INDUSTRIAL FORKLIFT FATALITY Husband, father dies in work accident (worker, 51, killed in forklift accident – he worked there for nearly 8 years and was on the nightshift – he was operating a forklift, moving a steel beam that collided with a pile of beams nearby knocking the forklift back crushing his body – crews worked to remove the beam off of the forklift but it was too late – he just celebrated a birthday last week, his wife is in disbelief) EXPLOSION w/ AMPUTATION Two workers severely injured in Ajman factory explosion (two workers have sustained severe injuries following the explosion of an engine cooler at an aluminum factory – the leg of the first worker was severely injured due to the blast and it was amputated while the second worker sustained burns at various parts of his body – the explosion suddenly occurred while the labourers were working very close to the cooler inside the factory, which caused injuries) CRUSHING FATALITY Tank Crushes Worker at Kansas Worksite (worker, 22, is dead after a 6-ton tank shifted unexpectedly and fell on him while he was sandblasting it – he was under a tarp working on the tank when it fell onto him) GUARDING/LOTO FATALITY Worker fatally injured at Kansas plant (worker, 51, died from injuries sustained in an industrial accident – he was found by a co-worker immediately after the accident happened about 2:10 p.m. – one of the machines at the manufacturing plant made contact with a part of his body – he was rushed by ambulance to a local hospital where he was pronounced dead) LOTO FATALITY Worker crushed to death (worker was crushed to death when a machine he was inspecting for water leakages switched on – according to the preliminary investigations, the supervisor was called by one of the CNC [computer numerically controlled] operators to inform him about water leakages at his machine – supervisor stopped the machine and went inside the machine to check where the water was coming from – while inside it, it switched on, crushing the man) CONFINED SPACE FATALITY – FLASH FIRE caused by STATIC Flash fire at EOG facility kills one, injures three (a flash fire at a natural gas production facility killed one contract worker, 35, while injuring two employees and another contractor – the time of the accident was listed at 1:56 p.m. and was described as an explosion at a natural gas storage tank that the four workers were cleaning at the time – preliminary investigation suggests there was a flash fire, caused by static buildup – this marks the second death in 2014 at an EOG facility in Wyoming) GRAIN DUST EXPLOSION Grain bin accident injures one worker (one worker is injured after a grain bin accident around 2:45 p.m. involving a small explosion in a grain leg – one worker was taken to the hospital with what appears to be non-life threatening injuries – cause of the explosion remains unknown) ARC FLASH Worker injured in fire at Primex sheet plant (worker was burned in an electrical accident when there was an arc associated with an electrical panel he was working on at the plastic sheet extrusion facility – they were working on an electrical panel, and when they fired it up, the panel arced) WAREHOUSE FATALITY Minneapolis man killed in Supervalu warehouse accident (worker, 51, died when he was pinned by machinery at a Food Distribution Center – preliminary investigation indicated no criminal action – he worked as an order selector) FATALITY Narmco Employee Dies In Industrial Accident (worker, 55, was killed when bins fell onto a worker – no official cause of death has yet been released – another employee was taken to hospital with chest pains, but no connection has been made between the two incidents) ELECTROCUTION WORKER ELECTROCUTED (a shop worker, 38, was electrocuted whilst taking down a sign from a shop frontage – he accidentally touched an electric cable and received an electric shock) LOTO/GUARDING FATALITY Gemini plant worker’s death is company’s ‘darkest day’ (worker, 26, died from his injuries after being trapped in machinery – victim had only been with the company for eight or nine months, after a change of career – he was transferred by road in an ambulance, under police escort, to a Hospital where he later died) CRUSHING FATALITY Worker killed in Dalton (worker, 24, was killed in an industrial accident as he was helping to move “large manufacturing equipment” – the machinery fell on him – the machinery was very large and top heavy, and the weight apparently shifted while they were moving it, and it struck him) FORKLIFT OSHA investigates forklift accident at P&G distribution center (a forklift accident seriously injured a worker at a distribution center – he was flown from the scene and taken to a Medical Center for treatment of a serious leg injury – he was trapped between a forklift and a shelving unit) GUARDING Worker falls into piece of equipment at Carmen Industries (a worker fell into a piece of equipment – the man became trapped between two steel plates and has been freed – his injuries are considered non-life-threatening – cause of the accident is under investigation) ARC FLASH Official: Injured worker airlifted to hospital after explosion at Nevada plastics plant (one employee is injured after an explosion at a plastics manufacturing business – the person was burned and was being airlifted after the blast – person’s condition and gender were not known – the “flash explosion” apparently stemmed from an electrical breaker failure, but it did not cause a full-fledged fire) DUST EXPLOSION and HOTWORK Arizona Pet Food Factory Explosion burns Four Workers (four workers have suffered burns in a grain dust explosion at a pet food factory – workers were welding in a grain elevator – grain dust caught fire, which further resulted in explosion – one worker was in critical condition and was airlifted to local burn center – another worker was transported to the same facility by an ambulance – third worker had moderate burns and was transported to a nearby hospital, while the fourth worker was released from the same hospital after treatment) CHEMICAL Spirit AeroSystems worker burned in face (worker is recovering after suffering a burn on her face – she initially refused further treatment, but was transported to a local hospital as a precaution with what’s described as a minor injury) FATAL HOTWORK EXPLOSION Worker killed in explosion at S. Idaho dairy (worker, 30 , died in an explosion at a dairy – the explosion occurred at about 6 a.m. as he did welding work on a trailer – the explosion damaged the shop and other equipment – it’s unclear what caused the explosion, and authorities are continuing to investigate) FATAL EXPLOSION @ STEEL MILL Second worker dies from U.S. Steel explosion; OSHA investigating (a second employee has died from injuries in an explosion at the company’s facility – three employees were transported to a Hospital when a pipe exploded in the Q-BOP where molten steel is made – the first victim died shortly after arriving – the third victim remains in critical condition – OSHA is investigating the incident – it marks the eighth OSHA investigation of the facility since 2003) CHEMICAL 2 hospitalized after being exposed to chemical fumes at McDuffie Street business (fire fighters and Hazmat team responded to an unusual odor – the source of the odor was produce by a residual amount of ethyl acrylate left over in one of the transport tanks – two persons were taken to the hospital and six were treated and release – the fumes escaped when a valve on the tanker was opened) EXPLOSION Pipeline blast is IISCO Steel Plant injures 24 (more than 20 people have been injured, some of them grievously, when a blast occurred – the explosion occurred at a gas pipeline at the steel plant around 12.15 p.m. in one of the units of the plant – preliminary enquiry has revealed that the incident occurred during a planned shutdown taken for carrying out rectification jobs in no. 11 pre-heater of the New Coke Oven Battery) AMMONIA Ammonia Leak Forces Workers To Evacuate In Sanger (firefighters responded to an ammonia leak, forcing workers outside and about a dozen people had to be taken to the hospital for breathing problems – the leak caused no major traffic issues and crews spent the morning clearing the vapors and locating the cause – no immediate word on workers’ conditions) Broken Valve Causes Warehouse Ammonia Leak (an ammonia leak was caused by a broken valve – it was a tier two hazmat situation – when officials arrived, they found a high concentration of ammonia inside the compressor room) AMMONIA Crews Cap Ammonia Leak At Blazers Ice Centre In OKC (emergency crews stopped an ammonia leak inside an ice centr – three employees were inside the facility when they first noticed the smell in the building – one of the employees told our news crew on the scene he had trouble breathing and his eyes were burning – crews were there in the last couple days to do routine work on the cooling system, but the cause of the leak is not known – firefighters were able to shut off the valve and are working to ventilate the building) CHLORINE TANK FAILS 1,100 gallons of chlorine spills in Cocoa Beach (more than 1,000 gallons of chlorine spilled from a plastic tank at a pool service store, causing the evacuation of a nearby apartment complex – no injuries – upon arriving at work at about 7:15 a.m. employees discovered the 1,500-gallon tank behind the store had broken apart emptying 1,100 gallons of chlorine into the parking lot and a storm drain – the tank breached around 5:30 a.m. – no dead fish or other dead marine life were reported in the River – about 20 people in a nearby apartment complex were evacuated for about two hours) CONSTRUCTION FATAL WALL COLLAPSE Two workers killed as wall collapses at building site (two workers, 35 and 45, were crushed to death when part of an old wall collapsed on them as they were digging a pit for the foundation of a building being constructed – they were attempting to remove part of a wall attached to a temple when it collapsed on them) CRANE Worker injured at construction yard in North Kingstown (a construction worker was injured after a crawler crane fell on top of him – he suffered injuries to his legs – a subcontractor was repairing the crawler crane when the track apparently slipped and came down on top of him) EXCAVATOR FATALITY Man dies in industrial accident in Kihei (worker, 61, died after he was struck by an excavator at 1:31 a.m. – co-worker reported finding the victim unresponsive on the ground after it appeared he had been struck by an excavator) BULLDOZER FATALITY Equipment operator killed after bulldozer flips, pins him underneath in Tennessee (construction worker, 58, was killed when a bulldozer flipped on top of him – he was operating the bulldozer too close to a ledge when it flipped over and pinned him underneath – he was later pronounced dead at a local hospital) CRANE FATALITY Lorry driver crushed to death by crane arm (driver crushed to death by crane arm – a crane’s arm crashed into a waste collection lorry, killing the driver, 55, on the spot – he suffered serious head injuries – crane was believed to have been lifting a generator to the complex roof for a cinema extension project when the accident happened) CRUSHING FATALITY Worker Dies in ‘Hell’s Kitchen’ Construction Site (worker was crushed to death at a construction site after a concrete slab fell on him – he was removing soil from underneath a building when a 4-foot by 8-foot piece of concrete slab from an adjacent 14-story building fell on him – his co-worker escaped with minor injuries – worker was pronounced dead at the scene) CRANE Worker injured in Vancouver following construction accident (construction worker has reportedly been injured by a falling beam – workers were installing a piece of equipment, possibly a window, about 16 to 20 floors up, when it appears the beam fell – worker is in critical condition) FALL from ROOF Worker injured in fall at Legacy Farms (a construction worker was seriously injured when he fell 15 to 20 feet from staging on a house being built – a medical helicopter crew flew him to a Medical Center’s trauma unit – extent of his injuries and how he fell are still under investigation) FATAL FALL down STAIRS Construction Worker Seriously Injured in Fall (Worker dies in Roswell construction accident (a worker, 40, died in a construction accident when fell down some stairs) BACKHOE FATALITY Manchester worker dead after Eagleville school accident (a construction worker, 58, died after a backhoe flipped over on him – he died after being pinned under the backhoe around 2 p.m. at the school) WORKZONE FATALITY 3 workers hit, 1 killed in highway construction zone crash (construction crews were working on a paving project when a pickup truck drove through the construction zone and hit three workers – one worker died on the way to the hospital – the other two workers were rushed to the hospital, but their conditions were not made public – driver of the pickup truck was not injured in the accident and he has yet to be charged – police are currently investigating whether fatigue played a role in the accident) REBAR One worker remains hospitalized after construction accident in Brattleboro (a construction worker injured in an accident at a bridge project – the other three injured workers were sent home from the hospital – two of the four workers suffered serious injuries, while the other two suffered minor injuries – accident occurred due to a large amount of rebar that fell; investigators are still probing how that happened) AERIAL LIFT FATALITIES 2 workers dead, 1 injured after lift accident in Edmond (two workers, 54 and 56, have died, and another worker is injured after an industrial accident – a 21-year-old also suffered injuries during this incident – he was treated in a hospital for a broken arm – the three workers were working on the second floor of the complex when it tipped over – the three workers were all on the platform of the lift when it fell) TRENCH Construction worker hurt in trench accident in Oakville (construction worker, 20, was rushed to hospital after he was caught in a trench that partially collapsed shortly before 3 p.m. – he was in a trench when part of its wall collapsed, pinning him from the waist down – police described his injuries as being non-life threatening) FORKLIFT FATALITY Industrial fatality accident at Tyler construction site (a construction worker, 35, was exiting out a third floor window into a large wood box container sitting on top of a fork lift – the box became unstable and fell off the fork lift falling to the ground with the worker inside) HOTWORK Welding torch ignites massive Port of Los Angeles fire (fire broke out at a Port, burning part of a dock and threatening a warehouse and ship – flames broke out on the wharf about 6:40 p.m. shutting down the main channel – a welding torch started the fire that burned the chemical-coated pier, sending clouds of smoke over the sprawling complex and forcing worker evacuations and the closure of at least one school – no injuries were reported) FLASH FIRE 2 renovation workers hurt in Waco house fire, fumes apparently ignited, exploded (two renovation workers have been hurt in a fiery explosion at a vacant house as they tried to remove glue from the kitchen floor – one man remains in critical condition with burns over 90 percent of his body – the second worker has burns over 20 percent of his body and was listed in serious condition – accident happened as the men used lacquer thinner to try to remove glue from the floor – officials are trying to determine the source of the ignition) HOTWORK Blaze destroys cafeteria (fire ripped through a popular coffee shop and restaurant after a stray spark from a welder’s torch sent the roof up in flames – no-one was injured – Coffee Shop and Restaurant was completely gutted in the blaze that firemen fought for almost three hours to contain – it broke out at around noon and is thought to have been caused by a welder fixing a sign to the roof) TRENCHING and ELECTRICAL Longview man digging trench zapped by power line (worker, 51, was taken to the hospital after he struck an underground power line while digging a trench – he was working on a water main outside the apartment complex when the accident occurred shortly before 9 p.m. – a neighbor watching a security camera saw a flash of light, and went outside to find the worker lying across the line – the neighbor pulled him off the line, receiving a shock himself in the process – neighbor declined medical assistance – worker suffered serious injuries but he was conscious and breathing when an ambulance took him to a Medical Center) WORKPLACE VIOLENCE Three dead after uniformed ups employee opens fire at Alabama warehouse one day after he’s fired (the gunman, 45-year-old father, is among the dead – the suspect had been fired the day before – the two men killed by the gunman appeared to be higher-ranked employees -“ it appears that the shooter knew exactly who he wanted to target at the time” – this news followed a previous attempt by the company to terminate him within the past month – the disgruntled man appealed the decision, but lost in recent days) MISCELLANEOUS CAR LIFT Car rolls off lift, injures Seaford auto worker (worker, 48, is hospitalized in serious condition after a vehicle rolled off a lift and pinned him underneath) CARNIVAL FATALITY Worker killed while setting up for Texas Rice Festival (a woman, 30, who was part of a carnival work crew was killed while setting up for the Festival – she was struck by equipment about 4 p.m. on the job site – she was struck while removing a transportation brace as the crew was setting up the carnival) SEWER EXPLOSION Cesspool explosion at Long Island home sparks dueling lawsuits (a cesspool cleaner is suing a homeowner, who hired him to clean a clog at her home – her son who was helping the worker lift the lid to the tank is suing the cleaner – both men suffered severe burns to their faces – the sewer guy claims the homeowner poured a chemical in the cesspool of her home that triggered a methane explosion when he pried off the lid with the help of her son – the son has filed a $3 million lawsuit against the worker and his mother – he wasn’t keen on suing his mother, but he was required to do so because she is the owner of the property. Evans suffered permanent damage to his eyes) WOOD CHIPPER FATALITY Worker falls into wood chipper in front of his sons (man was working with an industrial-sized machine near a car dealership when the accident happened – it’s the type of wood chipper where the chute end actually will push the debris into the back of the truck so it can handle large branches – the worker simply tripped and fell into the machine – he was decapitated – the man’s two sons were also on the crew and watched as their father was killed) LOTO FATALITY in FARMING Man killed in farming accident in Genesee County (a farm worker, 23, was killed after getting caught in a piece of machinery at 6:49 p.m. – he became trapped in a round hay baler as he and the owner of the farm were trying to repair a mechanical problem – they had the back of the baler jacked up with a tractor when the accident happened – victim had to be extricated from the baler by Fire Department and was pronounced dead at the scene by the County coroner) MOWING Injured worker remains in serious condition (Public Works employee remains in serious condition three days after an accident – driver possibly looked down for a second and left the roadway and headed to the right crashing into a tractor – worker was mowing grass when the car going about 55mph slammed into his tractor, and sent him flying into the car’s windshield) FLASH FIRE Worker burnt in paint blast (a man is being treated for burns after an explosive fire – a drum of paint thinner exploded – the resulting fire had burned itself out before firefighters arrived – paramedics treated the man at the scene before transferring him to a Hospital – he is in a stable condition) PROPANE FLASH FIRE during TRANSFER Man Burned in Boyle Heights Explosion (worker, 40, delivering propane to an industrial customer was critically burned in a “flashover” explosion at about 11:50 a.m. – the man had burns over most of his body – he was driving a “large single tank propane service truck” and in the process of filling a large tank when the accident occurred – he suffered second- and third-degree burns) CRANE FATALITY Worker electrocuted after crane hits overhead power line on farm near Coleville (worker, 23, is dead after he was electrocuted while working on a farm – the victim was part of a five-man work crew that was assembling grain bins on a farm – the metal bins were being moved by a crane when the machine hit an overhead power line, electrocuting one of the workers below – emergency crews responded but were unable to revive the man – none of the other workers were hurt in the incident) HOTWORK Welding sparks blamed for morning fire (lingering sparks from a welding job were blamed for a small fire – it appeared that a smoldering fire started on large timber logs that had being burning overnight after some welding – “It’s not abnormal to see a fire start after some time has passed because of sparks that flew in the right place and being helped by the wind,” – he referenced a structure fire that happened earlier in the week at a Storage business after sparks burned through upholstery a couple of hours after the owner was using a grinding wheel) WELDING GASES EXPLODE inside truck Truck explodes in Northglenn (a truck exploded while its 21-year-old owner was only three feet away – the explosion blew the windows out of nearby homes and cars, and enveloped the truck in flames – the owner uses the truck for his plumbing business – he was taken to the hospital with minor injuries, and expected to be ok. The fire from the truck was immediately extinguished – the explosion came from two tanks used to fuel a torch for welding that were stored inside the cab of the Ford F350 – what caused the tank to explode is under investigation – debris from the explosion was scattered throughout neighbors’ yards, causing superficial damage to six homes but no structural issues) |
