UPDATE Report blasts Transocean over Deepwater Horizon disaster (federal investigators blasted Transocean, owner of the Deepwater Horizon, for poor maintenance of the doomed drilling rig, inadequate training of workers and safety lapses that may have contributed to the deadly Gulf disaster a year ago – “numerous systems deficiencies” and “omissions” ranged from bypassing gas alarms and automatic shutdown systems that could have prevented an explosion to failing to conduct enough emergency drills – "These deficiencies indicate that Transocean’s failure to have an effective safety management system and instill a culture that emphasizes and ensures safety contributed to this disaster,” a joint Coast Guard-Interior Department panel said in a preliminary investigation report on the Deepwater Horizon accident – the report focused narrowly on the explosions and fire on the Deepwater Horizon, worker evacuations, the flooding and sinking of the rig and the safety systems of the rig and Transocean – Transocean said in a statement that it “strongly disagrees” with the findings, noting the Deepwater Horizon was compliant with U.S. and international standards at the time of the accident. “We look forward to setting the record straight,” the company said. LOTO FATALITY Industrial accident claims life of Canton Drop Forge worker (an employee, 31, likely died of massive head and neck injuries in an apparent industrial accident – he was attempting to move a pallet from a machine that uses high pressure blastings to remove imperfections from forgings when a cable snapped, causing a hopper to pivot backwards at a high rate of speed – he was thrown into a metal structure and then fell into a walkway – the machine was not operating at the time of the incident)
FALL into CONFINED SPACE Warren Worker Falls Down Manhole (a wastewater worker fell about 30 feet down a pump station hole – he is a mechanic with the city{apos}s wastewater department – he and a co-worker were checking sewers shortly after 9 a.m. when the accident took place – he went to go in and lost his footing and fell – the other worker may have actually broke his fall – the radios the workers had do not work that far underground, so the partner had to climb out to call 911 – it appears he has injuries to his foot and leg and is expected to make a full recovery)
ELECTROCUTION Utility Workers Killed at Power Plant in Nicaragua (two workers, 42, were electrocuted and died at a power plant while trying to restore service to a section of the capital – one worked for the state-owned Enatrel and the other worked for private electricity distributor – accident happened when the workers moved one of the bars on an electrical panel that was live during a difficult and complicated repair)
HOTWORK FATALITIES 4 workers overcome by fumes pronounced dead in hospital (four men died after they were overcome by fumes from a fire in a telecommunications building – the blaze broke out in an empty equipment room which was being renovated – five workers were inside the room when, according to an initial investigation, sparks from a cutting machine ignited insulation materials – four were overcome by the smoke and fumes – another managed to escape – the four were taken to hospital but later pronounced dead – the city government said the fire had been caused by "workers{apos} improper operations" and that "no safety precautions had been taken")

FATAL FALL Worker falls to death in Ajman (a 27-year-old labourer died after falling from an under-construction building – he was working on top of a building in new industrial area when he lost balance leading to his fall and immediate death – the official explained: “The labourer was not wearing a safety helmet while doing his job leading to severe head injury and death" – PERSONAL COMMENTS… are you kidding me!!! They are upset because he did not have on a hardhat and not that he fell!!!

FATAL FALL Worker dies after falling through warehouse skylight in Monterey Park (a worker, in his mid-40s, broke through a skylight of a warehouse and fell 35 feet to his death – workers were rehabilitating the roof of a warehouse at the time of the accident – a preliminary investigation showed the man was vacuuming rocks off the roof about 1:30 p.m. when he broke through a 3-foot-by-6-foot domed Plexiglas skylight and fell to the concrete floor below – he was taken to a local hospital where he later died)

AMMONIA LEAK 4 treated, released after ammonia spill in NE Neb. (four people have been treated for exposure to ammonia – a mother and two children were flown there from Martinsburg after an ammonia leak – the father drove to the hospital – all four were treated and released by afternoon – a farmer was using anhydrous ammonia fertilizer on his field when a trailer carrying a 1,000 gallon tank unhooked and snapped a line – a shutoff valve on the tank failed)
