co-worker returning from a break found the worker – the worker was pronounced dead at the scene) CONFINED SPACE FATALITIES One more dies in manhole accident (the death toll in the manhole accident increased to two with the death of one more worker at a hospital – three sanitary workers who entered a 15-feet manhole fell unconscious after inhaling poisonous gases – of the three workers, a worker, 30, died on the spot and the remaining two workers were rushed to a Hospital – a worker, 35, succumbed while undergoing treatment at the hospital – one is still in the ICU and his condition is critical) OIL FIELD FATALITY Worker killed at Toole County oil-field site (a worker, 20, died in an oilfield accident – he and another worker were “using a wheel wrench on a well head when a piece of the wrench broke off, striking him in the chest” – CPR was administered until an ambulance arrived and transported him to a Medical Center where he was pronounced dead – cause of death was blunt-force trauma to the chest) OIL FIELD FATALITY Big Spring worker dies when rig owned by Abilene firm collapses (an oil field worker, 40, has been killed when part of a drilling rig collapsed – the victim was dead at the scene – a second man was transported to a hospital after the accident for unrelated injuries) ARC FLASH 4 Centerpoint Workers Burned in Flash Fire Stable, 1 Still Critical (four of the five workers badly burned in a flash fire are now in stable condition – the remaining worker is in critical condition – the accident injured five employees who were doing routine maintenance on a vault in the building’s basement, which stores equipment to serve underground power lines) WELDING ACCIDENT BURNS Two Injured in Foley Plant Accident (two workers were injured in a minor explosion at a plant just before 8am. – one person has been Lifeflighted from the scene with burns to the back – that victim was taken to a Medical Center’s Burn Unit – another worker was injured and taken by ambulance to a local hospital – their conditions are unknown – witnesses on scene say it was a welding accident contained to building one) OIL FIELD FATALITY Worker dies in second Howard County oilfield accident this week (a worker, 59, died in a fatal oilfield accident – employees were working on a large heater tank used to prepare oil for transport at about 10:33 a.m. when the device rolled onto the worker pinning him against another similar “treater” device – he was transported from the yard to a Medical Center where he was pronounced dead) LOADING DOCK FATALITY Police: Postal employee killed in Brooklyn Center (a postal employee has been struck and killed at a post office at about 7:30 a.m. – a semi driver was backing up to the loading dock when a postal worker was struck and killed) AMPUTATION in CONVEYOR Tyson worker injured in plant accident (an employee at a food plant lost his arm in an accident – the man’s arm was caught in a conveyor belt, and he was pulled into the machine – his arm was amputated from the elbow down – he was taken by helicopter to a hospital where he was in critical condition) UPDATE on SAWMILL CONVEYOR FATALITY Teenage worker dies in sawmill accident (worker, 18, became entangled in a conveyor belt at a planer mill – a shift supervisor who found the worker freed him from the equipment and, along with two other workers, administered first aid until an ambulance arrived – he had been working part-time on the overnight cleanup crew for a few months – he was transported to a hospital and pronounced dead shortly after arrival – he was conducting cleanup and, at the time of the incident, he was on the main floor of the mill near a waste-disposal conveyor) LOTO/GUARDING in CONFINED SPACE Worker hospitalized after becoming trapped in silo (a man working in a silo has been taken to a hospital after he became trapped in the structure – an electrical contractor was working on a grain elevator when his arm became trapped in a piece of equipment – he was taken by helicopter to a local hospital after the accident – few other details were immediately available) AMMONIA Six Pemex workers injured in ammonia plant accident (six workers were injured in an accident at a water cooling tower in an ammonia plant – staff were carrying out maintenance on a turbine at the tower in the petrochemical complex when a connecting part broke off, injuring the workers – accident did not cause any toxic emissions or fire – the workers received immediate attention from on-site medics before being transferred to a hospital – workers suffered various bruises) WATER TREATMENT PLANT FATALITIES from GAS Industrial Accident Kills Four in North Jakarta (four workers have been confirmed dead and five others remain in critical condition after an apparent industrial accident at a water-treatment facility – cause of the accident was not immediately clear, but police said a toxic-gas leak was the most likely cause of death – police were unable to provide further details on the type of gas or the reason for the leak – the incident took place when nine workers were conducting maintenance work – when the police arrived, they found four workers dead at the scene and five others in critical condition) FORKLIFT FATALITY Publix employee dies in forklift accident at Boynton Beach distribution center (worker, 56, died at a Medical Center about an hour after the accident at the distribution center – he was operating a forklift inside the distribution center when he made a turn at a corner and did not see a pallet on the floor – attempting to avoid hitting the pallet, he spun the steering handle sharply, throwing him off the forklift – then, the forklift crushed him into a riser column – initially, employees ran to help – they told police they did not see any injuries and that he was conscious but disoriented – as Fire Rescue crews transported him to a Medical Center, he stopped breathing, and rescue workers administered CPR – he died minutes later during surgery) METHANE POISONING @ WWTP 7 killed in Turkish sewage plant accident (seven workers were killed when they fell into a pool of sewage and succumbed to methane gas poisoning – the men were working at a sewage plant – six died at the scene and the seventh died en route to an area hospital) ELECTRICAL Injured GPL worker has left hand amputated (employee who was burned while working on a utility pole had his left hand amputated – the amputation was done at the Hospital and he is said to be in a stable condition) FATAL EXPLOSION @ FIREWORKS PLANT Fireworks factory explosion near Montreal kills 2 (at least two people were confirmed dead after a fireworks plant blast shortly before 9 a.m. – fire lasted three hours and caused heavy smoke, damage to nearby houses and trees – authorities evacuated nearby homes as a safety precaution, but no injuries were reported – cause of the blast remains uncertain) EXPLOSION Explosion at Postville factory injures 3 (a small explosion at a factory injured three workers, including one who was flown to a burn unit in critical condition – the plant’s sprinkler system kept the ensuing fires at bay, but the plant suffered an estimated $5 million to $7 million in damage – the most seriously injured worker was flown to the burn unit – one of the other injured workers was treated and released from an area hospital, and the other declined treatment at the scene – the explosion was reported at about 1:20 a.m. – building personnel shut down boilers and gas valves to help contain the fire) UPDATE on LOTO vs. CONTROL CIRCUIT FATALITY Machine dragged worker to his death (a vineyard manager’s tragic safety oversight led to him being fatally pulled through an automatic net roller – he died from massive blunt-force head trauma in April 2010 after the net roller he was working on unexpectedly restarted, dragging him in – he would normally turn the tractor hydraulics off or disengage the spindle motor when working on the net, but on this occasion he did not – instead, he relied on a net sensor to stop the net being pulled in, but this did not fully engage and the machine restarted, trapping his leg in the process – he was known to be a conscientious and methodical worker who was proficent and experienced in the safe operation of the NetWizz – “For reasons that can not be established, on the day of the accident he did not disengage the spindle motor before working on the net, an oversight that had tragic consequences”) LINEMAN ELECTROCUTION Utility worker dies in suspected electrocution (a utility worker, 44, died after apparently being electrocuted by exposed power lines – he had been called to a power outage and was found unresponsive near some exposed power lines at a private residence – he was taken by ambulance to the a Regional Hospital – it appeared that he had died from coming into contact with the power lines) LINEMAN ELECTROCUTION Power company worker dies in Armstrong County (a worker, 52, died after he was electrocuted while trying to repair a power line that had been knocked down by a falling tree – he accidently grabbed the live line – he was pronounced dead at the scene at 3:50 a.m.) CONFINED SPACE FATALITY – ONE ENTRANT and THREE WOULD-BE RESCUERS 4 workers die after inhaling poisonous gas in well (four workers died after inhaling poisonous gas in a well – a worker at the site went into the well to repair a pump but fell unconscious inside – after some time, another worker went inside the well to inquire but he too did not return – later, two other workers stepped below through wooden stairs to rescue them -they too fell unconscious due to poisonous gas emanating from the well – all of them were later rescued with the help of police and were rushed to a hospital where they were declared dead) CONFINED SPACE FATALITY Sanitation worker dies inside manhole, 2 fall ill (a municipal sanitation worker, 30, died and two others fell unconscious due to inhalation of toxic gases after they went down into a manhole – they went down into the manhole at around 8.15 AM to clean it but fell unconscious after they inhaled the toxic gases inside – one died on the spot, his co-workers were rescued and taken to a nearby hospital, where the condition of one of them is critical) HOTWORK Welding Sparked Cell Phone Tower Fire: Official (two welders were working 70 feet in the air on a cell phone tower when sparks set off an intense fire ten feet above them – they tried to put it out, but ended up having to race down to get help – the fire spread quickly and left the tower, which stands nearly 10 stories high, leaning over precariously) HOTWORK Spark from welder caused Amcor fire (a welding mishap caused this week’s fire at a Plastics plant at around 8 a.m. causing the evacuation of the plastics manufacturer – no one was injured, but one firefighter was treated for heat exhaustion – the company had been doing some remodeling work along the building’s north wall when a spark from a spot welder ignited a plastic coating around some duct work – the fire quickly spread before fire crews were able to extinguish it by 10 a.m.- damage was estimated at $80,000 including equipment and products) EXPLOSION Blast, fire rock chemical plant in Shanghai (a petrochemical factory, one of the largest acrylic acid producers in China, was partly engulfed in fire for nearly three hours – no one was injured and 50 fire engines were used to extinguish the blaze – tests showed there was no danger to neighbors from chemicals at the factory – the fire occurred after an acrylic acid reactor in a factory workshop exploded during manufacturing – just before the explosion, a reactor tube cracked and caused a leak) AMMONIA Fayetteville Plant Back Open After Ammonia Spill (plant is recovering after an ammonia spill that shut the facility down – the spill was not reported to local law enforcement or workplace safety officials – plant was evacuated after ammonia spilled into the sewage system on-site, which leads to the city Wastewater Treatment Facility – WWTP workers diverted the contaminated water from the food plant into a holding pond temporarily, to stop the ammonia from spreading down the line – there are no reports of injuries) AMMONIUM HYDROXIDE Ammonia leak at Aurora water treatment facility injures one (a Water Purification Facility tractor-trailer tanker had an ammonia hydroxide leak – one person was taken to an area hospital with unknown injuries – extent of the leak was not immediately known, but hazmat crews were on the scene of the water treatment facility) AMMONIA Workers evacuated at Rochester dairy plant (workers at a dairy plant were evacuated after an apparent ammonia leak just after 9 a.m. – worker had reported an ammonia release and crews then evacuated the building – the cause of the leak is being investigated as the crews continue their inspection) AMMONIA Seven treated after ammonia leak on farm (several people are rushed to the hospital after an ammonia leak at a farm – the leak happened inside a building that’s used to pack asparagus – it was a leak in a refrigeration line – seven employees were complaining of headaches, some had trouble breathing and they were taken to get treatment – workers in the building were eventually able to clamp off the line where the leak was happening and get it stopped) UPDATE on AMMONIA LOADING RACK ACCIDENT Injunction Filed Following Anhydrous Ammonia Leak (Attorney General and County State’s Attorney announced a court order will keep an anhydrous ammonia loading dock closed at a farm co-op where an ammonia leak occurred earlier this month until the extent of environmental damage is known – a driver delivering anhydrous struck and severed the overhead anhydrous pipe connecting the tank loading stations – a shutoff mechanism on the dock failed, resulting in an uncontrolled discharge of approximately 40,000 pounds of compressed anhydrous ammonia for approximately 12 hours – responders used approximately 100,000 gallons of water to mitigate the effects of the ammonia release – the resulting contaminated waste water flowed off site into a tributary – according to the joint complaint, the ammonia release and the shutoff mechanism’s failure presents a substantial danger to human health and the environment – in addition, there must be a detailed evaluation to determine whether damage to the air, soil and groundwater in and around occurred before the loading dock operations are allowed to resume – the immediate injunction requires the facility to cease any anhydrous ammonia loading activities, maintain the dikes and temporary barriers constructed for containing storm water and continue remediation of the nearby South Branch of Kent Creek that contains waste water runoff) CONSTRUCTION STRUCTURE COLLAPSE Workers injured in construction accident at Texas A&M (several construction workers were injured when a portion of the Equine Center under construction collapsed – workers were in elevated positions tying steel together on the 35-foot-tall, 300-foot-long barn when they came down – a 35-year-old man was admitted to a Regional Health Center in critical condition – two 22-year-old men had been admitted in stable condition and a 32-year-old man received treatment and was later released – an additional worker had been transported to a Medical Center and that worker was in stable condition – between 25 and 30 people were working at the scene at the time of the collapse) FALL from STEEL Pa. Firefighter, Construction Worker Hurt at Incident (firefighter suffered a seizure and a head injury while helping a construction worker – a construction worker, 52, and fireman were seriously injured in a construction accident – the worker had fallen 30 feet from the steel frame of a building being built – he was rushed to a Hospital, where he was in critical condition) CRANE ELECTRICAL Electric Shock At Chemainus River Bridge Sends Worker To Hospital (a worker was electrocuted during work at the Bridge – he was airlifted to a hospital with serious injuries just after 2 p.m. – the man was working on the Bridge replacement project and appears to have suffered injuries consistent with a severe electric shock – a crane may have been involved, but few other details of the accident are available) GENERATOR BACKFIRES Worker badly burned in ‘freak accident’ (a construction worker at an area mall suffered serious burns to his arms and chest after he was splashed with gas during an industrial accident – the worker was working on a generator when his hair caught fire – generator backfired, caught his shirt on fire – he dropped and rolled and they had a fire extinguisher there and put him out immediately – he suffered severe but non life-threatening upper-body injuries and was taken by helicopter from the scene) SCAFFOLDING Worker rescued after Bethpage scaffold accident (a construction worker was rescued from a 40-foot scaffold after two rolls of roofing material fell and crushed part of his leg about 11:20 a.m. – the worker was on a scaffold when he was hit by 400-pound rolls that slid off the roof) GASOLINE FLASH FIRE Worker burned while working on new Wal-mart (worker was on the roof of the new building working on a gas generator, when some fumes ignited burning the worker over the upper part of his body including his face and arms – officials say either fumes ignited burning the man or he spilled gas on himself – he was med-flighted to a Hospital) STEAM EXPLOSION Citizens worker injured in steam leak (an employee suffered burns from stepping into some hot water while trying to fix a steam leak – the man is still hospitalized with burns below the knee – he was getting into the manhole to pump the water out, when he stepped into hot water about one foot deep – the steam leak caused the cover of a manhole to blow off) MANLIFT FATALITY Bridge inspector fatally injured on Interstate 279 on North Side (a bridge inspector, 58, has died after he somehow crushed himself while working beneath the overpass – he was taken to a Hospital in critical condition after the accident – medical examiner’s office confirmed that he had died – he was using special equipment to view what DOT said are recently completed concrete repairs – he was on a ‘cherry picker’ – apparently he got too close to the beams, and in some way he lost control of the equipment and crushed himself with one of the steel beams that he was inspecting) CRANE 13 injured as crane tips into house in Germany (a crane carrying a panoramic cabin tipped over at a festival and hit a house, injuring 13 people – the cabin suspended from the crane had been set up for a festival organized by a local school – the cabin collapsed into the roof of a residential building – all eight adults and five children in the cabin were injured; two of the adults suffered serious injuries and were taken to hospitals by helicopter – no one in the house was hurt – it wasn’t immediately clear what caused the crane to tip over) MISCELLANEOUS STUMP GRINDING ACCIDENT OSHA to investigate Portsmouth fatal stump grinder accident (officials are investigating a fatal accident involving a stump grinder that killed a tree service worker, 43 – he was working alone at the time – no other details) GARBAGE TRUCK Man injured in dump truck accident (a worker, 21, was hit about 7:40 a.m. – medical responders took the man to a Hospital at 8:15 a.m. – the man was in stable condition – he has a broken left calf bone and swelling of both legs) TRACTOR Worker Hurt In Freak Accident On Bourbon County Farm (a man was rushed to the hospital after an accident on a farm at about 10 a.m. – one of the farm’s workers was on part of a tractor, fell off, and was then run over by a manure spreader – he suffered serious injuries as the spreader actually ran over his chest, but his injuries are not believed to be life-threatening at this point) SANITATION Sanitation worker hit by minivan (a sanitation worker was recently struck by a vehicle while he was standing on the road next to his truck – the worker was in the process of directing traffic around his truck when the accident occurred – he was hit by an oncoming minivan, which was attempting to get around the sanitation truck, during his attempt to direct traffic – he was transported to a nearby hospital and reportedly suffered injuries to his head, back and knee) HYDRANT Fire hydrant accident injures city worker (a fire hydrant exploded and injured a city worker – a homeowner who witnessed the incident said that the worker was immediately taken to a Hospital, but the extent of his injuries are unknown) TREE TRIMMING ELECTRICAL Worker cutting trees critically injured in high-voltage accident (a worker, 33, cutting trees was critically injured after coming in contact with high-voltage power lines – he was in a lift bucket when the accident happened about 9:30 a.m. – firefighters rescued the tree trimmer from the bucket – the power line he touched carries more than 7,000 volts – he was taken to a Hospital and listed in critical condition) TREE TRIMMING FATALITY Worker for tree removal company dies on the job (a worker, 44, died on the job while removing trees around 1 p.m. – he was working for a tree-removal company – there were a total of four workers at the site, but no other injuries – no other details) HOTWORK FATAL EXPLOSION on FUEL OIL TANK Man from Blaine dies from burns while cutting oil tank (a man, 72, has died after being critically burned while cutting up an old heating oil tank with a welding torch – he died in the overnight hours after being flown to a Hospital with burns to 80 percent of his body – it appears the torch ignited vapors as he was cutting apart the tank at his home) FORKLIFT FATALITY Forklift accident claims life (an accident with a heavy machine claimed the life of a local resident, 73 – she was leaving a nail salon when she was struck by a forklift in the parking lot of a grocery store during the afternoon hours – the forklift’s operator was moving boxes at the time of the accident) |
