in a portion of the byproducts area of the Coke Plant, rocked the

rushed to the hospital after the accident – no word on his current

killed when a crossbar of an electrical pole struck him in the head
– medical staff worked on his injuries until 2:22 p.m., when he was
pronounced dead – preliminary findings indicated cause of death as
head injury with depressed skull fracture)
UPDATE on FIRE
Report finds Toone flare plant explosion likely made worse by
ongoing worker errors (state investigators have found no
definitive cause for a Sept. 14 explosion and fire that seriously
burned three workers at a flare plant, but their report says the
problems were likely made worse by employees overloading their work
stations with flammable materials – the state determined that
overloading assembly booths was a recurring problem at the plant –
report states several times that booths in building 35 "were most
likely greatly overloaded" with flare assembly materials at the time
of the fire – in fact, TOSHA received a complaint alleging
overloaded booths on Sept. 10, just four days before the accident –
the flare materials are highly flammable and can be ignited by
static electricity, which is what investigators have said they
suspect caused the explosion and fire – investigations into previous
fires and explosions at the company have pointed to friction, impact
and electrostatic discharge as likely causes)

from a platform on the truck when it apparently landed on top of him
– the worker was taken to a hospital, were he was pronounced dead)

falling into a machine that sorts recyclables – he was using a
forklift to drop recyclable items into the machine around 3 p.m.
when he apparently fell in – he was alone at the time of the
incident – he had been working with another individual who found him
soon after the fall – he was pronounced dead at the scene – the
separator into which he fell is about 20 feet long – rotating
implements inside of it divide recyclable items by size)

Man injured in industrial accident in Salisbury (worker was
injured on the job when his left foot was run over by a forklift –
he was hospitalized with injuries not believed to be
life-threatening)
CARBON MONOXIDE FATALITIES
10 die from carbon monoxide poisoning in N China (the death toll
in a carbon monoxide poisoning accident rose to ten people – the
accident happened at 1:24 p.m. at a power plant – fifteen workers
were repairing a boiler at the plant when the accident happened –
two others, who came to rescue, were also poisoned – seven victims
are in stable condition and are recuperating in a hospital)

he was badly burned and was flown to the burn unit at a Hospital –

caused some second- and third-degree burns to his arms and hands

West Yorkshire print worker death crush (a worker, 35, died
after heavy machinery weighing more than a ton fell on him in a
horrific industrial accident at an ink manufacturing company – he
was trapped under the machinery – he was freed and taken to a
Hospital where he died of his injuries)

Port worker killed in forklift accident at APM Terminals (a
worker, 38, died of injuries she sustained in an accident involving
a forklift – she was pronounced dead less than an hour after being
struck by a forklift on the waterfront – at the time she was hit,
she was working as a "slinger," or a guide to a crane operator – she
was standing on the pier and using a radio to guide a crane operator
when a forklift driver nearby picked up steel bins, likely used for
collecting locking pins removed from cargo containers as they come
off ships – when those bins fill, forklifts similar to those used in
warehouses and other industrial operations pluck them off the
concrete berth and move them out of the way – the forklift driver
moved the forklift forward, running into the worker – the driver{apos}s
vision was obstructed by the bins carried on the front of his
vehicle)

Workman injured in shipping container fall (a worker split his
head open as he accidentally fell out of the container he was
offloading – he was using a forklift to move bikes from the
container into a company warehouse when the accident happened at
about 2.30pm – he suffered a large gash to his head as he slipped
and fell backwards out of the container at the warehouses –
colleagues carried out first aid at the scene and the injured man
was taken to a Hospital by ambulance)

by helicopter to a hospital after an accident at the plant just

CONFINED SPACE FATALITY
Worker dies after accident at ExxonMobil refinery (refinery
suspended maintenance work after the death of a contract worke – two
contract workers were discovered unconscious in an enclosed space
filled with nitrogen – one of the workers, 34, succumbed to his
injuries and died – the other is in critical condition in hospital –
as a result of this incident, all work relating to the turnaround at
the refinery has been suspended)

Keokuk man killed at Pinnacle Foods (a worker, 30, died as the
result of an accident – he had worked at the plant since September
of 2007 – he was cleaning an auger when he got caught in the machine
– it took several hours to get him out of the machine)
FLASH FIRE
1 burned in flash fire at Gallatin industrial plant (one person
was injured in a flash fire at a plant that produces powdered metal
products – this is the second such flash fire at the facility since
January – one person sustained burns in the accident and was
transported to a nearby hospital – two maintenance workers were
burned, one fatally, in a flash fire at the plant on Jan. 31 – in
that incident, investigators determined the fire was caused by an
electrical short that ignited materials in an elevator machinery
room, burning one worker on approximately 95 percent of his body and
the other on approximately 70 percent)

became trapped in a piece of machinery – co-workers called police
after they found him at about 4:30 p.m. – he was last seen just
after 1 p.m.- he was pronounced dead at the scene)

from the volume of earth caused significant chest, back and

STEAMROLLER FATALITY
Worker crushed to death by steamroller (a worker, 18, died under
a steamroller after allegedly jumping off the moving machine in a
steep, swampy area – friends found the body with his crushed head
partially buried in the ground – the victim was carrying out some
road works when he lost control of the machine, resulting in the
mishap – the victim tried to jump off the vehicle in a bid to save
himself and he landed behind the back wheel of the steamroller – he
died of severe injuries to the head)

Police appeal after New Malden explosion (an explosion
hospitalized a 24-year-old workman – he was in critical condition
when he was taken to hospital with head and arm injuries – he was
working to repair a collapsed sewer and was digging up the side of a
road when there was a suspected electrical explosion in a junction

Minor fire erupts at IPL stadium (a minor fire erupted at the
new cricket stadium being built – some 80 boxes containing LED
screens and tiles were destroyed in the fire – no one was injured in
the incident – labourers were busy completing the welding job,
sparks emanating from the metal fell on the boxes kept in the
storage room on the ground floor through the ducts kept for
ventilation – people were alerted on seeing the smoke emerge from
the room)

he was injured when he fell while climbing scaffolding – he was just
on his way up and then he slipped and fell – hospital kept him
overnight just to be safe, but they were going to release him in the
morning – the employee was a relatively new hire)

MBTA worker rescued after fall (an employee of the area{apos}s
transit agency was injured in a 30-foot fall down a shaft –
firefighters pulled the veteran worker from inside a hollow concrete
support column at about 7 a.m. – the employee worker for the
agency{apos}s power department – he was working on an elevated trolley
track at about 4:30 a.m. when he fell between the tracks and down a
2-foot-by-2-foot hole that widens at the bottom)
ELECTROCUTION
Worker electrocuted while washing widows at Huntington Beach medical
office (the worker was standing on the ground and washing
windows of the three-story building when the accident occurred – the
equipment he was using came into contact with a 12,000-volt
electrical line – he was using a long extension rod with a brush at
the end of it and he had to step around a tree – it appears as he
did so, he brought the brush and the extension rod away from the
building, and he came in contact with the 12,000 volt power lines
above him)
HOTWORK EXPLOSION – Cutting Drum
Man injured in explosion at Clarksville hardware store (an oil
drum that was believed to be empty exploded when the worker tried to
cut into it using a blowtorch – worker was taken by ambulance for
treatment for head injuries – the wounds were not thought to be
life-threatening)
Fumes beneath hotel send four to hospital (noxious fumes and a
powder sent four people to a hospital for decontamination – the
fumes may have been sewer gas – three men who were working beneath
the hotel started having trouble breathing, and paramedics were
called about 9:40 a.m. – when the workers came out from under the
building, they were covered in a white powder, possibly lime that
had been spread on the dirt a year ago to absorb moisture – the
workers complained of burning lungs and eyes, and one vomited – a
female emergency medical technician breathed in the dust that wafted
off one patient in an ambulance – she and the workers showered and
scrubbed behind privacy screens at an emergency decontamination zone
set up outside the emergency room – the workers were admitted to the
hospital for treatment but were expected to be released soon)
