INDUSTRIAL FATAL FALL from HELICOPTER Oil worker dies after falling 2,000ft from helicopter into North Sea (an oil worker has died after falling almost 2000ft from a helicopter above the North Sea – accident investigators are probing the circumstances of the man’s death, but it is believed an emergency door opened during the flight and he plunged through it) TRAPPED in FREEZER Man survives over four days in freezer in southern Vietnam (worker, 23, was rescued from a minus degree storage freezer after more than four days – doctors said the seafood company worker had reached stable condition after spending 103 hours in a freezer – he and three colleagues were cleaning and rearranging a 3,000 square meter cold storage unit when the shelving units collapsed – two of the workers managed to escape the room – two other workers were found later that night) ELECTRICAL Industrial accident sends one to hospital (worker at a Plastics business was taken to hospital after an industrial accident – worker was taken to hospital with non life-threatening injuries – she told emergency crews she thought she was electrocuted, but they are unsure of what caused the accident) LOTO/GUARDING FATALITY Worker dies at JSR synthetic rubber plant (worker, 19, has died following an accident at a styrene-butadiene synthetic rubber plant – employee was caught in manufacturing equipment at around 12:25 local time – after being freed, the man was taken immediately to hospital but was confirmed dead on arrival) Worker dies in accident at Galax mirror plant (a worker, 37 died at an industrial plant after becoming trapped in a piece of machinery at about 2:45 p.m. – victim was pronounced dead at the scene – cause of the incident is still unknown) Work orders issued in Tecumseh industrial accident (a worker was taken to hospital with non-life threatening injuries after his boot got caught in one of the factory’s machines – a stop work order, attached to the order regarding cleaning, oiling, repairing or maintaining a machine) MINE FATALITY Mine safety in question as 20-year-old worker dies of suffocation (worker, 20, died of suffocation at a micro-mine – another worker, 40, was also found dead due to lack of air in a mine at the same district last year in May – the victim was pulled out from under 100 meters below the ground – District’s inspectors and the police worked at the accident scene and discovered that the opencast mine’s power was cut off) FRONT-END LOADER FATALITY Man dies following industrial accident in Brooksville (a worker, 51, has died following an industrial accident at a cement plant – he was driving the first truck of the day to dump a load of ash – as he was exiting his truck, the driver of the front-end loader, who was moving debris from one bin to another, did not see him – as the driver of the front-end loader attempted to return to the bin, deputies said he backed into the worker, pinning him against the cabin of his own semi – he was transported to a hospital, where he died) UPDATE on FATAL PLANT COLLAPSE Investigators: Overloaded Bins Caused Collapse (overloaded storage bins on the roof of a livestock feed manufacturer’s plant caused the building collapse that killed two people in January – nine storage bins on the plant’s roof were overloaded, and the additional weight caused three floors to collapse inward in about 30 seconds – the Jan. 20 collapse killed two workers, 47 & 53 – several others were injured – four workers were trapped in the rubble after the collapse and had to be rescued by firefighters) MINE ELECTRICAL Worker sent to hospital in potash mine accident (worker at the potash mine was sent to hospital with serious injuries after receiving a serious electrical shock at around 9:30 a.m. – not (worker, 45, was rushed to hospital after being seriously injured in an industrial accident after he got his leg caught in machinery – worker is thought to be a trainee on his first day of work) FORKLIFT FATALITY Employee killed in forklift accident at Benton Co. plant (a maintenance worker, 58, has died after a forklift accident – the employee was reportedly working to repair the forklift when it fell on him – he had been with the company for two years) LOTO on AUGER Waterford Worker Injured on Farm (worker, 57, was flown by helicopter to a hospital after falling into auger equipment used to move crops – employee was doing cleaning work when he fell into the auger – he was trapped for 30 minutes and suffered multiple traumatic injuries) HOTWORK EXPLOSION Glycol flash burned welder at Mattydale chemical facility (officials suspect a welding torch ignited fumes, causing a small explosion at an industrial facility, which burned a welder and knocked him from a ladder – the man had been welding a large tank) FIRE Fire at Consol Energy Site in Pennsylvania Contained Quickly (a faulty valve that leaked gas is thought to be the cause of a fire that burned briefly at a pump station near a four-well pad before it extinguished itself – no injuries were reported at the site and all personnel were accounted for – the wells were not damaged) FATAL FALL from CATWALK Worker falls to his death at TECO plant (a contractor, 31, fell to his death from a catwalk at a power plant – he fell through a metal grate on the catwalk in the 1:15 p.m. accident – he fell 40 to 50 feet and died at the scene – the plant had previously been converted from coal to natural gas power generation, and workers were dismantling an old boiler that no longer was needed) SKID STEER FATALITY 200kg bobcat load kills worker at Wodonga foundry (worker, 42, has been crushed to death after a 200kg load in his bobcat’s bucket crashed through the vehicle’s windscreen – he was working at a foundry at about 11pm when the contents fell from the bobcat’s bucket and through the windscreen) Employee dies at Gaston County plant (a man has been killed while working shortly after 2 a.m. – the worker had died from a crushed skull when emergency workers arrived – no word on cause) HOTWORK and COMBUSTIBLE DUST Man welding in Ensley injured in flash fire (a flash fire burned a man who was welding on top of a storage container – the incident happened around 2:45 p.m. – the fire was caused by dust that ignited in the area where the man was welding on top of the container – victim fell approximately 35 feet and was taken to a Hospital to be treated for second degree burns to his face and arms, as well as a broken leg and other injuries) REFINERY TANK FIRE Mexico oil refinery fire leaves 23 injured (a fire at an oil refinery in northeastern Mexico left 23 workers injured before firefighters managed to put it out – the fire at the MJN-T-510 storage tank at the refinery has been extinguished – workers at the refinery rushed to help fight the fire when it broke out – two suffered light burns and 21 were treated for dehydration and exhaustion – 21 of the 23 workers who received medical attention have been released – authorities had evacuated families from the surrounding area as the army set up a perimeter around the refinery) UPDATE Fertilizer makers blame city for plant explosion (two fertilizer companies sued following the massive explosion in West last year that killed 15 people are claiming the small town is partly to blame – they contend in court filings that West failed to properly train first responders and had lax protocols in place to battle the April 2013 blast – motions filed in state court in Waco seek to designate the city of West as a “responsible third party” in lawsuits filed against the companies) ETHANOL PLANT EXPLOSION No one injured in Arthur, Iowa, ethanol plant explosion (an explosion in a distilled grains drying area shut down the plant to allow officials to determine the extent of the damage – no one was injured) FATAL EXPLOSION One of those injured in Puerto Rico power plant explosion dies (one of the three employees who were injured in a July 13 explosion at a power plant died on Tuesday of the severe burns he suffered – he was the most critically injured of those hurt in the explosion at the power plant) AMMONIA Ammonia leak closes Houston Ship Channel, sends 4 workers to hospital (ammonia leaked from a vessel and forced the closure of a Ship Channel and nearby bridges while sending four workers to a hospital – the tanker was docked when a gasket blew and anhydrous ammonia leaked – a tugboat was refueling the ship when the leak occurred, and four of its crew members were exposed and required hospitalization) AMMONIA One transported to hospital following ammonia leak at Tyson (an ammonia leak impacted four people – one of the workers was taken to a Regional hospital via ambulance, but it is unclear the extent of their condition) CONSTRUCTION FATAL SCAFFOLDING COLLAPSE Scaffold collapse kills 2 Vietnamese at Taiwanese steel mill (two workers, 23 & 45, were killed and three others severely injured after a scaffold collapsed at a construction site around 7:20 a.m. when a group of workers was working to build a water plant at the site – three others were rushed to hospital – one had brain injury while two had their thigh bones broken and sustained other injuries) FATAL FALL inside POWER PLANT STACK 2 Dead, 1 Hurt in Mishap at Power unit Construction site (two construction workers, 26 & 31, of a power company died after they fell from a high chimney – construction of a thermal power project was underway – work was being carried out inside a chimney and workers were attached to the structure with ropes – the workers fell to the floor and died on the spot – sources said the ropes broke, leading to the death of the two workers – another worker, 18, lost his leg due to the impact of the fall) TRENCH FATALITY Body of worker killed in Ft. Bragg trench collapse recovered (part of the side of the trench collapsed around 11 a.m., covering a worker – the trench was dug into the top of an earthen ammunition bunker – a manmade hill that stores ammunition inside – the worker was in the trench to connect two ventilation pipes) CRANE Houston crane operator injured 200 feet in the air (Fire Department crews use a crane to rescue an injured construction worker high above the ground – the crane operator was apparently injured shortly after 4 p.m. – he was brought down to the ground about 5:20 p.m. – he was about 200 feet up in the air when the accident occurred – details of his injury weren’t immediately known) CUT THROAT Construction worker was taken to the hospital after his throat was cut on the job (it happened around 8a.m. – the 30-year-old man did receive a serious injury but is expected to be okay – we have not yet been told how this accident happened) TRENCH FATALITY Construction worker dies of positional asphyxia (a construction worker died after becoming trapped in a trench at about 2:45 p.m. – fellow workers tried to dig the man out but could not free him – the man could not breathe and died of positional asphyxia) FATAL FALL from WATER TOWER Georgia construction worker dies after fall from Brookhaven water tower (worker, 41, has died after falling from a water tower – company specializes in the repair and renovation of water storage tanks – it has a contract to sandblast and repaint the tower – he was working with a crew when he fell from a scaffold and plunged 70 to 90 feet) WORKZONE FATALITY Trash truck kills construction worker in Reno County, hits vehicles stopped for construction (a construction worker, 22, died after being hit by a trash truck – he was removing construction cones when he was hit – before the truck hit the worker, it rear-ended one vehicle and sideswiped two others that had stopped for the construction – the deceased and two others were sent to hospitals) WORKZONE FATALITY Road Construction Worker Dies on I-84 near Boardman (at approximately 11:12 p.m. a worker, 64, a member of a construction company contracted with ODOT to install rumble strips at a paving project site was killed – he was working outside of a 1999 Dodge pickup operated by another employee when the pickup, which had a piece of equipment in tow, began to slowly move forward and he was run over by the rear dual tires) FATAL FALL from BRIDGE Bridge worker killed in Kansas City identified (worker, 53, died after falling from a bridge) AERIAL LIFT Worker sent to hospital after accident at Kenosha Amazon construction site (an accident at the construction site of a distribution center has sent a worker to the hospital – the man was pinned between a beam and an aerial lift about 3:30 p.m. – he was freed by co-workers and taken to a hospital with a shoulder injury – no other workers saw the accident) FATAL FALL from SCAFFOLDING Worker falls to his death at Intel (worker, 62, died after falling off a scaffold – he fell about 20 feet) TRENCHING Worker rescued from collapsed trench (cameras captured the dramatic scene of rescue crews pulling a worker, 36, to safety after a five-foot trench caved in on top of him – he was working on a fire hydrant at the time of the accident – it took 30 rescue workers more than 30 minutes to pull the man to safety – he was rushed to the hospital in critical condition) WORKZONE St. Petersburg worker critically injured in tractor accident (a construction worker, 64, was seriously injured when he was backed over by a brush tractor as he attempted to climb aboard at about 7:45 a.m. in a construction zone – as he climbed up, the tractor somehow slipped into reverse – he was thrown off the back of the tractor, which stands about 6 feet tall – his leg was pinned under a front tire and he was dragged – the tractor then rolled over his upper body) FATAL FALL off LADDER (KNOCKED OFF LADDER) One dead, two hurt in Lauderdale construction site accident (one worker was killed, two others were hurt in a construction accident when a beam collapsed – the death, which apparently occurred when the three men were knocked off a ladder, marks at least the sixth workplace fatality in Broward County in the past seven weeks) FATAL FALL from LADDER Worker killed in construction accident at Bellevue College (a construction worker, 31, was killed in an accident at the College – the worker was on a ladder on the fourth story of the new Health Sciences Building when he fell about 50 feet about 10:30 a.m. – he died at the scene – he had a safety harness on, but it is not known if it had been attached to anything when he fell – witnesses said he was on a ladder, fell from that ladder and down several stories to the ground below) WORKZONE Shovel from Truck Hits Highway Worker (a highway worker suffered head injuries after being hit by a shovel that fell out of a truck – employee was in the median, placing construction signs along the southbound lanes when a shovel flew off a passing pickup truck – worker required nine staples in the top of his head – he was released from the hospital the same day and is recovering at home – police believe the driver was switching lanes when it happened) ELECTROCUTION Lawrence accident investigation: Man likely electrocuted when guttering touches power line (worker, 54, was killed as he worked to repair guttering on a building – investigators suspect he got too close to the power lines as he was working on the building – a fellow worker made the 911 call – worker was pronounced dead at the scene shortly after first responders from the police and fire departments arrived at about 8 a.m.) FATAL FALL down ELEVATOR SHAFT Worker killed in construction accident near UofL campus (worker, 34, on a construction project has died after a fall – he was found dead at the bottom of an elevator shaft – it appears he had fallen) GAS CYLINDER EXPLOSION Two injured after mine blast (two employees were injured in an explosion at an excavation project – a gas cylinder exploded – employees who were in the vicinity were being assessed for possible gas inhalation) WORKPLACE VIOLENCE Hospital shooting suspect charged with murder (a man accused of fatally shooting his caseworker and grazing his psychiatrist at a hospital complex before the doctor returned fire has been charged with murder – the man, 49, was ranting about the hospital’s gun ban before he shot a 53-year-old caseworker during an appointment – a psychiatrist crouched behind a chair and pulled out his own gun, firing several shots – he had 39 unspent bullets on him when he was wrestled to the ground – two guns, including a revolver that police said he used to kill the case worker, were found at the scene) MISCELLANEOUS ELECTROCUTION Man Electrocuted In Mowing Accident In Bartlesville (a mowing attachment became tangled in an electric pole guy wire – he was mowing with a tractor when the mower deck became tangled in a guy wire supporting a utility pole – his co-worker, 66, attempted to free the guy wire from the mower when it became electrified – 7,600 volts passed through hiss body, killing him instantly – the other man was not injured, because he remained on the tractor and didn’t touch the guy wire) ELECTROCUTION Worker Electrocuted at California High School (a man was electrocuted and another badly shocked during a construction accident at a high school – a construction crew was working on the High School’s athletic field when one of the workers was electrocuted – a second worker attempted to help the first and climbed onto the scaffolding they were building to hang a banner – he received an electrical shock and was thrown 30 feet onto the ground – both workers were taken to a Hospital – the first worker was in his early 20s and was pronounced dead at the hospital – the second is also in his early 20s and is in serious condition) ELEVATOR FATALITY Teen worker dies in freak lift accident (a worker, 19, at a medical shop was killed when the bottom of a goods’ elevator hit her head – she is believed to have looked into the lower part of the lift shaft, which is inside the shop, when the incident happened about 7.20pm – initial investigations revealed the girl could have unintentionally pressed a button which caused her head to be trapped between the iron poles and the gate where she was standing) TREE CUTTING HEAD INJURY Worker injured by falling tree branch (a worker, a tree cutter, was injured around 10 a.m. after he was struck by a falling tree limb – he was hit in the head and taken by ambulance to hospital – condition is unknown at this time) CRUSHED by CAR Auto shop worker killed when car falls on him (an auto shop worker, in his 30s, was killed when a car he was working on fell on top of him – people on scene tell News 4 that a man has been renting the space there and working countless hours as a mechanic – he had jacked up a Lincoln Town Car on grass and was working on it when it fell, killing him) HOTWORK Men suffer burns after welding tool sparks fire at Burslem business (two men needed immediate hospital treatment after suffering burns when a piece of welding equipment set a car on fire shortly after 2.05pm – it is believed the blaze was started accidentally after a welding tool set a saloon vehicle alight) |
