INDUSTRIAL CRUSHING FATALITY Mill worker dies in crushing accident (a worker, 46, has died in an accident at a sugar mill – the man was crushed between two cane bins at the Mill receiving severe injuries to his pelvis – he was taken to a Hospital by ambulance and treated there by rescue helicopter medical staff, before being airlifted to another Hospital – he was in a serious condition when he arrived at the hospital and died at 2.30pm, some two and a half hours after the accident) LOTO/GUARDING MEAT GRINDER Man Caught In Meat Grinder (fire crews and hospital emergency workers combined their efforts in the operating room to save a worker’s hand trapped in a meat grinder – the man’s fingers had been caught in a food meat grinder – the accident happened as the man, a grocery store worker, was processing meat – the first responders set to work dissembling the machinery down to its smallest component so the man could be taken to the hospital -the man will likely lose “at least all of his fingers, if not his hand) LOTO/GUARDING CONVEYOR Man is hurt in Yukon grain mill accident (a worker’s arm was severely injured when he was caught in a conveyor belt at a Flour Mill – employees were loading a tractor-trailer with canola grain about 4:30 p.m. when the man’s clothing became caught in a conveyor in the basement of the mill – he was lifted 12 feet from the basement in a basket and was taken to a Medical Center) PAPER MILL FATALITY Worker killed at Muskogee Georgia-Pacific plant (officials are investigating after a worker was killed in an accident at a paper mill shortly before 11 a.m. – the man was working for an electrical contractor – no details of the incident have been released) UPDATE on CONFINED SPACE FATALITIES Sewage hell: SACP calls for action (four workers apparently tried to save a colleague trapped at an overflowing pump station – while their colleague succumbed to the methane gas fumes, the four workers then also became trapped in the rising sludge and died after rescue bids failed) PROPANE FLASH FIRE Man injured in propane accident north of Chico in “bad shape” (the state agency investigating a propane accident plans to give the burned worker weeks at the hospital, if not a month, before interviewing him – the employee had burns on much of his body and was in “bad shape” – OSHA was unsure what his exact condition was at the hospital – the man had filled up his delivery truck with propane from a tank when the fuel ignited, burning him – state investigators are still unsure what started the fire – a truck driver sitting in the office put out the fire and helped the man cool himself off with a hose while the owner called 9-1-1 – a helicopter took the man to a Hospital’s burn unit) FATAL EXPLOSION – ACETONE Worker spends six hours under rubble after plant explosion (plant explosion killed two workers at a chemical factory – a woman is in satisfactory condition after an explosion that tore apart – the woman dove under a desk that shielded her as the building collapsed around her – she remained buried for at least six hours under smoldering rubble, with toxic fumes covering the entire area – two bodies were also found in the rubble and 19 other workers were injured – two workers remained in hospital with severe burns – one worker suffered burns to 90% of his body and was airlifted to a burn unit – about 15,000 litres of acetone, a common solvent, ignited and gutted the factory – firefighters spent much of the night controlling the fire and trying to stop a nitrogen leak – the blast weakened an adjacent building that is at risk of collapse) UPDATE on ELECTRICAL – CPR and AED SAVE a LIFE!!! PSE&G worker shocked by 13,000 volts now is resting at home (a utility worker shocked by a 13,000 volt line is resting at home with his family after CPR, defibrillation and a trip to the hospital saved his life – the accident happened at a facility and the lineman immediately got help from fellow workers who performed CPR and from a police officer who responded and worked on him with a defibrillator – the entrance wound was in one hand, the exit wound was out the other, so it’s pretty clear he had 13,000 volts go through his body – the man will get further medical assessments later and could need surgery for his injured hands) FATAL EXPLOSION Third person dies following explosion, fire at Quebec plant (a third person has died from injuries related to a powerful explosion and fire at a manufacturing plant – a worker at the plant, died at the hospital Saturday evening – he suffered burns on 90 percent of his body and was in an induced coma – the bodies of two other people were found in the ruins of the plant after the explosion – 18 others were hospitalized – three people remain hospitalized with non-life-threatening injuries – the plant produces and exports health products) EXPLOSION Two badly burned in explosion at Bombardier plant in Quebec (the blast occurred about 20 minutes after midnight and appeared to come from a lab in the facility’s research centre – the victims are a 38-year-old employee and a 54-year-old security guard – both were taken to hospital and remained in a medically induced comas, although one is considered to be in stable condition) BOILER EXPLOSION @ POWER PLANT Blast at Electrabel Dutch power plant (a blast rocked the electricity plant sending clouds of steam into the sky – no one was hurt and power supplies to client were not disrupted after a boiler exploded, scattering debris across the facility – police advised residents in the area to keep their windows and doors shut and to remain inside, but the warning was lifted within two hours) AMMONIA Gas leak at ice-cream factory (an ammonia gas leak was reported at an ice-making factory – a team in Hazmat suit was sent to inspect the factory premises after a call was received at 10.30am. – a worker was evacuated from the building and the area was cordoned off from the public – he was sent to a Hospital for fear of being exposed to the gas hazard – leak was traced to a valve which was believed to have been opened by a worker) Ammonia leak at Owens Corning contained (an apparent ammonia leak at a warehouse was contained around 2:20 p.m. -fire and hazardous materials crews responded to the plant around 2 p.m. – the on-site response crew had the situation mostly contained when units arrived – no injuries were reported) CHLORINE DIOXIDE Evacuations in Jackson, Ala. Due to Chemical Leak (a small chemical leak from a tank at a paper mill has forced the evacuation of nearby residents – the chemical is chlorine dioxide CLO2, which started leaking from a tank at the paper mill around 8:55 a.m. – a small RV park near the plant was evacuated as a precaution – crews have been unable to stop the leak, but say they have been able to keep it from getting worse – the plant is on the Tombigbee River and the wind has been blowing steadily toward the river, and away from populated areas – still, authorities are on standby to order more evacuations, should they be needed – they will utilize a reverse 9-11 telephone calling system in addition to going door-to-door – there are no reports of any injuries) CRANE Police Blotter (rescue workers freed a man trapped beneath a dump truck on a logging site – the man, whose name was not available, suffered two broken legs and was flown by helicopter to a Medical Center – the accident happened around noon as the crane operator tried to lift the trailer of the tractor trailer dump truck that got stuck in the mud while it dumped a load of stone for a road to the site – the crane operator tried to lift the trailer and it toppled onto the crane trapping the operator – rescue workers had to cut holes in the dump trailer to free the man – it took nearly an hour to extricate him because of the amount of rocks still left in the trailer) FORKLIFT Forklift truck accident blacks out 2,000 homes (two thousand homes Burton were plunged into darkness after a forklift truck was driven into an electrical substation – the whole substation needed to be replaced after the accident and properties had to be disconnected from the grid while this work was carried out – the forklift truck, which belonged to a ‘third party’, shunted into the substation at around 6pm, causing it to move several feet – the safest option was to replace the whole substation instead of attempting to patch it up) CONSTRUCTION WORKZONE FATALITY State worker killed on NC 12; highway closed again (the highway that connects Hatteras Island with the mainland is closed again after a worker for the state Transportation Department was killed there – the accident occurred in an area where crews were removing sand in the wake of Sandy and the recent nor’easter) WORKZONE Worker Hit By Truck In Construction Zone (a worker, 56, was critically injured when he was struck by a vehicle in a construction zone – the man was reportedly setting up traffic cones about 7 a.m. when he was hit by a full-size pickup truck estimated to be traveling at least 45 mph – the force of the impact threw the man about 35 feet – he was airlifted to a Hospital as a “trauma alert”) WORKZONE Worker hurt in Hwy. 41 construction accident (a construction worker was hurt this morning in a Highway construction zone – crews were paving the new road, when the worker suffered a non life-threatening injury around 9:45 a.m.) FATAL FALL Construction worker who died after falling from SUNY Oswego structure identified (a construction worker, 37, died after falling from a structure – he was an ironworker – the incident was reported at 8:25 a.m. – he was transported to a Hospital where he was pronounced dead – he was installing metal roof decking on a connector between two buildings on campus as part of a renovation project) MANLIFT and FALL Worker injured in cherry picker truck accident (a worker, 45, suffered serious fractures and internal injuries after the cherry-picker bucket he was working in collapsed – the man fell about 10 metres to the ground at about 3:40 p.m., while the man was working on a job site – he was taken to hospital where he was in serious condition) WORKZONE FATALITY Worker killed in accident in Madera County (a worker digging a cable line was killed when he darted in front of traffic to get a shovel and was struck by an oncoming car – he died at the scene – the driver has not been sited) FATAL FALL Fort Collins worker fall to his death in home construction accident (a construction worker, 40, died in an accidental fall on the job – the victim was a framer and was working on the roof of a home when he fell through a stairwell and into the basement, landing on a concrete floor – he suffered fatal head and neck injuries) MANLIFT Ottawa worker hospitalized after 9-metre fall (a construction worker, 45, is in hospital after he fell nine metres to the ground while in the basket of a cherry-picker lift at 2:41 p.m. – he suffered fractures to his legs, as well as internal injuries – he was in a basket atop the double-armed cherry-picker lift when the basket collapsed – he was taken to hospital in serious condition) BACKHOE FATALITY Accident kills man at Galveston shipyard (a backhoe accident at a shipyard has left a worker, 40, dead – he was operating a backhoe from outside the cab when he became pinned by the boom – the crew was digging a hole on a project at the time of the accident) HOTWORK Welding accident causes small fire at O.C. boardwalk (a welding accident nearly caused a large fire along the Ocean City boardwalk – workers were doing some welding on the Ocean 17 Condominiums – at some point, the welding started a small fire underneath the floor – emergency crews say they had a hard time extinguishing the blaze because of it’s location, but were able put out the fire before it caused any major damage to the building, or the boardwalk – authorities are still investigating) MISCELLANEOUS FIRE 3 Injured in Gasoline Fire at Midtown Restaurant (a worker at a restaurant was carrying gasoline in five gallon soy sauce containers – three restaurant workers are recovering from burns they suffered at a restaurant where some Sandy relief workers stepped in to help put out a fire – a worker was carrying gasoline in five gallon soy sauce containers, when one spilled in the kitchen, sparking a fire – several upstate New York tree trimmers who were in town for Sandy relief efforts and happened to be eating at the restaurant, stepped in to put out the flames – investigators think one of the restaurant workers, who disappeared after the fire began, may have been hording the gasoline during this time of gas shortages – one of the restaurant workers suffered severe injuries in the fire) CRANE Worker crushed by crane (a worker, 29, has been taken to hospital after being crushed by a crane just before 1:30 p.m. – a worker removing trees from a residence got caught up in the crane – he was on the first day of his job, was jumping off the flatbed of the truck when the crane swung around and crushed him between the flatbed deck and the crane – he was taken to a hospital with serious but non-life threatening injuries) UPDATE on ELECTROCUTION Worker electrocuted at Tunbridge Wells M&S was fixing microwave (the man electrocuted in a retail store was trying to fix a staff microwave at the time – a contractor, 62, died at the scene in a staff-only area at the store – he was pronounced dead at the scene) HOTWORK Welding works cause of Naviti Resort fire (fire is said to have started as a result of some welding works being carried out in the resort’s main building – a spark may have most likely ignited with some material causing the fire to spread – it has also been confirmed that there were some repair works being carried out at the back of the kid’s club area earlier in the day which is where the fire is said to have started from) |
