Lucas worker injured while working on power lines (employee was injured while working on power lines – employee was transported to a Regional Hospital) Industrial Accident Sends Man To Hospital (worker, 21, was involved in an industrial accident and taken to a Hospital in critical condition – production at the Belvidere Chrysler plant shutdown for five hours after an accident at nearby supplier – the accident at the supplier forced Chrysler to “short shift” around 10pm last night, eventually sending workers home at 1am – officials have yet to comment on the nature of the accident at the plant) FORKLIFT FATALITY Brooklyn man gets crushed in deadly forklift accident (worker, 22, suffered fatal chest injuries when a co-worker’s forklift pulled back and pinned him against a wall at a Supermarket – he was rushed to hospital after the accident but succumbed to his chest injuries – he was was using walk-behind forklift to push a co-worker manning another forklift up a ramp leading to a rooftop parking area – the co-worker’s forklift rolled back and pinned him against the wall – he suffered serious chest injuries in the accident just before 6 p.m.) OIL FIELD FATALITY Manitoba man named as worker killed at Alberta Suncor site (worker, 37, died in an accident around noon – the heavy-machinery mechanic was doing routine maintenance on a bulldozer when an engine plate detached, striking him in the head – he died in hospital – he had been working in the oil fields, two weeks on and two weeks back home, for the last 6 years – 4 workplace deaths at site in 2014 upsets union – the four deaths since January at the site are not believed to be linked, but the province’s Occupational Health and Safety is investigating) MINE FATALITY Worker killed in Idaho mine accident (worker died when he jumped on a mining skip that was not tied off and was crushed between the mine shaft and the skip – activities at the mine have been suspended pending the investigation) PLANER MILL Chemainus mill worker seriously injured (details about the condition, identity, and injuries of a worker, hurt in a planer mill were unknown at press time – a man in his mid- to late-50s was seriously injured at 11:33 a.m. in the firm’s mill, then airlifted to hospital) CHEMICAL Chemical reaction incident at Strongwell sends 1 worker to hospital (a reaction inside a chemical processing room resulted in one worker being hospitalized – the company is still investigating to determine if a freezer malfunctioned or if a catalyst that cause a prescribed chemical reaction began its process too early, but 14 gallons of resin vaporized into the air – three workers in that area were exposed and immediately put through a decontamination process and other accident protocols – a chemical reaction emitted a vapor that some workers got into close contact with – a male worker was taken to a Medical Center for further treatment and was being held for observation for 24 hours) LOTO/GUARDING FATALITY w/ AUGER OSHA investigating fatal grain mill accident (a fatal industrial accident claimed the life of a grain mill worker around 1 a.m. – medical crews responded to a report of a man who had lost his leg and paged a county coroner shortly after – he had died from blood loss after stepping in an auger) Man killed in industrial accident at Poole paving firm (a man in his 60s was found unconscious and bleeding from his mouth when found – he was taken to hospital but later pronounced died – no other details) FALL Worker seriously injured at LI work site (a man was seriously injured after falling 8 feet at a work site – happened around noon at a business which makes concrete drainage trenches and retaining walls – the worker was hooking a forklift to a 4-foot square drain when he slipped and fell at least 8 feet – he was taken to a hospital with life-threatening injuries – three drains were stacked on top of each other, and the employee was standing on the second one when he fell) MINE FATALITY Worker in fatal mine accident is identified (worker, 25, died from injuries sustained during an accident at a mine – he was caught in a hydraulic scoria crusher, according to preliminary reports – this is the seventh fatality at a coal mine this year and the first in Wyoming) LINEMAN ELECTROCUTION Florence utility worker dies after electric shock (a utility worker has died after being electrocuted while working to repair a damaged electrical line – the electricity department employee was on an electrical pole and touched a hot line – he died at a Hospital shortly after the accident – a tree had fallen in the area, disrupting power in parts of the city) MINE FATALITY Falling rock kills contract worker at Capstone’s Cozamin mine (a falling rock killed a contract worker – the worker, who was working on the active mining face, was immediately transported to a hospital and pronounced dead on arrival) LOTO/GUARDING Man Injured in Chicken Plant Accident (a worker at a chicken plant was injured when he got his leg caught in one of the machines – he was taken to the hospital and underwent surgery) FLASH FIRE Asian worker dies, two injured in separate incidents (worker died and two other received first-degree burns at two separate locations – first incident involved a worker, 30, who died while cleaning ships in the docking area – the cleaner opened a water hose which had very powerful pressure to clean the place after the departure of the ship – the intensity of the pressure hurtled him, causing him to have severe cranial injuries causing him to bleed to death – second incident involved two workers from a lubricant company – both received first-degree burns while on the job – they were cleaning chemically contaminated barrels, which erupted into fire that burned them) UPDATE Worker severely burned in steel plant accident dies (a worker severely burned in an accident at a steel plant in May passed away – less than a month ago, an accident at the company’s plant sent six people to the hospital – employee who died was the only one who suffered severe injuries – doctors released the other five workers the night of the accident) ELECTRICAL w/ BUCKET TRUCK Worker in Bucket Truck Shocked by Power Line in Ivy (a utility worker is being treated after the bucket truck he was in touched a power line – worker is recovering from serious electrical burn injuries at a Medical Center – the incident set the truck below on fire – fire crews rushed to the scene just after 1 p.m. and found the truck on fire with the worker trapped and tangled in the power lines above – he had burns over 30 percent of his body – he was a contract worker for a phone company installing fiber optic cables) GARBAGE TRUCK Sanitation worker killed in garbage truck accident in Moab (an investigation is underway into the death of a sanitation worker, 51 – he was helping a garbage truck driver unload his truck at a transfer station when the accident occurred – the truck’s lift gate was raised and its cargo was unloaded – he was crushed when he became caught between the rear of the truck and the lift gate when it lowered) ARC FLASH Two Injured In Edgefield County Electrical Accident (two worker were transported to a hospital after an electrical accident at a business – the men were working on a transformer when a large electrical arc knocked the men out – no word on the extent of the men’s injuries) MINE FATALITY Man ‘sucked into pipe’ in fatal mining accident in Cobar (worker, 26, has died after he was sucked into a pipe in a mining accident – he was working underground at a Copper Mine when the accident occurred about 11.15pm – the man was believed to be trying to dislodge debris from the pipe, which was attached to a piece of machinery, when he became trapped inside – the man was “sucked inside the pipe” – his colleagues freed him a short time later, before paramedics arrived at the mine site – he was taken to a Hospital, but could not be revived) MINE FATALITIES Seven gold miners die in Zimbabwe mine accident (seven gold miners died and 11 were injured when a hoist cage transporting them down a shaft broke loose and plunged into a pool of water – the accident happened on Monday as the workers reported for the night shift) UPDATE on FATAL COM DUST EXPLOSION Second of Seven Workers Injured in G-P ComDust Blast Dies (a second worker has died of injuries sustained in a suspected combustible dust explosion and fire that injured seven workers at a plywood plant on April 26 – worker, 58, died Friday morning at a Burn Center, more than one month after the incident – a week earlier on May 30, a worker, 56, who was also burned in the fire, died – they and five other workers were injured when the 25-foot tall bag house at the plant exploded – one other employee remains in critical condition – the newspaper also reported that the company recently filed an unemployment claim for approximately 275 workers at the plant, which is expected to remain shut down for at least several weeks while OSHA continues to investigate the cause of the explosion) FIRE No one hurt in blasts, fire at Plum Creek plant (an explosion and fire hit a Fiberboard plant – initial report indicated several dozen people could be unaccounted for, they all eventually were located and evacuated – 66 workers and two contractors were successfully FATAL EXPLOSION @ POWER PLANT Kosovo power plant blast kills three, many injured (at least three people were killed and many more were injured in a suspected gas explosion at a power plant – 13 people had been taken there with injuries – the blast, heard 10 kilometres away, was believed to have been caused by gas – plant is the second largest power station in the country with a capacity of 449 MW, more than 30 percent of electricity in Kosovo) ELECTROCUTION Electrician fixing neon sign electrocuted (an electrician was replacing a busted light in the signage of a restaurant when he was electrocuted – he was declared dead on arrival by attending doctors at a Medical Center – before the accident, the worker had crawled inside the tube-shaped neon sign of the restaurant to replace a burned-out light – he was still working on the neon sign when his co-worker saw a pair of pliers fall from the signage – co-worker looked up to ask about the pliers but instead saw the electrician shaking violently – he immediately turned off the electricity and sought the help of peace and security officers and other restaurant employees – he was pronounced dead on arrival) HOTWORK SHIP FIRE INJURES TWO TEXAS FIREFIGHTERS (two firefighters have been hurt while battling a cargo ship fire sparked by welding – both men apparently inhaled hot gas – one firefighter was treated and released following the fire at the port – the other injured firefighter was improving Wednesday at a hospital and should be released in a couple of days) FIRE Phillips 66 fire crew extinguishes blaze in crude unit (a fire broke out at the large crude unit of a refinery at about 1:45 p.m. – fire was extinguished in about 15 minutes by on-site personnel – no one was injured in the fire and employees were dispatched off-site while air quality was measured) FIRE Two injured in fire at Shell’s chemical plant in Netherlands (a major fire triggered by an explosion at a chemical facility has left two workers with minor injuries – fire crews managed to extinguish the flames that raged overnight at the plant – cause of the accident is not yet known – all personnel on-site at the time of accident had been accounted for – there seems to be a benzene leak but it is not known whether it was methylbenzene or ethylbenzene) CONFINED SPACE FATALITY UAE worker dies after inhaling toxic fumes (one worker was killed and four others hospitalized after they inhaled toxic gases while cleaning a diesel tank – the workers were trapped in the tank at their workplace – the police found four workers in a critical condition and helped them out the tank – they recovered the body of a worker who died of suffocation – the injured men are recuperating in a Hospital and are in a stable condition – primary investigations revealed that the men had entered the tank without taking any safety measures or precautions) AMMONIA Worker treated after ammonia leak released from hospital (a chemical leak forced workers to evacuate a little after 3 p.m., after a report of an ammonia leak – the leak was in the ammonia line in the attic area – workers did attempt to turn the ammonia off, but fumes over-bearing and they evacuated – emergency crews arrived and were able to turn off the valve – one person was hospitalized after inhaling ammonia, but has since been released – two others had minor injuries but refused to be treated – the weather helped to dissipate the ammonia) AMMONIA Ammonia Leak at Millard Refrigerated Services (an ammonia leak at a plant – all the employees have been evacuated from the building – officials are not sure how big the leak is at this time – authorities have also issued a full evacuation within a half mile radius of the building – Captain of the Fire Department said they believe a maintenance worker detected the leak – no injuries have been reported) AMMONIA Emergency services called to 300 kilo ammonia leak in Stevenage (a building was evacuated after supplies of liquid ammonia weighing 300 kilos leaked out at 1.35pm – the road was closed as a precautionary measure) AMMONIA 4 hospitalized in ammonia leak (four people have been hospitalized after being exposed to ammonia at a food processing facility – about 10 employees were evacuated from the building after the leak – employees were able to stop the leak and workers who were taken to a local hospital complained of minor injuries) CHLORINE Faulty gauge causes chlorine leak on Del. 9 (a chlorine gas leak sent a vapor cloud across a roadway – the road was initially reported being closed – fire personnel determined there was no need for the road to remain closed – more than 10 pounds of chlorine gas escaped about 8 a.m. from a longtime chlorine producer – the release was due to a faulty gauge – incident remains under investigation) CRANE FATALITY 2 workers die in Mongla crane fall (two workers, 36 and 52, were killed as a crane broke down and fell on them while unloading goods from a elevator shaft accident (one construction worker died after falling down an elevator shaft and a second was injured – the deceased worker fell five floors in the elevator shaft of a six-story building that was under construction -“Either his harness failed or he was not properly hooked in” said a person with knowledge of the investigation – he was pronounced dead while en route to a Hospital – a second worker who was on the fourth floor at the time of the accident, was injured by falling debris and is in stable condition at a Hospital) WORKZONE FATALITY Department of Transportation worker dies in accident (a Department of Transportation worker, 50, has died in a two-vehicle accident – the accident occurred when an 18-wheeler towing an excavator collided with MDOT work truck – the truck was following behind a work crew making repairs to U.S. Highway 61 – impact pushed both vehicles into the median where they caught fire – he was pronounced dead at scene) FATAL FALL thru HATCH Worker Dies After Fall From West Town Roof (a construction worked died after falling off a roof – the man, believed to be in his 40s, was part of a work crew at a construction site when he fell through a hatch or hole in the roof about 9:15 a.m. – he fell about 35 feet and was dead at the scene) EXCAVATOR FATALITY Worker pinned to death by falling equipment (a worker was killed when a hydraulic excavator fell on him during the demolition of a building) TRENCH Worker injured in trench collapse at SouthField (a worker was seriously injured in a partial trench collapse that buried him in dirt – by the time firefighters arrived, other workers had managed to free the man – he was transported to a Hospital – the man, who is in his 50s, was pinned against a concrete conduit containing high-voltage lines – he had been part of a crew working on digging up a water main – worker did not lose consciousness but suffered head and internal injuries) Monorail construction accident kills 1 in Sao Paulo; project originally part of World Cup plan (a construction accident on a monorail meant to expand the city’s metro network has killed a worker – the victim was hit by a large concrete support beam that fell while being erected) TRENCH FATALITY Officials ID worker killed in Mint Hill trench collapse (a worker, 46, was killed after a trench he was working in collapsed – no other details) ARC FLASH Two Burned in Santa Monica Explosion (an explosion and subsequent fire as burned two people, one of whom is currently in critical condition – first responders found two contractors with burns due an electrical explosion in a basement area of a five story building they were working on – one worker was taken to a Medical Center and hospitalized with critical burns over 40% of his body – the second worker was taken to a Hospital to be treated for moderate burns – blast and fire were confined to the basement and were quickly contained) CRANE Several wounded as construction crane topples in Swiss town (a construction crane fell over injuring several people – two people were trapped in the rubble – police did not immediately give details of the number of people hurt, nor the seriousness of their injuries, and did not say why the crane appeared to have tumbled) ELECTRICAL & AERIAL LIFT 2 shocked while working in cherry picker in Miami Beach (private contractors were about 30 feet in the air doing repair and remodeling work on a condominium when the bucket made contact with a high-voltage power line – both workers were taken to a Hospital’s Trauma Center and were listed in critical condition) SCAFFOLDING 2 Painters Fall in Scaffolding Accident (two house painters were rushed to the hospital after the scaffold they were standing on broke around 8:30 – both painters were taken to a Medical Center – no official word yet on the extent of their injuries, but a witness says they were both responsive and could move their arms and legs) Firefighters rescue construction workers after scaffolding collapses (two masonries were rescued from a building after the scaffolding platform they were standing on partially collapsed around 11 a.m. – one of two metal arms drawing cables to the platform running alongside Building #10, reportedly snapped-off the roof – the platform consequently dropped from a horizontal to vertical position, leaving the men dangling from two ropes, some 80 feet in the air – a truck operator then extended an aerial bucket to the eighth floor where firefighters swiftly pulled both construction workers to safety) MISCELLANEOUS FATAL FALL Worker identified in fall death at MGM Grand (a worker, 59, died in an apparent fall at a Conference Center – an accident resulted in a worker falling from the third floor of the Conference Center – circumstances of the accident were not released, but the accident is under investigation by both Metro police and OSHA) FORKLIFT FATALITY Limington man dies in accident on job site in Freeport (a worker for a landscaping crew was killed just before 9 a.m. – it appears a piece of equipment may have fallen on the victim – authorities will examine the loader to determine whether a mechanical problem could have contributed to the accident – he had been a mason for more than 30 years, and until recently owned his own company – he another man who was operating the loader were trying to remove its door when the forks dropped – he was killed instantly when the granite fell on him) UPDATE on HOTWORK FATALITY Barge captain convicted of negligence in explosion that killed deckhand and polluted canal (a barge explosion killed a 29-year old worker and released thousands of gallons of oil into a shipping canal – in a rare criminal conviction for a workplace fatality, a tugboat captain and his uncle’s business were found guilty of negligence charges for the death of a crewhand killed in a massive 2005 barge explosion – the captain, 35, was piloting a tugboat pushing a steel cargo barge on the Chicago Sanitary and Ship Canal when it exploded, killing deckhand, 29, sinking the barge and spilling 600,000 gallons of concentrated slurry oil – a petroleum byproduct – the captain illegally ordered the worker to use a small propane torch to heat a frozen discharge pump moments before the barge was to be unloaded – judge convicted the captain and his uncle’s company of one count each of negligent manslaughter and negligent discharge of oil – he faces up to five years in prison, while his uncle’s company faces probation and a fine of up to $200,000) HOTWORK Gas vapor explosion at salvage yard injures one (an employee at a local salvage yard was injured after an explosion just after 3 p.m. – employees smelled gas while welding in a part of the yard when the explosion happened – workers indicated they had an odor of gasoline in the area when there was an explosion – one employee was treated for minor flash burns to face area – no loss of property was reported) HOTWORK on TANKS Two men burned after trying torch 250-gallon tanks in Roane County (two men were burned while attempting to scrap two, 250 gallon tanks around 2 p.m. – the men were using a torch to cut the tanks into smaller pieces, so they would be easier to scrap – they were torching the tanks in a yard – it was evident that one man was able to cut a square in the tank – shortly afterwards, there was an explosion – that man holding the torch ran into the nearby creek in an attempt to put out the flames – he was transported to a burn unit with critical injuries – the second man suffered first and second degree burns on his body – fire marshal said the cause of the explosion seems to be an over-pressure rupture – contents inside of the tank looked to be crude oil, and tests ran on the substance appeared to confirm those findings) |
