INDUSTRIAL CONFINED SPACE FATALITIES – SIX WOULD BE RESCUERS Workers overcome by gases at paper mill, 7 die (workers were overcome with toxic gas from a paper mill’s waste pool and seven of them died – a worker who was cleaning the pool filled with pulp paper waste fell in and his co-workers rushed to help but were overcome with the noxious gas themselves – seven workers died and two were injured) BULLDOZER FATALITY @ WASTE WATER PIT Texas refinery worker killed after falling into 200-degree wastewater pit (a contractor died at a refinery after the bulldozer he was operating fell into a pit filled with 200-degree byproduct wastewater – a co-worker of the victim used his machine to pull out the bulldozer, but the man inside was already dead – victim likely was pushing petroleum coke, a byproduct of crude refining which is like fine graphite, into the pit – bulldozer somehow overtopped the levee) BUCKET TRUCK FATALITY UPDATE: State Police ID Worker Killed in Tipped Truck Accident (state DOT consultant , 43, died when his bucket truck tipped over and crushed him while he was performing a routine bridge inspection on Interstate 84) Goodyear worker dies in industrial accident (worker, 56, was killed in an industrial accident at approximately 9:30 p.m. – she was a 15-year associate of the company, worked as a windup operator and roll changer – no further details) FORKLIFT FATALITY Worker dies in Harrisburg industrial accident (worker, 40, was killed in an industrial accident while he was operating a forklift in the warehouse – he apparently collided with a stack of steel beams while turning the machinery just after 2 a.m. – he apparently was unaware that a bundle of steel had shifted and the 20-foot beams were protruding from the stack – the beams impaled him in the chest) FATAL FALL Contract worker dies in Natchitoches Parish industrial accident (worker, 50, was injured in a fall at a plant about 7:45 – he was pronounced dead at a Regional Medical Center) FALL into VAULT Worker suffers rib injuries at Travis construction site accident (construction worker was rescued after falling in a seven-foot hole at a substation suffered rib injuries – the contractor was securing infrastructure at the site to make it more resilient against storms) FORKLIFT Man Injured In Cookie Factory Forklift Accident (worker, 35, is recovering after he was injured in a forklift accident at a cookie factory – he became stuck between a forklift and a wall – co-worker found him trapped just a little after 6:00 a.m. – co-worker said that the forklift was in gear and the emergency break was not set – worker was operating the forklift when he stepped off it to shrink wrap a pallet – while his back was to the machine, it rolled toward him and trapped him against the wall) FATAL FALL into BARGE UPDATE: Police identify Winona barge worker killed in accident (worker, 24, was killed in a barge accident when he fell after a barge hatch shifted unexpectedly while being closed – initial investigation shows that the worker was on top of the barge and fell when one of the hatches shifted unexpectedly while they were being closed) FORKLIFT FATALITY Flint factory accident resulted in worker’s death, state says (worker, 56, was operating a powered industrial truck to flip over a fabricated metal automotive rack – while the rack was raised, the man backed up, and the top tray flipped open, hitting him in the back – he didn’t seek medical attention until two days after the incident, when he was admitted to a hospital – over the course of the following two weeks, his condition deteriorated) CONFINED SPACE ENGULFMENT Worker trapped in rail car filled with sand rescued (rescue crews successfully freed a worker who was buried to his chest in sand inside a railroad car where gravel and sand are loaded onto rail cars – worker got stuck at the bottom of the railroad car, which contained about 80,000 tons of sand – he was conscious and breathing, but was having difficulty due to the weight of the sand – Fire and Rescue used suction trucks to remove about 15,000 tons of sand from the rail car – some two hours later, rescue crews were able to hoist the worker out of the rail car – worker was taken to a local hospital in stable condition) EXPLOSION Medical conditions upgraded for burn victims in Renewable Energy Group plant explosion (two workers were injured in the fire – investigators attributed the start of the fire to a leaking pump that had undergone maintenance and was being brought back online at the time of the fire – two victims are now in good conditions, and one is in serious condition – the explosion was sparked from a hydrogen line undergoing maintenance – fire took five hours to extinguish completely and included among the injured) EXPLOSION Workers killed, burned in Basra power plant explosion (one worker was killed and four others suffered life-threatening burns when a pipeline exploded at a power plant – gas pipeline exploded during testing and maintenance operations on the gas system – two of the victims are contractors which has been rehabilitating and expanding the power plant) LOADING DOCK Police: Worker crushed between semis (worker crushed between two semis has a chance of surviving because of the actions of a deputy and first responders – worker had been crushed between two semis when one of the trucks backed up) EXPLOSION Explosion at Multiexport plant leaves 3 injured (three people have been injured after an explosion at a plant – the causes of the accident are being investigated – the three injured were sent to the workers’ safety institute, one of them with acoustic trauma and the other two with different injuries) LINEMAN ELECTROCUTED Electrical worker killed while working on telephone pole in Arab (an electrical worker, 36, was killed doing electrical work on a electrical pole – he was a contracted electrician and was helping to tie new lines on the electrical poles when he was electrocuted) FUMES Fumes overcome Amtrak workers in Mansfield electrical shed (three workers were rushed to hospitals after they inhaled dangerous fumes from a leaking battery in a small electrical building – worker entered the electrical building and there was a gas battery that had released prior to his entering – when he entered he became kind of incapacitated and dropped to the ground at the doorway – two other workers “went to pull him out of there and got a few whiffs” – the first worker who had entered was in the most serious condition) CONFINED SPACE FATALITIES Second fisherman dies after inhaling toxic fumes (a second fisherman has died after he and his co-worker were overcome by fumes in a trawler – he was pronounced dead at a Hospital – the 46-year-old went to rescue his colleague, 27, after he got into trouble on the fishing trawler – emergency services were called around 1.30pm – they found the two men unconscious in the hold of the trawler – one died in hospital several hours after being pulled from the boat, while the other died on Friday morning – not yet clear what caused the toxic fumes to be released but investigators believe that the two men inhaled ammonia and hydrogen sulphide gas from organic matter that had been rotting) FATAL CONFINED SPACE EXPLOSION w/ HOTWORK DSME fire kills two, injures seven (two people have died after a vessel under construction at a Shipbuilding & Marine Engineering Co. caught on fire – worker, 39, was found dead at around 5 p.m. in a fuel tank of the 84,000-ton liquefied petroleum gas (LPG) carrier – second victim, 30, who had been missing at first, was found about an hour later – the fire occurred on a vessel that is under construction also left an additional seven injured – the fire appeared to have been caused by a welding spark inside the vessel that was being constructed at one of the company’s building docks – seven have been hospitalized for treatment after they inhaled fumes from the fire, while 38 others are confirmed to be unharmed) EXPLOSION Propylene blast shuts down Czech plant (plant personnel were reported to have escaped without serious injury from the Aug. 13 event – up to 1,000 people were reported to be evacuated from the plant and surrounding area following the incident – five people, including four fire fighters, were treated for minor injuries at the site – the the gasoline production plant – a 52-year-old man was treated for headache, dizziness and weakness and a 36-year-old woman was treated for weakness and lethargy – a 40-year-old woman was taken as a precaution – workers in the area were immediately evacuated after the refinery had a brief chemical release – officials have not said what the chemical was) CHEMICAL – WRONG TANK Chemical accident at Ohio water treatment plant injures 4 (at a water treatment plant, chlorine was inadvertently mixed with fluoride in a container making it dangerous to breathe the air and sending four people to hospitals – the mixture was limited to the container, but caused the chemical reaction at the Water Treatment Plant – the threat was contained to one room at the plant and posed no danger to the city’s water supply – a truck delivered the wrong liquid chemical to the plant container – the plant was evacuated and the truck driver and three plant employees were taken to hospitals – emergency responders urged people in the area surrounding the plant to remain inside and warned drivers to avoid nearby roads as firefighters investigated the scene) LOTO/GUARDING FATALITY Industrial accident in Guelph leaves Kitchener man dead (worker, 26, was killed when he was pinned by a machine at a business – the electrician was working at the plant as a contractor – he was pinned by a machine inside the auto parts manufacturing facility) UPDATE on SCALPING Woman Gets Scalp Reattached After Horrific Accident (worker, 34, survived a horrific accident when her ponytail got caught in a conveyor belt – she was working at a recycling plant when her ponytail got caught in the conveyer belt, ripping off 70 percent of her scalp) AMMONIA Workers evacuated after ammonia leak at Mrs Mac’s Morley warehouse (workers at a warehouse were evacuated after an ammonia leak – it was believed a cooling tower valve was ruptured about 10am while workmen were doing refrigeration maintenance, causing ammonia to leak out – no one was harmed and the leak was contained by 10.52am) CONSTRUCTION FATAL FALL 1 construction worker dead, 1 injured at Vikings stadium site (one construction worker is dead and another is in serious condition following a roofing accident at a stadium site – the accident happened at about 7:45 a.m. while working on the north side of the building – the worker who died fell from the edge of the roof into what he described as a snow gutter below the main roof on the north side of the building – the man fell about 50 feet but said that distance still needed to be verified – the man was pronounced dead on arrival at the hospital – the other worker was injured while he was on the roof but he did not fall from the roof but “we don’t know exactly what caused his injuries”) FATAL FALL Worker dies after falling 40 feet to the ground (worker, 24, died after an industrial accident around 8:42 a.m – he was working about 40 feet off the ground on part of the warehouse’s framing when he fell and suffered traumatic injuries and died shortly after the fall) ELECTROCUTION Electrician killed in workplace accident in Connecticut (an electrician, in his 40s, has died after falling on live electrical wires while he worked at an indoor arcade – worker was removing light fixtures when he fell from a platform and grabbed the wires – workers had thought the power was off in the building, and had thrown the circuit breakers as an extra precaution) MOBILE EQUIPMENT FATALITY Construction worker killed at work site in Sayreville (construction worker, 67, died after he was struck by a piece of equipment shortly before 9 a.m.) WORKZONE Road worker hurt in South Bend school bus crash (road worker, 52, was putting up an electric directional sign when he was injured when a school bus struck the sign which then hit the worker – it all happened shortly after 6 a.m. – school bus was just starting its route with no children on board when it clipped the sign – the sign fell and struck the worker – the sign was not on but the worker was wearing a reflective vest) ELECTRICAL Contract worker shocked in NLR accident (a contract worker was shocked in an accident that also caused a power failure shortly before 11:15 a.m. – he was working on a 12,000-volt line when part of it fell on him – he was airlifted to a Medical Center estimated were burns to 20 percent of his upper body) FATAL FALL Man dies after falling into water at Bellevue construction site (construction worker died after he was found face-down in water at a building site at about 10:30 a.m. – co-workers performed CPR on the man – co-workers said they had found him face-down in a 10-foot hole that had two feet of water in the bottom – unclear what led to the accident) CRANE FATALITY Metro Worker Dead in Crane Accident (worker, 21, was crushed to death while trying to steady a steel load when he lost balance and fell – he was working near a crane whose boom was lifting a steel reinforcement load – the load was swaying wildly and he tried to adjust it a little – he suddenly lost balance and fell to the ground – the crane simultaneously tilted and fell to its right – the load fell to the ground and worker fell on it – the boom pierced his back, killing him on the spot) WORKZONE DOT Worker Hit in Norwalk I-95 Crash (two vehicles collided and one went on to hit a worker who was standing near a truck – a DOT worker sustained minor injuries after a two-vehicle accident) CRANE Worker injured in crane accident in Miramar, police say (worker was critically injured in a construction accident involving a crane – he was airlifted to an area hospital about 2 p.m. – it was not immediately known how the worker was hurt) Coroner identifies worker who died after accident at Beech Grove Amtrak facility (worker, 27, died after an accident at the maintenance facility – Fire Department responded to a call of a burned person at the facility and arrived and found a man suffering from burns – victim was transported to a Burn Center where he later succumbed to his injuries – no details) FATAL FALL Construction Worker Dies After Falling Down Elevator Shaft in Manhattan (construction worker, 30, fell four stories to his death after walking on wooden beams over a elevator shaft without a harness – he was found unconscious on his back in five feet of water at the bottom of the shaft, and was taken to a Hospital where he later died) FATAL FALL from SCAFFOLDING Worker dies after fall at UES co-op building (worker, 30, died after falling 13 floors from scaffolding – he fell from a scaffold on the 14th floor of the building around 4 p.m. and was taken to a Hospital shortly after 4:30 p.m., where he was pronounced dead) FATAL AERIAL LIFT Worker falls to his death at Jamul casino site (construction worker, 56, fell to his death from a platform at the top of a 38 ft man lift – he appears to have died instantly about 8:20 a.m.) FALL from SCAFFOLDING Worker injured after falling off scaffolding in Carteret, report says (worker was hurt after falling about 15 feet from scaffolding onto the roof of a building about 2:35 p.m.) FALL from BRIDGE Worker injured in fall off Iowa railroad bridge (a man working on a railroad bridge has been injured in a fall around 2 p.m. – he was wearing a safety harness when he fell 7 to 8 feet onto his back in a rocky creek bed – he was flown to a hospital for treatment) FALL into MANHOLE Construction Worker in Southwest Hospitalized After Work Accident (a construction worker was hospitalized after reportedly falling into a hole – worker was lifted out of the below-ground site on a stretcher and hospitalized with serious but non-life-threatening injuries) ELECTROCUTION w/ LADDER One man electrocuted, 2 others injured in Draper workplace accident (worker, 39, was killed and two others critically injured when they were shocked while working on the roof of a gymnastics center – employees of a roofing company were on the roof about 10:45 a.m. when an aluminum ladder fell onto a power line – one man was already on the roof, and investigators believe others may have been attempting to get equipment to him when the ladder hit the power line – at least three workers were in contact with the ladder when it hit the 7,200-volt power line – another worker went into cardiac arrest – crews responding to the scene were able to get a pulse back on that man after administering first aid – a third man was taken to a local hospital by ambulance in critical condition – both injured men remained in critical condition) WORKZONE Construction Worker Hit by Drunk Driver Early Friday Morning (worker, 25, is hospitalized and in critical condition – driver of the car involved has been arrested – the vehicle struck an employee at approximately 2:30 a.m., while he was working on an overnight road construction project) FALL Worker injured in fall from beam (worker, 52, was seriously injured in a work-related fall – Responding Life Support Services personnel found him conscious but suffering from serious injuries – he was transported to a Regional Medical Center where at last report he was in the Intensive Care Unit suffering from serious injuries to his spine) SCAFFOLDING COLLAPSE 3 construction workers injured in Venice scaffolding collapse (three workers were rushed to local hospitals when a scaffolding collapsed at a construction site – workers fell 15 feet from the scaffolding – no other details have been released) FATAL FALL from WATER TOWER Police identify worker killed after falling from Brownsville water tower (worker, 21, died after falling from a water tower – he climbed the water tower to perform routine maintenance on a satellite antenna – an ambulance rushed him from the water tower to a local hospital where he died) TRACKHOE FATALITY Worker found crushed to death between digger arm and cab in bizarre accident (worker, 46, died in an unexplained accident – he was found at around 11am with his upper body protruding out of the digger cab’s window, crushed between the cab and the vehicle’s arm – he was rushed to hospital, where he was declared dead – he did not have a license to operate such a construction vehicle, and they do not understand how the accident could have happened – he had been working on this particular construction site for the past year) CRANE Report: Crane error may have caused Bryant collapse (worker at the scene of a structure collapse told police crane operator error may be to blame for the accident – according to the worker, a crane operator pulled a steel beam too far, causing the structure to fall – officers at the scene were unable to immediately locate the operator of the crane – although the one worker pointed to crane error, the cause of the collapse has not yet been determined and an investigation is ongoing) FORKLIFT 3 Venice construction workers injured in fall (construction workers were hospitalized in a forklift accident – workers were moving an object with the forklift when the object fell, causing them to fall as well – no immediate word on how far they fell or the extent of their injuries) baggage handling shift when his cart crashed into a refueling truck — throwing him from the vehicle and trapping him under its wheels – he was transported to hospital in critical but stable condition with head and arm injuries) LAWNMOWER AMPUTATIONS Worker loses four fingers in lawnmower accident (worker, 23, lost four fingers on his right hand in an accident involving the use of a lawnmower in the local community – the temporary worker had been assigned to the Grounds Department – an effort was reportedly made, with limited success, to reattach the victim’s four fingers) ELECTRICAL Pest control worker injured from electric shock in West Dundee (a pest control worker, 21, was injured by an electric shock while he was using a 29-foot aluminum pole for routine outdoor maintenance at an office building when he was shocked – a Police Sgt. and a company supervisor were administering CPR to the unidentified man, who did not have a pulse, when the Fire Department arrived on the scene shortly after 2 p.m. – used an automated external defibrillator on the man, who started to show signs of improvement after the first shock – though the cause of the electric shock is not verified, investigators believe the aluminum pole may have touched a 7,200-volt overhead transmission line) WALL COLLAPSE Farm worker loses leg in accident (farm worker, 23, was seriously injured Aug. 22 when a wall fell on him while working near a pond at the farm – he was transported at 3:10 p.m. with a crushed pelvis after an eight-foot concrete wall toppled on him – managers at the farm told deputies they had no idea what caused the wall to fall over) GAS EXPLOSION @ MOTEL Gas worker injured in Bremerton motel explosion still recovering at Harborview (a motel exploded after a gas leak injuring a Gas worker, 59, who remains in the ICU – he was trying to fix a gas line at the Motel when it exploded – they had the firefighters surrounding him, pointing the hoses at him ready for worst case
