured in a construction accident is in fair condition – he was one of five men from local construction crew injured while framing a structure – a wall collapse – three framers were trapped under a double-layered drywall section on the second floor of a townhouse under construction – an additional two men were injured in the incident)

HYDRAULIC PILE DRIVER FATALITY Albertville construction worker killed in work-site accident in Guntersville (a worker, 41, was killed while working on a pier – he was struck in the head with a hydraulic pile driver, which killed him on impact – workers were working on moving the platform to another location when the accident happened)

Tape measure falls 50 stories, killing worker (worker fiddling with his tape measure on the 50th floor of a high-rise condo under construction site accidentally dropped it and it landed on a worker, 58, below, killing him – he wasn’t wearing his hard hat at the time, having left it in the back of his car – accident occurred as a construction worker was preparing a wall for windows on the 50th floor of one of two partially built residential towers – the worker was unfastening the 1-pound measuring tape clipped to his work belt when it slipped out of his hands – the instrument fell nearly 500 feet before striking a piece of construction metal about 10 feet above ground – the measuring device would have been falling at nearly 140 mph when it hit the piece of metal – the tool ricocheted off the metal and smashed into the victim who had been leaning into a car window to talk to someone and had just pulled his head out when he was struck)

FATAL FALL down ELEVATOR SHAFT Roofing Worker Falls Down Elevator Shaft, Dies In Mount Vernon (a worker, 28, fell to his death down an elevator shaft – he was doing roof work with four other men around 6:30 p.m. – he was apparently putting away his tools for the night when he fell through a screen covering the opening of the elevator shaft, and ended up landing on an elevator car several stories below)

Worker suffers head injury on Bristol city centre building site (worker, in his 40s, has suffered a head injury in an accident on the eighth floor of a building site – he had been hit by a piece of concrete)

Construction worker dead after accident (a construction worker, 39, died after falling from the fourth floor of a construction site at about 3:20 p.m.)

BUCKET TRUCK FATALITY Man, 54, dies in bucket truck accident; Investigation underway (a worker, 54, was in a bucket truck trimming branches at a cemetery when the accident happened – he was unconscious when medics arrived just after 11:30 a.m. – no one is sure exactly what happened – “The bucket truck came down. It snapped and the worker was injured” – victim was part of a four-man crew)

HOT WATER Two Injured in Industrial Accident in Wahpeton (two electrical workers were working on a scissor lift when hot water overflowed above them – both workers were taken to a Medical Center – their conditions are unknown at this time, but we do know one worker had to be taken to by Air Med helicopters)

FATAL FALL from BALCONY Construction worker dies 8 days after 30-foot fall from balcony of famous Ocean Springs home (a construction worker, 42, who fell from a balcony has died a little more than a week after the accident – he took a 30-foot fall – he had worked in construction all his life)

CONFINED SPACE Construction Worker Gets Trapped in Concrete Pipe in Beaumont (worker got trapped inside a concrete pipe – emergency crews had to work for more than two hours to free him – the trapped person was a construction worker who was doing some sort of work in the underground pipe – wasn’t immediately clear how they got stuck)

WORKZONE Construction Worker Hit By Vehicle In Stable Condition (construction worker was hit by a vehicle – he was reportedly lying in the street on a private drive looking down a manhole at the time of the accident – there were no lights, there was no indication of a construction zone and the man was behind a truck that obscured him from motorists – a mail carrier coming down the road ran the man over – no charges are pending)

FLASH FIRE Explosion At Texas Roofing Company Injures One Worker (a worker who evidently used a cigarette lighter to check a storage tank was injured in an explosion at a roofing company – the man evidently used a lighter to check the level of tar in a tank – the fumes ignited, leading to a brief flash fire – he was transported by ambulance to a hospital – nobody else was hurt)

MISCELLANEOUS

LOTO FATALITY OSHA investigating after maintenance worker trapped, killed in machine at Fairfield bowling alley (OSHA  investigators are working to determine how an experienced maintenance worker, 53, was killed at a bowling alley – he died after getting caught in a machine he was trying to repair – he was working on a pinsetter at about 2 p.m. when his clothing caught on something and the machine pulled him to his death)

FATALITY EMTA Mechanic Injured in Accident Dies (worker spent almost 30 years working as a mechanic at the Transit Authority – he died after an accident in the bus garage around 11:30 a.m. – he was taken to a hospital for a traumatic head injury and died just after 4 a.m. – no details)

DIVER DROWNING Maintenance Diver Drowns In New Albany Retention Pond (worker hired to dredge a series of ponds is dead after drowning in a diving accident – he was in dive gear for the job when he somehow got stuck underwater – the diver had been working in tandem with another employee – the diver who drowned was underwater longer than expected and lost contact with his co-worker – it is unclear what caused the accident)

FORKLIFT Forklift accident reported at Beaver County elementary school (a forklift accident injured a groundskeeper – he was driving the forklift on school grounds when he lost control – school officials believe the brakes malfunctioned – he crashed, and the forklift flipped on its side – he was not trapped under it)

ELECTROCUTION Man’s Wedding Ring Gets Him Killed (a good deed for his in-laws has ended in tragedy – worker, 33, was installing a new dishwasher at his brother-in-law and sister-in-law’s house when he was electrocuted – family members, who were sitting down to eat around 7pm, found him on his knees and leaning against the dishwasher’s metal door – he was “turning red” and unresponsive, but his left hand was still behind the dishwasher – they laid him on the ground to begin CPR, then called police – he was pronounced dead upon arrival – his wedding ring apparently touched something behind or in the dishwasher, causing the electrocution)
