Pa. plant accident (OSHA and the coroner were still investigating a fatal industrial accident – the company has described the incident as an “equipment related accident” that killed one worker and hurt two others – the death is the second at the facility about 100 miles northeast of Pittsburgh in three years – the other worker died in April 2012 when he was impaled on a fork lift while stacking mill in Grayson County (a worker, 55, has died after falling into machinery at a lumber mill – investigators believe his clothing pulled him into a conveyor belt and pulley mechanism as he was trying to clear lumber that had jammed) UPDATE on MINE FATALITY Worker Killed in Southern Illinois Mine Accident (federal mine safety officials finish their investigation into a deadly accident at a southern Illinois mine last July – a machine operator, 35, was killed when he got pinned between a coal hauler and the mine wall – the report shows the machine’s operator had reduced visibility and could not see that the victim was there – there were two other accidents at the same mine in 2011, but they didn’t result in death – investigators also blame reduced visibility for those incidents) CRUSHED LEGS Accident shuts down Ohio 7 (lanes of Ohio 7 were closed because of a work-related accident at a nearby industrial plant – the accident happened around 6 p.m. – a workers’ legs were crushed in the accident – a helicopter transported the victim to a nearby hospital) CONFINED SPACE FATALITIES @ POWER PLANT Accident at Essar Power plant in Salaya kills two (two workers, 23 and 24, were killed while two others were seriously injured in an accident while they were cleaning a boiler at a thermal power plant – they were killed after a metal object fell on them while they were cleaning boiler no.1 in the 1,200 Megawatt (MW) power plant – the other two workers who were also carrying out the maintenance work were seriously injured in the accident) FATALITY Worker dies after industrial accident (a man in his 40s has died after an accident at a factory around 11.15am. – officers found a man who had died after sustaining injuries while operating machinery – no-one else was injured during the incident) AMPUTATION Man loses arm in industrial accident in Hatch Beauchamp (worker is recovering in hospital after he lost an arm in an industrial accident – no details) GUARDING/LOTO AMPUTATION w/ CONVEYOR Laveen worker’s arm amputated after getting stuck in conveyor belt (fire crews were forced to amputate a man’s arm that was caught in a conveyor belt during a workplace accident just after 9:30 a.m. at a sand, rock and gravel business – paramedics called to the scene helped stabilized the man, and extrication crews were called to help free the employee from the machinery – after an evaluation of the situation, crews determined that a field amputation would be conducted on the man’s arm – the amputation was successful, and the man was transported to a hospital in critical condition) ELECTRICAL @ POWER PLANT Power-plant worker burned, in critical condition (a subcontractor is in critical condition four days after suffering electrical burns at a power plant – circumstances surrounding the incident currently are under investigation – the unidentified contractor was working on installing an auxiliary transformer at the power plant – when rescue units responded they determined that the man suffered electrical burns while working on a transformer) BURNS from OIL Mars Chocolate worker suffers third and second degree burns from peanut oil (worker was airlifted to a Medical Center after sustaining third and second degree burns to his hand and leg at 9:51 a.m. – the employee was working with and subsequently burned by hot peanut oil) CHEMICAL BURNS MINE Mine worker suffered chemical burns (an employee at a mine sustained severe burns on his body from a mixture of hot chemical solutions, and is now recovering at a Hospital’s surgical ward – a process plant operator was fastening bolt from a leaking filtering device used in gold stripping process, when suddenly the bolt’s thread loosened and the hot solution burst out onto his whole upper body – the mixture of hot chemical solutions, which consists of caustic, cyanide and hydrochloric acid, caused severe burns and scalds to his arms, abdomen, and face and had nearly left him without his sight) CHEMICAL BURNS Mint worker suffers chemical burns to face (a worker, 22, suffered burns to his eyes, face, ears and neck after being exposed to a mix of chemicals, including silver nitrate – his burns were first-degree, but paramedics said they could continue to affect the exposed areas on his body for hours – he was in stable condition in hospital) EXPLOSION Blast at Defiance plant injures at least 1 worker (a static-electricity spark may have triggered an explosion and fire that injured at least one worker at a food-products plant – victim remained in a hospital with injuries that were not life-threatening – the 4:30 a.m. incident at the plant closed nearby Streets for about two hours – the incident remained under investigation, but officials could not rule out a static-electricity spark as the cause – the fire was kept in check by a suppression system, then extinguished by area fire crews – the explosion caused mostly structural damage, while fire damage was “minimal”) FATAL BOILER EXPLOSION Boiler blast kills worker, injures 13 (a boiler exploded at a plywood factory killing one person died and injuring 13 others – a boiler burst and spilled hot water on Bath area dock worker reportedly crushed under pallet (a dock worker, 25, at a trucking company was taken to the hospital after being crushed under a pallet – he was reported as conscious but having trouble breathing and bleeding from the nose – he was not trapped by the pallet at the time of the call about 1:15 p.m. – the worker was being taken to an area hospital by ambulance) FATALITY w/ BURNS State officials to investigate Mooresville worker’s death (IN-OSHA is investigating the death of a woman, 57, from injuries she suffered at a car parts manufacturer – she died of complication of burns she suffered while working – she was working as a spot welder) LOTO FATALITY Mill worker dies during repair work (a worker, 45, trying to repair the belt of a grinder in a pulse mill died when he accidentally got trapped in the machine – he was repairing the belt of the machine when someone switched on the button of the machine – he lost balance and got entangled into the belt of a grinding machine – he suffered serious injuries and succumbed on the spot) FATAL FALL into RCAR HOPPER Worker Dies After Fall into Rail Car at Mich. Mill (a worker, 30, who died after falling into a railroad car at a grain elevator has been identified – he was loading rail cars shortly before 11 a.m. at the mill when he lost his footing and fell into a hopper car – co-workers pulled him out and started cardiopulmonary resuscitation, but he was pronounced dead) EXPLOSION Explosion and Fire Injure Safety Kleen Worker (one worker was injured following what appeared to be an explosion and fire shortly before 10 a.m. – heavy smoke could be seen rising into the air and police quickly shut down Pittsburgh Avenue from 14th Street through 21st – the company recycles used oil and also works with cleaning solvents) FATAL EXPLOSION Worker Killed When Tank Explodes In Johnson County (worker, 33, was killed in an explosion at a trucking company – workers were using a propane torch to thaw out a tank when the tank exploded – worker died when he was struck by a metal hatch plate that was blown off the back of the tank – no one else was injured – the tankers are designed to haul wastewater from drilling sites – authorities say it appears methane gas built up in the tank, causing the explosion) FATAL EXPLOSION Elk County Plant Explosion (one person died two other workers were hurt in an explosion – they were badly injured when an oven exploded at the plant – a man was killed in a forklift accident at the plant in 2012) HOTWORK Fire damages Ozark city well house (welder’s sparks ignited leaves, investigator says – no one injured – a welding project and a pile of leaves are to blame for a small fire at a well house behind the former community center building – a fire investigator determined that a welder working on the roof of the former Community Center building on the south side of the downtown square likely started the fire unknowingly – workers were welding on the roof just above the well house – a pile of leaves tucked against a wall on the northeast corner of the structure is where the fire began) Fire Destroys Biodiesel Refinery in Mississippi (a small biodiesel refinery that turns chicken fat into truck fuel was reduced to ashes by powerful explosions and a huge fire – fire broke out between 5:30 a.m. and 6 a.m. after a pressurized tank exploded – two workers who were at the plant when the first tank exploded escaped unharmed, and all of the plant workers have been accounted for – 25 homes and several businesses were evacuated – no injuries were reported) REFINERY FIRE Pemex workers hospitalized after refinery fire (two refinery workers were injured during a fire at the Lázaro Cárdenas refinery in the state of Veracruz – no details) FATAL CELL TOWER COLLAPSE Three Dead, Two Injured After Cell Tower Collapse (three men, 27, 28, 32, died after the collapse of a cellular phone tower – two men were killed and two men were injured as they were making structural repairs to the tower when it fell – a Volunteer Fireman was also killed when a second tower collapsed – the second tower was weakened by the collapse of the first tower – the two injured workers, and an additional injured firefighter were transported to hospitals for treatment for non-life threatening injuries – the three men approximately 70 feet above the ground, and a fourth man, about 20 feet above the ground, were on the tower – they were repairing and removing old structural supports, and before any of that could be done, the tower collapsed – all four were pinned in the wreckage for sometime) LOTO/GUARDING Firefighters: Man taken to hospital after accident at food container plant (a man was taken to the hospital after an industrial accident at a local food container plant – worker got his hand caught in a machine around 2:30 p.m. – officials have not said what may have led up to the accident) Man critically ill after meatworks accident (a worker, in his 60s, is critically ill after an accident at a meatworks plant – the man had been operating a stock loading race, a ramp to get stock off trucks, when it fell on him) EXPLOSION No injuries reported after gas explosion at ArcelorMittal plant in Steelton (a gas explosion completely destroyed a building on the grounds of a steel plant – no one was injured in the explosion and a subsequent fire, which was reported after 1 p.m. – the gas leak started in a building that is approximately 45-by-50 feet and is used partly as a washroom – no one was inside the building at the time of the explosion – the cause of the gas leak remains under investigation) PSV LIFTED Explosion rocks Tonawanda near coke plant (an explosion was caused by a safety valve activation due to a buildup of oven gas in a coke oven – there was a small fire in the facility, but it was quickly contained – there were no injuries) AMMONIA Ammonia leak closes Yuba City streets (an ammonia leak at a cold storage facility forced the shutdown of some city streets at about 6:30 p.m. – the company also had a similar leak on Friday night, to which firefighters responded – :It appears there’s some sort of failure in their piping system” – as many as nine firefighters were at the facility using water and portable fans to dissipate the odor) AMMONIA Homes evacuated after ammonia leaks at poultry plant (an ammonia leak forced the evacuation of the plant and 12 nearby homes at about 2:45 p.m. – the leak had been stopped a little more than two hours after a 911 call came from the chicken slaughtering and processing plant – no one was seriously injured – a plant employee, one of six who received medical evaluations, was taken to a Hospital for treatment of chest pain and breathing difficulty – the employee’s injury did not appear to be life threatening – plant employees trained in management of hazardous material management stopped the leak) AMMONIA No injuries in ammonia leak at Lyndon business (authorities responded to a leakage of anhydrous ammonia from a tank – no one was taken to the hospital – nine people were taken to the fire department to be checked out – no one went to the hospital) AMMONIA Ammonia leak probe under way at Kepak (plant is investigating a minor leak of ammonia – incident occurred at 10.30pm in a cold storage area of the facility – a small number of staff working in the plant at the time were evacuated while the leak was quickly rectified with no injury to staff) FORKLIFT FATALITY Man dies in forklift accident (a man died after being run over by a forklift – police believe the man’s foot became caught under the rear wheel of the forklift as he was making a left turn, causing him to fall into the path of the turning machine – several efforts to perform CPR on the man were unsuccessful, and emergency personnel pronounced the man dead on the scene at 4:54 p.m.) FORKLIFT FATALITY Forklift accident kills Jenks man (an industrial plant employee, 20, was killed in a forklift accident – he was run over by a forklift at about 3:15 a.m. – worker, 21, was driving the forklift in the business’ yard when the deceased, who was riding on the side of the forklift, fell and was hit by the vehicle’s back tire) CONSTRUCTION WORKZONE FATALITY Highway worker killed in I-695 crash in Pikesville (a highway worker, 54, was killed in an accident on the inner loop of Interstate 695 – he was completing landscaping for a State Highway Administration project when the collision occurred about 2 p.m. – he was killed when a 2001 Ford pickup truck veered off the road and onto the right shoulder – truck struck the worker and a piece of landscape equipment that he was using – Troopers are trying to determine what caused the pickup truck to leave the roadway) Worker killed at Cleveland Co. industrial site (a worker was killed when a pipe fell around 9:30 a.m. – the worker was a subcontractor working at the site – the accident occurred when the cap from a concrete casting was removed, causing a 10-inch pipe to fall and hit the worker in the head) TRENCH FRENCH VALLEY: Trench accident hurts worker (a trench accident at a construction site sent a worker to the hospital at 10:52 a.m. – the patient suffered only minor injuries and was freed from the hole at 11:37 a.m.) CRANE FATALITY One dead, one injured in accident at site of new Baylor stadium (worker, 55, died following an accident at the site of the new football stadium – another unidentified man was injured – the two men were working on a pedestrian bridge – the boom to which the two men were harnessed broke away from the crane – one of the men was able to unfasten his safety harness and was pulled to safety by emergency personnel – he was being treated for hypothermia at a local hospital – the other man was unable to detach his safety harness – divers found his body, still attached to the boom, four hours later) SCAFFOLDING COLLAPSE One dead, 10 injured in worksite accident at Sentosa (a scaffolding collapsed at a construction site leaving one worker dead, injuring 10 workers around 3.15pm – 11 workers were sent to the Hospital – one of them died in hospital, while nine others were in stable condition – another received outpatient treatment and was discharged) WORKZONE FATALITY Semi Driver Charged in I-88 Crash That Killed Toll Worker, Critically Injured Officer (driver had only gotten 3.5 hours of sleep during a 37.5 hour shift before the accident which killed a Tollway worker – driver has been charged with operating a commercial motor vehicle while fatigued or impaired – he is also charged with making a false report of record and duty status, failure to reduce speed to avoid an accident and failure to yield to stationary emergency vehicles – he is accused of crashing into a police car, which had its lights flashing, and a disabled semi truck, which led to both vehicles bursting into flames – the crash killed a tollway worker, 39, and seriously injured a 38-year-old state trooper) FALL from COLLAPSED STAIRS Workers Recovering From Stair Collapse (two men are recovering after a workplace accident that sent them tumbling from a building under construction when a stairway collapsed – temporary stairs collapsed and he fell 30 feet – he survived the 30 foot plunge without any broken bones – a co-worker who fell was also not seriously injured) FATAL FALL Indian construction worker falls to death in Sharjah (a construction worker, 24, has died after falling from the eighth floor of an under-construction building) FALL from SCAFFOLDING Apparent Accident Leaves Worker With Life Threatening Injuries (construction worker suffered life threatening injuries at a home construction project – worker fell off some scaffolding around noon – he was rushed to a local trauma center after the fall) FATAL FALL from SCAFFOLDING 1 killed in construction accident at Raleigh apartment complex (a construction worker, 30, was killed when he fell from the fifth floor of an apartment complex around 11:05 a.m. – he fell from scaffolding that was about five stories high and was pronounced dead at the scene) MANLIFT Worker injured at Matanuska Electric building site (a carpenter helping build a generating station was critically injured – the injured worker was one of two carpenters installing trim on a 10-by-14-foot rollup door – another worker was standing on a collapsible “man-lift” aerial platform raised 4 feet off the ground – the aerial platform rolled and pinned the injured man between the platform and the door – the worker underwent surgery to relieve pressure on his chest) CRANE FATALITY Police release name of worker killed at Red Lion’s water plant (an electrician, 55, was killed at the site of a new wastewater treatment facility being built – he was killed when an electrical switch gear box fell on him and trapped him underneath it about 10:58 a.m. – the box is about 7 feet by 4 feet in size and weighs more than 2 tons – it was not connected to an electrical source at the time – the facility is undergoing an expansion project and was installing the electrical switch gear box when it fell on him) FALLING OBJECT FATALITY Worker dies in construction site accident in Al Safa (a construction worker has died and four others sustained injuries when a concrete block fell form a height and led to a wall collapse at a construction site – one of the workers was rushed to the hospital with moderate injuries, while three others received medical attention on the site – workers were standing near a three-metre high wall, when some concrete blocks overhead fell down – one of them fell on the head of one of the workers, killing him on the spot – some of them crashed into the wall, causing it to collapse) SCAFFOLDING COLLAPSE 3 San Francisco Workers Injured in Scaffolding Collapse (three people suffered serious injuries in roofing accident – the men, 24, 50 and 58, were working on a home around 9:30 a.m. when a scaffolding collapsed, causing them to fall 25 feet – two are said to have potentially life-threatening injuries – the third is listed in serious condition) PLATFORM COLLAPSE Three Hurt In NC State Construction Accident (three construction workers have been hurt after a platform collapsed – the workers were helping a crane operator lift a steel beam when the eight foot platform they stood on collapsed – they fell about eight feet to the ground – none of the injuries was life-threatening – one person suffered a back injury, another had a leg injury and the third person had a head injury) MANLIFT FATALITY Worker tells of cherry picker horror in which man, 51, died (a worker who witnessed the aftermath of a man being killed after a cherry picker toppled over has spoken of his horror – the man killed was 51 years old – the man was ‘crushed’ when the cherry picker toppled over while being loaded on to a truck – the machine was being dropped off for another company fitting cavity wall insulation at a nearby home) Miscellaneous Framingham worker suffers serious burns in accident (a worker at an auto body shop was flown to the hospital after being severely burned in an accident – he suffered second-degree burns over 40 percent of his body – no other details) TREE CUTTING FATALITY Worker in East Texas dies in tree-cutting accident, struck by part of falling tree (a worker has died in a tree-cutting accident – the victim was dead at the scene after a 20-foot section of tree fell on him – he and several other people were part of the tree-cutting crew using heavy equipment and chain saws) TIRE EXPLOSION Burst tire rim sends worker to hospital (worker hit with rim of large truck tire – he was injured by part of a burst truck tire rim – worker was injured while repairing a tire on a large truck at the side of the road – they brought out a replacement tire and they were installing it at that point – it was a split-rim-type tire which has the potential of blowing the rim off the tire under pressure and apparently that’s what happened – he was hit with a piece of the rim and sent to hospital with injuries to his arm and legs) EXPLOSION Canister explosion seriously injures worker at Oregon coffee shop (an employee at a coffee shop suffered a severe head injury when a high-pressured canister behind the counter exploded – the woman was taken to a local hospital and then flown to another) HOTWORK Welding torch sparks fire at Greene Turtle (construction workers using a welding torch inadvertently ignited grease in an exhaust duct – the workers immediately grabbed a fire extinguisher and began putting out the fire – though the fire was small, use of the fire extinguisher caused “an awful lot” of damage) |
