INDUSTRIAL OIL RIG Line blast injures oil-rig worker (a worker, 25, is in a coma following an oil rig accident – he was working on the rig when a high pressure line broke and the force knocked him 10 feet away – he has had several strokes and swelling of the brain since the accident, and a ventilator is helping him breathe) LOTO/GUARDING Worker injured in accident at Boeing plant (employee was recovering at home after being seriously injured in an industrial accident at about 7:15 p.m. – he was trapped in a machine – crews freed the man from the machine and transported him to a field – he was taken from there by helicopter to a Medical Center – no further details will be released) ELECTRICAL Worker accident results in power outage (a power outage that affected about 13,000 customers happened when a worker accidentally came into contact with energized equipment – the unidentified subcontractor was transported to a Hospital – the worker was at a “tie in station”, a location where all the power lines come together) LOTO/GUARDING FATALITY Pirelli Factory Worker Found Dead in Autoclave (a worker, 48, was found dead inside an autoclave at about 6 p.m. – the autoclave in question was used in the curing of beads used in the construction of new tires – his presence physically inside the autoclave was highly unusual and is something that would only happen during maintenance – investigations are likely to focus on both how he came to enter the autoclave and how the machinery came into operation with him inside – unanswered questions include how the autoclave door was shut behind the deceased and how the machine was turned on – most autoclave machinery features fail-safe technology preventing the system from operating with an open door, so it is likely that the door had to be closed for the machine to operate) CONFINED SPACE FATALITY in WASTE TRUCK Refuse worker killed in bin truck accident (a refuse worker has died after becoming trapped in the back of a bin truck – the man was rescued from the vehicle but lost his fight for life in hospital around 11.30pm – fire crews used specialist equipment to get the man out of the back of the truck, and he was rushed to a Hospital for treatment, but later died of his injuries – it is understood the man who was aged around 40 was working for a private waste company) CONFINED SPACE FATALITY in WATER TANK Worker dies in water tank accident (one of two men trapped in a water tank at a power plant died after the floor beneath them collapsed – the men were contracted employees, sent to clean up the 15 by 10 foot tank – the men were standing on a makeshift floor inside the tank when it fell, breaking one man’s leg and killing the other worker, 54 – it was a routine cleaning – a repair the night before was done through welding, and the two gentlemen were there to clean up the remnants of the welding, and something happened with the floor – Cal-OSHA said the floor was temporarily made for workers to stand on during repairs, but it collapsed – there were no toxic fumes in the water tank – the tank was a watering filter system) FORKLIFT FATALITY Man dies in Pointe-Claire work accident (a worker, 29, died from injuries he suffered in a workplace accident – he got wedged between a lift-truck and a building just after 7 a.m. – his condition deteriorated on the way to hospital, where his death was confirmed) FATAL EXPLOSION Worker killed in blast at Qadian factory (a worker, 28, was killed and four others were seriously injured in a blast at a cracker factory – a powerful blast took place at the factory at about 2.45pm.) RACK COLLAPSE FATALITY Worker killed by pallets of paper (investigators are looking into the death of a worker at a paper distributor – the man was killed when a shelving unit stacked with pallets of copy paper gave way and fell on him at the plant – no one else at the plant was hurt) FATAL BOILER EXPLOSION during START-UP Wynnewood Explosion Kills Refinery Worker (a worker, 34, was killed in an explosion at an oil refinery when a boiler in the refinery exploded about 6:20 p.m. – another worker was injured in the blast and remained hospitalized – his condition was not released – the refinery had been shut down earlier this week for planned maintenance and that the boiler was being brought back online when it exploded) SETTING POLES LINEMAN FATALITY Orange County REMC lineman dies in accident (a local utility worker, 33, died on the job – he was doing a routine task loading a pole with the assistance of two other employees when it fell lose from the cradle and struck him in the head and he was killed instantly) UPDATE on LOTO FATALITY OSHA fines Alcoa $37K in worker’s death (worker, 45, who died at the plant in July was doing maintenance on a piece of machinery that wasn’t locked out – OSHA is citing the facility with six violations deemed “serious” and proposing a fine of $37,800 – OSHA inspectors discovered that about the time of the fatal accident in the facility’s sheet finishing department, a threader table attached to a coil slitter “was not pinned in place” after being positioned – the employer did not specifically address energy control procedures for controlling movement of the threader table due to gravity – worker was doing maintenance on the machinery when the accident occurred – he had been employed as a general mechanic at the plant since March) METHANOL TANK EXPLOSION Worker called in during Winnipeg fuel plant blaze saw fireball shoot into sky (fire investigators pored through the charred, twisted remains of the Speedway International fuel plant as a worker recalled seeing a huge ball of fire erupt into the sky – the start of a massive blaze that caused $15 million in damage – a methanol tank had exploded and firefighters were worried rail cars, trucks and large storage barrels would be next – some of the 60 firefighters on the scene started dousing the rail cars to keep them from catching fire, but could not attack the blaze directly for about five hours – businesses and homes in a one-kilometre radius were evacuated – officials gave the all-clear shortly after midnight and residents were allowed to return home) FLASH FIRE Worker burnt in blast (a worker, 25, is in serious condition in a hospital after a small explosion at a factory just before 9 am – the explosion occurred when a fuel source caught a light – worker was taken to a Hospital with “quite severe burns” to his face, hands and arms) FATAL EXPLOSION in DIESEL TANK Worker dies in plant fire (a worker at an asphalt plant is dead after an explosion occurred in one of the company’s diesel tanks – upon arrival, rescue units found the worker, 30, had been killed during the explosion – it is unknown at this time what caused the explosion – no other workers were injured and the explosion did not threaten the plant or the surrounding neighborhood) EXPLOSION Explosion at Japan chemical plant kills 1, dozens injured (an explosion at a chemical plant has killed a firefighter and injured dozens of people – plant workers first reported a large bursting noise and temblor after a chemical tank exploded – the cause of the blast was under investigation – at least 17 people were injured, including seven other firefighters, two police officers and seven plant workers) DUST FLASH FIRE Two Workers Recover From Burns After Columbus Plant Fire (two workers are recovering after being burned in a fire at a corn–processing plant – the fire happened when a cloud of dust at the plant ignited while the workers were moving smoldering corn – one worker was treated and released – the other is still in the hospital – they are expected to make a full recovery) HOTWORK Fire causes evacuation at Shannon facility (employees evacuated their facility around 10:30 a.m. after the filtering system for the facility’s welding system caught fire – the fire was relatively small and extinguished fairly easily – the fire was contained to the filtration system) HOTWORK Fire extinguished at oilfield service business (firefighters have extinguished a blaze at an oilfield service facility at about 2 p.m. – two buildings were on fire, one of which contains explosives – all employees were safely evacuated – there were no injuries – a welding spark ignited nearby building-insulation material) REFINERY FIRE Fire in US Largest Refinery push Crude and Gasoline Prices higher (a fire at the largest operating refinery’s diesel hydrotreater unit) UPDATE on FATAL EXPLOSION @ REFINERY Gas leak in Pdvsa refinery was detected more than one hour before the blast (a team inquiring into the causes of the explosion said, “It seems very clear that proper actions were not taken upon the leak” – task force explained a loss in the pressure and flow of olefin was detected before the deadly explosion in the refinery – they have declared that operation reports showed, “On August 24 at 11:30 p.m. olefins were being transferred from Amuay refinery to an alkylation plant in Cardón refinery when a loss of pressure and flow occurred” – therefore, there were signs of gas leaking at least one hour and forty minutes before the explosion) PRESSURE VESSEL CATASTROPHIC FAILURE Water treatment plant blows up (a pressurized compressor that sends water to residents in the privately owned development exploded, blowing up the entire treatment building – there is no evidence of criminal activity – preliminary investigation reveals the tank failed and the internal pressure blew the end of the tank off and threw the attached building, completely) AMMONIA Chemical leak at Marinette plant (a chemical leak at a chemical manufacturing plant a little after 6 p.m. – the chemical that was leaked was an ammonia based product – some employees became nauseous and started to get headaches – as a precaution, the plant shut down and sent all second shift workers home early – no one needed medical treatment) CONSTRUCTION WORKZONE FATALITY Highway Worker Killed At Work Site (a Township Highway Department employee, 62, died on a work site after being struck by one of the department’s truck – he was with a township highway department crew performing culvert repair – although no witnesses directly saw the event transpire, he was found by co-worker’s underneath a 1988 International Dump Truck belonging to the township – preliminary investigation indicates that the truck backed over him – he was pronounced dead at the scene) WORKZONE FATALITY Road worker dies in freak accident (a worker, 37, from the Highways Agency died while repairing a crash barrier in the middle of the night – it is believed the man was hit by a piece of machinery at 2.40am – he died at the scene) Falling Beam Pins Worker at Williamsburg Construction Site (an accident at a construction site left a worker pinned under a fallen beam around 2:30 p.m., when a beam fell on the worker, pinning his hand – the worker was rushed to a Hospital in serious but stable condition – the accident occurred after a worker cut through one of two horizontal support beams for the building – that cut created a ripple effect, causing the sliced beam to collapse, along with a vertical support column and the structure’s other horizontal support beam) WORKZONE FATALITY Road worker and motorist die in Indiana Toll Road crash (a road worker and a motorist died in a two-vehicle accident – the victims, 52 and 78, have been identified – the worker was a 17-year employee of the Toll Road – at 11:30 a.m. the 78 year old reportedly had been speeding and driving erractically—was westbound at the 39.8 mile marker when his 2005 Hyundai Santa Fe rear-ended a pickup truck parked on the inner shoulder – the worker had just exited the pickup to fetch maintenance equipment from the bed – the pickup had activated warning lights and an illuminated arrow board on it – tThere was also signage behind the pickup warning westbound traffic of workers up ahead – worker was airlifted from the scene to a Hospital where he died of his injuries) WORKZONE FATALITY DOT worker hit, killed by vehicle in Watauga (a worker, 55, with the N.C. Department of Transportation was hit by a pickup truck and killed as he attempted to remove a tree that had fallen onto the road – the accident happened just one day after a state law took effect requiring motorists to slow down or change lanes to avoid roadside utility or maintenance crews flashing an amber light – he died at the scene of the accident – he was a transportation supervisor and had worked with the DOT for 7½ years – the accident happened at about 4:40 a.m.) WORKZONE Construction Worker Seriously Hurt In SMART Bus Accident (a construction worker, 41, is critical condition – he was working on a bridge project and was carrying a 36-foot-long railing that stuck out into the northbound lanes – when a passing SMART clipped that railing, sending the worker flying into the air and landing on a crane – he suffered severe head injuries and was taken to a Hospital) EXCAVATION Workers Hit Gas Main (workers digging struck a gas main for the third time in about a year, snarling traffic and forcing people in nearby buildings to leave the area at 10:15 a.m. – the excavator pulled up a bucket of dirt then he heard a “hiss” – the excavator did not directly hit the gas main – instead, a wall in the pit they had dug gave way, dislodging a 1-inch, low-pressure gas pipe) CRUSHED Worker crushed to death between lorry and excavator (a worker, 52, was killed when he was crushed between a six-ton lorry and an excavator at a construction site – he was checking the excavator when the lorry suddenly reversed and pushed him against the excavator, killing him instantly) HOTWORK Roofer’s torch sets fire to home in Park Ridge (use of a torch during roof repairs is being blamed for a house fire last week that spread through the attic and left workers stranded atop a home) TRENCH Sioux City man rescued after construction accident (a city worker, 20, is hospitalized following a construction accident around 11 a.m. – he was working with a crew to dig a trench, when the dirt collapsed on top of him – he was rescued by his co-workers and emergency crews – he’s listed in fair condition) MISCELLANEOUS SPRINKLER FLOODS STORE Wal-Mart closes after construction accident (Wal-Mart closed after most of the store flooded when a contractor hit a sprinkler line about 7 a.m. – a worker using a hydraulic hammer hit the sprinkler line while removing a wall during construction to convert the store into an 146,002-square-foot Wal-Mart SuperCenter – there were no injuries in the accident) COTTON BUGGY FATALITY Man killed in industrial accident (a field worker, 48, was killed when a “cotton buggy” rolled over him – he had been working in a cotton field when a tractor hauling a trailer, or cotton buggy, crushed him – he was pronounced dead at the scene) Worker Airlifted After Pickup Falls on Him at Rogers Business (a workplace accident that left one man injured – the accident happened just after 10 a.m. – a 50 year-old man was working under a full-sized pickup that was up on a hoist, when the hoist gave way – the back end of the pickup fell on the worker – he was conscious and stable when he was airlifted, but did sustain a serious back injury) GUARDING Worker lucky not to lose arm in machine mishap (a man was rushed to a Hospital after getting his arm caught in a potato planter – his arm became caught in the large machine’s feeder mechanism – a co-worker driving the tractor pulling the potato planter saved the man from more serious injury – it is understood his arm may have been pulled in by the auger type feeder mechanism after his sleeve got caught – the man was cut out of the potato planter by Ambulance staff, who freed him by cutting his clothing off) BOOM TRUCK BASKET CLIPPED 2 western NY highway workers hurt in accident (two town highway department workers, 46 and 52, are hospitalized after they fell 30 feet from their truck’s basket platform when a tractor-trailer hit their vehicle while they were replacing a banner over a street – a tractor-trailer drove past and clipped the platform, sending the two men plunging to the street – they are at a Medical Center with serious but non-life-threatening injuries) FIBER OPTIC Workers with Fiber Optic Cables Rescued from Connecticut River (two men fell into the river prompting an afternoon river rescue – the men were trying to pull fiber optic cables across the river when the accident happened – they were placing the fiber that’s hanging down low and the boat capsized in the current) ARC FLASH Contractor suffers burns at Cheverly warehouse (a contractor has suffered second-degree burns while working at a warehouse – an electrical arc caused burns to the contractor’s face, neck arms and chest – his injuries were not life-threatening – several months ago, a worker at the warehouse accidentally ran a forklift into a storage rack which caused the roof to collapse – one worker was trapped and his body was discovered days later) LAB EXPLOSION Employee Injured in Laboratory Explosion (an employee at a laboratory was injured when a chemical reaction caused a small explosion in the building at around 2:30 p.m. – when firefighters arrived they found that the injured employee had already been taken to the hospital by a co-worker and that other employees had managed to put out the flames with a fire extinguisher) METHANE EXPLOSION Pig Barn Explosion Farm Worker Now In Critical Condition (one of the two farm workers, 25, that were victim to the pig barn explosion is now listed under critical condition – the other farm worker, 15, remains in serious condition – both men were airlifted to hospital after a methane explosion while draining liquid manure from a nearby pit) |
