1,000 workers evacuated after ammonia leak at Kentucky meat plant;
leak stopped 2½ hours later (an anhydrous ammonia leak at a
meat-packing plant forced 1,000 employees to evacuate and nearby
residents to stay indoors for hours – one worker, 40, was sent to
the hospital after complaining of burning eyes, nausea and vomiting
– there were no other reported injuries – about 1,000 workers were
evacuated at 11:30 a.m.- those living within a mile of the site were
told to stay indoors and to avoid using air conditioning or heating)
Japan atom plant worker received high radiation (one of the
workers struggling to avert a disaster at Japan{apos}s crippled nuclear
plant was exposed to a high radiation dose that may increase the
risk of cancer – authorities have also told the IAEA two prefectures
near the crippled plant were advised to monitor seafood products –
high levels of radioactive iodine and cesium were measured close to
water discharge points at the power plant – in a desperate attempt
to cool the reactors and their spent fuel ponds, workers have
sprayed or dumped sea-water into the plant{apos}s cooling system –
officials have acknowledged some of the water spilled back to sea –
the plant has still not been brought under control, and workers were
forced away from the complex when black smoke began rising from one
of its six reactors Wednesday)
SANITATION WORKER VEHICLE FATALITY
Sanitation worker killed by own vehicle in Carlsbad after falling
out of trash truck (a sanitation worker was killed when he
apparently fell out of the trash truck he was driving and was run
over by it – the fatal accident was reported shortly before 10 a.m.
– he suffered severe injuries to his torso and died at the scene –
he was working by himself when the accident occurred)
FATAL EXPLOSION
2 dead, 2 injured after Rubbertown explosion; cause under
investigation (two people are dead and two more injured after an
explosion and fire around 5:30 p.m. at the plant – everyone within a
one mile radius was advised to go and stay indoors, close all doors
and windows and bring in any pets after a chemical was feared to
have been released into the air – it was a Level 2 HazMat situation;
there are three levels – the chemical involved is calcium carbide
and it is an inhalation hazard – equipment at the scene did not show
any levels in the air that could do harm to people – there were 17
employees inside the plant at the time of the explosion, four of
which were taken to a Hospital – worker, 59, was pronounced dead at
the Hospital at 6:45 p.m. – a second man, 56, was pronounced dead
the following morning – both men died from burns sustained over the
majority of their bodies – the other two employees were released –
firefighters were told to evacuate the plant because the fire was
too large and was left to burn out – cause of the blast is still
under investigation)
MINE EXPLOSION
Mine engine explodes (two mine workers narrowly escaped what
could have been serious injuries in separate accidents – one case
included an explosion and the other case included a worker getting
his foot trapped in a ladder – investigations had begun into an
explosion incident reported to have occurred at about 3.30pm)
UPDATE on MINE FATALITIES
Former Massey Energy worker charged with felonies (a former
employee who had worked at the coal mine where 29 miners were killed
last year was charged with making false statements to investigators
and forging a document – worker, who left his job at the mine eight
months before the explosion, was charged with two felony counts of
making a false statement on an official document and making false
statements to an FBI agent and a federal MSHA investigator – he
fraudulently performed the duties of a mine foreman, which included
signing pre-shift examination reports – he signed his name on those
reports, using a foreman certificate number that did not belong to
him – during the course of the investigation into the disaster he
gave false statements to investigators)
CONFINED SPACE FATALITY
Worker killed by sewage fumes (one man, 27, has died and a
second, 32, was taken to hospital after a workplace accident – it is
believed the men were overcome by fumes while they removed sewage
from a septic tank at a commercial premises – he remains in a stable
condition)
UPDATE ON FATAL EXPLOSION @ HYDROPLANT
Investigators charge 7 executives over Sayano-Shushenskaya blast
(Russia{apos}s Investigative Committee has charged seven executives of
the Sayano-Shushenskaya hydropower plant with violations of
industrial safety rules that contributed to a deadly explosion at
the plant in August 2009 – specifically, investigators have charged
the plant{apos}s former CEO, his deputy, and five leading engineers at
the plant – the hydropower plant was seriously damaged in the
explosion, which killed 75 people and injured 85)
FATAL EXPLOSION
Explosion rocks Delta Steel Company, kills one (the explosion
occurred in one of the furnaces of the plant and blasted the
deceased in the Steel Melting Shop (SMS) department – another staff
of the company was also blasted and seriously wounded and is
presently under intensive care at a Hospital – the victims were
working under furnace two 33 KVA breakers in the control room in the
workshop when the furnace exploded and threw them apart)

ELECTROCUTION – SCAFFOLDING POLE INTO POWER LINE
Worker electrocuted in power line accident (a worker, 30, was
electrocuted when an aluminum pump jack he was setting up was blown
by wind into a 7200 volt power line – he was pronounced dead at a
Hospital shortly after the 8:30 a.m. incident – another worker, 43,
was admitted with injuries – the men began setting up to replace
vinyl siding on the duplex – the other two workers stood on the
ground and raised poles on the right side of the house to hold a
scaffold next to the duplex – that’s when the wind blew one pole
into the power lines, also located on the right side of the house –
one worker had burns to both feet and hands – the other worker was
more seriously injured – they tried to resuscitate him while they
awaited an ambulance)

Man rescued after fall at Pierce County treatment plant (a
construction worker, 37, was able to wedge himself in a 4-foot-wide
sewer line after he came loose from his safety line and wastewater
carried him 3,300 feet down the pitch-black pipe – hearing the man{apos}s
calls for help, crews at the Wastewater Treatment Plant were able to
find him and use ropes to pull him to the surface – he was rescued
about 300 feet from where the sewer line empties into the treatment
plant – he was conscious during the rescue and appeared to have only
minor injuries – he was taken to a hospital for further evaluation –
eployees from the construction company started work about 7:30 a.m.
on relining the sewer line – the worker had gone down a temporary
access shaft and was in the sewer line when he somehow became
unhooked from his safety line just after 7:50 a.m.)
BACKHOE FLIPS – WORKER DROWNS IN FLOOD

MnDOT Worker’s Body Found Along Hwy 169 (the body of a
Department of Transportation worker, 39, was found on the side of a
Highway – he was missing and presumed drowned after the backhoe he
was operating flipped over into a creek – he was a nine-year
employee with the department – according to the State Patrol, the
accident happened around 11:50 a.m. as he was clearing brush and
branches to help the creek flow – he apparently escaped from the
backhoe when it flipped, but somehow slipped into the creek and was
swept away by the current)
WORKZONE
Road worker on US 281 injured when struck by SUV (a man who was
part of a road construction crew working on resurfacing was injured
when he was struck by a sport utility vehicle – the man, an employee
of a company that had contracted to do leveling and laying hot mix
on the road, was complaining of injuries to his side before being
taken from the scene in an emergency medical services vehicle)

Killed At Hospital Construction Site (a man was killed at a
construction site when he was trapped between a backhoe and another
piece of equipment – no other details about the accident were
released)
WORKZONE
Collision on I-64 injures state highway worker (the preliminary
traffic crash investigation indicated that the driver of the Ford
pickup was westbound on I-64 when, due to a distraction inside the
vehicle, he struck the attenuator trailer (arrow stick) being pulled
by the highway maintenance truck – the highway maintenance truck was
stationary at the time of the crash, as highway maintenance workers
were repairing a damaged portion of the roadway – the driver of the
highway maintenance truck, 61, was transported to a Medical Center
for treatment of injuries he sustained in the crash)
CONCRETE AUGER
Worker trapped by machinery (a man, 55, became trapped by a
piece of concreting machinery – he sustained a compound fracture to
his right ankle when it became stuck in a concrete auger –
firefighters freed the man in about 20 minutes and he was rushed to
a Hospital in a stable condition)
ROLLER
Norwich man crushed under road roller in accident (worker, 40,
was left with a broken leg after the roller tipped over while he was
driving it – he was due to undergo a six-hour skin graft operation
to fill a big hole left in his leg following the accident – he had
the seatbelt on and it tipped over – there’s no side protection on
these rollers so both legs came out and he was twisted upside down
in the roller)
HOTWORK FIRE
Fire causes $50,000 in damages to Grandview P.S. (a small fire
on the roof of a school caused about $50,000 in damages – a roofer’s
torch sparked the fire at 2:47 p.m. – after fire crews removed the
aluminum siding in the effected area, they were able to put the fire
out on the roof before checking the interior of the school)

Framingham Man, 52, Falls From Crane in Boston (a man, 52, was
injured in a crane accident – he fell about 20 feet from a crane –
he was conscious after the crane tipped over – he was transported to
a Hospital – his injuries are unknown)
MISCELLANEOUS
STUMP GRINDER
Landscaper seriously injured in stump grinder accident (a
landscaper, 35, was critically injured when a stump grinder
activated while he was loading it into a vehicle – the worker
suffered life-threatening injuries to his torso and was air-lifted
to a Hospital for treatment – the team of landscapers were removing
trees when the accident occurred)

fire occurred just before 10 a.m. – the victim worked for a
contractor that details and prepares vehicles for sale – another
worker was hospitalized for smoke inhalation – fire officials said
the presence of solvents and other chemicals made the fire difficult
to tackle – no immediate word on a cause)
