At 4:15 p.m. on June 30, 2024, Employee #1, a maintenance technician, 27, was assisting with WLV pressure testing when a rod was ejected from the WLV system. The rod struck Employee #1 in the head, causing a fatal injury. Emergency services were called, but the employee died from the injuries.

a) On or about June 30, 2023, while pressure testing a 7″ wire line valve, employees used a crystal gauge without verifying if it was calibrated. The hazard will be abated through the following feasible means: Develop and implement a Management System Process to govern pressure control activities for the Wireline Product Service Line that requires Wireline Product Service Line employees to verify equipment calibration as part of proper equipment rig-up or setup. Train Wireline Product Service Line employees involved in pressure control activities on this Halliburton Management System Process.
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:
a) On or about June 30, 2023, while employees were pressure testing a 7″ wire line valve outside of a pressure testing cell or wellsite, the sources of pressure were not released before turning the wheel locks. The hazard will be abated through the following feasible means: Develop and implement a Management System Process to govern pressure control activities for the Wireline Product Service Line that requires Wireline Product Service Line employees to verify proper equipment rig-up or setup and to implement controls before engaging in any pressure testing activity. Train Wireline Product Service Line employees involved in pressure control activities on this Management System Process. Update Well Head Pressure Control Equipment Pressure Test Work Method to ensure that Wireline Product Service Line employees engaged in maintenance of wire line valves do not attempt to repair pressurized equipment and instead, must release and verify the release of all pressure before attempting any repairs, consistent with industry guidance which states: Tightening, repair, or any other work shall be done only after verification that the pressure has been released. Train Wireline Product Service Line employees involved in the maintenance of wireline valves on this updated Work Method.

a) On or about June 30, 2023, employees were exposed to struck by hazards caused by conducting pressure testing without guidance from procedures and policies governing how to confirm that pressure has been bled off.

a) On or about June 30, 2023, employees were exposed to struck by hazards when pressure testing a 7″ wire line valve outside of a pressure testing cell or wellsite when indicator rods failed. The hazard will be abated through the following feasible means: Update Wireline Product Service Line procedure for annual certification of single wire line valves to require referencing applicable manufacturer’s guidelines when reassembling single wire line valves to ensure correct assembly, as well as to require use of compatible hardware. Ensure employees involved in certifying single-wireline valves have the required competency prior to certifying a wireline valve.
