FATAL FALLWampum worker killed at Cemex had just returned from layoff (almost exactly a year after cement production stopped the worker, 60, was out of work – he was recalled May 5 – but his latest stint at the cement plant lasted less than a month – he was killed in an accident – an autopsy showed that he died from head injuries in the accident – just before 2 a.m. he was unloading a railroad car of cement – he had been inside the plant’s railroad unloading building and stepped out onto the train car to assist with the unloading when he fell around 20 feet from the rail car – he was pronounced dead at the scene)
FATALITY Del Monte Worker Killed In Accident (worker, 64, passed away at a hospital of injuries suffered at the plant – he worked at the plant for 33 years – company would not provide details of what happened)
CONFINED SPACE FATALITY – Oxygen Deficiency from Nitrogen Purge Two workers killed in Shintech accident’ investigation underway (state and federal authorities are investigating an accident that killed two workers, overcome by fumes – an employee and a contractor died in a vessel in a process unit that was not operational – the bodies of the two men were sent to New Orleans for autopsy and released to their families – a Guard called 911 at 11:30 a.m. to report one man had fallen in the tank, but was still alive – the men received immediate medical treatment at the site, and that local emergency medical services were called in – UPDATE on story Shintech plant manager: men apparently suffocated in nitrogen-filled container (investigators believe the contract worker was overcome by fumes while descending a ladder into the container to retrieve tools – the plant employee also went into the container, possibly to assist the contractor, and also was overcome – plant manager stated the men likely suffocated in the oxygen-deficient container that was filled with nitrogen as part of the process to produce vinyl chloride – authorities are awaiting autopsy results)
LOADING DOCK FATALITY Worker, 23, killed in Conroe loading-dock accident (a man, 23, died after a truck backed into him at a loading dock – he was unloading an 18-wheeler, and as the truck prepared to leave, he walked in between the rear of the truck and the loading dock – when the truck backed up, he was crushed between the truck and the loading dock – he may have been trying to repair the truck’s bumper when the accident happened – he was taken to a Hospital, where he died of severe injuries)
MOBILE EQUIPMENT FATALITYWorker killed at Nucor’s Memphis plant (a worker, 43, was killed at a steel facility – he was operating mobile equipment at the time of his death – plant has suspended plant operations while it investigates the accident)
AMPUTATION Railroad worker seriously injured in Boone (a railroad worker has had his arm amputated after he was struck by a passing train – the employee was getting off a locomotive when he was hit by a passing train – he was taken by helicopter to a hospital where his arm was amputated)
LOTO – AMPUTATION OSHA investigating accident that cost worker his arm (a night-shift maintenance employee’s arm was amputated by a team of surgeons after he became trapped by a piece of machinery he was working – the worker was performing routine maintenance on a sand system used to make molds at the foundry)
ACID SPILL Acid spill injures worker in Morris (about 100 gallons of hydrochloric acid leaked onto the floor of an industrial plant – one worker was hospitalized for skin injuries after the leak – the acid leaked from a 200 gallon tote – the building was evacuated, and hazardous material teams were called to the scene to ensure that acid had not leaked into the sewer system)
ELECTROCUTIONOhio Edison Worker Electrocuted During Repair (a power company employee, 53, died after being electrocuted while making a service repair on a power line – he was pronounced dead at the scene)
CONFINED SPACE BURNS Five workers burned at Midlothian cement plant (three of five workers burned in a cement plant accident have been listed in critical condition – the injuries were sustained in an industrial accident – there was no explosion – workers were injured while performing maintenance activities in the main processing area – the accident happened in a kiln, which is used in the cement-making process)
MINE FATALITY7 killed in China coal mine accident (seven people were killed in a coal mine accident – a total of 15 workers were in the mine at the moment of a gas leak there – eight managed to escape)
UPDATE DUST EXPLOSION FATALITY Worker Injured In Flash Fire At Plant Dies From Injuries (the second worker injured in a flash fire at an industrial plant has died of complications from his injuries – he died 16 weeks after he was seriously burned in the flash fire – he and another mechanic were fixing a broken elevator back in January when flammable metal dust that had accumulated in the area ignited, engulfing them in flames – the other worker died two days later)
LOTO Investigation launched after man crushed by heavy machinery near Hastings (a 40-year-old man is fighting for his life after his legs were crushed in heavy machinery – he was in a heavy duty car crushing machine when it was accidentally switched on – he was given an emergency anaesthetic by doctors at the scene and was flown to a Hospital by air ambulance following the incident – he is said to be in a stable but critical condition and his injuries have been described as life-threatening)
UPDATE FATAL EXPLOSION Enmore worker dies week after factory explosion (a Sugar Estate worker has succumbed to injuries he sustained more than a week ago after a dryer exploded in the factory – he had worked with the Sugar Corporation for more than 31 years – he succumbed to “internal injuries”)

Henley Bridge worker killed after concrete beam hits his head (a concrete rock was jarred loose from a support beam – the beam fell and struck the man in the head while he was working on the barge – the man was wearing a safety helmet at the time)
ANGLE GRINDER Angle grinder accident in Sunshine West (an accident with an angle grinder has left a worker, 67, with serious injuries – he was using an angle grinder when it apparently became snagged on his clothing and cut him just above is left ankle – workmates came to his aid – he then lost consciousness for a brief period – he had to be treated for blood loss and was taken to a Hospital in a stable condition)
MOBILE EQUIPMENT FATALITYConstruction Worker, 60, Killed In Skid Steer Accident (a man was killed when his leg was run over by a construction skid steer – he was taken to a Hospital, where he died during emergency surgery)

OSHA investigating worker’s death at Lowe’s (authorities are looking into a fatal accident that occurred at a home improvement store – the victim was pinned between a delivery truck and a trailer and was airlifted to a local hospital, where he later died from his injuries)
LAWNMOWER Man’s hand seriously injured in lawn mower mishap (emergency crews responded to an accident at a Golf Course after a maintenance worker had his hand injured in a lawn mower – a golf course employee suffered deep cuts to his hand after it was caught in the blade of the commercial mower he was operating)
COMPRESSED AIR Air hose pierces man’s rear-end and blows him up like balloon (a man slips and falls on an air hose, that pierces his rear-end, and the air blows the man up like a balloon until co-workers stop air supply – this very strange accident happened to a New Zealand truck driver, 48, who doctors say is very lucky to be alive – he was standing on the foot plate of his truck when he slipped and fell – the air hose broke off the air reservoir that powers the trucks brakes – when he fell, the air hose pierced his rear end and began pumping compressed air at 100 pounds a square inch into his body – his screams brought his co-workers running and one of the men released the safety value, which immediately stopped the air flow – he was rushed to the hospital with fluid in his lung and body swelling – the air had separated the fat from the muscle in his body – doctor’s also said he was lucky that the air did not enter his bloodstream – the air hose pierced his left buttocks and the pressure the hose was under continued to blow the air into his body)
ELECTROCUTIONWorker Electrocuted While Trimming Tree (a man, 45, working for a professional tree service was electrocuted when he came into contact with a power line – he was transported to a Hospital where he died – he was tied to a tree when a metal clip from his gear came into contact with the power line) |
