Industrial GUARDING/LOTO FATALITY Man dies in industrial accident on a ship moored in Milwaukee (a preliminary report from the medical examiner’s office says a worker, 60, died in an industrial accident aboard a docked freighter after his arm was severed at the shoulder – he was working alone by the conveyor belt three decks below the main deck when the accident occurred – the ship was delivering cement to various ports) FORKLIFT Worker run over by forklift airlifted (a worker was run over by a forklift and was airlifted to a Medical Center – the forklift ran over the man’s lower extremities) FATAL FALL or ELECTROCUTION Worker falls to death on TV serial sets (a worker, 26, fell to his death after getting an electric shock on the sets of a television serial – he had climbed about 15 feet to adjust the lighting when he got an electric shock – as a result, he slipped and was declared dead on arrival when taken to the hospital) POWER PLANT FATALITY Worker dies in mishap at Adani power plant (power plant authorities declared a holiday amid mounting tension among its workers following an accident in which a labourer, 45, died – one metal plate fell on one of the labourers who died on the spot) GUARDING/LOTO AMPUTATION Eastern Pa. man loses arm in industrial accident (a man, 46, lost his arm in an industrial accident in a factory that produces snack foods – when emergency crews arrived they learned that a man had his arm amputated by a machine – the worker lost his arm from the elbow down and was taken to a Hospital by ambulance – it was unclear if his arm would be able to be reattached) GUARDING/LOTO Industrial Accident Injures Worker in Brighton (a man was hospitalized following an industrial accident at a a metal finishing plant – the man had been injured in an accident involving a high/low – EMS provided medical care for the man’s injuries, which were primarily to his lower extremities, and called for LifeFlight transportation) FATAL AMMONIA EXPLOSION Ice plant accident claims three lives in Kannur (an investigation is on by the Factories and Boilers Department into the causes of the explosion of an ammonia tank in an ice plant which claimed the lives of three workers, 22, 23, and 25 and left another seriously injured – the accident occurred when the workers were carrying out maintenance work on the ammonia tank at the Ice Plant and Cold Storage – the injured, 21, has been admitted to a Hospital with 75% burns) SODIUM FIRE Sodium fire reported at Idaho National Lab site (one employee has been burned in a sodium fire – 10 other workers were evaluated on scene and released by medical staff – a department spokeswoman couldn’t immediately comment on the size of the fire, but suggested it may have been caused by a sodium reaction – there’s no more evidence of a continued reaction or fire – the cleanup worker burned in the reaction has been taken to a medical center – the incident comes several days after 16 cleanup workers were exposed to radiation when a box containing plutonium powder was opened) FATAL BOILER EXPLOSION One Killed as Boiler Explodes in Agra Factory (a worker, 50, was killed in a an explosion in a rubber factory – five other people were injured when a boiler exploded – police have not yet been able to ascertain the cause of explosion) MONOMER PLANT EXPLOSION Tosoh’s monomer plant caught fire after explosion (one worker is still missing at the No.2 vinyl chloride monomer plant – no schedule of when fire can be put out – the fire broke out after an explosion shortly before 3:30 p.m. – the cause was not known) Construction CIRCULAR SAW Worker hurt in circular saw accident (a worker, 19, has been injured when he was cut with a circular saw on a building site – he was working on scaffolding three floors up on the site when the saw sliced through his upper thigh – he suffered a deep cut – paramedics had to wait for rescue equipment to get the man down off the scaffolding) CRANE Worker injured after crane topples (a worker has been taken to hospital after being trapped for about two hours in the driver’s seat of an overturned crane at a construction site – the crane is thought to have toppled over after the ground it was standing on subsided – the worker had been operating the crane when the accident happened) UPDATE Las Vegas Fined $80,100 in Sewer Line Deaths (a woman is asking that criminal charges be filed after a trench collapsed killing her husband and another man while the two men were working last May – she is asking that her husband’s supervisor on scene that day face involuntary manslaughter charges – the City was recently cited by the state – the report indicated there were no support measures in place to prevent the collapse and no one inspected the trench before the men were sent in – the city government is facing $80,100 in fines for safety violations – on May 18, city employees, 32 and 49, were buried as they dug in trenches which, the state Occupational Safety and Health Bureau said, had not been stabilized according to code) MIscellaneouS CARWASH Greenwich car wash worker struck by SUV (a car wash worker was hospitalized with serious injuries after being struck by a sport utility vehicle leaving the Car Wash – his injuries are not believed to be life-threatening – the worker was standing near a Volvo S60 sedan and a Ford Explorer SUV, which were both parked after having left the car wash bays, at about 1:10 p.m. when a Lexus SUV apparently drove toward the man and the vehicles – the Lexus struck the car wash employee, sideswiped both vehicles and continued through the parking lot before coming to rest in the southbound lane of a Street – a carwash worker, who noted that the woman at the wheel of the Lexus was driving fast, said the worker who was hit was fortunate to have escaped more serious injuries) |
