INDUSTRIAL FORKLIFT FATALITY Worker dies in NC recycling plant accident (worker at a recycling plant has died after being struck by a forklift – the employee was pronounced dead at the scene) LOADING DOCK FATALITY 33-year-old worker dies after being struck by box truck (a box truck has struck and killed a trucking company employee, 33 – he had parked his vehicle on the rear parking lot and walked behind a box truck that was backing into a loading spot at the dock and was struck – he was pronounced dead at the scene) FORKLIFT FATALITY INDUSTRIAL ACCIDENT CLAIMS LIFE (an industrial accident has claimed the life of a worker, 61 – he died after a piece of heavy equipment fell and crushed him – he and his brother were working on a fork lift when the accident occurred – his brother was caught for a short time by forklift but managed to free himself) CAUSTIC FATALITY Natrium Plant Worker In Fatal Chemical Accident Is Named (one of four employees hurt in a September chemical accident has died – worker, 50, died from injuries he suffered during a Sept. 19 accident – four workers were performing a maintenance operation when they were exposed to a caustic soda solution – at the time, two of the victims were flown to a Burn Center via medical helicopter – a third patient was taken to a Hospital by ambulance, while a fourth employee was released following on-site medical treatment – the deceased was one of the two workers sent to the Burn Center after suffering chemical burns on his neck, arms, legs and torso – he had been hospitalized since the incident – company expects the other three employees to make full recoveries) WELL SITE FATALITY Worker dies following accident at Manitoba oil well site (worker, 20, died after being struck by weight arm of pump jack – a crew was working at the site when a worker was struck by the weight arm of a pump jack – he was taken to hospital, where he was pronounced dead) LOTO ELECTRICAL FATALITY Worker electrocuted at Guntown company (a maintenance worker, 46, at a frame shop died shortly before 11:30 am – he was disconnecting wires when the accident happened – he thought he had disconnected the power before beginning work on the equipment – he was taken to a Medical Center where he was pronounced dead) FORKLIFT FATALITY Worker Killed in Valley Forklift Accident (a worker at a big box distribution plant – he was using a standing forklift – at some point, the machine backed into a shelving unit and pinned him – co-workers freed him and began performing CPR on him – he was sent by helicopter to a hospital and pronounced dead) CRUSHING FATALITY Crushed metal worker loses battle for life (accident claimed the life of a worker, 76, when a large piece of heavy metal fell on top of him – while working to dismantle the U.S.S. Forrestal at a recycling yard a heavy piece of metal fell on top him – trapped underneath, rescue crews had a hard time freeing him) Worker injured in accident at Solomon Corp. (worker suffered a broken arm after an industrial accident just after 9 a.m. – the employee was cutting a transformer pad when the side blew out – he was transported to a Regional Medical Center) FATALITY BHP worker killed in Australian refinery accident (worker was killed at an alumina refinery powerhouse – the powerhouse at alumina refinery provides steam, power and air to the refinery, and includes a coal fired boiler/ turbine co-generation plant and gas turbine/waste heat boiler co- generation plant – no other details) LOTO/GUARDING on CONVEYOR Worker injured at Bear Truss in St. Louis (worker was cleaning something on the conveyor belt, got caught or pulled into it – he was alert and conscious when the ambulance arrived) Small Fire In Rockledge Injures Industrial Worker (a man was injured in a fire that started from an equipment malfunction at 9:05 a.m. – one of the workers was burned – he is in stable condition) MOBILE EQUIPMENT FATALITY Fatal Accident at Boise Cascade Plywood Plant (worker, 29, died in an industrial accident at a plywood plant – worker was killed when a piece of log yard equipment struck him – the equipment had a C-clamp, and somehow or another, the C-clamp was up and came down on him) MOBILE EQUIPMENT Worker injured in Fabius industrial incident (a helicopter was called to the scene of an industrial accident – person suffered a serious leg injury during the accident, which involved a tractor) LOTO/GUARDING on PRESS Worker burned when trapped in hydraulic press (worker sustained third-degree burns after he was trapped in a hydraulic press – he was taken by ambulance to a Hospital after the accident about 11:45 p.m. – he was trapped in the machine for about 10 minutes – he was able to use a radio to call for help to another worker) 2 burnt as spinning mill catches fire (two workers, 20 and 25, were burnt and another critically injured as fire broke out at a spinning mill – fire fighters brought the fire under control after an hour – another worker, 24, feeder man of the factory’s utility unit, also critically injured as he jumped from third floor of the factory building – the fire originated in generator room due to gas cylinder leakage at about 6:00am – the fire broke out because of the gas cylinder overflow) ARC FLASH Santee Cooper plant worker transported to burn center after electrical box explodes (worker at a plant has been flown to a burn center after an electrical box exploded and burned his face and right hand around 1:25 p.m. – he suffered second and third degree burns) STEAM BURNS FATALITY JBS Canada employee dies after receiving burns at meat-packing plant in Brooks (worker, 43, was using steam and water to remove rendering materials at a meat-packing plant – he was badly burned on the job in August at the facility and has died – he was using steam and water to remove rendering materials that were blocking a pipe at the meat-packing plant when it happened) EXPLOSION Explosion at Rockledge plant burns worker (a machine exploded around 9 a.m. – a worker suffered burns and was flown to a Regional Medical Center – he was able to walk outside to a waiting ambulance, which took him to a helicopter) CRUSHED Pro Saw worker crushed under stack of metal (an employee of a manufacturing facility has died after being crushed under a stack of metal – just after 10 a.m., the man was trapped beneath a large stack of heavy metal – emergency crews found the man underneath but not completely pinned) WAREHOUSE Fallen machinery breaks legs of Saddle Brook warehouse worker (worker, 19, at a beer, wine and spirits warehouse had both legs broken when a several-hundred-pound piece of machinery came loose and fell off a truck onto him) CRUSHED FATALITY Worker crushed and killed by air unit in Fort McMurray (worker, in his 50s, was killed after he was crushed by an air conditioning unit – he was on the roof installing an HVAC system at around 4:30 p.m. when the incident occurred – a colleague, who was working on the third floor, went to check on the man after he stopped communicating – he found his co-worker under the unit – he was transported to a Hospital where he later died) ARC FLASH Lisbon explosion leaves two seriously injured (an explosion left two people seriously injured – one of those involved in the explosion has been left with 50% – 60% burns to his body as a result of the fireless explosion which involved an electrical transformer station – the blast took place at 8.27am and left two men, one in their 30s and the second in his 40s, seriously injured – the most seriously injured man received burns to between 50 and 50 percent of his body while the second man received burns to his airways) FUMES lead to FATAL FALL Woman Is Overcome By Fumes In Wine Cellar, Drowns In Giant Vat Of Wine (a wine specialist, 25, who was overcome by fumes at an industrial cellar died after losing her balance and falling into a vat of wine – she was watching the fermenting process at a cellar when she was overcome by the fumes – she lost her balance and then fell into the giant vat – responders were unable to save her) SCAFFOLDING COLLAPSED @ SAFETY FAIR Four injured when scaffolding collapsed at safety fair (four people were injured when a gust of wind knocked over scaffolding at a safety fair shortly before 1:00 p.m. – one minor and one adult were transported for injuries – National Weather Service said the winds gusted up to 35 miles per hour – the fair was closed early because of the accident) EXPLOSION @ Start-Up Explosion hits PKN Orlen’s olefins plant in Poland (a small explosion occurred at a 16.3 million-tonne/year refinery and petrochemical complex – incident occurred at an unidentified remote installation of the olefins plant, injured six workers engaged in external renovation work at the site – the precise cause of the incident remains under investigation, the company said the explosion was related to a small ignition of volatile hydrocarbons during work related to the launch of start-up activities at one of the olefins plant’s two production units) EXPLOSION Smoldering Fire Reported At Valero Ethanol Plant Near Washington Court House (an explosion and subsequent fire broke out in a silo – the blast caused the roof to lift six inches off the silo – no injuries have been reported – Fire Department has evacuated a 1,500-foot area around the plant as a precaution) EXPLOSION Explosion at Stillwater energy plant injures four (four people were injured in an explosion at an energy company – explosion in a shop building was reported about 9:30 a.m. – one of the injured was flown to a local hospital and another was flown to a regional hospital, while the other two were taken to a hospital – the four are all employees of the company – conditions were not immediately available – the cause of the blast was not yet known, but that two of the injured employees were working on a tanker truck at the time of the explosion and that there was significant damage to the building) AMMONIA Ammonia leak prompts evacuations on NE Side (a small ammonia leak prompted an evacuation of workers from a building – workers were working on a valve when they discovered the leak outside a building – traffic was blocked briefly) AMMONIA Gas leak brings chaos to Norwegian town (an ammonia leak at a dairy led to nine people hospitalized and the town centre evacuated – the leakage was discovered at 8am by dairy staff and emergency services quickly cleared several streets – several people were exposed to the gas leak that came from a compressor room after maintenance work. – two people were directly exposed to the gas in its liquid form, while seven others were exposed to the gas from the leakage) AMMONIA Firefighters respond to ammonia leak at EFCO (employees smelled an ammonia leak and called for help shortly before 8 p.m. – nitrider equipment used to harden dyes on the extrusion line developed an anhydrous ammonia leak – firefighters in full hazmat gear, went in and turned off both the nitrider and the anhydrous ammonia source – 13 pounds of anhydrous ammonia was expelled in the leak – there were no injuries to treat) CHLORINE 30 people hospitalised after chlorine gas leak at HPC mill (at least 30 people, including women and children, were hospitalised after chlorine gas leaked from a paper mill – one of the chlorine pipes of the paper mill failed polluting the air and water causing 20 women and 10 children to be admitted to the hospital) CHLORINE Chlorine leak at Manitou Springs water plant sends one to hospital (a chlorine gas leak at a water treatment plant sent a worker to the hospital at 9 a.m.) FORKLIFT FATALITY Man, 19, dies in forklift accident (worker, 18, died after a forklift rolled on top of him just before 2pm – Paramedics were unable to revive the man – he was believed to have been operating the forklift) CONSTRUCTION CRUSHING FATALITY Concrete Slab Crushes Construction Worker in Midtown Manhattan (a concrete slab weighing thousands of pounds crashed to the ground trapping and killing a worker, 27, at a construction site – he was pronounced dead at the scene – cause of the accident was not immediately clear, but the man was working to secure the foundation of a building next to the site when a piece of concrete from that building came loose around 1:30 p.m. – another construction worker leapt to safety, but the victim was pinned beneath the slab, with only his arm visible as emergency workers rushed to the scene) ELECTROCUTION w/ LADDER Two die in Newfoundland industrial accident (two men are dead and another person is injured after they somehow came into contact with electricity around 3 p.m. – three workers somehow came into contact with electricity, leaving two of them dead and injuring another – no one knows how the workers came into contact with the electricity – the RCMP issued a statement saying the workers were believed to have been moving an aluminum ladder and there was mist and rain at the time of the accident) CRUSH FATALITY by STEAM ROLLER Construction worker killed by steamroller (construction worker, 64, was been killed in an accident that caused a mini steamroller to roll over and crush the victim – he was using the steamroller to flatten a dirt pile when part of the dirt pile collapsed – the collapse caused the steamroller to roll over and trap the worker) WORKZONE MoDOT worker struck on I-49 (Department of Transportation employee sustained non-life threatening injuries when his truck was struck by another vehicle around 10:45 a.m. on NB I-49 – the other driver sustained injuries to her knee and was taken to the hospital – she was not wearing a seat belt – prior to the accident the DOT worker was repairing a concrete barrier in the far left lane that was hit over the weekend – DOT had a truck in the first lane attempting to direct traffic to the center and right lanes when the car struck the rear of the truck) FATALITY Man dies in Heathrow accident (worker, in his 30s, died following an accident at an Airport – the construction worker was fatally injured when he was struck by a truck – incident happened in an area of the terminal where construction activity was still taking place) TRENCH FATALITIES 2 killed in New Jersey ditch collapse (two landscapers died after they were trapped in a large, excavated hole outside a home – accident occurred around 3:45 p.m. – a trench gave way while the workers were excavating a drainage pipe – one worker became trapped and another man jumped in to try and save him – both men were soon under about 10 feet of dirt – two bodies were recovered after a massive search and rescue effort) FALL Construction Worker Injured In Fall At Danbury Work Site (construction worker, in his mid-20s, fell 20 to 25 feet onto concrete at 8 a.m. – he was stabilized and transported to a Hospital) WORKZONE FATALITY Worker killed by dump truck on Georgia interstate (construction worker, 51, was killed after being hit by a dump truck – he was killed inside of a road work zone on Interstate 20 after being backed over by the truck – the truck was backing up alongside other equipment when he walked behind the truck – driver of the truck was apparently unaware of hitting him as after being hit, his body became lodged beneath the truck’s undercarriage – he was pronounced dead at the scene) Construction worker hurt at Steeles and Keele site (a construction worker was taken to hospital in serious but non-life-threatening condition after an industrial accident – a man in his late 20s was treated after he was crushed by falling material just before 8 a.m. – the incident happened at a subway construction site and the victim was stuck in a 30-foot hole and briefly trapped under a 6,700-pound beam – he had to be lifted out with a crane and suffered injuries to his torso) EXCAVATION FATALITY Tragic construction accident in Murcia (worker, 38, has been killed after being buried alive – an incline on the site collapsed resulting in the man becoming trapped under a mountain of earth – despite the rescue efforts of colleagues, nothing could be done to save his life – a large mound of earth on the construction site was disturbed by works being carried out, causing it to collapse, burying the man – the victim was pulled unresponsive from the ground and could not be resuscitated) WORKZONE Highway worker injured in latest 401 crash (worker done maintenance was taken to a hospital – the worker was in a service vehicle that was hit from behind) FORKLIFT FATALITIES OSHA investigates Edmond forklift accident (two workers were killed in the tip over and injured a third – workers, 54 & 56, brothers were in the platform area of a forklift, raising materials to the rooftop of one of the units when the machine toppled over – brothers were transported to a metro hospital along with a third worker, age 21, who suffered severe arm injuries – brothers died at the hospital) Tank falls on worker at New Canaan church (a 3,500-pound expansion tank fell onto a construction worker, 48, just before 1 p.m. – by the time emergency LADDER Roofer Killed By Electrocution, Co-Worker Injured (a roofer, 55, is electrocuted and another is severely injured just after 9:00 a.m. – workers from a roofing company were putting up a ladder at a corner home when it made contact with power lines – one worker died at the scene, his co-worker was taken by ambulance then airlifted to a hospital with injuries) MISCELLANEOUS GOLF CART FATALITY Philly Zoo employee killed in golf cart accident (worker, 57, was killed after becoming pinned underneath a golf cart at a Zoo around 11:10 a.m. – he was working under a 9-foot long heavy duty golf cart when it fell off the mechanical jack and onto his chest – no one was around when it happened – he lay pinned underneath the vehicle for an undetermined amount of time before he was discovered after failing to respond to his radio) FARM Farm worker breaks leg in harvester mishap (a farm worker, 68, was injured when he got tangled up in this potato harvester – he was seriously injured in the farm accident – he is believed to have broken his left leg after getting caught in a potato harvester’s deviner chain, which separates the vines and trash from the tubers) FALL @ DOCK Port of Los Angeles worker seriously injured after falling into cargo ship (worker fell 15 feet onto a break bulk ship and suffered head trauma at 12:45 a.m. while the dockworker was working on the ship – when he fell into the hold, he became wedged in the cargo – firefighters performed a rope rescue to extricate him – he was reported in serious condition) FARMING FATALITY WINERY WORKER KILLED IN VINEYARD ACCIDENT (winery worker, 54, has died after being run over by a harvesting machine – he fell beneath the wheel of a harvesting machine – he died instantly – the harvesting machine had just finished picking a row of vines and had emptied its load when the driver of the vehicle started to head back into the vineyard – it is believed the man, busy near the end of the row, slipped and passed under the wheels of the harvester) FATAL FALL @ FARM Fruit Farm Worker Killed (worker, 23, had spent the summer working at the fruit farm and had completed his work when the accident occurred – he was still living in a tent on the property and had planned to leave the same day he was killed – a full-time worker on the farm found him deceased at the base of a six-meter high pile of stacked fruit crates – preliminary investigation suggests he was climbing the stack when the top crate came loose, causing him to fall to the parking lot below) COMBINE FATALITY Upstate man dies while working on farm equipment (worker, 44, has died in accident that occurred while he was working on equipment at a farm – he was doing maintenance work on a corn harvester attachment for a combine at the farm when he became pinned underneath the equipment while working alone – he was later discovered by a co-worker – he was pronounced dead at the scene) TRUCK UNLOADING FATALITY Texas carnival worker killed by equipment (a carnival worker, 30, has died after a piece of equipment hit her while she set up for a barbecue cook-off – she was struck while removing a transportation brace as the crew was setting up the carnival -accident was not related to carnival rides, which hadn’t been assembled – she was transported to an area hospital where she later died) Levee workers sprayed with pesticide (officials are investigating a July pesticide-spraying incident that sent 10 construction personnel working on levees to seek treatment – incident occurred on July 22, when an apprentice pilot flying a helicopter applied pesticides Govern 4E and NU-Lure to a walnut orchard – pilot made multiple passes over the levee where 12 construction personnel were working on the Levee Project) HOTWORK Blowtorch
