Industrial MINE FATALITY Worker killed in Şırnak mine collapse as search restarted in Afşin (one person was killed and another two were injured when a coal mine collapsed – search teams located and rescued the two injured miners, while also recovering the other miner’s body – the injured miners were sent to a Hospital where they are receiving treatment) FLASH FIRE 5 hurt in blast at metal powders plant (three workers were critically burned in an explosion at a chemical plant – the accident was the third this year at the plant, where a January flash fire killed a worker – the facility employs about 175 people making metal powders for automotive and industrial uses – the two previous accidents occurred after flammable dust accumulated in the air and combusted – the fire occurred in the furnace room of the plant, where 10-15 people work – it was different than the fatal accident in January) UPDATE Week after plant accident, worker dies from injuries (a man, 23, has died from injuries he suffered last week in an industrial accident at an auto-parts manufacturing plant – he was working as a machinist May 19 when he was hurt – he died Wednesday – what sort of injuries he suffered and what caused them are not clear – a spokesman for the parent company said machinery was running when the accident happened, but could not elaborate) LINEMAN ELECTRICAL FPIA lineman injured in accident (a worker suffered minor burns after he accidentally came into contact with live wires – the lineman remained in the intensive care unit at the burn center as an apparent precaution – he was working with a crew doing line work around 9 a.m. when the incident happened – he reportedly received minor burns to both hands) ACID Worker in clinic after acid accident (one person had to be hospitalised following a chemical reaction at an industrial plant – he was rushed to a clinic after complaining of breathing difficulties when a mixture of acids leaked during work procedures at the site – fire fighters evacuated and decontaminated the area before removing the two burst 25-litre tanks of acid) LOTO/GUARDING Worker’s legs crushed at scrapyard (a worker, 40, was fighting for his life after his legs were trapped in a metal crusher – he is thought to have climbed into the machine after he dropped the remote control inside – he then stood on the button and set off the hydraulic press – both his legs were badly crushed – someone was working nearby and was able to raise the alarm – he was airlifted to a hospital where he underwent emergency surgery) MINE FATALITIES Roof fall kills 3 in south China coal mine (three people were confirmed dead in a roof fall accident at a coal mine -accident occurred at 3 p.m. trapping three workers who were conducting repair work in the shaft – by 6 p.m., the bodies of the three had been recovered, and the cause of the accident is under investigation) FUMES Workers overcome with gas at warehouse on Linden Avenue (six or more workers were overcome with gas at a warehouse in the metal management complex – it is unclear at this time how many workers came into contact with the gas – a hazmat team was dispatched to the site to determine the source of the gas – most of the workers present for the gas remained at the scene to await decontamination there before being transferred to a Medical Center for observation – three workers from the accident had been admitted to a Medical Center – the workers were stable, but were still under observation and awaiting further tests) MAGNESIUM FIRE Worker Burned at 2-Alarm Business Fire (a worker suffered burns to at least 50 percent of his body in a fire at a metal finishing business – the injured worker was taken by ground ambulance to a Hospital – the commercial building contained magnesium and since firefighters with the Volunteer Fire Department did not have the Class D foam used to extinguish metal fires, everyone inside the building was evacuated – the Emergency Operations Branch of Harris County Fire Marshal’s Office was dispatched to the fire with the Class D foam) CONVEYOR Man trapped as arm crushed in machine (a maintenance worker, 48, who had his arm crushed in a conveyor belt at the meat processing plant endured horrendous pain to tell his rescuers how to free him – he was working on the conveyor belt when he was pulled into the machine by his right arm, which became entangled around a roller – he was trapped for more than 30 minutes before another maintenance worker working in another area of the plant found him and raised the alarm – he was operated on and was in the Intensive Care Unit until he was moved to a surgical ward) HOTWORK FLASH FIRE cutting drum Employee injured in metal recycling business fire (an employee suffered underarm burns after materials he was cutting ignited in flames – details were sketchy but that the employee was using a battery-operated hand tool to slice up a 55-gallon drum in an area of the business where fuel is recycled – a spark from the hand tool, which was not a cutting torch, might have ignited some fuel or another liquid that was in the area) FUMES Toxic Fumes Infect Three Workers at Royal Canadian Mint (a shocking case of pilferage of toxic fumes came into light when three people at the Royal Canadian Mint were exposed to toxic fumes – one of the men lost its consciousness at the spot and other two were treated by the paramedics for trivial irritation – the three patients are reported to be stable) CONFINED SPACE Two rescued from dry docked tugboat (two men were rescued from a dry docked tugboat after being overcome by paint fumes – they were painting the boat when they began feeling ill due to the noxious paint fumes – firefighters had to use a ladder truck to reach the men, because the deck of the boat was fifty feet above ground – rescuers then had to maneuver through tiny passages to get to the victims, who were at the bottom of the boat – the rescue took about 15 minutes – the victims were taken to the hospital for treatment) EXPLOSION Five injured in explosion in Croatia (five people have been injured in an explosion in a plant for plastic materials – the blast occurred while workers were cleaning areas in the plant – two women are in a critical condition – cause for the explosion have not been identified yet – according to the preliminary reports one of the women found a metal object and threw it in a rubbish bin and then the blast occurred) FATAL EXPLOSION Explosion at chemical plant in eastern China kills 3 people, injures 8 (a blast at a chemical plant has killed three people and injured eight others – police were investigating the cause of the explosion at the chemical plant – the blast killed two people at the site while a third person died later at a hospital – eight other people who suffered slight injuries were being treated at a hospital) FATAL EXPLOSION – GAS WELL One killed in natural gas site explosion (one person has been confirmed dead and another has been airlifted to a Medical center with serious injuries, following an explosion on a natural gas processing site around 7 p.m. – the two men were the only ones in that area when the explosion happened – no evacuations are warranted at this time, however the plant has been shut down – the cause of the gas explosion is under investigation) CHLORINE 850 ill due to chlorine poisoning in Egypt (more than 850 people fell ill due to chlorine poisoning after toxic gas leaked from a shop in Egypt’s north – the incident occurred on the outskirts of a town – all the affected people were taken to hospital and rescuers were working at the scene of the leak) FORKLIFT FATALITY Local man crushed by forklift (worker, 58, was killed while carrying out his duties as a truck driver – he was picking up timber from the mill when he was involved in an accident with a forklift – it appears the forklift has rolled down a small incline and crushed him) Construction MANLIFT & NEW Definition of “Nearmiss” Worker injured in accident at chip factory (worker spent three nights in hospital after jumping from lift truck – he was injured after a boom lift truck he was operating nearly fell out of an opening of the yet-to-be-completed computer chip factory – the 16,000 pound vehicle nearly fell out of a fourth-floor opening – workers were taking the lift truck to the fourth floor of the building using what’s known as a lifting cage – when the cage reached the fourth-floor opening, the lift truck operator got into what’s known as the boom basket and started to drive the truck out of the cage into the building – but when the front wheels met the concrete, the rear wheels pushed the cage away from the building, and the lift truck fell between the building and the cage – luckily, the boom basket got wedged into the ceiling inside the building, keeping the lift truck from tumbling to the ground – the worker jumped about 12 feet from the basket onto the landing inside the building – the worker appeared to be OK, but was treated by medics and taken by ambulance to a Hospital – a company manager whose family owns the business said it was a near-miss type of accident) FALL into PIT Worker falls six metres into pit at Saudi embassy (a construction worker, 31, fell more than six metres into a mud pit at a construction site – he’s in stable condition, he’s coherent – he was in serious but stable condition when he arrived in hospital – he was extricated from the pit by a team of firefighters and paramedics and suffered injuries to his head, neck, back and extremities, but did not lose consciousness) WORKZONE City worker hurt in wreck at construction site (semitrailer driver disregarded stop sign, reports indicate – worker, 28, an employee in the city water department, was injured around 8:30 a.m. when he was struck by a semitrailer that reportedly disregarded a stop sign he was holding near a water main break – four water maintenance employees were preparing to patch a driveway when a semi collided with a city dump truck that was parked in a driveway – worker fractured two cheekbones and his jaw when he jumped out of the way – driver was cited for assured clear distance) WORKZONE Construction worker struck on HWY 43 near Kingsburg (a construction worker was hit in a busy construction zone – a company spokesperson she says she cannot confirm the details of what happened in the accident or the extent of injuries – he was flown to the hospital – the construction worker may have been run over by a water tanker – the truck may have backed over the man) LOTO FATALITY Caterpillar platform crushes worker (a worker, 37, was crushed to death trying to fix a vehicle’s broken hydraulic ramp – he died instantly after the platform carrying a 200-kilogram (kg) concrete pipe pushed him to the ground when it suddenly started operating) HIGH ANGLE RESCUE Worker rescued from Ottawa construction site roof (firefighters used a crane to bring a construction worker down from an eight-storey height – a worker, 21, on the building had injured his back – firefighters attached a rescue basket to an on-site crane and, with the aid of the crane operator, lifted a firefighter in the basket to the building’s top) EXPLOSION Explosion and huge fire rock Hudson plant (people within a 2-mile radius of the chemical plant are evacuated – part of U.S. 321 shut down – no one was hurt in the blaze – the blaze followed an explosion just after 3 p.m. at the plant which makes dyes, lacquers and other coatings for furniture – the cause of the explosion and fire was unclear) MIscellaneous CONFINED SPACE FATALITY – ENGULFMENT High school student killed in farming accident (an accident on a dairy farm has killed a high school student – the 18-year-old junior was killed inside a corn silo – he was inside the silo, knocking some of the corn loose that had stuck to the sides of the silo – another employee was working outside the silo when he sensed something was wrong – he climbed up in the silo and was unable to see the other employee and determined at that time he was probably trapped by the grain – that other employee climbed down the silo and found the entrant at an opening at the bottom, but because of the weight of the corn, could not get him out – fire personnel arrived but were unable to pull him to safety) ETHER FIRE Ether fire burns tire company worker (an employee at a Tire plant returned to work this week on light duty after he was burned last week when a customer sprayed a tire with ether, which exploded – the customer came to the store because he was having problems inflating the tire – the tire would not hold air, so the customer pulled out a can of ether, which the store does not use – the employee offered to hook air to the tire, but told the customer he’d have to use the ether himself – ether can be used to seal a tire, but it is dangerous – before the employee could walk away, the man sprayed ether on the tire and ignited it causing an explosion that burned the employee’s face, neck and arms – the customer, who also was burned, left in a white work van) |
