Industrial Sanitation Worker Killed On The Job In Laurel County (a sanitation worker, 36, was killed on the job – he was killed when he was pinned between the garbage truck and a car while checking the hydraulic system on the truck that had apparently failed – he was suffocated) FATAL FALL from SCAFFOLDING Worker killed at Cemex plant (a worker, 29, has been killed in an industrial accident at a cement plant in the early hours – he was working as a scaffolder for a sub-contractor and was killed around 1.45am – paramedics arrived on the scene to find the man with a serious head injury and in cardiac arrest – another man suffered a broken arm and had to be taken to hospital for treatment – the cement plant is currently in its annual shutdown period for regular maintenance – there are a number of contractors working on the site as a result) FALL from PLATFORM Worker seriously hurt in fall at TTC yard (a worker, 41, was seriously hurt when he fell four-and-a-half metres from a platform onto the pavement around 5:15 a.m. – he was taken to a Hospital, semi-conscious, with a serious cut to the forehead – he is recovering in hospital – no word on what caused the accident) BUCKET TRUCK TIPPED Cleveland Public Power worker thrown from bucket truck (a Public Power worker was thrown from a bucket truck as he was working on power lines around 3:10 a.m.- the truck tipped over into the woods along the highway, throwing the worker out of the bucket, according to a police officer who witnessed the accident – the worker landed on some railroad tracks and was able to crawl across the tracks where he was picked up by EMS and taken to a local hospital – condition is unknown – the officer that witnessed the truck tipping over said everything was moving in slow motion and he couldn’t believe what he was seeing – it is unclear what caused the truck to tip) SAWMILL BOP sawmill accident under investigation (a machinery accident which left a young man, 20, with serious crush injuries – the man had sustained “crushing” injuries – the injured worker was a contractor, but would not provide any further details on the incident until investigations were complete) FATAL FALL of UTILITY WORKER Worker dies in fall from Saco transmission tower (a worker, 44, fell to his death while working on an electrical transmission tower – he was working high up on one of the poles that is part of a line upgrade in the area when he fell about 55 feet just before 4 p.m. – he survived the fall but by the time they got him to a Medical Center shortly after, he passed away) PRESSURE TESTING GONE BAD Air tank explosion injuries three (three people were sent to a Medical Center after a pressurized-air tank explosion at about 8:25 a.m. – they were pressure testing a tank that had been built and it broke loose and exploded – the tank was six feet in diameter and about 20 feet tall – it was unclear how close the three were to the tank – there was no gasoline or chemical explosion) OIL RIG FIRE Chevron Says Two Workers Still Missing as Fire Burns Off Nigeria (a search and rescue operation continues for two contractors missing after a fire at a rig – the rig was drilling a natural-gas exploration well in about 40 feet of water – there was a possible failure of surface equipment during drilling work that led to a loss of well control, based on initial indications – the rig has partially collapsed and the well is burning, adding that it can’t estimate how long the fire will continue – two workers are hospitalized because of minor burns, and others are being kept for observation) Worker injured at steel firm in Quarryville (a worker sustained a “severe” leg injury in an industrial accident around 6 a.m. – he suffered a severecrushing leg injury – the subcontractor was working in a welding shop when the accident occurred – he was moving something around and apparently made a mistake and a piece of steel hit his leg) ARC FLASH Electrical Short Severely Burns Maintenance Worker at Area Hospital (an electrical short is blamed for severely burning a maintenance worker at a Hospital – a maintenance employee was burned by an electrical short in a room on the roof – he suffered 3rd degree burns and had to be taken to the emergency room – the short sparked a small fire that other employees put out before fire crews arrived – the short happened when employees were taking the cover off an electrical panel for inspection – the burned employee suffered serious burns on the hand and arm) FIRE Worker injured at Bowmanville auto wreckers fire (one worker was burned and a local business was left with more than $500,000 in damages in a fire at an Auto Recycling facility – the fire broke out in one of the garage buildings at 8:30 a.m. – there was an accidental ignition in one of their garages where they do some of their work, removing gas tanks – the garage was destroyed by the fire – one worker was injured and taken to a hospital with minor burns – he was treated at the hospital and then released) ELECTRICAL Sheboygan Falls’ Bemis plant worker injured after electrical shock (a man, 26, was hospitalized after suffering an electric shock while working in the plant at about 12:30 p.m. – emergency responders found the man conscious and alert but suffering from electrical burns – he was taken to a hospital – the man was working on a machine and experiencing a continuous shock when another worker saw him and pushed him away from the machine, breaking the electrical connection) CONFINED SPACE FATALITY – ONE ENTRANT and TWO WOULD BE RESCUERS – ATMOSPHERIC Three workers die after inhaling poisonous gas (three workers, including an engineer, of a chemical factory died after allegedly inhaling poisonous gas – a worker, 50, was cleaning a boiler of the plant when he felt suffocated and collapsed – his son who tried to rescue him and an engineer who went to their aid also met with the same fate – fire service personnel retrieved the bodies and sent them for post mortem) CONFINED SPACE FATALITY – ONE ENTRANT and ONE WOULD BE RESCUER – ATMOSPHERIC 2 choke to death inside sewer line (two persons died of suffocation while cleaning a drainage line – a contractor, 35, and his worker entered the drainage line one after the other and apparently got choked to death after inhaling noxious fumes in the drainage line – preliminary investigation revealed that around 10 am, one worker entered the 18-feet deep drainage line – when he did not come out, the boss too entered the drainage to check but he also got stuck inside – a citizen at the spot who saw the two entering the drainage informed the police) ELECTROCUTION T&TEC worker electrocuted (worker, 42, was electrocuted while engaged in what is known as “hot work” around 11 am while he and other crew members were installing an airbreak switch on high tension wires – he was rushed to a Health Facility where he was pronounced dead on arrival – he had been employed with the commission for over eight years – on the scene, workers said all protective equipment was worn by the crew and all precautionary measures were taken – the high-tension wires which he worked on contained 12,000 volts – the purpose of the switch was to isolate electricity from the lines for future work) EXPLOSION Man wounded in explosion at St Marys industrial complex (an explosion at an polyurethane-resin plant left a worker, 50, with shrapnel wounds to his head, chest and arms – he had been working near a bulky container of a flammable liquid – he was taken to Hospital in a serious but stable condition – a large plastic container of a flammable liquid used to make polyurethane had exploded and blown apart the container and its metal housing – workers were changing out a liquid container that must have had vapour residue still inside, which exploded) HOTWORK Four workers suffer burns in BIA fire (hundreds of travelers had a narrow escape when a fire broke out at an International Airport around noon – the fire erupted after sparks that flew from a welding worksite set tarpaulin sheets ablaze near the VIP lift in the departure terminal – four workers are in a critical condition with 80% burns – the travelers were waiting at the check-in counters, a mere 30 metres away – the sheets went up in flames as they fell on the four workers. – the fire was put out in 10 minutes) HOTWORK EXPLOSION cutting a METHANOL DRUM 1 injured in explosion (one man, 48, was injured in an explosion and fire at 10:09 a.m. – upon arrival on scene, the crew discovered there had been an explosion at the Oil Well Service business, but the fire was out, and the employee reported to have been injured in the explosion had left the area – worker was transported to a Hospital and then flown to a Burn Center with second-degree burns to both legs and a possible broken arm – the explosion occurred when he was using a cutting torch near a 50-gallon barrel marked “Methanol”) HOTWORK Small fire at OxyChem extinguished quickly (the fire was out on arrival – a contractor doing torch work ignited a small fire that was immediately put out) TANK LEAK Chemical leaking at Carteret fuel refinery (emergency workers are trying to stop a leak at a fuel refinery that has released 3,000 gallons of combustible liquid – all of the liquid spilling from a tank was contained in a dyked area – the product, reformate, which is used in blending fuel products, was first discovered leaking from a storage tank flange about 8 a.m. – the tank holds 3 million gallons of the product – the liquid was being transferred another tank, and an environmental cleanup company was at the scene to remove the liquid that leaked from the flange) AMMONIA FATALITY Chemical leak in Codegua leaves one dead and 122 injured (an ammonia leak has left one person dead and caused the evacuation of nearly 150 workers, 122 of which have been taken to the hospital with injuries of varying severity – the most severely injured worker has been airlifted to a Hospital with burns on 25 percent of his body – the one fatal victim, 20, was found in a refrigeration chamber at the plant – the leak may have been caused by a broken pipe that fed into the refrigerated chamber) FORKLIFT FATALITY Western Pa. man run over, killed by forklift carrying scrap (a worker, 53, has died after being run over by a forklift carrying scrap metal at a stainless steel recycling company – an autopsy shows he died at a hospital about a half hour after he was crushed by the forklift) Construction DEMOLITION Worker ‘critical’ after Christchurch Grand Chancellor demolition accident (a man has been seriously injured while working on the demolition of a quake-damaged hotel at about 8.30 am – the worker was taken to a Hospital’s emergency department in a critical condition, but was unable to give further details of his injuries) WORKZONE State DOT worker hurt in icy I-405 crash (a state Department of Transportation employee was injured while working at any icy crash scene near the intersection of Interstate 405 and Interstate 5 at about 7 a.m. – the employee has been taken to a Medical Center – the transportation worker had responded to a crash at about 6:45 a.m. in which a car slid into a concrete barrier – as the worker was helping the driver, another car lost control and hit the first vehicle, which in turn struck the worker) DUMP TRUCK FATALITY Dump Truck Runs Over Worker (a construction worker, 57, is dead after a co-worker ran him over with a dump truck around 12:25 p.m. – a preliminary investigation indicates the driver was backing up while the owner of another trucking company directed him – the driver suddenly accelerated and ran over the owner, killing him at the scene – the victim’s wife was helping at the construction site and witnessed the accident) FALL from LADDER Worker Falls 30 Feet From Ladder (a construction worker, 49, fell about 30 feet from a ladder that swayed in high winds at a work site – the worker was taken to a Medical Center where he would probably be moved to another hospital – accident occurred at about 1:30 p.m. – worker suffered serious leg and pelvic area injuries – he had been doing repairs on the roof, was on a ladder and he fell when it tipped in gusty winds – he landed on a rusty metal container that used to be a compactor) CEILING COLLAPSE Indonesian killed in ceiling collapse (a worker, 25, died on the spot while his colleague sustained serious injuries in an accident at a construction site – he was working on a structure said to be a guardhouse when the ceiling fell on him and crushed him to death in the 11.45 am incident – the injured worker, 37, was sent to a Hospital with a fractured leg and broken ribs) EXCAVATION Propane leak closes intersection in Spring Hill (a propane gas leak at the intersection caused a temporary closure of the intersection and the evacuation of about a dozen homes within a block of the damaged line – the leak was caused by a crew that accidentally struck the line just before 9 a.m. – there were no injuries and no fire) CRANE Beam from crane shears top off of van on N.J. Turnpike (a steel beam dangling from a crane swung into the northbound lanes of a turnpike sheared off the top of a van leaving the vehicle’s driver with facial lacerations – workers were operating a crane – they were attempting to lift a steel beam when it pivoted and swung into the right lane of travel – driver, 63, was driving north when the beam struck his Econoline van – the force of the impact sheared off part of the roof and overturned the van – he suffered lacerations to his face and was taken to a hospital for treatment – authorities did not say whether high winds were a factor in the collision) MIscellaneouS Car wash worker dies in accident (a man died when struck by a car at a carwash – a car wash worker was directing a driver of a vehicle to the wash tracks when he hit the victim with his car, crushing him against the wall) City Worker Hit By Garbage Truck (a city worker is recovering after he was hit by a city garbage truck – the worker was assisting a driver on a commercial route when he was struck by the truck – the city is trying to figure out what caused the accident and whether the truck was working properly – the driver has been with department for five years, and the other worker has been there for two years) FLASH FIRE lighting heater Worker Injured Igniting Store Heater (worker, 32, was flown to a burn center for treatment of facial burns – he suffered first degree burns to his face after a gas heater he was working on ignited – he was working on the heater located in an area in the ceiling of the store – a flash of fire ignited as he started the heater, burning him) FATAL FALL Worker Dies at Hunter Mountain Ski Resort (an employee working, 41, on snow-making equipment at a Ski Resort fell to his death – he slipped on ice, slid off the edge of the trail, and fell 200 feet down a steep embankment striking a rock and a tree – he was wearing a helmet and had crampon spikes attached to his boots) HOTWORK Fire aboard the Michigan causes $10,000 in damage (a fire aboard a fishing vessel docked at a Wharf caused approximately $10,000 in damage to the boat – the fire was reported at 4:12 p.m. – it was caused by welding operations on board the vessel – sparks from the welding caused the insulation to burn after which the fire spread to one of the fish holds – there were no injuries) |
