Amtrak worker burned in incident (a worker was burned in an incident at 11:41 a.m. at a maintenance facility – business would not provide any more details about the cause of the accident or the extent of his injuries)

ELECTRICAL AEP worker injured after touching power line (worker, 34, was critically injured when he touched a live power line at a construction site – he was flown to a Medical Center in cardiac arrest – the jolt was strong enough to spark a fire to the work truck and a nearby utility pole, and caused a power outage to more than 400 businesses and residences)

MINING Worker airlifted to hospital in critical condition after underground workplace accident (worker, 38, was been airlifted to a hospital with critical injuries just after 2 p.m. after two mining-type vehicles collided in an underground tunnel)

UPDATE CHLORINE FATALITY Worker dies after Spokane chlorine leak accident (one of the people hospitalized after a chlorine leak at a recycling company has died – the other three workers hospitalized after the accident have been released from the hospital – more than a dozen people became sick from the chlorine, with most of them reporting respiratory issues – the gas came from a big cylinder that workers were crushing for scrap metal)

WORKZONE and BUCKET TRUCK Electric worker injured after accident in Wildwood (an electrical worker working on power lines was injured after a car accident – worker was in bucket truck working on power lines near an intersection when a semi hit the truck, knocking him out of the bucket – he suffered serious injuries and was taken to a hospital)

UPDATE on FORKLIFT FATALITY Dockworker had been drinking prior to fatal port accident, sources say (a dockworker, 49, died after she was struck by a front loader – the driver of the vehicle that struck and killed her had been drinking before the accident, and criminal charges are expected to be filed – the worker bled to death after her leg was severed)

LOTO FATALITY by ROBOT WELDER Worker killed by robot in welding accident at car parts factory in India (worker, 24, was reportedly adjusting a metal sheet being welded by the machine when he was stabbed by one of its arms – he had been working at the factory for around 18 months when the accident happened)

Jaguar Land Rover worker injured in factory accident (worker, in his 30’s, was taken to hospital with head and chest injuries after an accident just before 9.45am – he had a chest injury and a minor head injury after an incident – no other details)

FLASH FIRE @ DRILL RIG Oilfield worker recovering after well fire (an oilfield worker injured in a fire at a well site remained in critical condition – another employee also suffered injury – that employee was treated and released from an area hospital the day of the accident – a worker for a different company was also injured)

MOBILE EQUIPMENT FATALITY Industrial accident victim fatally struck by equipment (worker, 47, died in an industrial accident at a scrapyard when he was struck by a piece of equipment while working in the yard – worker was pronounced deceased at the scene by the coroner – no further details were released)

Worker seriously burned at power plant in Indiantown (worker was severely burned at a power plant at about 8:40 a.m. when there was a fire in the generator room – worker was flown to a Hospital – the plant is a “330 megawatt pulverized coal-fired facility” that produces electricity)

EXPLOSION Explosion at China chemical plant leaves one dead and nine injured (one person has been killed and nine injured in an explosion at a chemical plant – fire was put out after about five hours and authorities said no contamination has been detected – factory produces adiponitrile, a colourless liquid that releases poisonous gases when it reacts with fire)

ASSEMBLY LINE Colleague of JLR worker crushed on production line claims ‘speedy intervention of co-workers saved man’s life’ (worker was crushed and rushed to hospital after body shell fell – injured worker was working on a section where F-Type bodies and engines are combined when the car body crushed him after break at 9.40am – days after Jaguar workers returned to the plant from their annual summer shut-down – co-workers rushed to the man’s aid, with between 20 and 30 of them helping to lift the car from him – he is now stable with multiple broken ribs, a broken collarbone and multiple lacerations and bruises)

FORKLIFT FATALITY Worker crushed to death by pallet (worker, 49, died after he was crushed by a pallet while on company premises – the loaded pallet got detached from a hoist for reasons still to be determined, crushing him to death on the spot – colleague who witnessed the incident was taken to the emergency room unharmed but in a state of shock)

COMBUSTIBLE DUSTS Worker burns face in ‘powder explosion’ at factory (firefighters found that a hopper, which is used for storage, was giving off heat – a member of staff working on the production line had received minor injuries)

CONFINED SPACE FATALITIES 2 Workers Choke to Death in Sewer Hole (two workers, 29 and 31, died of suffocation while cleaning a manhole, while a third was hospitalized – incident took place around 12.45 pm – first worker slipped into the sewage, second worker tried to rescue him, also fell into it – the third worker started screaming for help)

HOTWORK EXPLOSION Dozens of firefighters worked to put out fire after explosion; exact cause still not determined (two employees were welding on a large chemical holding tank when it exploded at about 11:15 a.m. – although firefighters were told the two men were welding, what exactly caused the liquid holding tank to explode is not yet known – the chemical holding tank contained a mixture of about 80 percent water, 20 percent sodium hydrosulfide – the assistant manager was welding on the outside of the tank with the help of a co-worker – the two men were working at the top of a ladder approximately 40 feet above the ground – explosion ripped open the metal top of the holding tank, sending pieces of it flying into the air)

REFINERY FIRE Oil Refinery Near Houston Struck By Lightning, Catches Fire; Flooding Reported (a lightning strike was blamed for an oil refinery fire – a lightning bolt struck a transformer at the site – no injuries have been reported, nor are nearby residents in any danger – fire would be monitored and allowed to burn itself out)

Kuwait oil refinery shut down after fire (an oil refinery has been shut down after a fire broke out – all personnel were evacuated from the refinery and that no one was injured in the accident)

Delaware City Refinery fire under investigation (an equipment problem touched off a fire at a Refinery – no injuries were reported – a compressor malfunction caused the fire at about 1 p.m.)

Fire hits HollyFrontier’s Tulsa refinery (a fire at a 125,000 barrels per day refinery is being handled by the company’s in-house fire crews)

EXPLOSION Investigators: explosion at Houston chemical plant accidental (an explosion at a chemical plant warehouse is believed to have been triggered by oily rags stored at the warehouse, or a problem with electrical wiring or gas-powered equipment – fire started at about 4:30 p.m. inside a locked warehouse – a storage tank holding an unidentified chemical near the building ruptured during the blaze, causing an explosion that added to the thick, black smoke in the air – fire, which took fire fighters about two hours to bring under control, caused an estimated $3 million in damage – no one was injured in the accident)

FATAL EXPLOSION Five Killed in Mexico Pipeline Explosion (five workers at a contracting company died when machinery they were using cut through a gas pipeline, causing an explosion and fire)

AMMONIA 10,000 residents evacuated after ammonia leak at Sichuan chemical plant (a pipe at a fertiliser plant sprung a leak exposing 10,000 people to ammonia liquid – a government order to evacuate all the residents followed – some people who were very close to the plant at the time passed out from inhaling the ammonia fumes)

CONSTRUCTION

AERIAL LIFT FATALITIES 2 construction workers killed in Ellicott City (two construction workers, both 31, were killed when they were thrown from a lift machine that tipped over at a site at about 7:45 a.m. – both men were in the bucket of the lift machine when it tipped over on its side – investigators don’t yet know why the machine tipped over – the men were found on the ground suffering from serious injuries and taken to a Hospital, where they were both pronounced dead)

BRIDGE FATALITY Asian worker crushed to death by falling steel (a construction worker was crushed to death when massive steel bars collapsed on him during maintenance of a bridge – workers used cranes and ropes to pull the body out after it was buried by tonnes of steel used in the construction of the bridge – a colleague suffered from medium injuries and was taken to hospital)

FATAL FALL Construction worker dies after 60-foot fall at Madison site (construction worker, 25, died after falling 60 feet at the site of a future apartment building around 11 a.m. – worker died later at a hospital – he had worked as a carpenter for the company for three years)

WORKZONE FATALITY W.Va. construction worker killed in US 50 work zone accident (construction worker, 48, was fatally struck by a road milling machine in a work zone – he was pronounced dead at the scene following the accident)

Police: Worker killed in West Beaver Street industrial accident (victim was working with a backhoe or end loader when the equipment fell onto him about 7 p.m.)

SAW FATALITY Construction accident kills worker (worker, 39, was fatally injured while operating a saw – worker was fatally injured when a shard from a pipe struck him in the neck)

AERIAL LIFT and ELECTRICAL Police: Cherry picker worker possibly electrocuted (worker in a cherry picker was possibly electrocuted when his cherry picker came into contact with live wires)

FALL Construction worker injured after 9-foot fall onto concrete in West Point (construction worker was home recovering with a bruised hip after an accident at a work site when he fell 9 feet onto a concrete slab – no information available on what caused the worker to fall onto the concrete slab)

FALLING SHEET ROCK Construction worker injured at S.A. convention center (construction worker has been seriously injured in a downtown accident after a pile of sheet rock reportedly fell on the man – nobody else at the site was injured in the incident)

AERIAL LIFT FATALITY Worker killed after construction lift tips over in Taunton (worker fell to his death from a construction lift that tipped over – the fatal incident took place where a construction lift was operating on a slope)

POSSIBLE ELECTROCUTION Construction Worker Killed During Tesla Supercharger Installation (a contractor working on the installation of a Supercharger station was killed and police are investigating the death as a possible electrocution)

FATAL SCAFFOLDING COLLAPSE 7 injured, 1 dead after scaffolding collapses at Rio Rancho hospital (seven people were injured and one was killed in a construction accident, including three who suffered potentially-life-threatening injuries – four people are stable and were not seriously hurt – one person died from their injuries at a hospital – accident happened just before 1 p.m. when 21 feet of scaffolding buckled and collapsed – workers were working on installing windows and siding on each floor of the tower)

DEMOLITION FATALITY 1 dead, 1 critically hurt in demolition accident (worker, 54, was killed and another critically injured when a wall fell on them during a demolition project – the men were part of a crew tearing down an old Job Center – the 54-year-old man was pronounced dead at the scene and a 33-year-old man was taken to a Medical Center in critical condition – a 15-foot brick or block wall fell on the men)

TRENCH FATALITY Worker trapped in trench collapse has died (worker was trapped in a trench collapse about 1:40 p.m. – crew was working on sewer lines when the accident happened – worker was pronounced dead at the scene – no other injuries were reported)

ELECTRICAL Construction Worker Electrocuted in Farmington Accident (worker was repairing damaged to a severed water main from an earlier incident when he struck an underground electrical line – worker never lost consciousness and was able to walk to a waiting ambulance – extent of his injuries are unknown)

PIPE PLUG EXPLODES Worker injured in USCB blast released from hospital; OSHA considers investigation (construction worker was injured when a pipe exploded where a new parking lot is being built – employee was conscious before he was taken to a Medical Center for treatment just after 10:30 a.m. – the accident may have been caused by faulty equipment and said it involved an inflatable pipe plug becoming “overpressurized and exploding” – specific nature of the malfunction remains unclear)

HEAVY EQUIPMENT in WORKZONE TxDot worker injured in Victoria County (worker, 29, was caught and injured when the vehicle, which he was pushing, slipped and rolled because of slick tar from high temperature – worker was driving a roller machine that was packing asphalt – because of the slick tar in the high temperature, the machine, which was being operated in reverse, slid and rolled onto its left side, causing the worker to be injured)

FATAL FALL from SCAFFOLDING inside a CONFINED SPACE Worker falls 70 feet to death in Snyder County (worker, 32, was killed when he fell 70 feet inside a silo – no safety harness was used by the crew constructing the new silo and apparently he fell about 70 feet when a section of scaffolding broke)

SKID LOADER Construction Worker Run Over By Bobcat Loader (worker, 18, suffered injuries to his hips, back and legs at a construction site when he was run over by a Bobcat loader)

WORKZONE OTA worker injured in construction zone (worker was hit by a traffic cone on I-44 just before 10:00 a.m. – three cars were entering the construction zone that has been narrowed to a single lane – the first two drivers obeyed a flagger who was holding a “slow down” sign, but the third driver did not – this caused the second car to swerve into the cones to avoid being rear-ended and that’s when a cone went flying and hit the worker)

WORKZONE FATALITY Landscape worker killed while working off highway 174 (worker was killed along a highway when he was struck and killed by an SUV while doing landscape work in the median around 12:15 p.m. – the SUV left the road and hit the worker – worker was thrown eight metres after being hit)

ELECTRICAL and SCAFFOLDING Construction worker injured in Kitchener (worker, 31, suffered serious injuries in the accident around 3pm when he struck a live overhead wire while working with scaffolding)

FORKLIFT FATALITY Worker killed after construction lift tips over in Metchosin (worker, 42, was killed after a construction lift tipped over at a site – the machine operator was initially reported as the victim, but police later clarified that the operator suffered injuries while another worker was killed)

FATAL FALL Worker dies in workplace accident (worker, 20, died after hitting an iron pipe while working – he was installing a pipeline when he lost balance and hit the pipe – he was declared dead at a Hospital)

EXPLOSION Worker Lighting Match Sparked Gas Explosion at Bronx School (three workers were severely burned in a blast in science labs- the explosion at a high school complex was sparked by a worker lighting a match to test a gas line – the blast severely burned three workers – workers preparing science laboratories ahead of the Sept. 9 school start date were working on gas pipes feeding into Bunsen burners for several hours in an area that wasn’t vented – then a worker, apparently not realizing the room was filled with gas, put a match to the burner to test if it was working, causing the explosion)

CRANE Crane accident cuts electricity to Tesla Motors Fremont factory (crane accidentally brought down a power line running into a factory, shutting down the plant at 6:35 a.m. – accident happened outside the factory and no injuries have been reported)

MISCELLANEOUS

TREE CUTTING FATALITY Worker Killed In Tree-Cutting Accident In Sebastopol (employee, 21, of a tree-cutting business died when he was struck by a large section of a tree – owner of the company was cutting a large pine tree around 2:50 p.m. – he was trimming branches and was approximately 80 feet up the tree when he cut off a large section of the top of the tree – the deceased was on the ground below the tree when he was struck by a log eight inches in diameter)

Worker airlifted after accident at business in Madera (worker, 43, was working on a piece of equipment when her right foot got stuck – she was alert and crying before she was airlifted out, and they called Life Flight as a precaution – no word on what caused the accident, but the woman will not lose her leg)

FALL from LADDER Carnival worker injured in fall (a carnival worker suffered injuries in a fall on the job at the – worker fell from a ladder while working on the Ferris wheel on the fair’s midway – he suffered a lung injury and was flown to a Medical Center for treatment)

EXPLODING TIRE Worker hurt after tire explodes in face (worker was taken to a hospital after a tire exploded in his face around 3:15 p.m. at a tire shop – scanner reports indicated he was unconscious while other employees were performing CPR)

Montana Department of Transportation employee dies in work related accident in Missoula County (Department of Transportation employee, 45, has died in a work related accident around 4 p.m. – he and two other state employees were using a piece of equipment to mix sand and salt when it moved unexpectedly)

GUARDING/LOTO Man seriously hurt as arm stuck in pasta machine at Catering by Luigi (worker, 46, was rushed to a hospital after getting his arm caught in a pasta-making machine – paramedics and firefighters were on the scene for just over an hour-and-a-half trying to extricate the worker and treating him before he could be taken to a hospital – he was taken to hospital with serious injuries to his right arm)

ELECTROCUTION Elevator Worker Electrocuted in Downtown LA High Rise (man working on an elevator was found dead after being electrocuted at about 10:20 a.m. – he was found dead in an elevator machine room of a high-rise building)
