INDUSTRIAL CONFINED SPACE FATALITIES – ONE ENTRANT and TWO WOULD-BE RESCUERS Bangalore: Three Workers Die of Suffocation at Peenya Factory (three workers, 20, 24, and 26, at an iron forging factory got into an oil tank and died of suspected asphyxiation – five workers including the three victims, who worked as helpers, were on night shift when the accident occurred – the iron is melted in a furnace and moulded into bars and pipes at the factory for which furnace oil is used – the oil is stored in a tank in the underground sump behind the factory – usually, the oil is lifted from tank using a pump connected to the furnace – but on that night, the motor did not work and the workers thought of lifting the oil manually – one got into the tank first to lift oil in buckets – as he did not come out or answer any of the calls by those above, his brother and another worker entered the tank in search of him) 2 injured in Exxon Mobil plant accident in Texas (two workers were injured in an accident at a plant – the wounded were taken to a hospital where they are receiving treatment and undergoing evaluation – the victims suffered burns, and their conditions were not immediately known – they were employed by a contractor – the cause of the incident remains under investigation and the company hasn’t released any information about it) FLASH FIRE Worker burned in face while painting stripes on County Road 210 (a man suffered severe burns to the face after a flash fire occurred from a propane tank on his work truck – he was taken to a local hospital in a personal vehicle after receiving burns to his face – he was preparing to paint lines on the road when a flash fire occurred for unknown reasons – there was no fire and there were no other injuries reported) RAILYARD FATALITY Regina CP rail yard worker dies after train incident (around 3 a.m. a worker, 39, was fatally injured by a train movement) FATAL EXPLOSION of GAS CYLINDER Three workers injured in welding gas cylinder explosion (three workers were seriously injured when a gas cylinder meant for welding and cutting of iron exploded in a private steel mill – the engineer and two of his associates were working in the welding unit of the stage-II plant when the cylinder filled with oxygen and acetylene gas burst – the gas was being used for welding and cutting purpose when the mishap struck – the injured were first admitted to a hospital and then shifted to another hospital) MANLIFT Worker injured in equipment rollover (a contractor, 33, working at a landfill was airlifted to a hospital after being involved in an equipment rollover at 11:23 a.m. – he was operating an articulated hauler when it rolled down an embankment – the cause of the accident has not been determined, but it appears to have been the result of a mechanical malfunction of the hauler – the equipment rolled 50-60 feet down the embankment, causing the worker to sustain fractures and a head injury) REFINERY FIRE Equipment at Chevron El Segundo Plant Said Damaged in Fire (a fire at a refinery damaged equipment associated with the plant’s fluid catalytic cracker – the blaze, which ignited at around 6 p.m., damaged the selective catalytic reduction plant, a unit that removes particulates from gas produced by the catalytic cracker) REFINERY FIRE Flint Hills reports seal pump fire at Corpus refinery (the shut down process caused a flaring event – there were no injuries associated with the event, nor was there a call for the community to take action – the seal pump fire was extinguished quickly) MINE Accident at United Taconite Plant Sends One to Hospital (an accident sent one man to the hospital – a fuel truck overturned around 9:00 a.m, trapping one man, who is an employee of the mine – the man had to be extracted from the truck – he was transported to a hospital – no official cause of the accident has been determined yet, however he noted that road conditions were icy at the time the accident) FIRE Fire at ONGC plant, two dead in Assam (a fire swept through a unit killing two persons and destroying property worth lakhs of rupees – the rapidly spread through the entire GGS burning thousands of tonnes of oil – it took hours for the ONGC’s fire department to douse the flame – bodies of two workers were found – the exact amount of loss or the reason for the fire is yet to be ascertained) AMMONIA Ammonia leak leads to evacuation, no injuries reported (around 3:15 p.m., the fire department received a call about the pungent smell – Hazmat units arrived on the scene, went inside the facility and stopped the leak – firefighters evacuated children from a school, two houses near the school and closed off streets in a two-block radius from the facility – there are no reports of any injuries – the leak began in a stem on a valve inside the ice plant) CHLORINE Chlorine gas leak contained in Long View (several area emergency departments responded to a chlorine gas leak at a Water Treatment Plant – water treatment operators were changing out a large pressurized chlorine tank used in the partial filtration area of the plant when a small leak was discovered in the tank) CONSTRUCTION FATALITY run over Freak Accident Kills Worker (a construction worker, 23, was killed when he may have been attempting to jump onto a trailer when he lost his balance and fell – he was apparently crushed to death under the impact of two tires around 10.20 a.m. – he just starting working at the site three months ago and was a trenching guy) WORKZONE FATALITY Two killed in road accident while installing cables (two workers were killed in an accident involving a van and a truck at 8.30 a.m. – they were pronounced dead by paramedics at the scene – a third worker who was injured was found by the side of the road- he was sent to a Hospital in a conscious state – the workers were carrying out cable installation work at the time of the accident) WORKZONE FATALITY Road cleanup worker killed in hit-and-run accident (a road cleanup worker, 35, was killed in a deadly hit-and-run accident around 8:25 a.m. – he died at the scene – deputies said they’re looking for the driver that took off) Girder falls, kills metro worker (a metro rail worker was killed and three others were injured when one end of a giant steel launching girder, used to assemble the elevated viaduct, crashed at a work site – police arrested two site engineers and two safety officers in connection with the accident – the accident happened at 3.30 a.m. when the giant girder was being moved from one span to another for works on top of a tall pillar – the four workers were on top of the girder when it fell) CRUSHED by PIPE FATALITY Utah pipeline worker killed in accident (a worker, 62, at the construction site of a natural gas pipeline has died after he was accidentally crushed by a steel pipe – a machine that bends sections of steel pipe slid into the pipeline trench around 10:30 a.m. – he grabbed a piece of pipe that was in the machine to try and stop its fall – the machine still fell into the trench and the worker was crushed by the pipe as it fell to the ground) FATAL FALL 2 workers fall to death at construction site (two labourers, 20 and 23, fell to death from the sixth floor of an under-construction hotel – one of the labourers died on way to a hospital, the other succumbed to his injuries two hours later – the incident took place at midnight when the workers were disposing of construction waste from the sixth floor of the building – they were throwing the waste in a shaft which was extended from the second floor to the sixth floor of the building – one of the labourers lost his balance while working and fell into the shaft – in an attempt to save his co-worker the other labourer also fell) FALL from SCAFFOLDING Worker falls from scaffolding into River Hull dock (a worker, 40, fell from scaffolding into a dry dock – it is unclear how far the man fell – he had been carrying out work on the scaffolding in a dry dock at about 1 p.m. when he fell) MISCELLANEOUS POSSIBLE HEATSTROKE Heat stroke suspected in worker’s death (authorities are investigating whether a man, 38, died as a result of heat stroke – he died after falling ill at an industrial site – he had a heart attack on the way to hospital and was pronounced dead on arrival – authorities are investigating if the man was suffering from heat stroke) FALL from LADDER UM worker falls off stadium Jumbotron (the cold weather may have played a role in an employee falling off the stadium – the man fell six to eight feet off a ladder onto the catwalk below – the city fire department responded using high-angle rescue techniques to get the man down safely – the worker is being treated with minor injures at a hospital) FALL/FLIGHT Worker seriously injured while cutting down large tree in north Harris Co. (an accident seriously injured a worker who was cutting down a large tree – he was working to remove the tree when it suddenly began falling the wrong direction – the worker, who was tied to the tree, was thrown 20 feet into the air into another tree – he was flown to a hospital with serious injuries) |
