One dead in crane accident at Japan nuke plant (one person was
killed and four injured in a crane accident at a quake-hit nuclear
power station and four others were injured in a blast at another
stricken plant nearby – UN{apos}s nuclear watchdog said it had been
informed by Japanese authorities that the worker was killed and four
others injured in a {amp}quot;crane operation accident{amp}quot; at Fukushima No. 2
power plant – the agency said that at Fukushima No. 1 power plant,
which suffered an explosion a day after the massive quake, and where
authorities are battling a feared meltdown of two reactors, eight
workers had been affected)

man was killed after falling 33 feet from a tower at the Water Works
plant – the victim was a subcontracted employee who was working on
an upgrade project at the Water Filtration Plant)

was clearing a piece of equipment that had gotten plugged – incident
resulted in a non-life threatening personal injury and the
individual is hospitalized for treatment – the plant processes corn
and other grains into solutions for the pharmaceutical, human
nutrition, animal nutrition, paper, fermentation and industrial
markets)

worker killed at Kroger warehouse in Roanoke Co. (truck driver,
26, died after becoming pinned between two tractor trailers at the
warehouse – he ran behind a tractor trailer that was backing into a
parking space to alert the driver that the space was taken – he was
caught between the two tractor trailers and was taken to a hospital
where he died)
LOTO FATALITY on WAREHOUSE PICKER SYSTEM
1 Pepsi Worker Killed, 1 Injured in Tampa Industrial Accident
(an automated storage and retrieval system used to move pallets of
product collapsed – the lift of the system was about 20 feet up in
the air when it came down on two men – one man was trapped
underneath it and died – another man{apos}s legs were seriously injured –
paramedics treated him, and he was taken to a Hospital – people are
not usually in the area where this machine is working – both of the
men are contractors who were doing maintenance work on the system)
DRILL RIG FATALITY
Drilling
rig falls on barge killing worker (a drilling rig fell over on a
barge killing a drill worker – the worker, 30, was pinned in between
the rig and a mud tank following the rig accident – the operation
started out as a rescue but once officers arrived they found that
the worker had died and the operation became a recovery –
firefighters spent hours trying to remove him from the scene and
eventually got him out at 10:30 p.m. – firefighters needed to cut
through a big, steel tool box that had also fallen on him when the
rig fell over to get him out – five other drill workers were able to
escape the scene unharmed)
MINE FATALITY
Fatal mine accident victim identified – Second fatal accident in the
Camp in 2011 (a miner, 57, was killed in a fatal accident at the
gold mine – he was fatally injured when he was struck by underground
mobile equipment – he had been a certified electrician at the mine
for the past 11 years – this is the second fatality to occur in the
mine within the last two months – another miner, 28, died in an
underground accident on Jan. 6 – he reportedly died of a {amp}quot;crushing
injury to his chest{amp}quot; while he was using a jackleg drill in a
vertical raise about 250 feet above the 8,200 level of the mine)
AMPUTATION
Meadow Creek Cedar{apos}s mill is temporarily down while safety issues
are addressed (WorkSafeBC ordered the mill to stop using all
machinery requiring safe guarding – it follows an accident in which
a worker lost parts of three fingers – the employee, who was
reportedly young and inexperienced, caught his right hand between a
live chain and the drive sprocket of a belt conveyor – WorkSafe
officers were on site the next day and noticed “a number of power
transmission units” were unguarded, despite being brought to the
company{apos}s attention during previous inspections, most recently on
February 9)

Four steel workers burned in factory accident at Northgate (four
steel workers are being treated by paramedics with minor burns and
smoke inhalation after machinery caught fire at a factory –
hydraulic oil is believed to have caught fire – fire fighters are
still at the scene trying to stop the blaze)

Mechanical failure sparked Friday’s blaze (fire officials say a
mechanical failure sparked a blaze that burned an oil worker – a
worker was taken to a Hospital, but it’s unclear how seriously he
was burned – the blaze damaged an oil tanker and destroyed a nearby
building – the tanker was used to treat oil – fire caused about
$25,000 in damage)

Worker burned in Nuneaton factory acid spill (a man suffered
burns and scores of workers had to be evacuated when chemicals
spilled out at an industrial unit – fire crews used specialist
gas-tight suits and breathing apparatus to deal with a serious
nitric acid spillage – one man, believed to be a staff member, was
taken to hospital for treatment to acid burns and the area remained
closed for several hours, while a specialist contractor was called
in to dispose of the chemicals)

Mill injury in Brokaw (a worker, 47, was injured at a paper mill
when his leg was pinned between a paper machine and a fork-lift –
accident happened around 4am – he was taken by ambulance to a
Hospital, where he was treated and released – a forklift operator
was backing up, and didn{apos}t realize he was behind his machine)

Man killed in concrete mixer (a man died after he was crushed in
a concrete mixer – the man, in his early forties, was instructed to
clean a concrete mixer – a task he carried out regularly – a lack of
communication of some sort led to the piece of machinery being
turned on whilst the man was inside it still cleaning the mechanism
– when paramedics arrived on the scene, the man remained trapped in
the machine – he sustained fatal crushing injuries to his upper body
and was declared dead on the scene – it is the second incident in a
month in which a worker was killed in a concrete mixer – on February
23, two men were cleaning a machine at a concrete lintel factory
when it suddenly switched on, causing them to fall into it – the men
called for help, but by the time they were pulled from the machine
they had already sustained serious injuries – one man died on the
scene and the other was airlifted to hospital with severe injuries)
HOTWORK FIRE
Oil tank fire snuffed out (a fire was doused at an above ground
storage tank adjacent to an oil well – firefighters used foam to
snuff out the fire – the fire started when heavy crude oil in the
steel tank was ignited by a person cutting the metal tank{apos}s top with
a torch during maintenance work – some 200 gallons of heavy crude
were in the tank when the fire started but no oil escaped from the
tank and there were no injuries – the tank stayed intact)

Months (a strong odor accompanied a spewing gas leak at the
refinery – the refinery was shut down and vehicular access to a
Street was blocked – crews were able to shut the line down and
secure the area)
NUCLEAR PLANT EXPLOSION
Blast at Japan nuclear plant adds new threat (an explosion at a
nuclear power plant destroyed a building and added leaking
radiation, or even outright meltdown, to the threats menacing a
nation just beginning to grasp the scale of a catastrophic
earthquake and tsunami – radiation emanating from the plant appeared
to have decreased after the blast, which produced an intensifying
cloud of white smoke that swallowed the complex – it appeared to be
a consequence of steps taken to prevent a meltdown after the quake
and tsunami knocked out power to the plant, crippling the system
used to cool fuel rods – the blast destroyed the building housing
the reactor, but not the reactor itself, which is enveloped by
stainless steel 6 inches thick – inside that superheated steel
vessel, water being poured over the fuel rods to cool them formed
hydrogen. When officials released some of the hydrogen gas to
relieve pressure inside the reactor, the hydrogen apparently reacted
with oxygen, either in the air or the cooling water, and caused the
explosion)

homes ‘something fierce’ (a chemical plant has exploded, shaking
nearby homes and injuring at least four people – plant caught fire

Ammonia leak prompts evacuation in Hazleton (a leaking ammonia
tank discovered at a city bakery prompted firefighters to close down
an entire city block and disrupted school dismissals – as of 7 p.m.,
emergency personnel slowed a leak detected in a 1,000-gallon
cylinder – the property formerly housed the Armour Meat Packing
plant – medical personnel determined the three employees at the
bakery were fine – residents two blocks from the bakery were {amp}quot;held
in place{amp}quot; or asked to stay inside their homes with the windows and
doors closed – the former building owners abandoned the ammonia
cylinder, and the current owners may have bought the property
without realizing it was left behind – earlier in the day that three
workers at the bakery called 911 at 12:30 p.m. after smelling an
odor at the business)

Forklift truck accident injures man at Southport industrial firm
(a 47-year-old man was injured by a forklift truck in a workplace
accident – he was clipped by the reversing truck at the metal
fabricators and he was taken to a hospital for treatment but later
discharged to recover at home – company officials claim the worker
was wearing earplugs and couldn’t hear the forklift truck – when he
was hit he suffered an asthma attack and coworkers didn’t know where
his inhaler was, so they called the ambulance – quote…“The irony
is that if he wasn’t following health and safety rules by wearing
earplugs, he would have heard the truck.”)

site – he was a father of four, ranging in age from nine to 19 –
when crews arrived, they found a worker seriously injured – he was
taken to hospital where he was pronounced dead – the accident was
crane-related and involved hoisting and rigging)

Man dangles 30 feet above ground before co-workers rescue him
(an industrial accident left a sheet metal worker dangling in a
safety harness 30 feet above the ground at a construction site –
there were no injuries and construction workers rescued the man
without calling for outside assistance – incident happened between 7
and 7:30 a.m. and the worker was suspended for about 10 minutes

City suspends building work after fatal accident (worker, 34,
died after a drum that was being pried open by co-worker using
welding equipment exploded – they were working on the 21st floor –
as the worker started to pry open the cover, it exploded and hit the
worker in the chest)
FORKLIFT
Bridge worker averts injury in forklift fall (a construction
worker was in a telescoping forklift when it flipped over the side
of the bridge – he was uninjured, however, thanks to the bridge’s
railing – the accident occurred when the lift reached out too far
while trying to put a 32-foot-long, 1,400-pound steel girder in
place for a walkway on south side of the bridge – several other
similar girders had previously been installed without a problem, but
the telescoping lift was leaning at a different angle because the
lift had to reach between other beams on the bridge – the driver of
the low-profile forklift was buckled into his seat and rode it out
as the weight of the beam pulled the forklift over – the forklift
caught on the bridge railing, which kept it from going over the side
– attached to the lift by a cable, the girder was left dangling over
the river near the east end of the bridge)

Worker dies at road construction site (a 50-year-old truck
driver was killed when a steel plate struck him in the head just
before 8 a.m. – an excavator was loading a large steel plate onto a
truck when the plate slipped and fell, hitting the driver – he died
at the scene)

Worker Burned in Flash Fire at Cincinnati College (a small
explosion and flash fire caused minor injuries to a worker – an
outside contractor was working in a room at the Advanced Technology
and Learning Center and apparently touched a wire which arced,
causing a small flash or explosion – the worker suffered burns to
his face and hands and was transported to a nearby hospital)

New Orleans construction worker electrocuted in St. Bernard Parish
(a construction worker, 27, died of electrocution while working
underneath a house – he was raising a home about 3:45 p.m. when an
electrical wire touched his hand and face)

Man Falls on Scaffolding at Financial District Construction Site
(a 40-year-old man fell from the roof onto a seventh-floor scaffold
was taken to a Hospital in serious condition – he fell about 10 feet
off the roof of the site shortly after 10:30 p.m. – he fell
one-story from the roof and apparently landed on a construction
scaffold that was set up on the building{apos}s seventh floor – he is
listed in serious condition)

DUMBWAITER FATALITY
Woman dies in fall down dumbwaiter shaft (a restaurant employee,
21, died three days after falling down a dumbwaiter shaft –
investigators, initially told it was an elevator shaft, discovered
it is actually a material lift not intended for passenger use –
while elevators require yearly inspection, dumbwaiters are only
required to be inspected on installation – the building containing
the dumbwaiter was built in 1947 – she was loading or unloading the
dumbwaiter on the first floor when it had a mechanical malfunction –
the dumbwaiter fell to the basement and took her with it)
TREE TRIMMING FATALITY Huntington Man
Dies in Tree-Trimming Accident (a man, 39, died in a tree-trimming
accident – he died after being struck by a tree – he and fellow
workers were doing a job for a local power company to remove the
tree from a line – accident happened around 12:45 p.m.)

Man dies after tree trimming accident (worker, 29, had been
trimming the top of a large tree when the tree splintered and caused
him to fall nearly ten feet before his safety harness stopped the
fall – even though the harness stopped his fall, the tree continued
to separate over the top of him as he hung nearly twenty feet from
the ground – he was found dead by rescue teams who had climbed the
tree to free him)
CARBON MONOXIDE
Carbon monoxide exposure hospitalizes 5 workers at Atlantic County
farm (five workers at a farm were hospitalized after being
overcome by carbon monoxide fumes – the five were found outside a
greenhouse on the farm around 8:20 a.m. – they were taken to a
Hospital for treatment and were expected to fully recover – the leak
apparently was caused by a faulty heater or pipe in the greenhouse,
but officials say the investigation is ongoing)
Worker freed from Port of Seattle crane (firefighters rescued a
worker who got stuck in a crane elevator at the Port terminal – the
worker was taking the elevator to the operator{apos}s cab when it became
stuck about halfway up the 200-foot-tall crane – fire crews brought
the man down with a ladder – the crane is used to load and unload
containers from cargo ships)
