Industrial REFINERY FATALITY Worker killed at Exxon’s Fawley,UK refinery (a worker was killed following an accident at a refinery – the worker, a contractor, was reportedly trapped under pipes at the facility and died as he was taken to the hospital) Streetsboro industrial worker seriously injured in accident (an industrial worker, 43, was seriously injured while on the job around 8:30AM at an industrial business – EMS responded and found the victim with a crushing arm injury sustained when a large piece of steel fell on his arm – the victim suffered a large laceration and other unknown internal injuries to his left arm – Lifeflight transported the victim to the trauma center) FORKLIFT FATALITY – PASSENGER THROWN from forklift Another worker dies (another accident inside the ship building facility claimed the life of a worker, 26, around 12:15 am – worker hitched a ride on a forklift being driven by a another worker to go to the mess hall of the production department center for their midnight meal – the report described that the driver “was driving the forklift in an over speeding manner” – the forklift then hit a concrete ditch cover and the riding worker was thrown off head first into the ditch) ELECTRICAL Worker injured in possible electrical accident at MT Refining Company (a worker at a refinery was taken to a hospital after an electrical incident just before 11 am – the refinery worker was found on the ground near electrical equipment – the worker was treated on scene by Fire Rescue personnel and was then taken to the hospital by ambulance – extent of his injuries is unknown) AMMONIA Ammonia leak scare causes two-hour evacuation (an anhydrous ammonia leak at a plant prompted the evacuation of a compressor room and surrounding businesses shortly before noon – about 45 people were working in the compressor room when a valve started leaking – because of high winds, authorities evacuated nearby businesses in addition to evacuating the compressor room – the evacuations were precautionary and no one was injured – authorities contained the leak by putting a new packing gland into the valve – in 1987, an ammonia leak and explosion at the plant gave two employees and two firefighters minor burns – it cost the plant $500,000 to repair and forced it to shut down for a month) MINE FATALITY Russian coal mine accident kills two (two miners have been found dead after part of a coal mine collapsed – a third worker rescued from the mine is in hospital in a stable condition, while the remaining 76 miners were able to escape immediately – a preparatory output section partially collapsed as a result of strong natural pressure) Lufkin Industries employee in stable condition following weekend accident (an employee was injured when a 2,000-pound container fell on him – he is recovering at a hospital, in stable condition – the report shows an approximate 2,000-pound container fell on a worker and it appeared he suffered significant injuries to his back, as well as internal injuries, as he had trouble breathing) ELECTRICAL Shipyard employee shocked in dry dock (officials are investigating an incident in which an employee, 35, received an electric shock while working at a dry dock at about 12:30 p.m. – the shock was described as minor – he was working in the pump well of Dry Dock 2 – a pump well houses pumps used to remove water from the dry dock – the worker was released from the hospital and returned to work later in the day) LOTO/GUARDING Man injured in abattoir accident (a man sustained serious hand injuries when it became caught in machinery – the man’s hand became caught in a sprocket chain – workers stopped the machine and were able to free the man’s hand before emergency services arrived – the man was wearing gloves – his thumb was severely damaged) LINEMAN ELECTROCUTED BGE Contractor Dead After Industrial Accident in Lothian (worker, 39, died after he was electrocuted around 2:45 p.m. – witnesses on scene told officials that he had climbed an electrical pole with his pole-climbing gear and began work on a portion of the pole near the wires – he then came into contact with a live wire – workers began a rescue effort and brought him down from the pole – Fire Department personnel began CPR – resuscitation efforts were eventually discontinued and he was pronounced dead at the scene) ARC FLASH Electrician burned by shock (an electrician is recovering in a hospital after he suffered a shock and second degree burns during a routine installation job – the electrician and an apprentice were installing a 600-volt generator when the accident happened – a wire from the generator came loose and struck an open panel, causing an electrical arc – the apprentice was released a few hours later with second degree burns to one arm – the second worker was taken to a Regional Hospital for further treatment – his injuries are non-life threatening) STEAM Man Released from the Hospital After Being Burned at Alon Refinery (a man suffered second degree burns while working at the refinery – he was burned when steam was released and hit his body – the employee was working on a leak near the boiler – when he tried to lower the level of the boiler, he was sprayed with hot water – he suffered second degree burns to over nine percent of his body – he was released from the hospital around 3 a.m. the following morning) Worker severely burned at AK Steel (worker, 25, suffered severe burns when his work vehicle reportedly overturned and caught fire – he was operating a truck that ignited after backing too close into a molten slag dumping site – Engine 6 personnel used a jump line from their truck and extinguished the passenger side rear tire assembly, which had begun to burn after contact with the molten material – he transported to a Hospital and was later sent to a Children’s Hospital’s burn unit, where he was listed in good condition) CONFINED SPACE FATALITY LA worker crushed in industrial clothes washer (a worker has been crushed to death in an industrial clothes washer – he was pronounced dead at the scene shortly before 12:30 p.m. – the death is under investigation) H2S Leak at Shell refinery (a hydrogen sulphide leak at an Oil refinery) EXPLOSION Chemical explosion closes Olympia-area plant (three people were injured after a small explosion at a chemical plant – crews transported three people – their injuries are not considered to be life threatening – the explosion happened when peroxide was improperly placed in the wrong location – a small explosion happened and released a small cloud into the air – the cloud dissipated once it left the plant – DHEC asked the plant to close the section where the explosion happened to ensure the structural integrity of the plant) UPDATE on AMMONIA INCIDENT Ammonia Distributor Settles With EPA, Agrees to Install 14 Leak Detection Systems (a national ammonia distributor has agreed to install and operate $345,000 of ammonia leak detection systems at 14 facilities across the country to settle claims by the U.S. Environmental Protection Agency that the company violated federal clean air regulations on chemical risk management at plants in Rhode Island and Michigan – business will also pay the EPA $56,700 to settle the claims – EPA says the business did take some actions to address the risks of an accidental release of ammonia at its facility in Rhode Island. But the company failed to address the risks associated with its policy of not staffing the facility except when ammonia is being received or distributed there, the agency said – the Rhode Island facility is about a tenth of a mile from a residential neighborhood – the company’s Michigan plant has similar staffing problems, the EPA said – the business also failed to coordinate adequate emergency response plans with local emergency response agencies to ensure that the public would be protected in the event of a release of ammonia – the new ammonia leak detectors the business agreed to install and operate will alert residents in surrounding communities of accidental releases of ammonia – alarm signals will also be sent to emergency response personnel so that they may address accidental ammonia releases – the agreement involved two separate settlements with EPA regional offices – in a settlement with EPA New England, the company agreed to install and operate ammonia leak detection systems at six facilities: Butner, N.C.; East Providence, R.I.; Tamaqua, Penn.; Philadelphia, Penn.; New Castle, Penn.; and Natalbany, La. – the company also agreed to a $28,350 penalty to settle claims of violating the Clear Air Act at its Rhode Island plant – in a separate settlement with EPA’s Region 5 office in Chicago, the company agreed to install and operate detectors at eight other Tanner facilities: Lincoln, Ala.; Inkster, Mich.; Belvidere, Ill.; Council Bluffs, Iowa; Neosho, Mo.; Morgantown, W.Va.; Mount Hope, W.Va.; and Fayetteville, N.C. This settlement also requires the company to pay a penalty of $28,350 for violating the Clean Air Act at its Inkster, Michigan plant)
Construction WORKZONE Freeway worker injured, driver jailed (roadway worker, 52, struck is in critical condition – a suspected drunken driver careened down a closed highway entrance ramp, critically injuring a freeway construction worker – he was working on the ramp when the driver, 21, drove down it in a tan 1998 Mazda – the on-ramp was closed and was under construction – the Mazda glanced off a moving Caterpillar loader, hit a concrete k-rail and then struck the worker – some members of the construction crew honked the horns on their trucks, but driver never responded – workers kept the driver at the scene until police arrived, after she got out of her vehicle yelling) TUNNEL FATALITIES Eight feared dead in Manali cloudburst (at least eight people have been feared killed and twenty others injured in a flash flood triggered by a cloudburst – accident took place when eight workers were washed away at the construction site of a tunnel – one body was recovered eight kilometres downstream from the cloudburst spot -search is on to locate the others – the workers were involved in construction of the tunnel) FORKLIFT FATALITY Forklift truck driver dies (a construction worker, 41, was killed after being crushed when the forklift truck he was driving toppled over – he was pronounced dead at the scene – accident happened at about 9.10 a.m. where a £150 million new shopping centre is being built) WORKZONE UPDATE: Highway clean-up worker struck, killed by truck (a man who was seriously injured after being hit by a pick-up truck has died – incident happened just after 8:30 a.m. – the 74-year-old man was a volunteer with the group Adopt a Highway and was working on the side of a Road when the incident happened – police have determined the pick-up truck went off the road on a slight curve and struck the victim, who was standing on the shoulder – the man was taken to a Hospital where he was pronounced dead – all Adopt a Highway volunteers have to wear safety vests, put up warning signs along the highways they are working on and even use brightly colored garbage cans) AUGER FATALITY Vermont man dies in construction accident (a worker, 46, has died of injuries caused by heavy machinery in a construction accident – he was operating an auger as he worked to install a 3-foot-diameter pipe – the auger became bound in the pipe and he was thrown to the ground and pinned there by the auger – another construction worker used a backhoe to lift the auger off him – two workers administered CPR until an ambulance arrived to take the worker to the hospital, where he was pronounced dead) GRANITE Worker Injured When Granite Falls On Leg (a worker, 53, was injured when a 700-pound slab of granite fell on his lower left leg – crews responded to the scene and found the slab had already been removed from the man – ambulance transported the patient to a nearby hospital) WORKZONE Officers investigate report of highway worker hit (a worker on a highway project was hit by a vehicle around 3:30 p.m. – the condition of the worker wasn’t immediately known but the injuries weren’t believed to be life-threatening – the driver of the vehicle didn’t stop after the accident, but was pulled over and arrested) FATAL FALL Four workers die in freak accident (four construction workers died when the rope snapped on the lift that was carrying them – doctors declared all the four dead as they were rushed to a hospital) HOTWORK Non-injury fire breaks out at LAX Terminal 6 (a non-injury fire broke out in an escalator area of an airport around 2:30 a.m. and was put out by airport-based firefighters – it appears that welding was under way where the fire occurred – firefighters used fans to blow the smoke out of Terminal 6 and adjoining terminals) TRENCH Workman buried in Melbourne trench collapse (a worker, 25, was buried in dirt when a trench collapsed on him – he was been dug out by his quick-thinking mates and a paramedic – he is in a serious condition after the accident – he suffered suspected fractured ribs, a suspected collapsed lung and leg injuries after the walls came down on the trench – paramedics arrived to find workmates digging him out of the trench)
MIscellaneouS Tree service worker spiked in head while working at Lakeview Plaza (the man, 37, who was working for a tree-trimming service was taken to Mercy – he was trimming a large oak tree in the complex’s court yard when his safety line slipped – he slid, and the spike from his shoe pierced the back of another employee’s head – its estimated the spike was an inch to an inch and a half long) GASOLINE FLASH FIRE 2 landscaping workers hurt in explosion on truck (a flash fire inside a truck injured two landscaping workers – the accident happened around 10:30 a.m. – Video from WSB-TV showed the roof of the truck burned through – the injured workers were part of a three-man crew of landscapers – one of the men tried to start up a lawnmower inside the back of the truck when a spark from the lawnmower ignited gas fumes that had built up – the two injured workers were taken to hospitals) GUARDING/LOTO FATALITY Fair worker killed in machinery (a fair employee, 40, was killed in a generator fan accident – he was using a large diesel generator when he suffered severe injuries – at around 3:30 p.m. the victim was transported by EMS and was pronounced dead after resuscitative efforts failed to revive him en-route to a Hospital) UPDATE Welding banned in Ottawa school after fatal blast (a school where a student died in a shop-class explosion this spring has been ordered to stop all welding and related activities until certain safety standards are met – Ministry of Labour applied the ban on welding, grinding and other “hot” work at the High School – the ban follows preliminary work in an investigation into the death of an 18-year-old who died following an explosion on May 26 in the school’s shop class – four other students and one teacher were also injured in the explosion – students in the class were using empty oil drums to make barbecues – officials said they believed that vapours from residual oil might have ignited as the students cut through the drum using a hand grinder) HOTWORK Small fire caused Fort Myers City Hall to evacuate (firefighters determined there was a small fire in one of the elevator shafts that had been extinguished prior to their arrival – workers doing maintenance on the elevator were using a cutting torch and some of the hydraulic oil used to operate the elevator heated-up and caused smoke to fill the first three floors of the four-story building – the building was evacuated and no one was hurt) FORKLIFT FATALITY Man dies in Vic forklift accident (a worker, 40, has died while working on a farm using a forklift to lift fertilizer into a bin when he fell from the machine, sustaining head injuries – he died at the scene of the accident at around 3.50pm) |
