INDUSTRIAL ELECTRICAL FATALITY Duke Energy worker dies in accident at Carmel substation (employee, 59, died following an accident at a substation – when emergency responders arrived on the scene he was in cardiac arrest – he was transported to a Hospital but he was pronounced dead later that evening – he had been with the company for 37 years, where he was a substation maintenance supervisor) UPDATE on SCALDING INJURY ArcelorMittal worker dies after February incident (steelworker, 62, has died from injuries suffered when he fell into scalding water at the mill in February – he died Friday as a result of complications from thermal injuries after falling into scalding water) Worker hurt in heavy machinery accident in Robertson (a worker, 33, was injured in a machine accident shortly after 5 p.m. – at the time of the accident, he was operating heavy machinery weighted down with several large boxes – he and the boxes were about 30 feet up in the air when something happened and the whole thing dropped straight down – worker dropped with it, sitting in the operator’s chair – the victim may have lost consciousness at some point, but he was awake and alert when emergency officials arrived on the scene – he complained of loss of sensation on one side and was airlifted to a Medical Center with a possible spinal injury) FATAL FALL down ELEVATOR SHAFT Worker Dies In Fall Down Elevator Shaft Of Downtown Boston Building (a worker died after falling down an elevator shaft in a building just after 10 a.m. – the worker fell 30 feet down the freight elevator shaft – the man was unresponsive when first responders arrived, and rescuers were working to remove his body after a police investigation – the man was pronounced dead) MINE FATALITY Worker killed at southwestern Indiana coal mine (worker, 41, has been fatally injured at a mine about 2 a.m. – he and another employee were doing maintenance work on a piece of underground mining machinery when he was injured by an unexpected movement of the equipment – the other worker wasn’t hurt) CRUSHED EXCAVATOR Worker Trapped In Excavator Under Boulder For Hours At N.J. Quarry (a worker was trapped in an excavator under a boulder at a Rock Quarry – crews were excavating some very large rocks on the on the side of a cliff, and a large boulder came down onto the cab of a piece of construction equipment, and trapped a worker – worker was trapped from 2 p.m. until he was finally rescued around 4:45 p.m.) MACHINERY FATALITY Woman dead after industrial accident in Vaughan (a woman is dead after an industrial accident at about 5 a.m. at a plastics manufacturer – she was pinned under a piece of machinery – no other details) CO2 FATALITY Worker killed in gas leak at Samsung (a gas leakage sparked by a malfunction of the fire extinguishing system left one worker dead – the building’s safety system released carbon dioxide after it wrongly detected fire, suffocating a 52-year-old man who worked for a partner company – the worker was already in cardiac arrest upon arrival at the hospital) AMMONIA FATALITY @ Ice Plant Worker killed, 7 others fall ill following gas leakage in factory (a labour was killed and seven others were taken ill following leakage of Ammonia gas in an Ice Factory – those who fell ill are rushed to the hospital where they are now out of danger – incident took place around 1 am when wall of the Ammonia gas plant started leakage due to which a labour, 35, working close to wall fell unconscious on the spot – the leakage impact was so wide that three others workers working in plant and four of their family members living in same factory premises fell ill – an official from the police station said that prima facies suggest that company owner negligence who do not conducted proper maintenance work due to which incident happened but further probe is on) PROPANE TANK San Leandro: Exploded propane tank smashes through car, driver unhurt (a 61-year-old man napping in his car before work narrowly escaped serious injury after a propane tank from a nearby facility exploded, flew across a street and smacked into his vehicle – the chunk of metal traveled approximately 0.2 miles before shredding through the man’s new Honda Civic, leaving the car looking like a sardine can with the lid pulled off – a metal fabricator at an industrial sheet metal company said his co-worker narrowly escaped death because he had leaned forward to turn off his cell phone alarm at the same moment a piece of metal came hurtling through the car’s back window just before 7:30 a.m. – the 5-gallon propane tank that was being cut down came from a metal recycling facility – Cal/OSHA is investigating the incident to see if proper safety protocols were followed) OIL WELL FATALITY Man dies on oil well drilling site (a man, 32, died at an oil well drilling site while working on a drilling ring – he was struck by “blocks” – emergency personnel arrived at 6:52 a.m. – he was located on the floor of the drilling rig, which sits approximately 30 feet above ground – employees had been performing CPR – CPR was stopped at 7 a.m. and the victim was pronounced dead at the scene – Coroner is performing an autopsy to determine the cause of death) HOTWORK EXPLOSION Worker suffered burns in explosion at town of Hudson business (an explosion was touched off when a worker attempted to cut off bolts on the underside of a liquid propane tanker at a business – the worker suffered burns to his face and hands in what was described as a flash fire than accompanied the explosion – the commercial building the business is located in also sustained some structural damage, including garage doors blown off their tracks – there was no fire damage to the building – the tanker was inside the building when the explosion occurred) MOBILE EQUIPMENT FATALITY WorkCover investigates death of cotton gin worker (a man died in an agricultural accident at a cotton gin – initial investigations indicate a truck driver, 53, was run over by a module truck, commonly known as a moon buggy, while unloading cotton bales from the vehicle – early investigations indicate the module truck hit the trailer’s load stop – the man sustained fatal injuries and its investigation into the accident is continuing) ELECTRICAL Man in hospital after touching power line, residents lose power (a man grabbed onto a live power line – the victim’s hand was burned, discolored, and cut from the power line just after 10 a.m. – TxDOT says a company was carrying the naptha drum – the 20 foot high and 24 foot wide load had to go through some power lines, which forced a few workers moving the drum to move the lines for safety reasons – one man grabbed onto a live wire and was shocked on impact – the worker then dropped the wire, which caused a small fire to erupt and heavy traffic in the area) RIG FIRE Officials investigating oil rig fire near Watford City; 1 worker burned (a drilling rig fire resulted in one worker’s injury – authorities responded to a report of a rig explosion at 4:16 p.m. – when responders arrived, all workers were clear from the area and one man suffered burns to his hands and head that were not life-threatening – initial cause of the fire is believed to be a pressure build-up that caused material to escape and ignite) PRESSURE COOKER EXPLOSION Meat worker seriously injured after pressure cooker explosion (a man, 59, is in a serious but stable condition in hospital after a pressure cooker exploded at a meat processing plant) NITRIC ACID TANK EXPLOSION Acid tank explosion injures 9 at C. Ind. plant (an explosion involving a tank of nitric acid has injured nine workers – a 250-gallon tank of nitric exploded about 1:30 p.m. releasing a cloud of vapor that left nine workers suffering from acid burns and breathing problems – eight of the workers were hospitalized, but none had life-threatening injuries – one worker was treated at the scene) FATAL ANHYDROUS AMMONIA EXPLOSION Explosion Kills One in Tecumseh (a grain elevator worker, 63, has died in an anhydrous ammonia explosion that injured another worker around 6 p.m. – rescue crews took two workers to a local hospital, where one of then later died – two others, including a sheriff’s deputy, were treated for non-life threatening injuries or exposure to anhydrous ammonia) SPILL Energy from waste plant says there was carrier truck attendants, 23 and 47, were accidentally electrocuted while inside the Ice Plant – investigation disclosed the victims went to the aforementioned ice plant to load some ice – truck went inside the premises of the ice plant and the victims took a steel bar to fix the cover of the ice box on top of the truck – victims failed to notice that the steel bar touched the electric wire causing both of them to be electrocuted) Malfunction At BP Refinery Causes Oil Spill On Lake Michigan (crude oil spilled onto the shore of Lake Michigan following an accident at the refinery – the EPA responded and found an area of about 5,000 square feet covered in crude oil – a disruption in the refining process resulted in a crude oil discharge from the Refinery’s cooling water outfall into Lake Michigan at approximately 4:30 p.m.) EXPLOSION @ WWTP Methane explosion causes extensive damage to Goshen water treatment plant (methane gas ignited, causing extensive damage at a wastewater treatment plant – methane gas ignited around 5 a.m. in the sludge thickener building connected to the plant’s administrative offices – no one was injured – the blast moved the wall facing west roughly 6 inches, broke glass windows and warped the plastic coverings on lights hanging from the ceiling – “I’ve been doing this 36 years, and I’ve never seen anything like this before,” plant superintendent said – the explosion was caused by methane that had built up in the thickener building – explosion happened an hour before staff arrived at work) FORKLIFT FATALITY Man killed in forklift accident at RSG Forest Products (a man was killed after being run over by a forklift operated by a mill employee – a 50-year-old truck driver had arrived at the mill around 1 p.m. and was waiting for his truck to be loaded with lumber when he was hit by the forklift used for loading lumber onto the trucks – this was the driver’s third trip for the day – the truck driver had gotten out of his truck and had begun to secure the load with straps while the truck was still being loaded, and the forklift backed into him – CPR was attempted, but the man was pronounced dead) CONSTRUCTION FATAL FALL Worker dies in Sao Paulo World Cup stadium accident (worker died while helping to build the stadium that will host the opening match at this year’s World Cup in Brazil – he is the third construction worker to perish while working on the Arena and the seventh to die in building Brazil’s World Cup arenas – he fell about 25 feet to his death – he was installing floors on the temporary stands being constructed at the ground that will host the opener) TRENCHING FATALITY Fukushima worker killed in accident, cleanup halted (worker at Japan’s wrecked Fukushima nuclear plant died after being buried under gravel while digging a ditch – the worker was in his 50s – the company confirmed it had hired the worker through another subcontractor) SCAFFOLD COLLAPSE OSHA Investigating Worker Accident At Fayetteville Bridge (investigation is in the preliminary stages but will examine whether safety regulations were being followed when the worker fell on Thursday (March 27)- he was transported to a Medical Center with serious injuries – worker fell about 30 feet from the bridge onto the surface below – he was on a piece of scaffolding cutting rebar from concrete barriers when the scaffolding gave way, dropping him from the bridge onto the ground below – no one else was hurt in the incident) PIPE HANDLING Worker suffers life-threatening injuries in accident (a man was hurt in an industrial accident around 8:15 a.m. – workers were moving a pipe with an excavator when the pipe fell, hitting the man in the head – his injuries were believed to be life-threatening) CONCRETE CUTTING Construction accident in Rogers County sends concrete worker to hospital (a construction accident at the site of a razed County Courthouse sent a man to the hospital – workers were sawing on concrete about 11:45 a.m. when a roughly 4-by-8-foot piece fell on a man, trapping him – didn’t believe that the man’s injuries were life-threatening) WORKZONE FATALITY Road Worker Killed in Accident in Copiah County (flagman, 39, is dead after a two car accident on Highway 51 near Crystal Springs – car was waiting for the flag man to release him so he can pass and a truck came over a hill and hit him from the back and pushed him in the ditch – the car ran over the flag man who was directing traffic while workers were paving Highway 51 – Department of Transportation officials also investigated the scene for any work zone safety violations, however, they found nothing wrong) COLLAPSE High winds blamed for construction accident at former GM site in Linden (high winds are being blamed for a construction accident at the former General Motors site in which three workers were injured when two steel columns and several joists fell – about a dozen people were standing on the edge of the construction site shortly after 7 a.m., preparing to start their shifts when the columns and joists came crashing down on top of several construction vehicles, including boom lift and a high-reach forklift – officers arrived to find three workers on the ground, including one who suffered a serious head injury -that injury was not considered life-threatening – the three workers, men in their 30s, were taken to a Hospital – two of the workers were treated and released – the worker with the head injury remains hospitalized) SAW Worker Hospitalized After Accident at CL&P in Hartford (worker was hospitalized after being hit in the face with a saw – he was starting the saw when he was struck – the worker’s injuries are not life-threatening) MANLIFT FATALITY Worker dies in building site accident (worker, 23, was killed at a worksite after being pinned between two pieces of heavy construction equipment – he was pronounced dead at the site – preliminary findings showed he was conducting checks on a boom lift when it suddenly moved, pinning him between the boom lift and a crawler crane parked next to it) EQUIPMENT REPAIR FATALITY Worker killed at West Vancouver road construction site (a worker, 56, was killed when the equipment he was repairing shifted, trapping him underneath around 2:40 p.m. – he was performing repairs on excavation equipment at a road construction site when the equipment accidentally shifted – he could not be revived) FATAL FALL Two workmen plunge 500 feet to their deaths after gondola splits in two at The Peak (two workmen were killed and two others escaped injury when the gondola they were working on fell apart – two workers – a man and a woman – plunged about 50 stories to their deaths from a luxury residential building after a working platform they were on snapped in two) WORKZONE 82-year-old driver charged after worker struck flagging West End traffic (a county worker sustained a head injury, but was expected to make a full recovery, after he was struck by a car while flagging traffic – he was flagging traffic around a crew removing leafs from a ditch – a driver headed east struck him and continued to drive – the driver was located close to the accident scene and compensation (a worker, 48, was killed in an accident in the milk processing plant – accident happened while the worker was engaged in dismantling a defunct chimney of plant-II – the deceased was a contractual worker – the 50-feet high chimney accidentally fell on the worker while he was trying to dismantle it with a gas cutter – he died on the spot) AMPUTATION Worker flown to hospital after thumb nearly severed in Annapolis (a worker, 48, was flown to a hospital after his thumb was nearly severed in an industrial accident at 3:30 p.m. – an employee of a mechanical contractor, he was Fire burns through roof of building in Midtown (a second-alarm fire burned through the roof of a building – no injuries were immediately reported – flames broke out in a closed clothing store that is undergoing renovation – authorities say the fire might have been started by workers – it appeared there were workers in the store with torches – took crews just under 90 minutes to bring the fire under control – cause of the fire is under investigation) HOTWORK FD: Roof workers cause fire at Norfolk business (crews were dispatched for a report of a fire just after 12 p.m. – there they found smoke coming from the roof of the building and the fire had extended inside – construction workers were on the roof prior to the fire and they were using a torch to make repairs when they accidentally caught the insulation from the roof on fire – no one was injured) Miscellaneous FARMING EQUIPMENT FATALITY Plantation worker dies in workplace accident (a man who was trapped in plantation machinery had already died when police arrived – the man, believed to be aged in his 50s, became trapped in machinery at the plantation just before 4p.m. and died as a result of his injuries) LANDSCAPER Phoenix landscaper critical after being crushed by truck (two teenagers drag racing may have caused the accident that left a landscaper in critical condition – two boys, 16 and 17, will face aggravated assault and endangerment charges – the 16-year-old is also facing a charge of leaving the scene of an injury collision – driver of the Mustang drove through a stop sign at the intersection and collided with a Toyota pickup truck that was traveling southbound – the Toyota truck veered off the roadway, sending it through a cinder-block wall, where it struck the 61-year-old landscaper as he mowed a lawn) OVEN EXPLOSION Oven explosion injures worker at Tommy Bahama’s restaurant (an employee, 30, was injured after an oven explosion at a restaurant around 9:30 a.m. – a woman working in the kitchen went to light the oven when it exploded, causing blunt force trauma injuries to her head, chest, neck and arms – she was taken to a Hospital and is expected to recover – the explosion was caused by a gas leak in the top oven, where the pilot light was off – gas collected in the oven overnight, and then additional gas was pushed into the oven as workers prepared for the lunch crowd – when the employee flipped the switch to turn the oven on, the gas ignited, blowing out the doors) HOTWORK EXPLOSION w/ SAFETY CAN Auto body shop worker injured in gas can explosion (a man is in stable condition following a gas can explosion at an auto body shop – the man was cutting on a car with a torch shortly before noon when a gas can exploded – fire investigators say they think slag from the victim cutting on the car struck the can, causing the explosion – victim told firefighters he tried putting the fire out with a fire extinguisher, but the can exploded before he could get the fire out – fire caused about $50,000 in damage to the vehicles and the building) CHEMICAL DUCT WORK Caustic Gas Leak Forces Evacuation At UCSD; 3 People Hospitalized (a leak of toxic fumes forced the evacuation of a building at a University and sent three people to a hospital – an employee of a private contractor was in a ground-floor laboratory shortly before 9:30 a.m., servicing the structure’s ventilation system, when he accidentally cut a duct that carries a caustic gas – the man left the building, went into a trailer used by his company and passed out – a co-worker found the unconscious man and made an emergency call – medics took the stricken worker to a Hospital along with another man who had been working at the building and a woman employed as a faculty assistant there) HOTWORK Welding blamed in South Side apartment fire (a welding torch sparked an apartment fire – workers were using the torch to repair plumbing in the laundry area of the four-unit building around 1:15 p.m., when the fire started – the fire was between the floors of the building – no one was injured – damage estimates have not been released) |
